I own a 2016 Kia Optima EX with VIN [XXX] . The engine failed at 85,763 miles due to a rod bearing seizure. This matches the known defect in Kia’s 2.4L GDI engines, which has been the subject of recalls and service campaigns in the past. Despite being covered under Kia’s extended engine warranty, Kia America denied my engine replacement claim. They cited "vehicle reconditioning" and "multiple
... ownership" as reasons — neither of which are valid grounds to void warranty coverage, especially for a known defect. A certified Kia dealership (Mark Kia in Scottsdale, AZ) approved my vehicle for warranty replacement, and submitted the request. Kia corporate denied it. This creates a serious safety concern and shows that Kia may be wrongfully avoiding responsibility for this defect. Please investigate. Generally, for the entire time we've owned the car, it has ongoing and systemic electronics malfunctions. This ranges from the car slamming on its own brakes while driving to systems not working to the seat shifting on its own while driving to false positives in the NAV system, etc. A lot of the malfunctions are funny, some are dangerous, but the common thread is that in spite of months in repai
... r and countless supposed repairs by dealerships, the problems continue. And, yes, some of these absolutely put safety at risk (slamming on the brakes for no reason, false positives on detecting objects in road, seat moving during vehicle operation, speedometer ceasing to work, etc). Chrysler has confirmed and acknowledged these problems in the form of various settlements in spite of our requests for a buyback, offering us money, but leaving us saddled with the lemon. What I'm complaining here about, however, is that for the second time the vehicle has apparently caught fire in the engine when started from being off. The first time, it was supposedly fixed by replacing a sensor, but it did it again today, stranding my wife and young son in a parking lot. This car is a death trap and will, sooner or later, hurt or kill someone.Read more

While driving less than 10 mph, (slightly down hill) approaching the house garage, the car suddenly accelerated and the brake was applied to stop but the car continued and smashed through the garage door, proceeded through garage, smashed through the back of garage, hit a Weber gas grill, proceeded over a low-level deck and hit a large tree. The impact force of the car (left front) against the tr
... ee caused the car rear end to rise up and move 30 degrees to the right and land on the low-level deck. The driver was the only person in the car and suffered a broken right wrist and contusions to her chest. Both Front air bags did not deploy, side air bags did deploy. There was no warning given and the Car collision avoidance system did not stop the car. The damage to car was total. Car Emergency Voice activated and assisted in notifying ambulance and police. Insurance has reviewed the damage, but I am not aware that they are investigating for cause. There has been one other sudden acceleration reported of a 2025 Crown Signa. There is history of malfunctions of the Electronic Throttle Control (ETC) (per Google). There is an Event Data Recorder (EDR) on the car, but I don't know what is on it. This needs to be investigated. (If this unintended acceleration happened in a grocery parking lot, or on a highway, there could have been serious injuries or a death).Read more

I have a 2022 Chevrolet 1500 Trail Boss with the 6.2. Intermittingly when starting off at a red light or from a stopped position, when you press the gas pedal to go the engine will stall and then will suddenly pick back up after a second or two. This intermittingly issue makes it very dangerous when turning in front of oncoming traffic, or in a situation when sudden acceleration is needed. The mot
... or ticks even at idle when running. Also, when you come to a stop at a redlight, the engine shuts off, when you release the brake to go, the auto start restarts the engine. On my truck, when the starter restarts the engine, the starter will not disengage. You have to put the truck in park, turn the switch off and manually restart the engine to make the starter quit spinning over. This problem is also intermitting. Sometimes it does it, sometimes it dont. I took my truck to Mike Bell Chevrolet twice in Carrollton Georgia, where I bought it. The first time I took it to them was 2-10-25, it was for the starter hanging up in auto-start, they charged me $199.00 to do a diagnostic, told me they could not replicate the issue. Then I took it back to Mike Bell Chevrolet on 7-2-2025 after my wife had called the service department and told them that the engine stalls sometimes when starting off, the woman at Mike Bell said that this is one of the symptoms of the recall on my truck. She said we need to bring it in. They called me back and said they could not get it to replicate the issue and wanted to charge me another $399.00 for checking it. We refused to pay this because it is an active recall and we were advised to bring it in. If this engine stalls in traffic, I want it documented that the engine intermittingly stalls when starting off sometimes in case there is an accident.Read more

While driving the engine seems to "go out" which is a risk to the driver, passengers, and other motorists near by because it is not expected and can happen anywhere (two lane road, major highway, while turning, etc.). You are able to press the gas and hear the engine but the car does not move - it acts as if it were in neutral. There are no safety or warning lights on the dashboard before or duri
... ng this issue meaning that there is no way for the driver to prepare. There have been times where I was able to pull off the road and restart the car to temporarily remedy the issue but safe and accessible pull offs are not always available (bride, two lane road, etc). I travel with small children and there is a fear that if this continues to happen it can result in fatal consequences. In addition to the safety issue mentioned above there is a perpetual leak coming from the engine. This has progressively gotten worse and after speaking with a representative at Ford they named off a few "possible" causes but were unable to make a definitive decision. I was also informed that the issues that I am experiencing were not unique to just my vehicle but many Ford Escape 2013's in the area that had come to the dealership for guidance and help. While I have stated that the issue occurred in 2013 - it is still happening and continues to get worse.Read more

Dear Sir or Madam, I am writing to file a formal complaint regarding a critical safety failure of my Fisker Ocean One. This incident involved a sudden and total loss of power while driving, which created a severe safety hazard for myself and other motorists. Date of Incident: [Date the incident occurred] Location of Incident: [Name of Highway, City, State, e.g., Interstate 35, Austin, TX] Detai
... led Description of the Incident: On [Date of Incident], while I was operating my Fisker Ocean One on the highway, the vehicle experienced a sudden and complete loss of motive power without any prior warning. The vehicle decelerated and came to a stop in a live traffic lane, rendering it a dangerous obstruction. Immediately following the power loss, I made multiple attempts to restart the vehicle and engage "Drive." The vehicle's systems were unresponsive. Despite repeated efforts, the gear selector would not shift into drive, and the vehicle remained immobile. Shortly after these failed attempts, the vehicle's entire electrical system powered off, leaving the car completely disabled on the highway. This catastrophic failure created an extremely dangerous situation, leaving me stranded in a vulnerable position and at high risk of being struck by other vehicles. The inability to move the car, even to the shoulder, constitutes a grave design or manufacturing defect. Given that the manufacturer, Fisker Inc., has declared bankruptcy, I am not filing this complaint with the expectation of a warranty repair or corporate response. Instead, I am submitting this report to formally document this dangerous defect for public safety records and to urge your agency to investigate this issue. The sudden and complete failure of the powertrain and electrical systems in the Fisker Ocean presents a significant risk to all owners and the publicRead more

Component/System That Failed: The 10R80 automatic transmission is malfunctioning. It causes harsh shifting, delayed acceleration, hesitation, and unpredictable surging. These issues have been confirmed as part of Ford’s internal TSB 23-2250, which directly references this transmission defect. The vehicle is available for inspection upon request. Safety Risk: This transmission failure poses a se

The delayed response time compromises my ability to move safely through traffic. Reproduced/Confirmed: Yes. The issue was confirmed by Liberty Ford in Fayetteville, NC, after I paid for a diagnostic. The service department acknowledged the symptoms but refused to cover the diagnosis or repair under warranty since no recall was showing under my VIN. Inspected by Others: The vehicle has not been inspected by the manufacturer directly or by law enforcement or insurance. However, it was diagnosed and documented by an authorized Ford dealership (Liberty Ford in Fayetteville NC). Warning Lamps or Symptoms: There were no warning lamps or error codes. The symptoms began subtly but became consistent over time. The first noticeable event was approximately in “early 2025”, with increased severity since then. I submitted a written complaint to Ford on June 9, 2025, via USPS Priority Mail (Tracking #[XXX]) and also filed a certified safety complaint with NHTSA (Tracking #[XXX]) but have received no response. All doors were unlocked when I placed the baby in her seat and closed the right passenger doors. I walked around to open the drivers side door when I discovered all doors were locked. The vehicle is designed to not be able to lock with a key inside. We have been unable to manually reproduce the incident which leads us to believe the problem is in the electronic coding and needs attention and service from the manufacturing level. This could have easily ended in a fatality. I do not want this to happen to anyone else. I submitted another complaint to Mercedes - this time via letter on their website. At any given time without warning the car will down shift into 4, 3, 2, or 1. It is intermittent but in the last 3 weeks has become very frequent. I have taken it to the dealership. They state that it is the internal clock spring in the the steering column. They have told me that my power train warranty and/or technology extended warranty will honor the replacement, so I looked up possible r
... ecalls on this model vehicle and found there is in fact a recall on the steering column internal clock spring but the concern was on the airbag. The issue I am having with the downshifting is going to cause someone to rear end me. There is no pre-warning. I have driven on service roads, the interstate, my neighborhood streets, and on highway. It seems to happen more as I am following the curve of the road, or when I am turning or changing lanes/merging onto interstate. If I am at a complete stop and turn either left or right it will downshift to 1. I am also concerned to the extent of damage this may be doing to my transmission and engine, as it happens during speeds of 45 to 75 mph on the highways and interstate. I do have video on my phone of this behavior and I can send it to you if I have an email address you have access to. I have placed the most current date of this behavior in the incident box, however this has happened numerous times over the past few months, It just wouldn't give a code to any dealerships for diagnosis.Read more

On [XXX] my Jeep started making a grinding noise near the rear axle. Initially, thinking something had broken when I had hit a pot hole earlier that evening. We parked the car at a garage and the next day inspected it and said it sounded like the rear differential may be effected, advising it should be safe to drive in “sport” mode. I drove it to Blueridge Chrysler Jeep in Abingdon VA where I
... informed them I had discovered a Recall 45A on the Jeep, they kept the Jeep and called me in a few days saying it was only for an “software update” and it was fixed. Om [XXX], I drove my Jeep and upon getting less than a mile down the road it sounded like the backend was dragging. They called a tow truck & took it back to the garage & now with the MOPAR Extended warranty they would have to replace the PTU. It was part of the initial recall and it could affect the PTU and the RDM (rear differential). This part took months to be delivered and during that time I had to rent and borrow a car to get back and forth from work for over two months. In May, the service dept. called saying the Jeep was still making the dragging noise and may be the transmission or the Rear Differential, but they’re still looking into it. In June, after re-working the PTU, Blueridge has determined it is the Rear Differential that now needs replaced and when I asked about it all being a part of the MOPAR extended warranty and they told me I needed to call Chrysler Customer Care. I followed up and called and spoke to several people advising that the dealership would have to call MOPAR warranty. Today, 7/7/25, I spoke with Blueridge again and they are saying the Recall was only for an update. I referred to the details of the initial 45A recall that specifically says that if the update is not complete, it would lead to problems with the PTU & the rear differential. My stance is that MOPAR warranty should cover both the PTU and the Rear Differential under Both recalls the 45A and 20V343000. While parked at home, my Tesla Model 3 developed a spontaneous windshield crack with no known cause. The car had not been driven recently, and there was no visible chip, impact, or external force involved. I believe this is a stress or pressure crack, which compromises visibility and may reflect a structural flaw in the glass or how it was mounted. I submitted a warranty claim through Tesla’s a
... pp and preserved the vehicle in its exact state for inspection (no washing, no driving, no impact). Tesla denied the claim without conducting a physical inspection or providing any supporting evidence. This kind of uninspected warranty denial is unacceptable, especially for a crack that can impair driver visibility. I have found similar reports from other Tesla owners online, indicating a potential pattern. If this issue is systemic, it could pose a broader safety risk related to poor windshield durability or design tolerances. I am asking the NHTSA to investigate this issue and Tesla’s handling of it.Read more

On [XXX] at [XXX] I dropped my wife and 2 children off at the Funland Arcade in Deep Creek, Maryland. The parking lot was full from the holiday weekend and I was planning on taking the Lightning to a nearby charger while the family gamed so I double parked perpendicular to the arcade entrance, put the hazards on, and let them out. After the 3 of them got out, I placed the Lightning in reverse to
... back up 5-10 feet so that I could exit the parking lot through the same way I came in. Once I had enough space from reversing so I could make the turn to exit I placed the Lighting in drive. The moment the Lightning switched to drive it accelerated at a very high speed as if I was flooring the gas pedal, which I was not. The lightning was in one-petal mode which I always use unless I’m on a highway. I immediately began pumping the brakes but the truck would not stop, I probably slammed on the brake petal 4-5 times to no avail. I even tried shifting the truck back into park but it would not shift out of drive. I was able to luckily steer the truck into a hillside approximately 150 feet away. The truck ran over numerous small trees and shrubs before crashing to a stop approximately 16 feet up the hill. Shortly after the shock wore off, I slowly reversed the truck down the hill, exited the truck, and informed dozens of onlookers who came out of the arcade to see what the noise in the parking lot was from, that the truck just took off on its own but that I was fine. I called 911 and the police came. Luckily no people or vehicles were between the arcade entrance and the hillside or the outcome would have been significantly worse. This crash was witnessed first hand by wife and 2 children. I have grave safety concerns about this truck now and will not put my family or self back in it until Ford fixes this problem. I am making this complaint in case this information can help prevent future crashes of this vehicle. The vehicle was a 2023 Toyota RAV4 hybrid. When the driver attempted to park the vehicle in a parking lot, it suddenly began moving forward without use of the accelerator. The driver applied the brakes but the pedal went to the floor without braking. The vehicle seemed to be moving faster than expected without use of the gas pedal and went through the parking lot. The driver stopped the vehicl
... e by running up over a curb, through a traffic island with soft dirt and mulch and finally crashing into a metal storage unit. Two air bags deployed. Toyota was notified of the crash and a Toyota representative made the 911 call. A New York state trooper and two fire trucks arrived. The vehicle was towed and eventually collected by our insurance company. The tow truck driver said the brake pedal went to the floor when he tried it. The car was declared a total loss. 1. The notable failure was in the braking system. The car also seemed to be traveling faster than it should and didn't slow down. The insurance company has the vehicle now. 2. The safety issue was loss of speed and braking control of the vehicle. Steering was normal. Fortunately the parking lot had few cars or people in it. 3. No investigation of the problem aside from assessment of damage that I know of. 4. Liberty Mutual assessed the damage. The NY state trooper made a collision information form and gave us a copy. 5. No warning lights or related symptoms were noted prior to the car traveling on its own. After the crash, a local Toyota dealer called and said that they were notified that the check engine light was on and there was a problem with the hybrid system, along with other warnings.Read more
