The component I think that is faulty, is the, "shifter return spring." That component is believed to be apart of the power train system. With this spring being generally located inside of the power train itself, it call only be inspected when the power train is taken apart. A physical demonstration could be performed to show the defect in action. With me being unable to downshift, on demand as th
... e operator is certainly a hazard to myself and all other vehicle operators. I definitely feel like a hazard to public safety and others while riding. The problem has not been reproduced or confirmed by a dealer as of yet. Since the warranty has expired, getting the issue looked at and or resolved, out of warranty will end in a costly repair. The vehicle has not been inspected by manufacturer, when I emailed Kawasaki directly about the issue and my concerns, they simply didn't show much interest into my concern, and just shoved me off to deal with my local Kawasaki Dealer/Shop. I then emailed my local shop about my concern before the warranty expired, no response was directly back to me. Leaving me to operate a motorcycle that has a down shifting defect. They are no lights or warnings present. Since the issue seems to me mechanical, but on the interior of the power train. The first event reference I have happened about a few months ago. It all of a sudden out of nowhere, stopped returning the shifter lever back to its neutral position when downshifting. I couldn't understand in that very moment was happening, and nearly panicked! Being unable to downshift on demand, without extra foot manipulations, could be scary while operating a motorcycle. Even as I ride to this day, the defect is readily present. I'm not the only bike owner of this year, make and model that has this issue.Read more

While proceeding through a four-way stop, my vehicle unexpectedly lost driveline engagement. I had to shift repeatedly between Park and Drive before the vehicle would respond and move again. This is not an isolated incident. The same failure occurred pr
... eviously in early 2024. The vehicle has also exhibited persistent transmission issues, including gear hunting and harsh downshifts, particularly a hard clunk during the 4–3 shift on deceleration. The dealer has dismissed these symptoms in the past, stating it was due to the transmission being "cold." However, the intermittent loss of driveline engagement presents a serious safety risk to myself and others, especially when it occurs in active traffic situations. This is the second time the vehicle has left me stranded without warning. I’m extremely concerned that Ford has not addressed this recurring safety issue. The condition is unpredictable and dangerous. Immediate investigation and corrective action are needed.Read more

Issue Description: While driving during daylight hours, especially when the sun is positioned at a certain angle, I have observed an intense reflection from the dashboard onto the inside of the windshield. This reflection is bright and widespread, directly obstructing my field of vision and making it difficult to see the road ahead clearly. The glare is particularly noticeable on clear or sunny da
... ys and significantly increases the risk of an accident. This visibility issue appears to be related to the dashboard material and design, which may not adequately diffuse or absorb sunlight. I believe this condition may affect other drivers as well, potentially leading to widespread safety concerns. I respectfully urge the NHTSA to investigate this matter and consider whether this condition warrants further examination or manufacturer guidance to reduce the risk associated with glare-induced visibility loss. Please let me know if any further information or documentation (such as photographs or video recordings) would be helpful in reviewing this issue. Thank you for your attention to this matter and for your continued efforts in promoting road safety. Subject: Safety Hazard – Turn Signal Button Placement on Tesla Model 3/Y (Newer Models) Hi there, I am reaching because the newer Tesla models have eliminated the turn signal stalks and instead incorporated the turn signal controls as buttons directly on the steering wheel. This became a critical safety hazard multiple times when taking a turn and had to give the turn indication. The issue happ
... ened because the turn signal buttons are not fixed and they rotate along with the rotation of the steering wheel. It makes us look for the turn signal buttons every time the steering wheel is turned and becomes a safety hazard when in motion. Also, it is very challenging when we have to make a right/left turn while on the ramp to take an exit because the buttons are not fixes unlike the turn signal stalks. There were times when we were unable to give the turn signals. It is not only dangerous for the Tesla drivers but also for the other drivers on the road to effectively communicate. Please, I request you to re-assess this configuration. Thank you!Read more

Incident date: Jul 25, 2025
Two weeks ago, I had my 2022 Toyota Highlander serviced at Romano Toyota for my state inspection and regular maintenance. The dealership passed the inspection and did not mention any problems with my vehicle. However, just two days after the maintenance, the check engine light came on unexpectedly. The vehicle began to hesitate and shift roughly, with noticeable jerking during acceleration and de
... layed gear engagement. This created a safety hazard, especially when merging into traffic or crossing intersections, because the SUV would not respond quickly when pressing the accelerator. A diagnostic scan revealed transmission-related trouble codes (P2714 – Pressure Control Solenoid D, and/or P2757 – Torque Converter Clutch Pressure Control Solenoid Performance). I returned to the dealer, and they confirmed the issue but could not provide an immediate fix due to parts availability. They advised continuing to drive, which I feel is unsafe. The symptoms persist, and the transmission sometimes feels like it is slipping, which could result in loss of power in high-speed situations. This issue appears similar to other UA80 8-speed automatic transmission problems reported by Toyota owners from 2017–2022. Publicly available NHTSA records show over 200 similar complaints involving hesitation, rough shifting, and transmission failure. These reports suggest a potential widespread defect that may affect vehicle safety. Given the safety risk of delayed acceleration and possible transmission failure, I believe this defect needs investigation and corrective action.Read more

So I just had my oil changed. And when they shut the hood, the hood stop fell out the bottom. They really just glued the hood stop on? This is allowed? And they didn't even do it properly. And I'd know. I'm a former ASE Certified Master Tech. First, they used the adhesive as a spacer to make the square bracket sit 90 degrees to the ground on a curved panel. Unnecessary since you can't see the bra
... cket with the hood closed. Second, they didn't rough up either surface for adhesion and, they didn't put any adhesive BETWEEN the bracket and the body, (except for where they used the adhesive as a wedge spacer). They just put the panel bond over the bracket. Looks like they used half a tube on each one. The bracket has 2 holes in it for #8 screws. Why didn't they use those? The panel adhesive costs like $80-$100 bucks a tube. 4 bolts with washers and nyloc nuts are about $2. Imagine if that had fallen off while driving and bounced up and punctured the LP line, or fuel line, or tire, or G*d forbid a BRAKE LINE. Or even worse. What about when that chunk of steel come bounding out from under my vehicle and goes right through the car windshield behind me. And you just know it would have happened at the worst possible time like going down hill in a construction zone at night in the rain. Now those things did not happen this time. But have they? I pulled the remaining adhesive off BY HAND with very little effort. How many other parts have they just “glued on”? They make a lot of motorhomes. Is this their normal practice? How many accidents have they caused they don’t even know about? Please investigate this company’s production practices.Read more

Dear [NHTSA or Honda Customer Service], I am submitting this complaint to raise a serious safety concern regarding the automatic engine shutoff feature in my 2023 Honda Pilot EX-L. While parked and talking to the dealer, I left my vehicle running with the air conditioning on and my older children in the back seat. Despite the key being inside the car and the engine running, the vehicle shut off
... automatically after approximately 30 minutes, disabling the air conditioning without any clear warning. This happened in the desert heat, creating an extremely dangerous situation for my children, who were sitting in a vehicle that quickly began to overheat. At no point were we warned by the dealer, the vehicle itself, or any documentation that this idle shutoff would activate even when occupants are inside the car. This is an unacceptable and unsafe default behavior. While I understand the intention behind the feature may be related to fuel or emissions concerns, it should never override the need for cabin cooling when passengers—especially children—are present. This feature should: •Include more prominent audible and visual warnings •Be easily disabled or extended by the driver •Be reconsidered entirely when rear seat occupancy is detected I urge the NHTSA and/or Honda to take this issue seriously and issue a recall, software update, or safety bulletin to allow owners to override or disable this idle shutoff function under safe, supervised circumstances. This is a family vehicle, and no feature should put occupants at risk of heat-related injury. Description of Issue: We have experienced multiple episodes of violent shaking in our 2024 Toyota Grand Highlander while stopped at red lights or in intersections. Both my wife and I have had this happen independently, around two to three times each. Most incidents occurred while our young children were in the vehicle. The most recent incident occurred today, when my wife experienced it twice in
... a row while stopped at an intersection. Our children were not in the vehicle during today’s event. Component or system involved: UNKNOWN at this time. We suspect a major mechanical or drivetrain-related failure. The entire vehicle shakes violently and feels unstable. We are willing to make the vehicle available for inspection upon request. Safety risk: The severity of the shaking is extreme and it feels as though the car may explode or fall apart. We have had to put the car in park, shut it off immediately, and in some cases, exit the vehicle in the middle of intersections or while stopped at a red light. This places everyone in the vehicle at serious risk, especially our children, and could result in a crash or secondary accident due to the abrupt stop and unexpected behavior in active traffic. Has the problem been reproduced or confirmed by a dealer or service center? The issue has not yet been reproduced by the dealership. We brought the vehicle to Toyota of Escondido after a previous incident. They inspected it but stated they could not find anything wrong. Has the vehicle or component been inspected by others? The vehicle was only inspected by the dealership (Toyota of Escondido). It has not been reviewed by any other parties such as law enforcement or insurance representatives. Warning lamps, messages, or other symptoms prior to the issue: There were no warning lights, error messages, or abnormal behavior prior to the onset of the violent shaking.
