I was beginning to slowly back out of a parking space in a super market parking lot. I had not put my foot on the accelerator because my car begins to slowly inch backwards once it is put into reverse. As the car began to inch backwards, I saw a car approaching down the parking lot driveway behind me. I immediately stepped on my brake, but instead of stopping, my car began to race backwards, accel
... erating at a fast speed. The car never responded when I put my foot on the brake, so I was unable to stop the car. This caused me to crash into the car driving past me, and that finally stopped the car. Because I was almost perpendicular to the car I hit, and the driver was unable to get out of his car, I then put my car into drive to move it out of the way. My car accelerated out of control, again, this time forward. Again, it did not respond when I put my foot on the brake. My out of control car was quickly heading toward a large metal fire gate. In complete desperation, I stomped very hard on the brake, pressing it to the floor as I was about to hit the gate, and the car finally stopped. In both incidents, my brakes completely failed to stop the car accelerator which was acting on its own. The problem has not been reproduced by Toyota, which says there is nothing wrong with the car. However the data report they gave us is suspect because the data shows no reverse movement of the car and labels the accident a rollover, which it was not. In addition, the inspector told my husband there were more tests he could do, but Toyota had not authorized him to do them. There is a police report, and a witness who observed the whole thing. She said that because of the way the car was moving, it looked like a hit and run accident with the driver trying to make a speedy getaway. Timeline: • Late April 2026 (while driving ~300–400 miles from home): Vehicle stalled without warning at a red light. • May 5, 2026 (while driving on freeway ~50–75 miles from home): Check Engine Light illuminated. AutoZone scan and dealer diagnostic tool both confirmed DTC P107B. • May 5, 2026: Dealer performed health check using Toyota GTS+ tool (see attached screenshot taken at 15:58:01). Screen clearly shows “PERMANENT: YES” confirming a permanent stored fault code. • May 5–15, 2026: Crown Toyota (Ontario, CA) confirmed the code was present on arrival (Repair Order 2842115 dated 05/15/26). They later cleared the code, performed a short test drive, could not duplicate the fault at that moment, and closed the repair order as “inconclusive.” They refused the low-pressure fuel pump replacement under Customer Support Program 23TE04 even though corporate cases #260505002096 and #260515002110 had previously confirmed eligibility based on the prior pump replacement (9/11/2021, RO #1764477). The issue is known to be intermittent and occurs under load/highway conditions. This creates a serious safety risk of sudden loss of power while driving. T-SB-0050-20 specifically directs dealers to replace the low-pressure fuel pump assembly when P107B is present on 2018–2019 Highlander Hybrids, and T-SB-009-23 provides the official repair procedure under CSP 23TE04. My car is only at 125k miles and I have spent about 20k purchasing and servicing this vehicle in the last year alone. I travel out of state frequently with my children to visit my relative who is in active chemotherapy. If my transmission were to die in the middle of Donners Pass I would be left stranded with my two young children. This is my daily driver and I can’t imagine what would happen if my car completely shut down in the middle of traffic or out of state as I was given certainty that my car ran perfectly until this just recently happened. I am concerned about prolonged exposure to contaminated air inside the passenger compartment, especially during regular use of the air conditioning system. Toyota should investigate this recurring HVAC odor condition and determine whether a design or manufacturing defect exists involving the evaporator, condensate drainage, or ventilation system. Motorized struts to lift and close the rear lift gate have failed. The struts do work to open the lift gate but they do not work to close it. Manually closing the lift gate is extremely difficult because of the electric motors inside of the struts. A known problem by Toyota as they have issued a TSB. If my wife would inadvertently open the lift gate, which does work, after leaving the grocery store to put all of the groceries in the back, she would not have the strength to manually close it. Obviously, the vehicle cannot be driven with the open tail gate. This is not any different than if any of the doors could not be closed due to defective hinges. I have a 2005 Toyota 4runner with frame rust that has caused everything under my 4runner to rust.
