Vehicle stalled approximately 15 times between April 17 and May 4, 2026, including once on Interstate XXX at approximately 50mph with minor children ages [XXX] and [XXX] in the vehicle. The oil and battery lights illuminated simultaneously at each stall. Diagnosed as ABS module failure first, then left front wheel speed sensor failure on the 2nd occasion, both confirmed by a Hyundai dealership.

While attempting a slow right turn from a complete stop at an intersection, I heard a loud thunk sound followed by an immediate loss of power steering response. The steering wheel spun freely with little resistance while the front tires did not respond proportionally. I was able to safely pull over. Upon restarting the vehicle it produced an abnormal wheezing sound suggesting additional mechanical
... stress. At the time of the failure the vehicle had approximately 40,000 miles, which I believe represents a premature failure for a safety-critical steering component. The vehicle was brought to a Hyundai dealership for inspection. The dealer pulled diagnostic fault codes, found none, performed a 20-minute test drive, and returned the vehicle stating they could not find anything wrong. No physical inspection of the steering components was performed. The failure condition — a slow full-lock turn from a complete stop — was never specifically replicated during the dealer's test drive. The problem has not been confirmed or repaired by the dealer. The vehicle is available for inspection. My safety and the safety of other drivers was put at risk by the sudden and unexpected loss of steering control. No warning lights appeared prior to or during the failure.Read more

Vehicle primarily failed while parked at owner residence [XXX] and remained inoperable for approximately three weeks prior to tow to Cadillac dealership. The ignition key became trapped and could not be removed. A manual override release beneath the driver-side steering column was required to free the key — confirming the lock was held electronically, not mechanically jammed. Fault confirmed by
... authorized Cadillac dealer: intermittent electrical fault within the shifter assembly lock mechanism. A similar intermittent concern was previously documented on this vehicle in 2011 (logged as "Customer Concern Not Duplicated"), establishing a recurring pattern. GM Technical Service Bulletin 07-07-30-032D (2006–2011 Cadillac CTS, automatic transmission) explicitly documents this exact condition: key sticking, Shift to Park DIC message, shifter binding, and battery drain. OEM replacement part is now discontinued. Manufacturer declined goodwill or recall coverage. Dealer refuses repair unless owner assumes full financial responsibility for a used part with no warranty and no guarantee against recurrence. Safety concern: ignition/shift interlock failure causing vehicle immobilization, key entrapment, and loss of transportation for an elderly owner dependent on the vehicle for medical care. Defect evades shop diagnosis due to intermittency. No safe OEM remedy exists. Prior recall: NHTSA 14V394000 / GM Campaign 14172 addressed same interlock system. Current failure represents recurrence beyond that recall's scope. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

My 2025 Tesla Cybertruck AWD (VIN: [XXX] ) has a recurring failure of the right rear seat occupant detection system, which governs airbag deployment and is certified by Tesla to meet FMVSS 208. The same alert (internal code RCM2_a698) persisted through two official repairs at Tesla Palo Alto: 1st repair (Invoice XXX, 8/22/2025): seat belt buckle replaced. Alert recurred 2 days later, August
... 24, 2025. 2nd repair (Invoice XXX, 9/18/2025): harness assembly replaced. Tesla then performed a Visual Quality Check, recording in the invoice: "Technician performed a visual quality check to ensure its condition meets our standards." Vehicle returned as repaired. Relying on Tesla's written certification, I reinstalled my [XXX] son's car seat in that position. He rode there daily for approximately 7 months (September 2025 - April 2026). On April 21, 2026, I tested the seat and found the sensor still actively failing - unable to detect an adult occupant. My spouse witnessed this. I have 30-second video. Alert RCM2_a698 remains active in Tesla's systems today, confirmed by screenshot. This is my family's only vehicle. My wife currently uses the right rear seat. Airbag deployment for that seat cannot be relied upon. Tesla has been notified and has not resolved this issue. I would like to report a serious safety concern regarding Tesla Full Self-Driving (FSD) on HW3-equipped vehicles. My vehicle repeatedly performs sudden and aggressive braking while FSD is engaged, even when there are no visible obstacles, vehicles, or hazards ahead. In some cases, the vehicle slows dramatically or comes to a complete stop unexpectedly. This creates a significant rear-end collisio
... n risk, especially on higher-speed roads where surrounding traffic does not anticipate abrupt braking behavior. This issue has been occurring for approximately one year and appears to have worsened over time and TESLA hasn't released a single FDS software update for over a year for HW3 vehicles. I recently brought my vehicle to Tesla service, paid for diagnostics/repair, and the issue still remains unresolved. I have also personally experienced the same behavior in another HW3 Tesla vehicle provided as a loaner from a Tesla service center, which suggests the issue may not be isolated to my individual car. Based on my experience, this appears to affect multiple HW3 vehicles operating with FSD enabled. The unsafe braking behavior causes surrounding vehicles to tailgate closely and react suddenly, increasing the likelihood of accidents. In my opinion, the current FSD behavior on affected HW3 vehicles presents a safety hazard for both Tesla owners and other road users. I believe this issue warrants immediate investigation. Until the root cause is identified and corrected, Tesla should consider disabling or restricting FSD functionality on affected HW3 vehicles to reduce the risk of accidents caused by phantom braking and unsafe autonomous driving behavior.Read more

My 2022 Honda Pilot failed to engage Drive after reversing, despite the Drive button being pressed. This resulted in an unintended backward motion & a collision with another vehicle. 1.What component or system failed or malfunctioned…? The Electronic Transmission Shift-by-Wire system (push-button shifter) failed to respond to a gear change request. Additionally, related electrical systems includ
... ing the dashboard infotainment system & the integrated USB charging ports have malfunctioned. The vehicle & its components are available for inspection upon request. 2.How was your safety or the safety of others put at risk? The vehicle experienced unintended motion. While attempting to move forward in a parking lot, the vehicle remained in Reverse despite selecting Drive. This failure led to a low-speed collision with a truck’s tow hitch. This defect creates a high risk of crashes, injuries to pedestrians, or property damage because the vehicle does not move in the direction the driver has selected. 3.Has the problem been reproduced or confirmed by a dealer or independent service center? The vehicle was previously serviced at a Honda dealership for a radio/infotainment recall. Since that service, the transmission has failed to engage Drive multiple times at random. 4.Has the vehicle or component been inspected by the manufacturer…? The vehicle was inspected during the initial recall fix. And brought back because after the fix because the charging port stopped working/continued screen glitches. Following the recent collision, it is awaiting further inspection by a Honda dealership to determine the extent of the mechanical failure. 5.Were there any warning lamps, messages prior…? Symptoms began after the recall fix & included intermittent electronic failures of the charging ports/screen. There were no specific transmission warning lights prior to the gear selection failure; however, the persistent electrical issues in the dash area suggested a systemic electronic malfunction.Read more
