1. The car's steering could be malfunctioning . The car is available for inspection upon request. 2. The steering wheel occasionally and without warning suddenly FEELS loose and "disconnected" from its ability to steer the car -- until I turn the steering wheel about a quarter to a half an inch and the steering FEELS normal again. I worry the issue (which so far has not interfered with steering th
... e car) may worsen and possibly become hazardous. 3. I took the car to my regular licensed mechanic who did not experience the issue when he drove it. Nevertheless, the issue reported here exists as my wife and I have each experienced it. 4. The steering has not been inspected by Subaru, the police, insurance reps or others, other than my regular mechanic who has found no issues with the steering. 5. There were no warning lamps or messages prior to the issue first occurring except for the replacement of a tie rod by my regular mechanic, which may be coincidental. 6. This issue has existed since at least last February with no worsening of the condition. My regular mechanic has assured me there is no danger as he has checked the mechanical aspects of the steering (linkages, tie rods, etc.).Read more

The vehicle was taken to a dealer, but it was not diagnosed due the manufacturer only allowing certain vehicle to be repaired at this time, even with the failures that had transpired, and no repairs were made at that time. The contact stated that while driving home in pitch darkness at approximately 30 MPH on a windy road, the vehicle stalled. While attempting to steer the vehicle off the road, it remained halfway in the street and nearly caused over 15 accidents. One near-accident involved a motorcyclist who came around a turn, narrowly missed the vehicle, fell into the street, and was almost hit by oncoming traffic. No police reports were filed as the police could not assist at that time. The contact and her daughter did not sustain injuries but were severely traumatized. It was uncertain if the motorcyclist sustained injuries. The contact stated that when the vehicle was being towed, it would not go into neutral, and the contact observed the frame cracking while being hauled on the tow truck. The manufacturer was notified of the failure but did not provide assistance. The failure mileage was approximately 18,000.Read more

I was driving down the highway, and my wife and I decided to stop at Costco to get protein bars and gas for the drive home. We also had a [XXX] , [XXX] old, and [XXX] in the car. As we were approaching the end of the exit ramp at approximately 30 miles per hour, the engine jerked the car two or three times then died. When it died, I lost power steering and the car slammed to a stop. The car also

I stopped trying to restart, and got my family onto the hill beside the exit while waiting for a tow. The tech at the GMC dealer stated that the engine had seized on me while driving. The cam shaft can rotate a small amount one way before getting blocked, and likewise the other way. Tech believes that the bearing is responsible, but will not be doing a tear down until September 2. When turning my car on, I received warning lights for the ABS, Stability Track, and Traction Control systems. These stated they had all been deactivated. The Check Engine light also illuminated. I was in a safe, open lot location so I put my truck in drive to confirm this was not a dash/technical issue. The brakes were in fact almost completely deactivated. It took me over 100 feet to come to a st
... op. The brake pedal had slightly retracted also causing more issue in trying to reach the brake pedal. Once stopped, I checked the VIN and discovered this recall for the vehicle. The major complaint comes from the recall repair process with Ram/MOPAR. I took the truck to my nearest Ram dealership. Once in, they advised I needed two sensors replaced in the front wheel-well per the recall guidelines. The service department of the dealership further stated they could only order ONE sensor per month per dealership. Not per vehicle, per person, etc. They could as a dealership only order one sensor a month, meaning nobody else could come in for this recall repair while my FIRST sensor was being replaced. Now I am stuck waiting over a month for the second sensor to be ordered. They refused to give me a courtesy vehicle to drive for the time being so I am forced to drive my Ram with this recall issue with a major safety concern. I contacted Ram/MOPAR about this to find out if the dealership was correct in the part ordering statement and they could not give me a straight answer. Considering the brakes are about the most critical piece of safety equipment in a vehicle, I find this by Ram completely unacceptable and opening the door for potential bad things down the road.Read more

Immediately after an over-the-air (OTA) update on Aug 8, 2025, my vehicle began showing camera connection errors and disabled multiple safety/ADAS features. Over the following days, I repeatedly received alerts that AEB was unavailable. The vehicle remained drivable, but AEB and related safety functions were inactive. Chronology: Aug 8, 2025: OTA software update to 2025.26.6 completed. Right aft
... er completion, the touchscreen displayed camera connection errors and disabled AEB / Autopilot / cruise control. Aug 11, 2025: While driving, I again received an “AEB unavailable” alert. Similar alerts appeared intermittently throughout the week. There was no water intrusion, impact, or external damage. Cameras were clean and unobstructed. I scheduled the earliest available Tesla Service appointment and reported the AEB failure as a safety issue. Service diagnosis: Tesla diagnosed a failure of the Autopilot computer (HW3) and ordered a replacement. Cost/coverage: I requested that the repair be covered by Tesla because the failure started immediately after the OTA update and I am a paid Full Self-Driving (FSD) purchaser. My request was denied; I was charged ~$1,600 (slightly discounted) for the computer replacement. Safety risk description: The failure disabled AEB and other forward-collision mitigation features without an external cause, beginning minutes after the OTA update. I consider this a safety defect because an advertised crash-mitigation system became unavailable during normal use, increasing crash risk. The timing suggests a possible software–hardware interaction (software change triggering or revealing a hardware fault) that could affect other vehicles.Read more

... hich is a critical component for engine timing. The distributor itself can fail for a number of reasons, including: Internal bearing failure: The bearing that the distributor shaft spins on can fail, causing the shaft to wobble or seize up. This disrupts the signal from the camshaft position sensor. Failed or failing camshaft position sensor: The sensor itself can fail due to age, heat, or internal electrical issues. Ignition coil failure: The ignition coil is often integrated into the distributor housing on this engine. As it heats up, it can fail and lose spark, causing the engine to stall. The reason this issue is often related to hot weather or a hot engine is that the heat can cause the internal components of the distributor to expand and fail, and it can also cause the ignition coil to break down more easily. Symptoms of Distributor Failure The symptoms for a failing distributor on the 2.4L are very similar to those of the V6 engine: Stalling, especially when hot: This is the most common symptom. The engine may run perfectly when cold, but after it has been running for a while and the engine compartment is hot, it may stall and refuse to restart until it cools down. Engine runs poorly: You may experience a rough idle, misfires, or a loss of power. Hard start or no start: A failing distributor can cause the engine to crank but not start, as it is unable to produce a spark." I initially brought my car in for a weird clicking noise. This noise went away after the car was towed to the Service Center. During service at Tesla Service Center in LA on August 04, 2025, technicians identified structural deformation of the left-hand rear subframe toe link mounting side wall. This deformation prevents proper alignment of the suspension bolt and makes a 4-wheel alignment impossi
... ble. Tesla refused warranty coverage, claiming “impact damage,” citing only cosmetic wheel rim scrape as evidence. However: 1. Torque paint marks remain intact, proving the bolt has not shifted since factory assembly. 2. No collateral suspension damage (control arms, bushings, underbody) was found, which would normally occur if an impact strong enough to bend a subframe ear had happened. 3. This failure mode is nearly identical to Tesla’s prior recall (21V-835 / 23V-235) on the front suspension lateral link bolts, which involved subframe mounting holes deforming and preventing proper alignment. Safety Concern: The subframe is a critical structural component of the suspension system. Deformation at the toe link mounting point risks the following issues: improper suspension geometry, loss of wheel alignment, reduced vehicle stability and tire wear, and potential sudden loss of control in emergency maneuvers. Tesla has declined to repair this under warranty, leaving the defect unresolved. I believe this represents a potential systemic defect in the rear subframe design, similar to the previously acknowledged front subframe defect.Read more

Summary of Issue: The vehicle has experienced intermittent drive disconnections without warning. This occurred multiple times on August 9–10, 2025, during trips between Palo Alto and Monterey. The vehicle is available for inspection upon request. What component or system failed or malfunctioned, and is it available for inspection? The suspected component is the drive unit (covered under Tesla’
... s 8-year Drive Unit & Battery warranty). The vehicle and its systems remain available for inspection by NHTSA or Tesla. How was safety put at risk? The intermittent drive disconnections can result in sudden loss of propulsion without warning while the car is in motion. If this occurs at highway speeds, it creates a serious safety risk of collision or loss of vehicle control. Has the problem been reproduced or confirmed by a dealer or service center? Tesla Service has attempted to inspect the vehicle but reported “no fault found.” Because the issue is intermittent, Tesla did not reproduce it during their inspection, though I documented it on August 9–10, 2025. Has the vehicle/component been inspected by the manufacturer or others? Yes, the Tesla Service Center inspected the vehicle 25 August 2025. However, Tesla has refused to provide a detailed record of what diagnostic steps were actually performed (log review, physical inspection, or drive unit stress testing). Were there any warning lamps, messages, or other symptoms? No warning lamps or messages appeared at the time of the failures. The vehicle resumed propulsion after several seconds, clearing the condition without leaving an active customer-facing alert. Concern: Because the failure is intermittent and leaves no persistent alerts, Tesla has returned the vehicle without resolving the problem or providing adequate documentation. This leaves a potential safety-critical drivetrain failure unaddressed. Requested Action: I request that NHTSA investigate potential drive unit intermittent disconnection failures in Tesla Model YRead more

While merging onto a highway, my 2020 Hyundai Palisade abruptly lost all engine power and stalled. This immediate and complete failure occurred at a critical moment, posing an extreme safety risk. All dashboard warning lights illuminated, but provided no specific diagnosis. The vehicle was towed to a dealership, where technicians determined the engine was consuming oil at an abnormal rate, despit
... e a lack of visible leaks. I was instructed to participate in an "oil consumption test," a three-part process that required multiple oil changes and inspections over several months. Each time, the vehicle had burned over a quart of oil—a clear indication of a defect. Despite this evidence, Hyundai denied my warranty claim, accusing me of negligence for being unable to produce oil change records between 29,600 and 46,900 miles. This is a common and unacceptable tactic used to avoid accountability for a widespread issue. My experience is not isolated. Hundreds, if not thousands, of owners of 2020-2023 Hyundai models, including the Palisade, report identical engine oil over-consumption issues and subsequent power loss. This issue is a **severe safety hazard** that can lead to sudden, catastrophic engine failure and potential accidents. Hyundai's refusal to acknowledge this defect and issue a recall is **gross negligence**. They are knowingly allowing thousands of unsafe vehicles to remain on the road. There are currently federal lawsuits against Hyundai for this very issue. The transportation authority must force Hyundai to issue an immediate recall and remedy this critical defect before a tragedy occurs.Read more

Emergency braking saves lives at speed. However this feature is absolutely outrageously unwarranted when I'm trying to back into a parking space at 0.05 MPH and the car slams on the emergency braking system because I'm 12 inches away from a stationary unoccupied abandoned shopping cart. My only remaining option is to change parking spaces to one I can pull into, or leave the front car bumper sticking out a good 12-14 into the lane of travel risking potential damage to the car. This has been occurring since the car was purchased and has been growing in annoyance over the years each time I try to back up to park. Let's find a way to permanently disable this feature when speeds are under half a mile per hour. I'm already using the backup camera and other visual aids to ensure I don't hit anything, the last thing I need is the sudden grinding jerk to a stop to break my focus and concentration on completing a task.Read more

I believe this is unsafe because I lose visibility of all driving information and access to basic controls. Tesla now refuses to repair these ongoing problems. After I raised complaints about the severity of the issue, I was dismissed and mocked for my speech disability during phone calls, and Tesla placed me on a service restriction. As a result, I am unable to obtain further repairs for this safety defect. I am afraid to drive the car. I am filing this urgent complaint regarding a life-threatening defect in my 2022 Toyota RAV4 SUV that gravely endangers my family and the public. This is a desperate plea for intervention. For over a year, my vehicle has repeatedly displayed a terrifying, unreadable red alert and piercing buzzer, occurring 3-4 times without warning and not real collision threat. These incidents are terrifying pre
... cursors to catastrophic failure. The Toyota dealership dismissed my concerns, stating they "cannot do anything" without replicating the intermittent problem. This dangerous negligence compounds my frustration, especially after prior unaddressed issues like radio malfunctions and door water infiltration. Crucially, the dealer's own CAR DTC logs confirmed C14FE14: "steering angle sensor supply voltage circuit short to ground or open." This documented electrical failure directly impacts the steering system. Toyota's own webpage [XXX] ) also details similar warnings, confirming my fear of brake system activation. My paramount fear is a false Pre-Collision System activation. Imagine driving at high speed when the brakes violently engage or steering falters due to this electrical short. The unthinkable consequences could be a devastating collision, causing severe injury or death to my family and others. This is a documented defect, met with manufacturer and dealer refusal to act. I cannot feel secure driving this car, nor can I responsibly sell this ticking time bomb to another family. I implore the NHTSA to launch an immediate, comprehensive investigation into this critical safety defect. This is a public safety matter demanding urgent attention.
I was in a parking lot and activated Autopilot while the car was next to a stationary pillar near a parking space. Immediately after activation, the vehicle failed to detect the pillar and drove directly into it. The impact happened so quickly that there was no practical opportunity for human intervention to avoid the collision. There may or may not have been a warning, but even if a warning was
... displayed, it occurred too late to allow the driver to react before the impact. The system did not provide effective parking collision warning or apply automatic emergency braking. This incident caused damage to the driver-side door and mirror. My safety and the safety of others were at risk because the system failed to prevent a collision with a fixed obstacle. No inspection has yet been performed by Tesla service.Read more

I had gotten a new battery because one point after work, my car would not start. I then got installed a brand new battery, and it still wouldn’t turn on. The only things that turned on were the dashboard, but no noise coming from the front engine compartment. I tried turning it on after, but it wouldn’t start at all. I then got it towed to an auto shop and they installed a new starter. The new
... starter worked for a month before another incident in a Walmart parking lot where it would not start after turning the key multiple times. After a few times it started and I drove it again to the same auto shop. The auto shop had no trouble starting it, they turned it on and off several times without any fault. That was in July of 2025. Fast forward a few weeks later, there was two instances in which the car did the same thing, the engine was not turning on only the front dashboard lights. After the first try it turned on. Now on 8/22/2025 as I was driving on the highway going 75mph the car stopped decelerating. Pressing the gas pedal did nothing. It stopped completely, thankfully I was able to stop on the side of the highway but it was still a very dangerous situation had it been another time during the day. This time, the engine compartment, once the key was turned was simply making a noise but not starting or “turning over”. I lifted up the hood and noticed that when the key was inserted the engine compartment was shaking. I got it towed to the same auto shop awaiting the results. I looked up the symptoms for a faulty fuel state and my car is showing to have a lot of the same as it would be that.Read more

I was driving on the highway at 55 mph and my truck downshifted to fist gear and the speedometer went to zero with the wrench light coming on. The system that malfunctioned was the molded lead frame for the transmission that was a customer satisfaction program with Ford that I was not notified about. My safety was at risk because I was traveling at a high rate of speed before my truck downshifted
... into first gear. My problem has been confirmed by my local ford dealer and repaired at my own cost. The vehicle has been inspected by my local Ford dealer. No other symptoms prior to the failure.Read more

My CVT Transmission started having problems with the Valve Body inside the transmission. On acceleration from a complete stop my vehicle would start "bucking" hard causing a violent back and forth rocking or "jerking" of the drive train and vehicle. I did not request a inspection of the failed Valve Body when I paid to have it replaced. The life and safety of myself and others are at severe risk o
... f injury or death resulting from a complete failure of the Valve Body component when driving on highways or other roads because without any warning the vehicle will come to a complete stop which would cause a rear end collision possibly at highway speeds. Also I'm concerned about the Torque Converter having a failure because of the intense pressure from the failed Valve Body with the constant "bucking" on acceleration. I took my vehicle to both a independent transmission shop and a Subaru Service Center and the Independent Transmission Shop told me I needed a new transmission and the Subaru Service Department kept telling me they didn't know what was causing my vehicle symptoms. Subaru of America extended the warranty on my vehicle to 100,000 miles because of the problems with their CVT Transmissions but because I bought my vehicle used with over 100,000 miles on it they refused to either pay for 50% or 100% of the replacement cost. No warning Lights came on but when the Valve Body completely fails then the warning lights will illuminate. My Subaru Case Number that was DENIED is (Case# 250820-1801058) for Subaru extended warranty for CVT Transmission.Read more

On the 23-25MY Subaru Solterra there is piano black reflective material plastics on the center console. It is angled slightly and when the sun is shinning through the windshield from directly above the car, such as noon/lunch time. The sun reflects directly into the drivers eyes, the plastic area is large and needs to be covered by something while you are driving or else you need to keep moving
... your head or body around. Covering it quickly with your hand is an option if you like to drive with one hand and distracted by trying to cover the glare. This would be easily duplicated if sunny and time of day is right. Multiple other Solterra owners noted this issue as well, the 26MY Solterra is redesigned and fixed this issue by changing the plastic and interior design.Read more

After installing the most recent Tesla software update, my vehicle immediately displayed multiple alerts: traction control disabled, stability control disabled, and reduced steering assist. Cameras were disabled, navigation was disabled and my vehicle location was wrong. The vehicle lost core safety systems that are critical to safe driving The car functioned normally before the update. Immediate
... ly afterward, these safety systems were disabled. Tesla Service has since diagnosed and confirmed, They state “an internal short within the drive assistance module (computer). The internal short is causing excessive current draw, resulting in loss of driver assistance features. The latest software has revealed the underlying root issue within the car computer.” This indicates that the vehicle was operating with a latent defect that Tesla’s diagnostic update exposed. The update did not cause wear-and-tear failure; it surfaced a hidden safety defect inside Tesla’s own hardware. Why This Is a Safety Concern Loss of traction control, stability control, and steering assist creates a significant risk of loss of vehicle control. If this had occurred while driving at speed, the sudden loss of these systems could have led to an accident. Attempting a reboot while driving (Tesla’s standard troubleshooting suggestion) would have created further distraction and risk. luckily when the car catastrophically failed and was unable to turn, I was in my driveway and not on a highway or any other roads.Read more

Engine began jumping during driving and loss power. No dashboard illumination warning lights and the temperature gage showed normal operating temperature. Car was not accelerating properly. Had to maneuver carefully to prevent being ran over. Engine light illuminated showing misfire. Replaced ignition coil pack and spark plugs. Driving again, car stalled. No dashboard warning lights. No high tem
... perature on gauge. Narrowly missed being hit by a semi as there was absolutely no warning and we were on an Interstate bypass ramp with nowhere to pull over!! Towed car to dealership for diagnosis. Results: engine blown, turbos gone, and the catalytic converter is holding oil. Dealer is stating this is not covered under the extended warranty from Kia as its not connector rod bearing issues and that the piston rings failed. However, they stated the turbos got "too hot" and they failed. The gauges have never shown that the car was running hot. I have a sworn statement from my mechanic stating this is absurd and a valiant attempt to not pay for what would be the second warranty replacement of my engine. The piston rings failed as a result of the connector rod bearing failure. The "too hot" temperature that damaged the turbos was likely caused by the same bearing issue. The heat from the bearings unseated the piston rings and allowed the engine to dump its oil causing the engine to fail. I highly recommend that you open an investigation into these issues. The car not accelerating and losing power at interstate speeds is a very serious and dangerous issue.Read more
