The transaxle, torque converter, and/or PCM/TCM have suddenly, and rapidly started wearing and degrading to the point of requiring replacement. Available for inspection upon request. The vehicle had troubles shifting, finding/selecting gears, and as a result, would lurch or come to abrupt stops at low speeds, have difficulty accelerating which made it difficult to enter or exit intersections wher
... e there is great risk of collision. I had to very carefully choose and wait for the opportune time to enter/exit roadways where no other vehicles were coming in case the vehicle would not accelerate for 3-5 seconds when using the accelerator pedal. Problem has been reproduced since first occurrence and a dealer has inspected the components indicating that replacement is needed. There were no indications, warnings, alerts, or messages to suggest that an issue was occurring until AFTER the first several instances of failure to shift. I tried to use a scanner to find OBD2 codes, but there were none. A generic alert via the driver display was given, but it took 500+ miles for a more specific warning and OBD2 codes to be generated. From my understanding, there have been hundreds of drivers that have experienced the same issues with the same/similar models of Ford Edge vehicles that use the same transmission. NOTE: I had experienced shifting difficulty/roughness, RPM variation, slow gear changes, delayed acceleration at ~60k miles. I had brought the vehicle in for service where transmission fluid was flushed. Approximately another 55k miles later is when my vehicle experienced the severe failures described above. This is similar to many other vehicle owners experiencing these issues at ~60k miles.Read more

I bought a tesla model Y 2025 and got my car delivered on [XXX] I barely drove it and within a week, I noticed all sensors stopped working (ALL cameras and camera-related functions stopped working, including the legally mandatory backup camera for new cars, GPS has stopped working, making the premium connectivity package I paid not usable). Not only was I not able to use the self driving capabilit
... y, which I paid $8000 for, but also the car lacked the most basic safety features. I contacted Tesla service center, they refused to admit that the sensor malfunction is a security concern, and still put me on an appointment three weeks later (dec 13). After the first appointment, they did not fix the problem at all and noted it was a major computer issue, as reported by several media outlets [XXX] ). So my brand new car has a defective core component upon delivery. I was scheduled to meet them after they get the computer part on Dec 27. The appointment was rescheduled to Jan 14 again for missing parts. During the process, I found some weird tricks online and semi-fixed my car’s navigation but the sensor issues still exist. And on Jan 14 again, they claimed the computer part was missing again and I had to wait until that upcoming weekend to fix it. To summarize the situation: I was given a car that is defective upon delivery on its most important core component for 2.5 months, when the car lacks even the basic legally required safety features, as well as premium features that I already paid off! During the process, I also spent a lot of time dealing with their customer service, and rescheduled appointments (I even had to drive to their service center several times to get a loaner and had to return the loaner early before they did not have parts to fix my car). Their negligence on safety, and promised features is simply astonishing and unacceptable. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

On Tuesday, [XXX], at approximately [XXX] PM, I was driving southbound on [XXX] near Coalinga, CA, when my truck suddenly experienced a failure. I noticed smoke in the side mirror and asked my passenger to confirm. Moments later, a "Low Fuel Pressure" message appeared on the dashboard, and smoke began coming from the hood. I immediately pulled over and attempted to pop the hood, at which point f
... lames erupted from the engine bay. The fire quickly spread, fully engulfing the vehicle within minutes. Emergency responders, including Cal-Fire and CHP, arrived on the scene. The truck was towed by Cal-State Towing Service in Huron, CA. The Fire Captain on site provided an incident card, but the full fire report is expected to take at least a week to obtain. There were no prior indications of mechanical issues before this incident, and no recent repairs or modifications to the fuel system or engine. However, I recently became aware of an ongoing NHTSA investigation into fuel leaks and fire risks in Ford Super Duty trucks equipped with the 6.7L diesel engine (TFL Truck Report). The investigation highlights a potential issue with fractures in the secondary fuel filter housing, which could lead to fuel leaks and fire hazards. Given that my truck displayed a Low Fuel Pressure warning immediately before catching fire, I strongly suspect this failure may be related to the issue under investigation. The truck is currently in tow yard storage and is available for inspection upon request. This incident posed a serious safety risk to myself, my father and others on the roadway, and I believe it warrants further examination by NHTSA. I sent following to Tesla I had an accident at around 6:30 PM on Sunday, March 2nd with my Tesla Model 3 and asking to see if you can conduct a diagnostic check into this incident. I was coming home at that time with the Tesla and about 100 yards from my house, I experienced a sudden acceleration with the car going from about 20mph to, I think, over 40 mph and crashed into the garage area of
... my house. Around the time I started experiencing that unwanted acceleration, I heard a whining sound. As it started that sudden acceleration, I checked below to make sure my right foot was pressing on the brake pedal and it was. Since I have the regenerative braking with my tesla, the car should have decelerated but, instead, it sped up. I continued to press the brake pedal but I could not mitigate my car’s continual acceleration. It eventually crashed through the lowered garage door and went through the right side of the garage wall about half way out and came to the stop. I was driving myself and not engaged in self-driving feature. I like to see if the Tesla has any resources available to retrace last few seconds and be able to glean more information of why that sudden acceleration might have happened. The car is still located in my garage but currently the car is disabled and powered off. The car’s power was on and it automatically turned on emergency light for about 30 minutes after the accident before it eventually powering off. I am also curious to know why none of the airbags were deployed in this accident. I have many photos available of the accident. The fire department and police came and assisted. I do not know if this matters but the couple of nights before the car had one of those over the air updates by Tesla and also about couple of minutes before the accident I drove through about a minute of hard rain in otherwise light rainy drive. I also wonder why auto-braking did not engage against the house.Read more

Description of Incident(s) On March 5, 2025, I discovered that my 2023 Fisker Ocean had undergone an unauthorized over-the-air (OTA) software update to version 2.2.2 without my knowledge or consent. I received no prior notification from Fisker or American Leasing, the current entity managing Fisker vehicles following the company’s bankruptcy in 2024. Additionally, I have no access to the vehicle
... ’s cloud system or update logs to understand what changes were implemented, leaving me unable to assess the scope or purpose of the update. Following this update, I experienced two significant safety-related issues with my vehicle, detailed below. Driver’s Door Handle Overheating After the 2.2.2 update, the driver’s side door handle began exhibiting extreme overheating, reaching temperatures exceeding 130°F as measured with a non-contact infrared thermometer. This poses a serious burn hazard and makes the handle unsafe to touch for entry or exit. The issue appears to be electrical in nature, potentially related to a malfunction in the door handle actuator or power management system altered by the software update. The handle remains excessively hot even when the vehicle is powered off, suggesting a persistent fault. This defect compromises safe access to the vehicle and presents a risk of injury. Rearview Mirror Fan Constantly Running Since the update, the fan associated with the vehicle’s digital rearview mirror now runs continuously, regardless of whether the mirror is in use or the vehicle’s thermal conditions warrant cooling. This creates unnecessary noise, potential wear on the fan component, and a possible drain on the vehicle’s battery. While the digital mirror functionality remains operational, the constant fan operation suggests a software glitch affecting thermal regulation or sensor feedback loops, likely introduced by the 2.2.2 update. This issue, while not immediately hazardous, indicates broader concerns with the stability of the vehicle’s software systems post-Read more

Incident Details: Date & Time: [XXX] Driver: My spouse, Purpose of Trip: Personal (heading to a nearby soccer park) Location: [XXX] Weather & Road Conditions: Clear weather, dry roads, normal traffic conditions Driver Safety: The driver was wearing a seatbelt at the time of the incident Incident Description: My spouse was driving to the nearby soccer park with the vehicle in FSD mode. Upon appro
... aching a railway crossing, the signal was green, indicating it was safe to proceed. However, instead of maintaining a controlled speed, the car suddenly accelerated while attempting to cross. Before my spouse could take control, the vehicle struck the boom barrier, which was either lowering or had already lowered at that moment. The incident happened so suddenly that she did not have enough time to react and override the system. Fortunately, no injuries occurred, but the vehicle sustained damage to the front bumper, windshield, and frame. The unexpected behavior of FSD at a railway crossing raises serious safety concerns regarding its decision-making. Safety Concerns: I have been a long-time appreciative user of FSD, but this incident has shaken my confidence in the system. I now fear using FSD. Given the serious nature of this incident, I request an urgent review and response regarding my concerns, as well as coverage for the necessary repairs. Request for Investigation & Cost Coverage: Access to Vehicle Logs: I request access to the vehicle's telemetry and camera recordings to determine why this happened. FSD System Review: Please investigate whether this was an FSD-related miscalculation and what corrective actions are necessary. Cost of Repairs: As this incident was caused by an unexpected FSD action, I request Tesla to cover the repair costs for the vehicle and any associated damages. I own a 2022 Mazda CX-5 Carbon Edition and have recently noticed a persistent noise coming from the left headlight. The noise resembles a fan or vibrating sound. Coincidentally, this issue started only a month after I bought out the vehicle following the lease expiration. Component/System Failure and Inspection: The issue appears to be caused by a malfunctioning auto-leveling or adaptive headligh
... t motor. The component is still installed in the vehicle and can be inspected upon request by a dealer or manufacturer. Safety Risks: While this issue has not yet caused immediate driving hazards, a malfunctioning headlight system could pose a safety risk if the auto-leveling function fails, leading to improper headlight beam alignment. This could impact nighttime visibility or cause glare for other drivers, increasing the risk of accidents. Reproduction and Confirmation: The problem has not yet been confirmed by a dealer or independent service center in my case, but many other customers have reported similar issues, some of whom have had the component replaced under warranty. Inspection Status: As of now, the vehicle has not yet been inspected by the manufacturer, police, insurance representatives, or other parties. Warning Signs or Symptoms Prior to Failure: No warning lamps, messages, or dashboard alerts appeared before the noise started. The issue began suddenly and has been persistent ever since.
