I have a 2005 Toyota 4runner with frame rust that has caused everything under my 4runner to rust. Toyota knows these frames have problems with rust and corrosion and have done nothing, I am far from being the only one with this complaint or problem. It is up for inspection anytime. I have taken it to get work done at shops and been turned away because of the amount of rust on all bolts, lines,
... all metal under truck causing it to be a liability. I cant even get my skid plates off to fully address the issue. The metal is starting to split on the main frame by the tire, it is rotting from inside. Rust has always been there but last couple years has gotten really bad. So much rust my gas tank strap has broken but so much rust I cant get the skid plate off to fix it.Read more

While attempting to cross 3 lanes of oncoming traffic from a standstill the vehicle lost all power and barely moved 2 feet, irrespective my efforts of stabbing at the throttle, and almost got us killed. Fortunately I slammed on the brakes and barely missed being hit head on. No warning lamps or codes popped up. This is now the 3rd such incident in the 2 months we have owned this vehicle.

I am filing a complaint regarding Toyota’s handling of a known paint defect affecting my 2014 Toyota RAV4 Limited in Blizzard Pearl paint. Over the past several years, the paint on my vehicle has progressively peeled and deteriorated. Approximately 50% of the vehicle’s paint is now damaged. This issue is clearly consistent with Toyota’s known Blizzard Pearl paint defect program. However, I
... was never notified by Toyota about this defect program through mail, email, or phone call. In addition, I regularly serviced my vehicle at authorized Toyota dealerships during the years the program was active, yet no dealership or Toyota representative informed me that my vehicle qualified for repair coverage. I only recently learned about the existence of this program after visiting a Toyota dealership regarding another issue. At that time, I was told the program had already expired in April 2024 and that Toyota would no longer provide assistance. I believe this is unfair because: - Toyota acknowledged this as a manufacturing-related defect. - I was never properly notified. - Toyota dealerships had multiple opportunities to inform me during regular service visits. - The defect became progressively worse over time. I am requesting that Toyota reconsider my case and provide goodwill assistance or repair coverage despite the expiration date, due to the lack of notification and the severity of the defect. Toyota Brand Engagement Center still told me the recall expiated.Read more

ubject: URGENT SAFETY INCIDENT REPORT: Engine Seizure at Highway Speeds – Case #xxx To the Toyota Safety & Compliance Department: I am writing to formally report a critical safety incident involving my 2024 Toyota Tundra (VIN: [XXX] ). Incident Description: While traveling at highway speeds, the vehicle’s engine suffered a catastrophic internal seizure and shut down completely without any p
... rior warning. This resulted in an immediate loss of motive power in the middle of high-speed traffic. Because the engine died, I also experienced an immediate loss of power steering assist and power brake vacuum, making the vehicle extremely difficult to control. I was forced to perform an emergency maneuver to avoid a rear-end collision and struggled to steer the truck toward the shoulder while other vehicles swerved around me at high speeds. Safety Hazard & Manufacturing Defect: This failure is identical to the conditions described in NHTSA Safety Recall 24V-381 (Toyota Recall 25TA14). The dealership's diagnosis at AutoNation Toyota Weston confirmed that the bearings are discolored and "spun," which is a direct consequence of the manufacturing debris that Toyota has already admitted exists in these engine blocks. Bad-Faith Warranty Denial: Despite the life-threatening nature of this event, the dealership is attempting to blame "abuse" based on a software log from the Brake Override System. They are ignoring the fact that a metallurgical failure of the main bearings has no mechanical link to pedal input. Using a safety software record to deny a repair for a known manufacturing defect that nearly caused a fatal accident on the highway is unacceptable. Requested Action: I demand that this case be immediately reviewed by Toyota’s safety team. My vehicle manifested the exact defect described in the federal recall during a high-risk situation. I am requesting an immediate resolution to remove this dangerous vehicle from the road and to honor the terms of the INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FORead more

... and for Buyback To the Representatives of CarMax Corporate and NHTSA, I am writing to formally report a severe safety incident and a gross failure of the CarMax Certified Quality Inspection process. The negligence displayed by the CarMax Orlando staff not only violated the terms of our purchase contract but actively endangered my life and the lives of others on the road. 1. Timeline of Events and Staff Negligence On 04/30/2026, I purchased the above-referenced vehicle from CarMax Orlando. Upon attempting a test drive, I was denied by sales consultant Boris Iniquez, who stated the vehicle could not be driven until it was cleaned. On 05/01/2026, I took delivery and immediately noticed the brakes were non-responsive and emitting a loud "air brake" sound. I returned to the dealership on 05/02/2026. Mr. Iniquez performed a ride-along, personally witnessed the brake malfunction, and was informed of my immediate need to drive 450 miles to Atlanta, GA, for my role as a Site Inspector. Rather than providing a safe loaner or grounded vehicle, Mr. Iniquez and the acting manager explicitly instructed me to drive the vehicle to Atlanta, asserting that the car had "passed all inspections." 2. Mechanical Safety Failure Upon reaching Atlanta, the vehicle's condition became untenable. On 05/07/2026, CarMax GA inspected the vehicle and found: - Total failure of all four brake pads. - Front rotors requiring immediate replacement. It is mathematically and mechanically impossible for a vehicle with metal-on-metal rotors and failing pads to have passed a legitimate 125+ INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FRead more

The check engine light illuminated on my 2020 Toyota Camry XSE 2.5L with approximately 132,798 miles. An OBD2 scan revealed fault code P26AE — Engine Coolant Bypass Valve 'B' Control Circuit High. This is a known widespread defect affecting 2018-2022 Toyota Camry, RAV4, and Corolla vehicles equipped with the 2.5L Dynamic Force engine. The plastic valve casing is prone to cracking, allowing coola
... nt to seep into the electrical connector and corrode the pins, triggering the fault. Toyota has acknowledged this issue through a Customer Support Program offering free replacement on qualifying vehicles, however many owners are still being quoted $900-$1,300 at dealerships. The faulty valve affects the engine coolant flow control system and transmission cooling circuit. Warning light appeared suddenly after a routine fuel fill. Vehicle has not yet been inspected by dealer. Component is available for inspection upon request."Read more

I am writing to report a safety defect regarding the 8-speed automatic transmission in my 2017 Toyota Sienna. The vehicle exhibits severe hesitation during acceleration, harsh shifting, and a high-pitched whining noise, specifically consistent with the 'whine of death' described in Toyota's Customer Support Program ZJC and related Technical Service Bulletin T-SB-0160-18.Despite these identical sym
... ptoms, my vehicle’s VIN is not included in the coverage list. This hesitation during acceleration creates a serious safety issue, as the car fails to accelerate unexpectedly when attempting to merge or cross traffic. My vehicle is equipped with the UA80F/E transmission, which is documented to have a faulty assembly (improperly bent washer tab). I request that NHTSA investigate the exclusion of my VIN from this program, as the defect poses a significant risk to passenger safety and the safety of others on the road.Read more

The main instrument panel on numerous occasions remains locked on “Experience Drive Connect”, usually when starting the vehicles, but occasionally when the vehicle is being driven. Sometimes when we can turn off the car and restart, the screen will begin normal functions. Yesterday the screen remained locked after 5 shut-downs and we had to drive with a locked screen. This causes the loss o

We should not have to trade a higher end 2024 vehicle with 10K miles for another brand to remedy this issue. Additionally, the vehicle may lose trade value with a pending recall that prevents dealers from certifying the auto.Read more

"The vehicle suffered a critical failure of the shift interlock safety system. After parking in a driveway, the ignition key was able to be fully turned to the 'off' position and removed from the ignition cylinder, even though the transmission was not successfully secured in 'Park.' After the key was removed and I and went into the house the van rolled away. It traveled downhill, crashed through a

This failure could potentially cause serious injury or death and this was the first time that it had occurred and is exhibiting the same failure as described. Immediately following the collision, video evidence was recorded showing the key in hand while the dashboard indicators and shifter remained out of the 'Park' position. This indicates the safety mechanism designed to prevent key removal unless the vehicle is in 'Park' failed to operate, creating a life-safety rollaway hazard."Read more

Description of the Problem Vehicle: 2014 Toyota Prius v (Mileage: 70,000) Problem Description: The vehicle is experiencing a critical failure of the Electronically Controlled Braking (ECB) system, specifically the Brake Actuator and Booster Pump assembly. The dashboard currently displays multiple warning indicators: ABS light, Traction Control light, and the Brake System Warning light (Master War
... ning "!"). Symptoms: Mechanical Failure: The internal high-pressure accumulator is failing to hold pressure. The electric brake booster pump is heard cycling (straining) every 10–20 seconds even when the vehicle is stationary and the brake pedal is not depressed. Loss of Braking Safety Features: The system has disabled regenerative braking and Anti-lock Braking (ABS), causing significantly increased stopping distances and a "wooden," unresponsive brake pedal feel. Predictability of Failure: This failure matches the exact symptoms described in Toyota’s Customer Support Program 20TE07 (and previously ZJB), which acknowledges an internal defect in the brake actuator. Safety Risk: The sudden transition from power-assisted braking to manual hydraulic braking during a drive creates an immediate safety hazard. As the pump motor continues to cycle excessively to compensate for the internal leak, it is at risk of total thermal failure, which would lead to a near-total loss of braking assist while the vehicle is in motion. Additional Notes: Despite the vehicle having only 70,000 miles (well under the 150,000-mile limit acknowledged in Toyota’s support programs), Toyota has declined to provide assistance because the vehicle has surpassed the 10-year age mark. I am filing this report because a primary safety component (brakes) should not experience catastrophic internal failure at 70,000 miles, regardless of the vehicle's age.Read more

COMPONENT: Electronic Throttle Control System (ETCS) / accelerator pedal. Vehicle is available for inspection at Autocentro Toyota, San Juan, PR, where it has been retained since 2/28/2026. INCIDENT: On 2/28/2026 at 7:45 AM, while entering a private driveway at under 5 MPH to park, my 2025 Toyota RAV4 Hybrid SE (VIN [XXX] , 3,164 miles) suddenly and involuntarily accelerated without driver input.
... I categorically affirm I did NOT press the accelerator. The vehicle struck a metal gate, causing frontal damage. Airbags did not deploy. OEM floor mat was properly secured; no aftermarket parts installed. SAFETY RISK: Sudden unintended acceleration at low-speed parking maneuver. Had a pedestrian, child, or another vehicle been in the path, this would have caused serious injury or death. The defect is unpredictable and the driver cannot anticipate or override it. INSPECTION: Vehicle inspected 3/20/2026 by Engineering Analysis Associates (EAA), a firm contracted and paid by Toyota — NOT independent. EAA downloaded electronic data and concluded the accelerator was pressed 1 second before impact. I dispute this finding: I did not press the pedal. No independent inspection has been allowed. Full EAA report was never provided to me. On 4/23/2026, Toyota Motor North America (Ms. Delve L., Legal Claims Administrator) declined assistance based solely on EAA's data. WARNING SIGNS: UNKNOWN. No warning lamps, messages, or prior symptoms appeared before the incident. The vehicle had functioned normally since purchase on 4/25/2025. ADDITIONAL: Vehicle retained by dealer 73+ days without repair or substitute vehicle. Consumer complaint filed with Puerto Rico DACO (Case XXX).
