Component/system: Suspected engine lower end/connecting rod bearings. Vehicle is currently being towed to Keeler Honda of Latham, NY for diagnosis and is available for inspection upon request. What happened: On Friday night, [XXX], I was traveling about 63mph in the 65mph zone on the [XXX] which is a moderate uphill. the engine all of a sudden began making a very violent knocking noise and the t
... ruck started to lose power very fast. I activated my hazards and coasted to the shoulder narowly sideswiping the cars near me. The malfunction created a huge collision risk. There is no crash or fire. Confirmation by shop/inspection: JMR Auto is a well-known and long-running independent shop located in Nassau, NY and is also the first place I towed the vehicle to. They stated based on listening to it it is indeed rod knock. Official Honda dealer confirmation is pending, the vehicle is currently being towed to Keeler Honda of Latham, NY where they will inspect and confirm internal connecting rod bearing failure. Warnings: No warning lights before the event. At failure, the engine light along with some driver assistance lights were displayed. Context: Symptoms match Honda Recall 23V-751 (prematrue connecting-rod bearing wear/seizure) reported on this model/year, but my VIN shows no open recalls. I understand NHTSA has Recall Query Rq24-013 into similar failures outside 23V-751's VIN range. I am reporting this for safety tracking and potential expanded recall consideration. Maintenance: Vehicle reasonably maintained per Hondas' Maintenance Minder; some services were at shops that do not report to CARFAX. Documentation being gathered. Dealer RO/case numbers to follow. I DON'T KNOW EXACTLY THE PROBLEM PART, BUT IT IS UNSAFE TO DRIVE. I ONLY GOT IT FOR ONE WEEK. I DID LET THEM KNOW SINCE THE SECOND DAY I HAD THE CAR, IT WAS KEEPING BREAKING DOWN ON THE STREET. THEY TOLD ME TO KEEP DRIVING IT TO GET COMFORTABLE WITH THE TECHNOLOGY IT HAD. THINKING IS WAS TECHNOLOGY LIKE THEY SAID I KEEP DRIVING IT AND I HAVE EXPERIENCED 3 OCCURRENCES OF A SERIOUS SAFETY CONCERN.
... THE CAR JUST LOSES POWER IN THE MIDDLE OF THE ROAD and picks back up and continues like nothing happened. I could have been hit by another car. I bring the car back to them because I'm scared to keep driving it, my 3 kids have only me as family to support them, I do not want to die and leave them on the street. They have the car back the same week, and they have my $6700 down payment, as well as my trade Toyota CHR for another amount of $3700. The Sales manager told me I have to pay for the loan anyway, not to lose my credit. He said he had already spent his to get my credit okay, so I should pay for the car anyway because the loan is in my name. Or to sell him the car and pay the rest of the money after he values the car. It's unfair to me to lose my $6700 plus the $3700 from the trade plus my credit. I need a car, but not an unsafe one, which was the reason why I spent all this money and ended up with nothing. i need help to get another car or my money back to find another Brand, that's not my first time victim from Toyota.Read more

While accelerating uphill approximately 65 mph the car suddenly started to shutter and slow down to 35 mph. I forced me to move rapidly over to the right lane on a busy highway almost getting hit by a semi truck. Luckily I was able move to the shoulder and park the vehicle. I shut the vehicle off and tried to start it back up which it did but struggled to turn over. Using my OBDII scanner I found
... that multiple codes had appeared which included P0087 Fuel Rail/System Pressure to low and P0299 Turbocharger Underboost. Vehicle was towed to my residence where it currently sits until I am able to repair it. Prior to this happening there were No warning lamps or messages or symptoms. The failures first appeared while driving up hill. It was not inspected by manufacturer, police, or insurance. The problem was not taken to dealer or independent service center to confirm since the car is out of warranty and cost to diagnose is ridiculously high. I myself diagnosed the failures and found that the mechanical fuel pump Part# 16790-5PC-H02 seized up and was not providing enough fuel to engine. And diagnosed that the waste gate actuator arm was to loose due to worn out bushings. (Poor engineering in my opinion since there is no way to replace just the bushings on the actuator causing customers to replace the entire turbo.) After replacing the mechanical fuel pump the vehicle was able to start up without issue and drives perfectly fine. I currently still have the fuel pump and the turbo is still in the vehicle if needed for inspection.Read more

I have had my new car only two weeks and have driven only 500 miles. Yesterday, while driving on a freeway at constant 65 mph speed, I suddenly experienced COMPLETE POWER FAILURE. I coasted across three lanes of heavy traffic and came to a stop on the shoulder. There were no warning lights on the dashboard. I turned the car off, turned it on again, and the car drove normally. I checked on my
... Hyundai app and it said under Vehicle Health that all systems were normal. I called the Santa Monica Hyundai service center and was advised to drive home to Santa Barbara (on the freeway) and get in touch if the problem recurred. I drove my car some 50 miles to Santa Monica to be evaluated and the service rep said that all makes of EVs have this problem, not unusual. After some hours, I spoke to the shop foreman who said he had never seen this problem, that the diagnostics had shown no problem, that there was no indication that this had indeed happened, that he could do nothing further unless there was proof that I had experienced complete power failure, and that if it happened again I should obtain proof by videoing the dashboard and my foot pressing down on the accelerator without any response. I called the Oxnard Hyundai dealer where I had purchased the car and was advised to bring my car there for a second opinion. I drove the 60 miles to their service center. I was told by the service rep that he had never heard of this problem. I was given a loaner car to drive the 40 miles home and my Ioniq 5 was kept for evaluation. I discovered on Google that sudden power failures in Ioniq 5's are a known problem by both Hyundai and the NHTSA and that there have been recalls as a result. It seems reprehensible that Hyundai representatives in two different dealerships claimed that they had never heard of this phenomenon. I was at risk of a potentially deadly accident by losing power on a freeway. I am lucky to be alive.Read more

... re was no oil in my vehicle which matches the oil consumption defect identified in Kia's Theta II GDI engine recalls and nationwide class action settlement. Kia has acknowledged in public filing that is defect poses a risk of engine seizing and fire. Although my VIN is not currently listed under any active recall, my vehicle exhibits the exact same defect pattern as recalled vehicles, including sudden loss of power at highway speeds which is a significant safety hazard. Manufactures Response: I contacted Kia Corporate to request an engine replacement consistent with the recall and the settlement remedies and they refused to cover the repair solely because my VIN is not included in the recall list despite the defect being identical. Safety Concern: The sudden loss of power creates a risk of rear-end collisions, loss of control and potential fire. This defect can be life threatening and should not be excluded from recall coverage simply due to VIN range. Requested Action: I am requesting that NHTSA investigate why vehicles with identical engine defects, including mine have been excluded from recall coverage and compel Kia to provide a no-cost repair. On [XXX], I identified a severe and unacceptable loss of driving range in my 2020 Tesla Model Y (VIN: [XXX] , mileage: 59,127 miles). The vehicle now shows only ~180 miles of range at 100% charge, compared to the original advertised 316 miles — representing a ~43% loss of range. This degradation is far outside normal industry standards for EV battery health. This loss of range significantly imp
... acts vehicle safety, as trip planning becomes unreliable and there is a heightened risk of being stranded on highways or in unsafe locations without charging availability. This could endanger both myself and other drivers, especially in rural areas or during extreme weather conditions. The issue has been repeatedly reported to Tesla, but they have failed to provide a technical explanation, battery replacement, or corrective action. Tesla service claims it is “normal degradation,” despite the magnitude of loss and the vehicle’s mileage being well below expected battery life thresholds. No warning lamps appeared prior to the loss of range. The problem is ongoing and reproducible — the vehicle consistently charges to ~180 miles of estimated range. I believe the battery is defective or has been subject to software-imposed range limitation. I request that NHTSA investigate this matter as it may indicate a broader safety risk or a systemic defect in Tesla batteries that could affect other consumers. NHTSA Safety Complaint (Loss of Emergency Connectivity) Subject: 2025 Chevrolet Equinox EV — OnStar/telematics failure disables Emergency Services & Automatic Crash Response; part on national backorder VIN: [XXX] Make/Model/Year: Chevrolet Equinox EV, 2025, RS trim Mileage at first occurrence: 4000 Date problem first occurred: [XXX] Frequency: Continuous (services remain unavailable) Injuries/
... Crashes: None to date Component(s) affected: Electrical system — telematics/communication (OnStar); emergency calling/SOS; crash notification (ACR). Description of the safety defect: The vehicle’s OnStar/Connected Services display “Services Unavailable.” The dealer diagnosed a failed telematics module. Replacement is on national backorder with no estimated timeframe. While in this condition, Emergency Services and Automatic Crash Response are disabled—the SOS button does not connect, and the vehicle will not automatically contact OnStar after a crash. Parts availability (scope/delay): My dealership and multiple GM/Chevrolet and OnStar owner forums indicate 3+ month waits for the telematics module with no ETA, leaving vehicles without emergency connectivity for extended periods. What has been done so far: Dealer confirmed module failure and opened a parts order; advised no ETA due to national backorder. Manufacturer has not provided an interim fix or loaner vehicle. Issue remains unresolved as of Aug 11, 2025. Why this is a safety concern: Loss of emergency calling and crash notification defeats safety features marketed as critical for post-crash response. Owners cannot rely on the vehicle to summon help after a collision or medical emergency. Requested NHTSA action : Track this failure and parts unavailability, assess scope, and determine whether a safety investigation or recall is warranted. This motorcycle has been garage kept. It is in mint condition with 2114 miles on the odometer. It was purchased new in 2018. In 2024 the ABS light on the instrumentation illuminated. I tested the brakes to see if the ABS still worked, but it does not work. The rear tire can be locked up to skid, and the ABS does nothing. It used to work in years prior, as I had tested it in the past by apply
... ing heavy rear brakes. According to the manual, the error code #35 indicates a faulty ABS unit. The trouble-shooting is to test for 12 volts at the connector to the ABS unit. I put the bike into service mode, I got error code 35, and I tested the voltage to the unit and it was over 12 volts. I went online and discovered that ABS units failing on this motorcycle is not a rare event. I also learned that the old ABS unit 55610-28K00 has been superseded with part number 55610-2K01. This would indicate an issue with the earlier unit. The replacement cost is list price of $986.42, sale price of $828.59. This part can be found under "Hydraulic Unit" on the OEM parts schematic. Non-functioning ABS, especially on the front wheel, can be very dangerous and lead to serious injury or death during an emergency brake application on a motorcycle. I contacted Suzuki Motors of America. They have no recall on the ABS unit, and they offered no assistance due to the warranty being expired. Yet they have a new part number, replacing the old part for some reason. They must be aware of a high failure rate of the original ABS unit. I feel that it is very irresponsible for Suzuki Motors to send out motorcycles with faulty ABS units and not do something to remedy the dangerous situation this creates. The consumer's only option at this point is to have the ABS unit replaced at well over $1000 when factoring in taxes and labor charges. If you can, please check to find out how many ABS units they have sold due to this issue. Thank you.Read more

The panoramic sunroof on my 2020 Toyota Camry XSE failed to close on two separate occasions. The most recent incident occurred about three months ago. After opening the sunroof, the glass panel would not close, leaving the vehicle fully exposed during heavy rain. ... from exposure. This failure poses safety risks, including: Driver distraction if the failure occurs while driving. Possible water damage to electrical components, potentially affecting vehicle operation. Reduced visibility in heavy rain. Security risks if the vehicle cannot be secured. Online research and discussions with other owners revealed that this problem appears to be relatively common with Toyota Camry models equipped with a panoramic roof. My initial repair estimate was approximately $12,000, which is unreasonable given the vehicle’s value. I ultimately had the sunroof permanently sealed shut at my own expense. I have video evidence and documentation from both incidents, including an earlier occurrence on August 29, 2020, which was reported to a Toyota dealer but dismissed at the time. I believe this should be investigated as a potential recall.Read more

1. Component/System Failed: Halogen headlamp assemblies (low beams) malfunctioned due to delamination of the reflective surface, causing dimming. Available for inspection upon request. I have had the issue for years. Hard to see at night. 2. Safety Risk: Dim low beams reduce nighttime visibility, increasing risk of collisions. Using high beams to compensate may blind other drivers, creating furt
... her hazards. 3. Problem Reproduced/Confirmed: Not yet confirmed by a dealer or independent service center. Issue matches known defect in 2013-2018 Altima halogen headlamps (TSB NTB15-111). 4. Inspections: No inspections by police, or insurance yet. I did have it inspected with the Santa Rosa California (Jim Bone) Nissan dealer inspection under Voluntary Service Campaign (PC861/PC890). That dealer said it was not bad however the headlights were bad before and after that so I believe they lied or didn’t test it correctly. That was in 2022 I believe. 5. Warning Signs/Symptoms: Gradual low-beam dimming noticed, first observed around 2017 perhaps. I just thought it was bad but it got worse. I saw in 2020 or 2021 some notice about a class action lawsuit (Suarez?) that Nissan lost so I tried to get them to fix it but they had various dates they said claims had to be done and it was confusing. I called Vallejo Nissan and they said I had to wait. Later it kept getting worse. No warning lamps; low beams too dim for safe driving, requiring high beams. 6. Description (if uncertain): Low beams are dim, compromising safe nighttime driving. Issue aligns with reported delamination defect in halogen headlamp assemblies. Exact onset date/mileage UNKNOWN. Sought dealers inspection for replacement under Nissan’s campaign but they didn’t take care of it. Nissan should be held responsible if any deaths, injuries or damages result from this issue. Sometimes when driving the steering wheel will tend to “jerk”, or appear “stuck” when trying to turn. This has started to happen recently and I have noticed it when NOT using lane “assistance” this issue has not only been seen from me, but rather by many others as seen in forms such as the following: [XXX] This issue has yet to cause a wreck for me, but this does pose a safety risk as
... it could prevent me from moving out of the way of cars, or worse prevent me from turning causing me to hit someone or something around my environment. I hope that this is something that can be fixed or recalled for as I am not the only one experiencing this and does genuinely pose a risk. I’ve been told that it could possibly have somet

Shortly after my warranty expired last september ([XXX]) my 2020 Audi A4 Allroad started to experience random episodes of the car's main entertainment touch screen rebooting in the middle of the car being operated. This usually would only happen once or twice during maybe an hour long car ride. Then this spring, starting in mid-March the car started to experience the MMI reboots more often, say 6-
... 7 times over an hour long car ride. This started to happen more often, and then it would be around 4-5 times in a 20 minute car ride. I noticed that when this would happen, it would cause issues with the backup camera (the camera would stop function during the 60 second reboot. Also, if a parking sensor (or cross-traffic warning sensor, incorporated with the backup camera) was detecting something near the car during the reboot and the camera turning off, it would throw a safety system communication error on the Audi Virtual Dashboard. It was clearly a safety issue and it made maneuvering to park on a busy street very dangerous for folks outside the car and inside. I went to the dealership twice to get the MMI reboots fixed, and received two software updates and a 5F module replacement (quoted at $5k, Audi offered the part 90% off) - all three attempts resulted in the car with a MMI rebooting even faster. It now reboots and has the same issues as before with the car MMI rebooting between every 2-5 minutes during operation. You can find reports from numerous other 2020 Audi A4 owners with this same rebooting MMI issue here, many of whom have already submitted their own NHTSA safety problem reports in this long thread: [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more
