This incident happened on [XXX], I just got the Tesla data and police report. Here are the events as I remembered: - I was driving my 2023 Tesla Y with FSD supervised (v13.2.8) along [XXX] entered from [XXX] and driving close to north of [XXX] without any issue - Noticed a car in front of me braking hard (about 6secs from the recording dashcam footage) - Realized FSD was not braking hard enough
... (or at least it did not feel like it) - Slammed on the brake for what felt like an eternity (about 2 secs from the dashcam footage) - Rear end collision on the front car at ~25 mph, airbag deployed. I remembered feeling the brake pedal vibrating but did not stop the car at all. By analyzing the raw data provided by Tesla, it is found brake, accelerator, AEB, ABS and TSC were completely messed up after FSD failed, please refer to the attached analysis report. Note: The dashcam video file and the original Tesla data are available for reviewing if any need as I cannot upload along with this report. I spoke to a sales rep about trading my car in for new but he said engine issue still not resolved. He advised me to lease Escalade for $900 because by the time my engine blew my lease would be up; if not GMC fix under warranty!! Mechanic said new engine but GMC will take care of it. I waited 3+ months for GMC to authorize repair. GMC said they did not get docs but mechanic sent me verification of conversation and repair order sent to GMC. Mechanic received an unrelated warranty from GMC that said my issue was not covered. Mechanic said docs sent made no sense bc he sent the right warranty and he was sorry and embarrassed by GMC behavior. I finally paid over 14K for new engine. Picked up my car and drive a block and car still knocking and engine light came back on. I had returned my rental and no way home. In tears dealership gave me my rental keys back still all at my expense. The mechanic calls me two days later to tell me my torque needs to be replaced. I am hysterical, 14k spent, and car still not fixed. GMC told me to pay additional funds for torque converter or pick up. Threatened to charge storage fees. When I discussed their liability GMC became very rude and antagonistic. They cut off my communication with their lead mechanic. Took care of back to local repair shop. Tesla FSD Incident – Exton, PA ([XXX]) Just sharing a scary experience I had with Tesla FSD Beta yesterday — hoping it reaches the right eyes and helps others stay alert. I was driving on a main road in Exton, PA, using FSD with the “Avoid Highways” option turned on for the first time. This is a route I know well and usually follow using Google Maps. At one point, FSD turned on the left
... indicator, and I thought it was taking a regular exit — the road it chose looked like a narrow side street, so I didn’t question it. But within 5 seconds, I realized it had actually taken me into the oncoming traffic lane. It was shocking and totally unexpected. I immediately took over, stopped the car, and when the road was clear, safely steered back into the correct lane. This happened around 3 PM on a bright, sunny day, with normal traffic. I have so many problems with my truck and the surging engine as well as squealing brakes, lane assist is disabled but comes on and jerks the truck out of my lane and it's impossible to steer almost causing me to crash numeroys times. ... even sell this truck privately because it needs a new engine. I have 25,300 miles as of today and I've had it in the shop so many times and they say they can't find anything wrong with it. The headliner fell down onto my head and took it in for service and I got it back worse because they broke all the clips that hold the interior headliner in place and it's hanging down. Every time I bring it in for something they tell me the parts are not available but what they're doing is trying to make me run out of my warranty which is in 2 months. They're trying to sell me an extended warranty but they can't even fix anything that's under warranty right now. I really need your help here because there's so many other issues that I haven't even gone over. Everything is misaligned on this truck and they can't fix it my rear passenger door never latched correctly and they don't know what to do they just give it back to me the same way and I have small children that sit back there but they claim they don't know how to fix it.Read more

Over a year ago, my car developed an intermittent slow crank/starting issue. After driving 20–30 minutes and turning the engine off, it often wouldn’t restart on the first try, sometimes requiring 2–5 cranks. From my research (forums, Reddit, Facebook), I suspected heat soak affecting the starter, which can eventually harm the battery, alternator, or starter itself. About six months ago, the
... battery was replaced under warranty, though I don’t know the full details of what the dealership diagnosed back then. On April 25, 2025, a more dangerous incident occurred: while driving on the highway, my engine suddenly lost all power without warning, right as I tried to accelerate to merge lanes. I had to coast in neutral onto the shoulder, narrowly avoiding a dangerous situation on a busy road. There were no warning lights or advance signs. After getting towed, I suspected the alternator had failed due to the long-term starter heat soak issue. Both the tow truck driver and dealership technician thought the same, especially after the dealership checked the OBD2 and only saw low-voltage-related codes. After 1-2 weeks of diagnostics, the dealership shocked me with their findings: the engine had catastrophically failed. They suspect metal shavings, possibly due to a rod bearing or crankshaft failure, but the exact cause is still under investigation. They are going to replace the engine and will provide the full details once the work is done. What’s concerning is that the slow crank issue had been happening for over a year, yet no clear signs pointing to impending engine failure. Only hints were a static (not flashing) check engine light that came on and off a few times over the past year, usually disappearing after 1-2 days and not returning for months. The LT1 6.2l V8 in my 23 Camaro is essentially the same engine found in GM’s recent massive recall for L87 6.2l V8 engines in trucks/SUVs. This calls for investigation into the LT1 engines.Read more

I am reporting an issue with a 2020 Chevrolet Silverado 1500 equipped with the 3.0L Duramax diesel engine. The vehicle experienced a failure of the NOx Sensor Position 2, which significantly impacted performance and emissions compliance. GM previously issued Emission Recall N212351211 (March 7, 2023) for other diesel Silverado models regarding improper ECM detection of NOx sensor faults. The issu
... e in my vehicle appears functionally identical, yet this engine and model were not included in that recall. The sensor was replaced and reprogrammed at a Chevrolet dealership at the owner’s expense. After contacting Chevrolet, a customer retention check was provided, covering roughly half of the total cost. While appreciated, the remaining cost burden remains significant. If this model is eventually included in the recall, it may allow for full reimbursement and support for other affected owners. Online forums show that many 3.0L Duramax owners are experiencing similar failures, suggesting this may be a widespread and systemic defect. Due to the strong similarities with the recalled condition in other models, I respectfully request that NHTSA investigate whether this recall should be expanded to cover additional affected vehicles. This issue presents both reliability and emissions compliance concerns. In addition, the vehicle developed a separate mechanical/electrical problem shortly after the NOx sensor failure. Because it involves different systems, I will file a separate complaint regarding that issue.Read more

I own a Tesla Model X, and for the past two summers I’ve experienced a repeating safety-critical issue during hot weather. After driving for a period in the heat, multiple systems begin to fail. The car displays numerous warnings including: •“Parking brake requires service” •“Traction control disabled” •“Stability control disabled” •“Steering assist reduced” •“Automat
... ic Emergency Braking unavailable” •“Regenerative braking unavailable” •“Lane Departure Avoidance unavailable” When this happens, the steering wheel becomes very stiff and hard to turn, which severely affects my ability to drive and control the vehicle. I’ve brought the vehicle to Tesla service twice. The first time (when it was still under warranty), they said they couldn’t replicate the issue. The second time, I brought it in while the warnings were still active. Tesla only cleaned some sensors and returned the car, but the issue returned again recently with all the same symptoms. This only happens in hot weather. Once the car cools down, the errors disappear. This intermittent issue is difficult for the service center to replicate, but it presents a serious safety risk, especially because steering and braking functions are compromised. I believe this is a deeper electrical or software issue affecting multiple safety systems under thermal stress. The issue has not been resolved, and the vehicle remains dangerous to drive in high temperatures.Read more

I am writing to formally request reconsideration for including my 2017 Kia Forte in the existing recall (NHTSA Recall No. 21V-260, Manufacturer Recall No. SC210) affecting similar 2017-2018 Kia Forte models. Despite my vehicle not being initially included, I believe it should be reconsidered due to the following reasons: 1. Similar Engine Issues: The recall pertains to certain 2017-2018 Kia Forte
... models manufactured at the Kia Mexico plant between June 5, 2017, and July 24, 2017, equipped with 2.0L Nu MPI engines. My vehicle, manufactured at the Kia Mexico plant, is equipped with the same 2.0L Nu MPI engine and has experienced similar issues as described in the recall, including abnormal engine noise and warning lamp illumination prior to engine failure . 2. Consistent Maintenance Record: I have maintained my vehicle consistently, adhering to the manufacturer’s recommended maintenance schedule. The Carfax records demonstrate regular oil changes, brake inspections, tire rotations, and other necessary maintenance performed both at authorized Kia service centers and independent shops. The most recent maintenance was conducted at 112,530 miles, just before the engine failure occurred . This demonstrates that the failure was not due to neglect or lack of proper maintenance. 3. Premature Engine Failure: My vehicle’s engine failed before reaching 116,000 miles, which is significantly premature for a well-maintained engine of this type. Given the similar defect characteristics — oil pressure issues caused by foreign particles in the oil pump variable relief plunger — I strongly believe that my vehicle’s failure aligns with the recall conditions described . 4. Safety Concerns: The defect described in the recall poses a significant safety risk due to the potential for loss of motive power while the vehicle is in motion. My experience aligns with this risk, as the engine failure occurred without sufficient warning, putting me and others at risk.Read more
