Tailgate will not close when button on the back or by the driver's seat is pressed. After pressing the close button at tailgate or driver's seat with tailgate open, it will lower an inch then beep several times and lift open again. Toyota service department said, "there is a button on the hatch that will reset it. you will hold that button until it does a sequence of beeps (beeeeeeeeepp, beep beep beep) then let go an press it again like you would want to close it and you should be good to go! :)" (sic). Just got [XXX] yesterday morning. I have ~350 miles on this version now in ~24 hrs. FIRST TIME I've ever completely turned off FSD and went back to Autopilot Only. This release is Absolutely 100% DANGEROUS and needs to be pulled and rolled back ASAP Observations: 1: Does not Honor Turn Only lanes and drives straight through them. 2: CONSTANT lane switching no matter what setting it's on. 3: La
... ne changes without using turn signal. Example, in right lane, it FULLY changes lane to left lane using turn signal, then decides to just go right back to the right lane again without using turn signal cutting off other vehicles. 4: Speed issues, It can't decide on speeds. 45mph road. I do like 5mph over. With no other cars around, it will speed up to 55-60 (Above max setting) then drop down to 40-43mph. It's all over the [XXX] place! Even worse on Freeway. So it actively exceeds speed limits, as much as enough to get my license automatically revoked under Wisconsin Law! 5: Redesigned where set speed limit is displayed. No longer shows unless you actively change it. Then it disappears again. Need to constantly fiddle with set speed just to see how crazy it will decide to break the law, or how slow it will go and [XXX] other drivers. 6: Rolling stops, had a number of rolling stops today and one where it blew straight through a stop sign. 7: Route choices are absolutely alinidine. Seems to prioritize taking the least efficient routes. 8: Drives like a bucking bronco with accel and decell. 9: Ping pongs within the lane. 10: False emergency braking events. 11: Last release the double stop at stop signs had gotten better. This release they are full back again. It is once again stopping at both sides of the intersection. 12: Vehicle departing lane warnings when driving manually, even when my TURN SIGNAL IS ON so I can pull into a parking lot or my own driveway! With the above, there have been no other changes other than firmware update. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C.Read more

This happened on 2/18/2023. After I charged my car at a local supercharging station, I drove on the local roads for about 2 miles when the car started shutting down. I could barely steer, and the computer monitor stated that the brakes were turning off, and other parts, like cameras, were losing functionality. I pulled over into the local park. I was frightened that my car was no longer functi
... onal and that I could be in grave danger driving again. I tried to call Tesla but no one answered. I had to go through the app. My dad came and we found a Youtube video that said to walk out of the car to let it restart. Looks like it happened to another owner. We did that and the car started functioning again. Not sure why it did this after a charge. But it happened again 3 mos later when I was on the freeway. Luckily, I knew what to do from prior experience. But I am mindful of this whenever I supercharge my car. I wait after charging to drive. This shouldn't be the case and Tesla should tell people of this issue. I tried to tell Tesla service when I went in for another repair but they ignored my comments.Read more

This is my 4th safety report about the same issue: unexpected braking while on cruise control. Since my last report, Tesla offered another free month of auto pilot but I did not take it for fear of this braking issue. On Tuesday [XXX] I was returning to my home in Las Cruces NM from Colorado. I was on [XXX] toward Albuquerque, and there was very little traffic. I decided to see if I could get aw
... ay with the cruise control. As I was pulling into Bernalillo, there were some oncoming cars in a left-turn lane at a signal, and my car braked automatically...enough to lurch me forward. I decided to stop at the "new" Tesla dealer in Bernalillo (I got my car in Santa Fe) and ask about the automatic breaking and whether they'd fixed it. The 2 service employees looked at each other and one said "No". I then asked about whether I needed to re-boot my key card since my screen went blank while I was in CO, during a snow storm, parked. I luckily had my phone and on the Tesla App, under Roadside Service, it gave me the option to re-boot the screen! I did so. The serviceman said he didn't think the blank screen had anything to do with my key card, but I could re-boot it if I want to. (A prior complaint also reported the failed key card in less than a year and Tesla's unwillingness to replace it). I leased a new 2019 Ford f150 xlt sport package in the fall of 2019. During the first year, I experienced some "choking" of the engine when I would accelerate. Nothing big, but noticeable. I mentioned this experience to the dealer at regular checkups and they informed me that it was "normal" and that if anything was wrong, the computerized monitoring system on the truck would identify the proble
... m. I bought the truck after a 36 month lease. The choking continued to be a regular occurance, especially for the first few miles of a new drive. Around 30k miles, I noticed the issue getting more pronounced. I again asked the dealer about it and they provided me the same answer as before and didn't offer to explore the issue. Between 30K and 50K miles, the issue became more frequent and would impact the truck whether it was warm or cold. But now I experienced a hard shift between 1-3 gears and sometime 3-5-- jerking and slowing and struggling to move to the next gear. It will also bog down at times when changing into the higher gears with no telling when this would occur. Again, I asked the dealer about it, and becuase there was no safety light, they wouldn't do a thing. Over the past month or two, at over 50K miles, the frequency was several times on every drive. It started to jump from 8-1 and back up to 8. It would jerk and slow in high traffic situations, and finally it just would not move into gear at a stop sign at rush hour. I had to drop it into low gears and move to the side of the road after 10 minutes of stoppage; then the check engine indicator finally flashed, after an incident that could have caused harm to me or others. I slowly drove it to my destination. Since then, I have not allowed anyone to drive the truck because it is dangerous to operate. I took the truck to the dealer and they informed me that it needs a new transmission which will be ~$5kRead more

The Head Unit, which is the center built-in touchscreen of the 2018 Subaru Outback, began glitching after 5 to 6 years after this vehicle was purchased. It would not register actual touches, and then it would register touches that did not occur, it became so severe that while driving, the radio would change on its own, navigation/GPS application would become unusable, and other distracting actions
... would occur without any way of fixing them. After some online research, matching photos of the screen showing bubbles underneath, and similar reports we have found including a class-action lawsuit against Subaru, the vehicle was taken in for official diagnosis of Delamination. Subaru Of America (SOA) agreed to pay $1000 of the approx $1500 cost to replace the Head Unit, and recommended we reach out directly to NHTSA to report a safety concern to have a potential recall started. This issue is a dangerous distraction and safety concern, and it seems to be a common occurrence with these Head Units. There were numerous incidents before getting the screen repaired where it caused driving distractions with the erratic movements and changes on the screen, which could cause an accident. Please recall these so no one ends up in an accident or ends up having to pay for this faulty part. Thank you for your time.Read more

My 2014 Kia Forte experienced random stalling/loss of speed. The vehicle was taken to the mechanic multiple times- the crank shaft sensor was replaced, new batteries were put in, and a starter was replaced. When starting the vehicle sometimes it would not turn over or click a few times to start. A ticking noise was noted aswell and relayed to the mechanic-mechanic could not find any issues. The oi
... l light began coming on and off-especially when stopped the light would stay on, when starting to drive if would go off. Took the vehicle to the closest mechanic-the mechanic said he has diagnosed another Kia with the same issue. He said to look up recalls. I found out that my car is experiencing the exact same issues as other Kias listed as NHTSA Recall Number: 20V750- It was diagnosed at a Kia dealer in which they noted the rod is coming lose in the engine and that the whole engine would need to be replaced. When calling Kia I was told I was not going to qualify because I have a 1.8 Liter engine, however the symptoms of the car are identical to the recall listed above. The car needs a whole new engine replacement. I tried multiple times to get my car diagnosed and was told by a few mechanics that nothing was wrong with the car. I do not feel that it is fair that this car is not included in the recall. It is a 2014 Kia Forte, and other 2014 Kia fortes are listed in the recall. The cost of repair is more than what the car is currently valued at. The car has just recently hit 104,000 miles. All the maintenance on the car was routinely taken care of and inspections/emissions have always been up to date.Read more

I am submitting a complaint regarding a serious safety issue with my 2023 Tesla Model Y. The vehicle experienced a sudden and complete power loss while in operation, creating a hazardous situation. ... lectrical system power reduced – Vehicle shutting down •VCFRONT_a192: Electrical system unable to support all features – Switching off features to conserve energy •BMS_a151: Vehicle may not restart – Service is required •DI_a227: Please wait while high voltage system powers up •Safety Risk: The unexpected shutdown posed a serious safety risk. If this had occurred in high-speed traffic, it could have led to a crash, endangering my safety and that of others on the road. •Reproduction of Issue: The issue has not been reproduced yet, but it is an ongoing concern. I have not received a resolution from Tesla at this time. •Inspection by Authorities: The vehicle has not yet been inspected by Tesla, law enforcement, or insurance representatives. However, I am willing to make it available for investigation. •Warning Signs: There were no prior warning signs before the failure. The alerts appeared shortly before the vehicle lost power completely. This issue raises concerns about the reliability and safety of Tesla’s electrical system. I urge the NHTSA to investigate this matter to prevent potential accidents caused by similar failures in other vehicles.Read more

Pedestrian Warning Sounds Gone I am writing to express my deep concerns regarding the ongoing issues that are being seen with the NHTSA recall campaign numbers 24V865000 and 24V623000. Following an attempted OTA software update for the safety recall, I have noticed that the pedestrian alert noises are missing, which poses a significant safety risk not only for those around the car but also for the
... occupants inside the vehicle. This situation is unacceptable and raises serious concerns about the vehicle’s safety features. Moreover, it has come to my attention that a concerning number of owners have reported their vehicles as undrivable following OTA updates, with 2%-5% of vehicles experiencing such issues weekly. The current 48-hour update window renders my car unusable, and now I require another session, an additional 48 hours to complete the update only adds to my frustration. Based on owner surveys, it appears that complete updates, including the correct sounds, have been successfully implemented in less than 30% of vehicles attempted. Given these concerns, I urge you to look into the situation with my vehicle and the entire process. I should not have to pay out of pocket for the safety recall and restored function of pedestrian alert noises, nor the cost of a rental car.
