On I95 by the Secaucus Train station there is something sending a signal to my Tesla and other Teslas to disable all of its ADAS systems. All of the functionality related to ADAS gets disabled thus making the car very unsafe to drive. Due to this disability it also disables the camera systems in the car. So if an accident were to occur there would be no way to record.

The contact owns a 2026 Tesla Model Y. The contact stated that while driving manually over 40 MPH, the Lane Departure Avoidance system jerked the steering wheel abruptly to the other side while the vehicle was still inside the desired lane. The failure was a recurring issue. The dealer was contacted and informed of the option to turn off the feature. However, each time the vehicle was restarted, t

My 2018 Tesla Model 3 displayed alert RCM_a056, “Front Passenger Safety Restraint System Fault.” The warning may affect correct detection of a passenger and operation of the passenger restraint/airbag system. Tesla quoted approximately $705 but has not identified the failed component or explained whether the condition is related to known front-seat wiring or occupant-classification failures.
... Tesla Repair Bulletin SB-21-17-005 recognizes that electrical harnesses in 2017–2019 Model 3 front seats can be damaged through repeated seat movement and trigger restraint-system alerts, although that bulletin specifically lists codes RCM_a021 and RCM_a022. I am reporting RCM_a056 because it affects the same general front-passenger safety-restraint system and appears to be reported by other early Model 3 owners. I am concerned that an occupant may not be properly classified or protected in a collision.Read more

I am submitting this complaint regarding two serious safety-related failures with my Tesla Cybertruck: a braking-system malfunction and a rear suspension/subframe failure at approximately 12,500 miles. The vehicle previously experienced a significant braking problem that required repair by an authorized Tesla Service Center. The initial repair did not properly resolve the issue, requiring the veh
... icle to return for additional service. Because the problem involved the braking system, I was concerned about the vehicle's ability to stop safely and predictably, creating a risk of a serious collision. Tesla's service records should document the diagnosis, repairs, and any related fault codes. At approximately 12,500 miles, the vehicle experienced a major failure involving the rear suspension/subframe. The rear suspension separated/dropped in relation to the subframe, causing significant damage. Tesla inspected the vehicle and performed extensive repairs involving the rear subframe, including component/part number 1250562-00-E. This failure created a serious safety risk because the rear suspension/subframe affects vehicle stability, wheel positioning, handling, and control. If this failure had occurred while traveling at highway speed, it could have caused loss of control, a serious collision, injury, or death. Due to the condition of the vehicle, I had it towed rather than driving it. To my knowledge, there was no meaningful advance warning that a major suspension/subframe failure was about to occur. Tesla should have diagnostic logs, photographs, technician findings, repair records, and information concerning the failed components. I also have photographs and service documentation. Having experienced both a braking-system problem and a major suspension/subframe failure at low mileage has caused serious concerns regarding the safety of this vehicle. I am requesting that NHTSA investigate whether similar failures have occurred in other Cybertrucks.Read more

While using Tesla's Actually Smart Summon feature via the phone app in a hospital parking garage, my unoccupied 2017 Model S initiated an unexpected turning maneuver and struck a clearly visible parked SUV (a 2019 Toyota RAV4). I was within line of sight and actively supervising per Tesla's instructions, but the vehicle turned into the parked car without any meaningful opportunity to intervene bef
... ore impact. The failure appears to involve the Summon system's object detection, path planning, or control logic. The incident occurred at low speed. Both vehicles sustained property damage; there were no injuries. The vehicle's own cameras captured the entire incident; full video is available on request and still frames are attached. I reported the incident to my local Tesla service center the same day; no investigation was performed. On 3/13/2026 I formally requested all data associated with my vehicle for the day of the incident, specifically including moment-by-moment vehicle system data: Summon session logs and command history, telemetry and event data recorder data, sensor and object detection outputs, and path planning/control decision data. Tesla provided only basic account-level data and did NOT provide any of the requested vehicle-level event data. A formal notice of defect and evidence preservation demand sent to Tesla Legal on 5/2/2026 received only automated replies. Although I have contacted Tesla many times by phone and email since the incident (first written claim 3/30/2026 went unanswered), Tesla has refused to follow up: apart from two brief emails from a corporate case representative on 5/4/2026 and 5/11/2026, my litigation hold and data preservation requests and all subsequent emails and calls have received no response. The vehicle remains unrepaired and available for inspection. My insurer has paid the other party's property damage claim.Read more

Incident / Safety Defect Summary: A certified Tesla mobile service technician improperly calibrated the low-beam headlights on my 2018 Tesla Model 3, causing a severe reduction in forward night visibility and creating an immediate roadway crash hazard in direct violation of FMVSS 108 illumination standards. Detailed Description of the Safety Failure: Improper Calibration Environment: The technicia
... n attempted to perform the headlight alignment procedure inside a closed residential garage with approximately 12 inches of clearance between the front bumper and the garage door. Standard automotive and manufacturer procedures require either a level 25-foot standoff from an aiming target or a dedicated optical aiming tool. At 12 inches, optical beam geometry and cutoff steps cannot be evaluated. Extreme Downward Misadjustment: The technician bypassed standard cutoff alignment and manually adjusted both headlights down to their absolute lowest mechanical limit using the vehicle interface. Safety Impact: The usable beam throw is now restricted to only a few feet in front of the vehicle's bumper. At standard driving speeds, especially during early morning and nighttime commutes, the vehicle completely overdrives the low beams, rendering road hazards, lane markings, and pedestrians invisible until impact is imminent. Manufacturer Failure to Triage Safety Rework: When reported to Tesla Service as an urgent safety-critical failure caused directly by their technician, the service center refused immediate safety triage/rework and scheduled resolution days out, leaving an unsafe vehicle in service.Read more

My Tesla Model 3 has a persistent RCM_a056 alert indicating a problem with the front passenger occupancy detection system. The unresolved fault affects the operation of the front passenger airbag system. I paid Tesla approximately $1,000 for a repair intended to resolve this problem. Tesla replaced multiple components associated with the passenger seat/occupancy detection system, including the se

I returned the vehicle to Tesla for further diagnosis. Tesla tested the individual components and documented that they found “no issues present,” but the RCM_a056 alert remained. Tesla then recommended replacing the entire passenger seat assembly at a cost of approximately $3,000. When I asked what diagnostic evidence indicated that the seat assembly was defective, I was told that they had no specific evidence that replacing the seat would resolve the problem. They were unable to identify a failed component and indicated that replacing the entire seat was the only remaining repair they could suggest. I am concerned that this represents an unresolved safety-system defect. I have already paid approximately $1,000 for a repair that did not resolve the fault, and Tesla is now recommending another $3,000 repair without identifying the cause of the failure. The vehicle remains unavailable for normal use because of the unresolved passenger restraint/airbag-system fault. I am requesting that NHTSA document and investigate this potential safety defect, particularly if other Tesla Model 3 vehicles are experiencing the same persistent occupancy-detection failure.Read more

My 2022 Tesla Model 3 has experienced recurring intermittent loss/reduction of power steering assistance. The vehicle displays “Steering assist reduced – Steering may require increased effort.” The problem has occurred multiple times over a period of months and may disappear after the vehicle is power cycled or the alert clears. Tesla previously evaluated the vehicle for this condition. On
... July 8, 2026, Tesla documented the steering-assist-reduced complaint and recommended replacement of the RWD steering rack/steering gear, with an estimated repair cost of approximately $3,018. On August 20, 2026, the warning occurred again. Tesla Service Mode recorded fault EPAS3S_a005_sent2Mia, described as “Torque Sensor 2 signal not received,” with the potential impact listed as “Steering assist reduced.” The vehicle simultaneously generated UI_a020_SteeringReduced. The warning subsequently cleared again, but this is a recurring intermittent failure involving the vehicle’s electronic power-assisted steering system. I am concerned that the steering assistance could become reduced or unavailable while driving. Tesla has already determined that replacement of the steering rack is required, but the repair is customer-pay. I am reporting this because this appears to be a recurring safety-related steering defect and I am concerned that other vehicles may experience the same failure.Read more

In May 2025, my 2021 Tesla Model Y, with approximately 50,600 miles, developed a squeaking/groaning noise from the front suspension, especially while turning. I brought the vehicle to a Tesla Service Center because of the noise. Tesla inspected and road-tested the vehicle and confirmed the squeaking/groaning from the front suspension. According to the Tesla service invoice, the noise was caused b

Tesla also replaced the associated lateral-link bolts/hardware and performed a four-wheel alignment. There were no warning lights or dashboard messages that I recall. The suspension noise was the symptom that caused me to seek service. The problem was confirmed by Tesla during its inspection and road test. I paid for the repair myself. I am reporting this because I recently learned that NHTSA is investigating front lower lateral-link failures in 2021–2023 Tesla Model Y vehicles under Preliminary Evaluation PE26006. My vehicle is within the model years under investigation, and Tesla replaced both of my front lower lateral links after confirming my front suspension symptoms. I do not know whether my lateral links would have eventually detached if they had not been replaced. The original components were removed by Tesla during the repair and are not available for inspection.Read more

Previously, while driving from [XXX] to [XXX], the car jumped across freeway lanes unsafely. I reported it via the Tesla app and to Cathedral City Service. Technicians lacked diagnostic equipment, said they would escalate to the FSD team for research, and promised to realign the front cameras on a return visit. Months later: no follow-up and cameras still cosmetically misaligned. Camera Calibration menu only shows Clear option (I did not clear it). I have submitted a Service request and emailed Tesla requesting internal logs/telemetry and full escalation. Filing this for an independent record. This is what happened: While Full Self-Driving (FSD) was engaged, the vehicle approached an intersection and appeared unable to correctly follow the roadway. Instead of following the intended road path, the vehicle continued forward, drove over the curb, and struck a road sign. The impact knocked the sign down, and the sign subsequently struck the vehicle, including the roof glass. The roof glass
... shattered and small pieces of glass entered the cabin, exposing both the driver and front passenger to a safety risk. Vehicle data obtained after the incident shows that the vehicle was in ACTIVE_FSD leading up to the collision. FSD changed from ACTIVE to UNAVAILABLE approximately 1.5 seconds before the crash algorithm wake-up. Significant braking occurred only very close to the collision, and no Automatic Emergency Braking intervention was recorded during the incident window. The data also shows multiple substantial steering movements shortly before the collision. Accelerator-pedal data does not establish driver acceleration as the cause of the collision. The available samples show an accelerator position of 2.0% shortly before the collision and 0.0% in the next recorded sample. The driver applied the brake shortly before impact. There were no prior warning lamps or messages indicating that FSD was about to become unavailable before the vehicle departed the roadway. The vehicle has been inspected/repaired following the collision, and vehicle data relating to the incident has been obtained and preserved. Police responded to the collision, and the incident was also reported to insurance. A police report was filed, and the case number is 2026-07160174. I have not yet received the formal CR-4 crash report. Following the collision, I experienced significant psychological distress and trauma-related stress symptoms as a result of the incident.Read more

SAFETY DEFECT REPORT: UNMAPPED RECALL EXPANSION / COMPONENT COMMONALITY My 2021 Tesla Model Y experienced a sudden, dangerous loss of electronic power-assisted steering (EPAS) while driving, instantly displaying the alert: "Steering assist reduced / Steering may require increased effort."An official diagnosis by Tesla Service (Estimate #3000-009-0038060635, attached) confirmed a catastrophic fai
... lure of the steering gear assembly, requiring a full replacement of the unit. The specific component listed for replacement is STEERING GEAR - LEFT HAND DRIVE, Part Number: 1188832-00-B. This is the identical engineering architecture and part family (1188832) targeted in NHTSA Safety Recall 25V092 for an overvoltage breakdown of the internal EPAS printed circuit board. While my vehicle falls outside the current 2023 model year production window defined by Tesla for 25V092, it uses the exact same component architecture and suffered the exact same failure mode. This represents a clear pattern of failure and component commonality expanding into earlier model years (2021). I am submitting my official itemized Tesla repair estimate of $3,072.50 as physical documentation. I request that the NHTSA review this component match and investigate an expansion of Recall 25V092 to protect other owners of earlier model years from this sudden loss of steering control.Read more

My 2019 Tesla Model X has Full Self Driving (FSD) software version 12.6.4, which is the latest version provided by Tesla for my vehicle. While driving with FSD engaged, the car became confused by the yellow centerline road markings and possibly lines of black tar road repair and swerved across the centerline and into oncoming traffic. I had to take manual control over from the FSD software to swer

There are newer versions of the Full Self Driving software that Tesla is providing to newer Tesla vehicles but not to my older Tesla with AI Hardware 3. I believe the NHTSA needs to investigate Tesla's faulty FSD software.Read more

I am reporting multiple safety‑critical incidents involving my Tesla Model Y equipped with FSD (Supervised). The vehicle has repeatedly performed sudden braking in active traffic without any obstacles present, causing physical jolting and creating a risk of collision. These incidents occurred on [XXX] and XXX including a sudden hard brake at approximately 2:18 p.m. on XXX. Additionally, the veh
... icle has shown lane‑centering instability on the highway. On XXX at approximately 1:51 p.m., the vehicle drifted within the lane and failed to maintain proper centering while FSD was engaged, requiring immediate manual intervention to avoid unsafe movement. Tesla service has stated that no error codes were detected, but also acknowledged known FSD software behaviors affecting Hardware 3 vehicles. This indicates the system may be performing unsafe maneuvers without logging faults. I am reporting these incidents because they pose a direct safety risk to me, other drivers, and pedestrians. I request that NHTSA review these behaviors as potential safety defects in Tesla’s FSD (Supervised) system.
