Phantom braking (as it's referred to by experts) happened to me twice on the highway using Traffic-Aware-Cruise-Control. Very scary. From my research, this is a huge problem with Tesla vehicles that thousands of individuals are loudly complaining about online and now in court cases in the country. I see there are thousands of reports to your organization about this, but it says you have only complaints from 2021 and later years. However, I have a 2019 and it's happening to me. And it's happening to every single model year out there, just from researching and talking to Tesla owners. This is a major safety hazard, plain and simple. I was inches of being in a rear-end collision, and drivers behind me are extremely upset and vengeful on the road after an incident like this (understandably!). Our Tesla Model X is failing recently with several issues as listed below: 1. 'Autiopilot disabled' - Autopilot was not available to use since 09/2024 after upgraded to 12.x FSD upgrade. Tesla service diagnosed in 11/2024 with some error connected to evaporator fan, advised me to replace the kit at an estimated $580 2. 'Unable to start' - HV battery failed on 03/06/2025 with series of failur

'Hood unlatch' failure - while driving home from Service center on the same day at 70 mph on I-95 freeway, the 'hood' popped up blocking the front view, and had to pull vehicle to shoulder to safety. There was NO warning message on the screen at all. Took the vehicle to service immediately, and service team pointed that they forgot to turn on 'auto-latch' resulting hood failure. They offered the 'free' repair of hood. 4. 'ECU Autopilot failure' - while taking the delivery of 'hood' fix on 04/22/2025, I noticed the 'autopilot' was still NOT working. Serviced diagnosed with 'failed autopilot ECU' at an estimated cost of $1791.55. Currenlty the vehicle is with Tesla Service team. My vehicle is with service team since 03/06/2025, and has been identified with series of issues/failures. It has caused severe safety failure putting our lives at risk with 'unlatched' hood while driving on a freeway. Also with HV battery failure its completely unreliable on when it might fail during driving. Now with failed autopilot ECU, we are unsure of the vehicles ability to navigate, drive function and safety concerns. Service team is unable to provide precise root causes or fixes, or restore it to a functioning vehicle. Every visit with service team there is a new issue with vehicle. Software 2025.14.1, Full Self Driving 12.6.4 (FSD). Vehicle when using FSD continuously slows to 10 MPH below the speed limit. You can manually press the accelerator to speed up but FSD will not brake when doing so and warns you of the same. After a minute of being back at the speed limit, FSD again slows down. Everything happened so quickly: I could not control the steering wheel, and I applied the brakes in vain. The vehicle seized control, no matter how hard I tried to stop it. I regained control of the vehicle at the intersection of [XXX] and [XXX] , and was able to park the car at location A. We were very fortunate and blessed that we did not hit people at the bus stop, that there were no cars in front of us, that the opposite traffic on [XXX] was stopped by a red light, and that the traffic was light on [XXX] Without such luck, my wife and I, and probably others, would have been seriously injured, if not worse. Because [XXX] is one-way, we decided to move the vehicle to [XXX] , and parked at location B in the drawing. I called 911 and spoke to a female dispatcher. She asked us to remain at location B and wait for police officers to arrive. We waited there for almost 3 hours, but no police arrived. The dashboard display indicated that we had a flat tire(s). We called mobile tire services. After their inspection, most of our tires were alright, except the rim of the front-left wheel was badly bent, as you can see in the attached photo. The mobile tire service staff was skilled enough to carefully patch the damage and said it should be OK to drive back to Palo Alto. I purchased this vehicle on[XXX]. I was advised that there was an open recall for the O2 SENSOR. On 11/11/24, I took this vehicle to LOU BACHRODT in Coconut Creek,Fl. where they performed the recall. Shortly after having the recall done my vehicle's check engine light came on and then would turn off and continue to come intermittently. I took it to one of my local parts store that does the check e
... ngine light scan and it was giving the code for a faulty O2 sensor. I took it back to same dealership on 12/17/24. I explained to Charles Ramasir ( service advisor) what was happening. He ran a scan even though the check engine was not on at that moment. The code that was stored was a transmission modular code. He said I would have to bring the vehicle back when the engine light was actually on because he could not accept a scan/fix finder report from any other then this dealership. The problem continued and was now affecting the performance of my vehicle. I was able to get the vehicle back to Lou Bachrodt while the lite was on and their scan did indicate the O2 sensor. The part was reordered and the recall was done a second time by Lou Bachrodt Chevrolet. My vehicle continues to have the same problem. The engine lite continues to come on intermittently and the scans continue to indicate a problem with the O2 sensor: code on [XXX] =P0138 code on [XXX] =P2271 I returned to Lou bachrodt on may 3,25 while the lite was on. As I drove into the service area the service greeter advised me to drive up and wait for the service advisor-Charles. I proceeded to turn off he vehicle and wait. As soon as I turn the vehicle back on to show Charles the engine lite, it was no longer on. And once again I was told that if the lite was not on he could not run the scan or accept the scan report from Auto Zone from the day before. Common problem amongst Can Am Ryker riders. When making a sweeping left turn between 20-40 mph, the right front wheel will brake/accelerate on it's own and the bike will jerk. It also causes the right wheel to wobble forcing me to accelerate or decelerate to retain control of the bike. The buttons to the door locking and unlocking don't work unless I open and close my door a few times. The same thing with the back windows. But after you do the trick and use the intended button once, it stops working and have to repeat the entire process over again. Which is not safe especially when the car is in motion, I am not able to use those buttons at all.

This is particularly concerning because the car has shut off in the middle of traffic, at intersections, and while waiting at traffic lights, which poses a serious safety risk to me and other drivers. I understand that this issue may be common among other vehicles of the same make and model, and I respectfully request that it be investigated as a potential case for a technical service campaign or safety recall. By its very nature, the reboot catches the driver by surprise and requires diverting attention from driving.Typically, this user input is provided while stationary, immediately after starting the vehicle. The issue emerged sometime in February or March 2025. The vehicle had the software update done at Flemington Audi and problem still persists today. This issue constitutes a safety concern, as reboots can occur during reverse parking maneuvers, rendering the backup camera and obstacle detection sensors inoperable until the system reloads. Additionally, reboots completely disable the navigation screens and necessitate multiple user inputs to restore functionality, which can be hazardous while driving. Many states have enacted hands-free laws and requirements that mandate the use of dash-mounted systems to comply with these regulations. The reboots render such systems inoperative. The message/warning light keeps coming on: Stop/start Unavailable - Service stop/start system. This feature is suppose to help reduce the amount of harmful emissions released into the air! Some times this causes the engine/transmission to clunk. Seems to maybe a common issue with the Jeeps. Save fuel from burning while the car is stationary. Such a great feature and is doesn't work!
