My 2013 Kia Optima was running fine at 65 mph when suddenly the engine stalled. I then coasted to the shoulder and smelled burning plastic. As soon as I opened the hood the engine started being consumed in fire, it quickly went to a very large engine fire that consumed the front half of the car. There were no other vehicles involved with this incident. Vehicle is a total loss.

While driving my Ineos Grenadier in the middle of the Arizona desert, the steering suddenly became difficult to control due to a tie rod nut coming loose. The vehicle began wandering and required constant correction to maintain control, creating a serious safety concern in a remote area far from immediate assistance. There were no warning lamps, messages, or alerts displayed inside the vehicle bef
... ore or during the incident. Just one week prior, the steering and front end components had been inspected by an authorized dealer during service, and no issues were identified at that time. Upon inspection after the incident, the tie rod hardware was found loose, which could have resulted in a complete loss of steering control if not caught in time. This appears to be a known issue within the Ineos Grenadier community involving steering and tie rod hardware concerns. The failure presents a significant safety risk to vehicle occupants and others on the road, especially during off road or remote travel where recovery and emergency services may not be readily available.Read more

I am the original owner of a 2013 Hyundai Sonata (under 90K miles). In April 2026 the engine suddenly locked up without warning. The vehicle was towed to Massey Hyundai (Hagerstown, MD) and a claim was submitted to Hyundai Motor America for an engine replacement under Hyundai’s lifetime engine warranty program for affected 2.4L and 2.0L Theta II engines. I provided records I could obtain. Some o

I explained this & submitted all existing documentation. They denied my claim twice and closed both cases. I requested the official written denial, the date it was issued, the specific reasons for the denial, and the name and department of the individual who made the decision. Hyundai refused to provide any of that information, stating they “cannot disclose the information or documentation” and that the denial is “final.” Hyundai has not identified any specific maintenance item I allegedly neglected, nor provided any evidence supporting their claim of “exceptional maintenance neglect.” I have completed all Hyundai recalls and maintained the vehicle responsibly. Hyundai refuses to provide transparency or to reconsider the claim despite my documentation and original-owner status, I am requesting the Bureau of Consumer Protection investigate Hyundai Motor America for possible unfair or deceptive practices related to warranty denial & lack of disclosure. I ask that your office ask Hyundai to reopen & reevaluate my warranty claim for the Theta II engine defect based on the documentation I provided & my maintenance history as the original owner Provide the written denial, the date it was issued, the specific reasons for denial & the name title & department of the decision-maker Produce any evidence Hyundai relied on to conclude there was “exceptional maintenance neglect Hyundai can't substantiate its denialRead more

The car thinks the key fob is inside the vehicle when it is not. If you get out of the vehicle it will not self lock even if you have that feature set up. The most concerning thing is you can get into the

The car shifted itself into neutral while running between 65-70 mph on the interstate. Multiple vehicles
... were accelerating after the 55 mph work zone area was over, and we had to swerve between cars to get to the shoulder of the road because the vehicle lost all acceleration capabilities. We have taken the vehicle to the dealer for past recalls and software updates and mentioned this issue happening before and were told that the vehicle showed no codes concerning the electronic parking brake issue. Upon further investigation, there are multiple reports and forums of this particular vehicle having the same issue. Its a miracle we didn't get involved in a massive collision with semi trucks and other vehicles amongst us when the vehicle lost all ability to accelerate. This could cause a huge problem if it were to happen again in a more populated area or at increased speeds. We were stuck on the side of the road for 4 hours waiting on a flatbed to tow it because it locks up the AWD system with the electronic parking brake and doesn't allow the wheels to move. This is the second time we've had it towed for this same issue only to be told that it's not a problem. I'm willing to bet that the company knows exactly what the issue is but won't issue a recall because they don't classify it as a safety recall until people start losing their lives or cost the company stockholders money. This problem has the ability to cause major harm or death to people with this vehicle if it were to shift itself into neutral or lock up the electronic parking brake while in a heavy populated area. I hope they issue a safety recall and hold the company accountable before people lose their life.Read more

The suspension module failed due to water accumulation on floorboards. Suspect other electronics may be at risk. Water ingress cause by a combination of what dealer said was a worn out sunroof gasket and two of four drains that became plugged. Further examination from dealer found the plugged drains were crimped from when the vehicle was manufactured. The dealer blamed the excessive water ingress
... was further allowed by a failed sunroof seal. The vehicle has always been serviced by the dealer and problem areas like that were either not inspected or not reported. The seal appears as good as the day we purchased vehicle from dealer. It seems it needs a more expert eye to detect how it fails. The result was standing water in the floor boards and body cavities, which then caused the suspension module to fail. The entire interior had to be disassembled to verify there was no further electrical damage. The dashboard and other interior pieces needed to be removed to replace or correct the drains. This appears to be a common problem with this model although no service advisories seem to have been issued. Total estimated damage is about $5200. Vehicle has only 60K miles. Volvo has rejected responsibility for the compromised drain and failed sunroof seal. Service documentation attached, not including the estimated $1700 to remove dash for repairs.Read more

I am submitting this letter to formally express my serious concern and dissatisfaction regarding a safety issue that occurred and that, under only slightly different circumstances, could have resulted in a far more dangerous outcome. The situation I experienced was not only unacceptable but indicative of a potential systemic failure that warrants immediate investigation. The incident posed a clea
... r risk to my safety and the safety of others. It is deeply troubling that such a failure was able to occur at all, and even more troubling that—had the conditions been marginally different—it could have led to severe injury or worse. This is precisely the type of preventable hazard that NHTSA exists to identify and address before it results in tragedy. I am requesting that NHTSA review this matter thoroughly, determine whether similar incidents have been reported, and take appropriate action to ensure that this issue is corrected. Consumers should not be placed in harm’s way due to defects or oversights that compromise the safe operation of a vehicle or its components. This experience has left me with significant concerns about the reliability and safety of the system involved. I urge NHTSA to treat this as a serious complaint and to investigate promptly so that no one else is put at risk.Read more

The vehicle is experiencing sudden engine stalling while in motion, a known safety hazard. Despite a documented maintenance history with no issues, the dealer (Bayside Kia) is attempting to deny a Theta II Settlement claim by shifting the narrative to 'engine sludge,' which only appeared as a symptom in June 2025( Walmart oil change) alongside engine performance issues. The vehicle has active code
... s P0014 and P0017, which are secondary symptoms of the internal bearing wear and metallic debris contaminating the oiling system. KDS Diagnostic tests showed Cylinder 1 at 0.16mm (approaching the 0.20mm failure limit). This clearance proves the connecting rod bearings are worn and reaching the failure threshold; this mechanical wear is the root cause of the engine's inability to operate safely. The presence of metallic debris in the oil is the definitive signature of the Theta II manufacturing defect. Furthermore, the dealer acted in bad faith by attempting to charge for Airbag repairs (Codes B1346/B1481) that are covered under the 15-year Clock Spring Warranty Extension (CS1102). The dealer has dismissed my qualifying maintenance documents, claiming they are 'not official,' which is a direct violation of the Magnuson-Moss Warranty Act. A manufacturer cannot deny a warranty claim because services were performed at third-party facilities, nor can they deny a claim without proving 'exceptional neglect.' I am reporting a refusal to honor a court-ordered safety settlement and an attempt to return an unsafe, stalling vehicle to the road. There is no evidence of engine abandonment or exceptional neglect. I’m considered "in compliance" if I can show I didn't go more than 14 months without servicing or a certain mileage between oil changes. The dealer's dismissal of verified third-party receipts and bank statements is a bad-faith attempt to ignore an inherent manufacturing defect documented by a 0.16mm BCT result and metallic debris.Read more

The vehicle is scheduled for an appointment on Monday, May 11, 2026 at Porterville Ford for inspection. Approximately 50% of the glass came out, both inside and outside the vehicle. If my children were there, they would have been covered in broken tiny pieces of glass.
... This vehicle has just over 6,000 miles since new and has never had any damage to the glass. I have researched online and there are many other instances where this has happened, often siting incorrect or too tight of installations where the pressure from closing a door shatters the tempered glass.Read more

What component or system failed or malfunctioned, and is it available for inspection upon request? The flexible brake hose on my 2015 Ford Edge failed/broke shortly after I purchased the vehicle. The vehicle also began experiencing engine performance issues, including a rough idle and rough driving condition after the check engine light came on. The vehicle and failed brake hose should be availabl
... e for inspection upon request. How was your safety or the safety of others put at risk? The broken flexible brake hose created a serious safety risk because it could affect the vehicle’s braking ability and increase the risk of losing control or being unable to stop safely. The rough engine performance also caused concerns about the reliability and safe operation of the vehicle while driving. Has the problem been reproduced or confirmed by a dealer or independent service center? The issue has been observed and experienced consistently while driving the vehicle. The brake hose damage and rough engine performance were noticeable after the vehicle was purchased. (If a mechanic has looked at it, you can add: “An independent mechanic/dealership confirmed the brake hose was damaged.”) Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? I have taken it to the dealership and they said nothing about it but I got it diagnosed at autozone and the code came up right away. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Approximately 2–3 weeks after purchasing the vehicle, the check engine light came on and the vehicle began running roughly. Around the same time, issues related to the broken flexible brake hose were discovered. Symptoms included rough driving performance and concerns with the braking systemRead more

I am the owner of a Nissan Rogue subject to recall campaign R25D1, which addresses a manufacturing defect affecting engine bearings and/or engine components that may progressively fail over time, leading to engine damage and engine failure. I brought my vehicle to Headquarter Nissan in Columbus, GA (the same dealer that issued the recall notice) for diagnosis after experiencing engine problems. Th
... e dealer’s diagnostic findings document a complete engine failure consistent with the recall description, including: •EGR valve failure (valve could be opened to 70 degrees with no change in engine performance, when anything over 20 degrees should cause the engine to stall) •Engine overheating •Blown head gasket •Failed EGR cooler •Total coolant loss (vehicle arrived with no coolant; 2 gallons had to be added before fluid registered in the overflow) The dealer’s recommended repair is full replacement of the engine, turbo, catalytic converter, EGR valve, and EGR cooler, along with all gaskets and hardware. The estimate provided is $9,514.72. Despite the failure pattern matching the recall description (progressive component failure leading to engine damage), the dealer is denying recall coverage by attributing the root cause to the EGR cooler rather than the bearings or engine components named in the recall. This causation is also inconsistent with the diagnostic write-up itself, which describes the EGR valve as the initiating failure that led to overheating and subsequent damage to the head gasket and cooler. I am filing this complaint because the dealer’s denial appears to be a reframing of a recall-eligible failure to shift repair costs onto the consumer. I am requesting NHTSA review of how recall R25D1 is being applied at the dealer level and whether failures like mine are being inappropriately excluded.Read more
