I was driving my 2002 Toyota Sequoia on the freeway at approximately 65 mph on the [XXX] when suddenly the vehicle made a beeping noise and the VSC Light and TRAC Light came on. The vehicle suddenly lost control in the wheels and tires and suddenly decelerated out-of-nowhere. Due to the sudden, abrupt movement and loss of control, the vehicle nearly flipped over. It was an extremely dangerous, po

This happened before two times and this would be the third time. I have taken the vehicle to several different auto repair shops and they have been unable to pin point and fix the problem. Because this issue continues to happen and puts me into dangerous situations repeatedly, I firmly believe that this is a bigger malfunctioning/defect problem that Toyota needs to address and fix. I own a 2020 Infiniti QX50 that is included in NHTSA Recall 25V-437 for defective engine bearings that may cause sudden engine failure. My VIN has been confirmed by the dealer as part of the active recall. I have already noticed intermittent shaking at idle and a grinding sensation during acceleration, which are early warning signs listed for this defect. I am worried these symptoms indicate my e
... ngine may be starting to fail. My Infiniti dealer told me they “see the recall as active,” but that it is on hold and they cannot inspect or repair my vehicle at this time. They also stated they cannot provide me with a loaner or rental vehicle in the meantime. I believe this puts me, my family, and others on the road at risk because engine failure could occur suddenly while driving, leading to a crash. I am nervous to continue driving the vehicle, but I have no safe alternative since Infiniti has refused to provide a replacement vehicle until repairs are available. I am requesting NHTSA to investigate this delay and Infiniti’s refusal to provide a safe alternative vehicle while recall repairs are unavailable.Read more

I am writing to bring urgent attention to a significant and concerning issue affecting the 2016 Honda Accord, specifically the audio head unit, which is causing severe static noise through the vehicle's speakers. This issue poses a serious safety risk to drivers, passengers, and others on the road, and I believe it warrants immediate action, including a formal recall. The static noise occurs regar
... dless of whether the audio system is on or off. This interference compromises the ability to clearly hear essential sounds while driving, including emergency vehicle sirens, honking, and other important auditory cues that are vital for safe driving. The loud, distracting static significantly reduces the ability to focus on the road and may cause delayed responses to critical situations, including traffic accidents, pedestrian crossings, and other road hazards. Safety concerns include but are not limited to the following: inability to hear emergency alerts such as sirens and horns which could result in delayed or incorrect reactions when an ambulance, fire truck, or police vehicle is approaching, potentially leading to dangerous situations where first responders are unable to get through traffic quickly or safely, distracted driving, reducing ability to focus on the driving task, and ability to hear other critical sounds, such as tire issues (like a flat or blowout), or the sound of other cars around you, and can medically affect hearing of drivers. Given the severity of these issues, I urge an investigation to this matter thoroughly and issue a recall for the affected 2016 Honda Accord models and other year models that are affected. This would ensure the safety of drivers, passengers, and others on the road. Addressing this problem promptly will not only safeguard lives but also demonstrate a commitment to consumer safety and quality. I appreciate your attention to this urgent matter and trust that the necessary steps will be taken to resolve it as soon as possible.Read more

To Whom It May Concern, I am writing to formally report a potential safety issue with my 2022 Mazda CX-9, which is a Certified Pre-Owned (CPO) vehicle under limited warranty until March 2026 and powertrain warranty until March 2029. It has 36,000 miles on it. Recently, I noticed a crack forming near the front combination light on the front bumper. The vehicle has not been involved in any accident,
... collision, or external impact. Upon researching, I found TSB Bulletin No.: 09-022/20, titled FRONT BUMPER CRACKED NEAR THE FRONT FENDER/FRONT COMBINATION LIGHT, which addresses similar cracks in 2016–2019 CX-9 models (VINs lower than JM3TC******306028). Although my vehicle falls outside the bulletin’s specified range, the crack appears identical in location and nature—suggesting a possible design or material stress issue that may extend beyond the listed VINs. When I brought this to the dealership’s attention, they declined coverage under warranty, citing the TSB’s limited applicability. Mazda corporate echoed the same response. I find this deeply concerning. Why is this issue being dismissed based solely on VIN range, without a proper inspection or consideration of safety implications? The crack is adjacent to lighting components and could compromise structural integrity. This is not a cosmetic flaw—it’s a potential safety hazard. I respectfully request that NHTSA investigate this matter and help facilitate a resolution, including repair or replacement of the bumper. It is disappointing that safety concerns are being overlooked in favor of technical exclusions and cost considerations. I urge NHTSA to consider expanding the scope of investigation to include newer CX-9 models not currently covered by the TSB. Importantly, I’ve seen many other CX-9 owners reporting similar bumper cracks in vehicles outside the TSB VIN range, particularly in Facebook owner groups. This suggests the issue may be more widespread than currently acknowledged and deserves broader attention.Read more

Statement of Facts Regarding Tesla Accident [XXX] On [XXX], I was involved in an accident with a Tesla that I had rented directly from Tesla’s rental program. At no time during the rental process was I given warnings, training, or any overview about how these features work, the

On [XXX], I used the Summon feature for the first time in the parking lot of my business at [XXX] . The car was parked in spot [XXX], where I regularly park. I attempted to use Summon while standing in full view of the vehicle, but the software repeatedly displayed “lost connection.” After disabling Wi-Fi, I held the Summon button again and the car successfully pulled out of the spot, drove forward, and arrived directly in front of me. This demonstrated the feature was functioning properly on that day. On [XXX], while again parked in the same spot, I attempted to demonstrate Summon to my wife. I was standing in clear view, approximately 50–75 feet away, near the complex entrance. As before, the software repeatedly displayed “lost connection.” I attempted to reengage by hitting the summon button to reconnect the command. That did not work. When I walked up to the vehicle, I discovered that the front quarter panel near the wheel had come into contact with the covered parking pole. At no time was I aware that the car had hit anything, nor did the system provide any alert that it had engaged with an object. This stands in stark contrast to the way Tesla’s software consistently warns when the car is being manually driven — giving constant alerts when drifting out of a lane or approaching another vehicle. I was heading south on I-495 (Massachusetts) in right lane at just under 65 mph. I had just passed an entrance ramp and overpass. A car was entering I495 on my right and coming up to speed. The car entered my lane from his entrance ramp to the right most lane in front of me. The car was about 20 feet in front of me. The entering car's speed was was greater than mine. When the second the car l
... eft the entrance ramp and pulled into my lane about 20-30 feet in front of me, my brakes slammed on, causing me and my passenger to lurch forward. My seat belts protected me and my passenger from harm, but it scared the **** out of us. This kind of nonsense of so-called "safety features" has to stop. Its bad enough that I have to put up with a car that stalls out when the car stops, even for a second, but what happened above is unacceptable as a "safety feature." I could have easily reacted by jerking the steering wheel to the right and gone off the road at full speed, fast enough to roll the car causing considerable harm to life.Read more

Over the last year, recorded 13 incidences where car would not accelerate. (It occurred more times, but I did not record it). I wrote down date, time, weather, gas level, temperature, and what the car would not do. Each incidence was the same - the car would not accelerate, despite pressing down (strongly) on the gas pedal. The car would then jerk forward suddenly. This provided concern and dange
... r as it was often as I was trying to turn off a side road, onto a main road (state highway). This often needs to be done quickly as traffic approaches from both sides. There were a couple of instances when a car was approaching and the vehicle appeared to just stop in the intersection before finally picking up. I have definitely had concerns regarding being hit by oncoming traffic. I brought the vehicle in to Honda to assess in ~May/June 2023. They could not replicate the problem and ended up returning the vehicle without any noted actions. I began taking note of when this occurred starting 6/1/2024, and have 13 recorded incidences.Read more

The vehicle experiences a dual-clutch transmission shudder in low gears and at low speeds. This issue causes a sudden loss of speed in situations such as merging onto a highway, leaving a parking spot, or while driving at approximately 30 mph. The shudder occurs randomly and poses a serious safety concern. The problem first appeared in 2020 when the vehicle had fewer than 5,000 miles on it. Beginn
... ing in 2024, at around 20,000 miles, the condition began escalating rapidly, with the shudder occurring more frequently and with greater severity. All incidents have been recorded on dash cam and provided to the dealership’s service department. The vehicle has been serviced multiple times for this issue and was kept at the dealership for approximately six weeks in total. Despite this, the dealer refuses to acknowledge the problem, citing the absence of warning lights. When the clutch was adjusted at my request, the issue temporarily improved but returned about three weeks later. The shudder now occurs more often and with greater severity, frequently forcing the driver to pull over for safety. This ongoing transmission defect creates unsafe driving conditions, and the lack of acknowledgement or permanent resolution by the dealer further increases the risk to the driver and others on the road.Read more
