The 3rd brake light is known to leak on Chevy cars, which I found out when searching up the issues, which allows water inside trunk without your knowledge and does water damage to the battery and other electrical components that are located in the trunk that controls the dashboard, a/c, radio, power steering and probably even more. Safety risk: makes your dashboard stop working along with the powe
... r steering, while driving. There is another vehicle at the same dealer as mine right now with water in the trunk, it’s an impala. When you search up on the internet about dashboard not working along with radio and a/c there are quite a few different people pointing to the leaking 3rd brake light and water in the trunk damaging the electrical components in the trunk. The vehicle is at a Chevrolet dealer right now being fixed. No warning messages before it happened. I have only owned the car for 1 year bought it used June 2024. The bill I included is so far what they are saying needs to be fixed but I’m sure more to come as they get some components working.Read more

While traveling northwest at 37 mph, the automatic brake system engaged jolting the driver. The camera shows no objects in front of but there were two vehicles traveling southeast in the opposite lane prior to the brakes applying. There were shadows from trees. If there was a vehicle behind, and the driver did not react fast enough, this would have been a rearend collision. This is the third time
... that the automatic brake system engaged for no reason since we have owned this vehicle (Dec 2021) and each time it has been reported to the NTSB. In the past two cases, there were no vehicles or shadows from trees, since we were in the desert. The previous two times, the vehicle was inspected by the dealership with no defect noted (under warranty). There were no dashboard warning notices indicating an issue. The vehicle will be resubmitted to a dealership for inspection for this third incident (not under warranty).Read more

My Subaru Outback 2022 experienced a failure of the Supplemental Restraint System (SRS) sensors, resulting in the airbag warning light turning on and the airbags being disabled. The nature of the defect, the diagnostic trouble codes, and the location of the failed component are identical to Subaru Recall 24V227000 recently issued for the same model and year range. However, my VIN is not included
... in the recall. This is a safety-critical defect that prevents the airbags from deploying in a crash. The failure occurred under normal driving conditions and with all required maintenance performed. I believe my vehicle should be included in the existing recall, or a new recall should be issued to address this broader production issue. The defect puts occupants at risk of injury or death in the event of a collision. Please investigate this matter and require Subaru to expand the recall coverage to all affected VINs.Read more

awd hybrid vehicle makes a noise at speeds under 20mph , that sounds like an electric motor or bearing - salesman said the noise is a mandated law by nhtsa ? or ? month later the maintenance tech Forman took it for a test drive and said the same thing . that the noise was created (by a speaker ?) so the vehicle can be heard at low speeds / below 20 mph . i asked where the speaker is , he said he
... didn't know ! i asked if he could show me it on a diagnostics screen , he again refused . the noise sounds exactly like an electric motor . mg1 , mg2 ? i can't hear the noise when backing out of a driveway or similar low speed situations ? i need to verify if this is true - is there an actual law that the vehicle has to make a noise ? and it was okayed to use a noise that sounds like the hybrid electric motor or bearing ? even if this is true the computer logic should be changed so that the sound pulses so that it is obviously a speaker and not a mechanical issue . ( like no grease ) and it should always work when it is most needed , backing up and slow driving / all the time under 20 mph . service department should plug in a diagnostic scanner and prove to me that a speaker is making the noise . * why is this a safety issue ? because if they are lying , eventually ( when it is out of warranty ) this vehicle will break down . are ALL 2025 Toyota hybrids making some kind of noise ? and if it is a noise created by a speaker - Prove It are all 2025 Camrys making this same noise ? maybe they all have the same mechanical problem ? why wait till there is a crash , fire , or an accident ? as in the next section - Additional Details . and again if it is a legitimate noise generated for safety . at the least change the computer logic so it pulses and doesn't sound like a bad motor generator bearing . And have the maintenance techs trained to know where the speaker actually is . i don't want to share my personal information with toyota . if they are lyingRead more

What component or system failed or malfunctioned, and is it available for inspection upon request? The air conditioning (AC) condensate drain system is either blocked, misrouted, or improperly sealed, causing AC condensation to pool and leak into the front passenger-side footwell through the factory floor sealant. The vehicle is available for inspection upon request. How was your safety or the sa
... fety of others put at risk? The continuous leak has caused significant mold growth on the interior insulation behind the passenger footwell, creating a potential health hazard for occupants, especially those with respiratory conditions or allergies. Prolonged exposure to mold inside a sealed cabin environment poses a serious risk to occupant health. Has the problem been reproduced or confirmed by a dealer or independent service center? The vehicle was brought to a dealership for this issue, but the problem persists. I later reproduced the issue myself by running the AC for 30 seconds, after which water visibly seeped through the floor seam. Mold growth was also confirmed behind the interior trim. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? The dealership previously inspected the vehicle but failed to resolve the issue. No other parties (e.g., manufacturer representatives, police, or insurance) have inspected the vehicle at this time. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? There were no warning lights or messages. The only symptoms were damp carpet in the front passenger footwell and a persistent musty odor, which appeared prior to discovering the mold and leak source.Read more

For approximately the last year (2024-2025) my 2020 GMC Sierra 1500 AT4 with the 6.2L engine has had noticeable ticking, knocking, and unprovoked shaking upon start up and idle. The pickup has stalled out at stop lights and intersections in the past, posing risk. In June (2025) the pickup was taken to a certified mechanic for inspection who suggested possible lifter or valve failure. The vehi
... cle exhibits a persistent ticking sound from the engine, which I believe may be related to lifter or valve issues, similar to those reported in lawsuits against GM for 2019-2024 models and the recall for 2021-2024 models (NHTSA Recall ID: 24V-676). This ticking occurs regularly, including at approximately 54,500 miles, and raises concerns about potential engine failure or sudden loss of propulsion, which could be hazardous, especially at highway speeds. I have noticed approximately 2 quarts of engine oil usage per my last 2 oil changes (roughly every 6,000 miles). I recently had taken the pickup to a local GM dealership where they informed me that I am not longer covered under warranty and the recent recall is not applicable. I was advised to “keep and eye on it, and get a AAA membership”. I am aware of at least 10 class action lawsuits against GM for similar engine issues in vehicles like mine, suggesting a widespread defect. These issues, combined with the potential for engine failure or impaired vehicle control, warrant investigation to ensure driver safety. I have records of the ticking sound and can provide further details if needed. I am willing to cooperate with NHTSA for inspections or further documentation.Read more

Component/System: Engine and Cooling — Water Pump / Coolant System Vehicle: 2021 Audi SQ5 Engine Code: CWGD Mileage at Failure: 71,000 Date of Incident: 8/6/2025 Audi Denver I’m reporting a known defect affecting my 2021 Audi SQ5 with engine code CWGD. The engine experienced an internal coolant leak from the water pump housing, which is a known issue previously acknowledged by Audi in TSB #1
... 9Q5 2022 01, covering earlier 3.0T engines (DEWA/DEWC). Although my engine (CWGD) is not listed in the bulletin, the failure is identical in nature: the plastic thermostat/water pump housing fails internally, leading to coolant loss and potential engine overheating. This issue is widespread among Audi 3.0T engines, including CWGD, and poses a risk of engine failure and overheating. Audi has extended the warranty for earlier engine variants but refuses to cover mine, even though the defect and consequences are the same. This inconsistent coverage leaves owners like me exposed to high repair costs for a clearly known design flaw. I am requesting that NHTSA investigate whether Audi’s CWGD engines should also fall under the scope of the extended warranty or safety defect coverage already applied to DEWA/DEWC engine variants. Audi’s selective VIN or engine code exclusions appear arbitrary and may allow known safety-related failures to go unaddressed. On [XXX] Jim Ellis Kia of Kennesaw performed Recall SC284 on my 2013 Kia Soul (VIN: [XXX] ), replacing the HECU fuse. The next day, [XXX], the vehicle lost acceleration while driving, produced a muffled engine sound, and became barely able to move forward, creating an unsafe driving condition. At no point did the check engine light illuminate, even though the vehicle was experiencing severe driva
... bility issues. Prior to the recall, Kia’s own diagnostic (July 19, 2025) showed no catalytic converter or battery issues. An independent AAA diagnostic performed after the failure (July 21, 2025) ruled out pre-existing converter damage and linked the problem to the recall work. Despite this, the dealership denied responsibility and issued an unsupported estimate of $4,951.41 for catalytic converter replacement. The absence of a check engine light raises concerns that the recall work may have compromised the vehicle’s OBD-II diagnostic system, preventing detection of a safety defect. Furthermore, the evidence suggests that damage occurred to the catalytic converter while the vehicle was in Kia’s possession or as a direct result of the faulty installation of the recall fuse. This incident involves a safety risk, improper recall repair, and potential violation of federal recall law. I am requesting that NHTSA investigate the adequacy of Recall SC284 installation procedures and Kia’s handling of this case. I am reporting a safety concern related to the suspension system in my 2017 Tesla Model X. The front tires are rubbing against the wheel arch liner, especially when turning or driving over uneven terrain. I have not made any aftermarket modifications, and the vehicle has been maintained in accordance with Tesla’s recommendations. This issue appears to match the known problem involving front sus
... pension fore link failures documented in NHTSA investigations and prior complaints for 2016–2017 Model X vehicles. While I have not experienced a complete suspension failure, the current symptoms are consistent with reported cases where the fore links weaken or fail, causing the wheel to shift and contact the fender liner, leading to tire damage, unusual noises, and potential safety risks. I have reported this to Tesla, but they have not acknowledged this as a known issue for my VIN. However, NHTSA previously identified over 400 instances of similar failure and recommended that Tesla expand their service bulletin to include more vehicles. This is a critical safety concern as it can lead to premature tire wear, loss of control, and further suspension failure. I am requesting that NHTSA review whether Tesla should issue a recall or cover this repair under a broader service campaign.Read more

I am filing a complaint concerning serious safety risks associated with my 2020 Audi Q7 (VIN: [XXX] ) purchased as a Certified Pre-Owned vehicle from Audi Dominion San Antonio. It is currently under service at Audi Central Houston, where multiple concerning events occurred. Timeline and Issues: - Initial diagnosis identified a fire-damaged electrical harness, estimated at $1,400 for replacement. -
... After repair attempts, the vehicle began smoking, and the diagnosis expanded to include additional compromised electrical lines. The revised quote skyrocketed to $4,700 (later reduced to $3,100). - After nearly two months in service, I was informed that the AC compressor is now deemed "bad" due to debris from the harness, an issue not mentioned in previous inspections. Safety Concerns: 1. Inconsistent Diagnosis: There was no mention of compressor damage during initial assessments. This late-stage discovery indicates negligence or potential additional damage during servicing. 2. Risky AC Activation: A technician attempted to clean the compromised AC compressor and activated the system, a procedure that is dangerous given the debris. This could lead to refrigerant leaks and electrical shorts. 3. Conflicting Statements: Audi Central Houston claims the compressor was previously damaged but also attempted to restore it. Such contradictions raise serious concerns about service integrity. 4. Certified Pre-Owned Credibility: The emergence of these costly issues questions the reliability guarantees of vehicles sold under Audi's Certified Pre-Owned program. Given that the vehicle began smoking shortly after being parked and had not undergone prior electrical work, I underscore the severe safety risks posed by potential electrical fires and undisclosed damage. These issues could endanger lives and property if not properly addressed. To Whom It May Concern, Subject: Formal Complaint – 2011 Hyundai Sonata GLS Tail Light Meltdown / Fire Hazard Date: August 3, 2025 I am writing to formally report a serious safety issue with my 2011 Hyundai Sonata GLS, specifically concerning the tail light assembly, which has melted and poses a potential fire hazard and traffic safety risk. Approximately a year and a half ago, I noticed mel
... ting in the tail light housing and took the vehicle to a Hyundai dealership to address the issue. At that time, the car had approximately 208,000 miles. Despite the obvious manufacturing defect and safety implications, the dealership refused to repair or inspect the issue without charging a $275 inspection fee, stating the vehicle was out of warranty. I believe this to be an unfair and irresponsible business practice. Tail light assemblies should not melt under normal use, and such a defect could lead to: Electrical fires Brake light failure (increasing the risk of rear-end collisions) Potential traffic citations for inoperative brake lights Damage to other vehicles or individuals I have already notified Hyundai Motor Corporation about this issue, placing them on notice that should any damages or incidents occur as a result of this defect, they may be held liable. This is not a minor cosmetic issue but a major safety defect, and I strongly urge the NHTSA to investigate whether this is an isolated issue or part of a wider trend affecting Hyundai and Kia vehicles. If so, a safety recall may be warranted. I also intend to report this situation to the Indiana Attorney General's Office as a matter of consumer protection and product safety. My passenger and I, and the car behind us were put at serious risk of collision at ~ 85 mph, due to the completely unexpected and extremely rapid braking. This was under simple autopilot, no FSD or other driving assist. Nothing has been inspected yet, or reproduced past these 2 incidents. No warnings or symptoms occurred before or after this issue. Slightly terrified to use autopilot now, as if a semi-truck had been behind me, we would have been flattened. The vehicle experiences sudden and unexplained battery drain even after regular driving. The vehicle is available for inspection upon request.
... Safety Risk: This issue has caused my vehicle to fail to start on at least five separate occasions over the past month. I have been stranded in public places, including with my children, and now must carry a battery jump starter at all times. This is a safety concern, especially if the failure happens at night, in a remote location, during extreme weather, or in an emergency. The vehicle cannot be trusted to start reliably, putting me and others at risk. Confirmed by Dealer or Service Center: Not yet. I have not reported this specific battery issue to the dealership yet, as I have been tracking the frequency and pattern of failures before pursuing service. However, based on research, this appears to be a recurring issue with 2020 Nissan Armadas and may relate to an electrical or alternator defect. Inspected by Manufacturer, Police, or Insurance: No. The vehicle has not yet been inspected by Nissan corporate, law enforcement, or insurance. It has not been in a crash. Warning Lamps or Symptoms: There were no dashboard alerts, warning lights, or error messages before each battery failure. Absolutely. Here is the revised, anonymous version of your NHTSA complaint about the acceleration and incline issue—with all identifying info removed: ⸻ Component/System: Engine performance and drivetrain. My 2020 Nissan Armada experiences repeated loss of power when accelerating, especially on inclines or under load. The RPMs spike erratically, but the vehicle does not gain speed and often
... slows down unexpectedly. This condition has persisted despite a prior repair and continues to occur. The vehicle is available for inspection upon request. ⸻ Safety Risk: This issue places me and my children at serious risk while driving. The vehicle slows down when acceleration is needed most—such as when merging onto highways or climbing hills—creating a real danger of rear-end collisions or failed merges. The erratic RPM surges add instability and make it unsafe to rely on the vehicle in critical traffic situations like passing or freeway driving. ⸻ Confirmed by Dealer or Service Center: Yes. I brought the vehicle in for service in early 2025 after experiencing this issue. It was diagnosed with code P0025 and the exhaust valve timing control solenoid was replaced at a cost of over $1,000. However, the issue was not resolved and continues exactly as before. I returned for follow-up and was told an engine replacement might be needed, without receiving documentation or a clear explanation. ⸻ Inspected by Manufacturer, Police, or Insurance: No. The vehicle has not been inspected by Nissan corporate, police, or insurance. It has not resulted in a crash, but it remains an ongoing safety concern. ⸻ Warning Lamps or Symptoms: There were no consistent dashboard warnings. The issue began in early 2025 and occurs frequently—particularly during acceleration on hills or when the vehicle is under load. The RPMs surge, but the vehicle slows or struggles to maintain speed.Read more

2017,Kia soul plus brakes failed with no warning and we crashed into an intersection and hit a dump truck. I suffered severe broken bones including a compound femur fracture and broken arm. I was trapped in the car for over a hour. Both my arm and leg have rods and pins now. I also suffer from severe ptsd and nightmares. I hospitalized from 6/25-7/4, and in a nursing home until 7/4-8/1, learning
... to walk again. I received the voluntary recall letter on 7/11when I was in the nursing home. It was sent too little too late. I’m contacting an attorney and Kia deserves to pay. Kozlowski towing took the care away. I do not know if they still have itRead more

On May 15, 2025, at around 11:30 AM, my Tesla was involved in a collision while Full Self-Driving (FSD) was engaged. The incident occurred at the Tesla West Covina Dealership. The vehicle struck a barrier wall without any collision warning, suggesting both a failure of the sensors and FSD system. After the incident, Tesla staff directed me to their West Covina Service Center. A service advisor re
... viewed the dashcam footage with me and acknowledged the crash appeared to result from FSD malfunction. I was told Tesla will cover the repair. However, when I returned on May 26, 2025, I was informed that the dashcam footage was no longer available. I did not delete it and suspect it may have been removed by Tesla personnel. They also then claimed they will not cover any repairs. I have written to their legal department to formally request the video footage and any other data they have regarding the crash. Despite follow-up, Tesla has not responded or released any data. I am reporting this as a possible safety defect and mishandling of evidence. I request NHTSA investigate: The failure of Tesla’s FSD and collision avoidance system. The disappearance of crash footage after review by Tesla staff. Tesla’s lack of response to requests for data and documentation. Thank you for your attention.Read more
