In January 2021, I purchased a new 2021 GMC Yukon Denali from a GMC dealership, which currently has only about 27,000 miles. To date, several recalls have been assigned and completed for my vehicle. per the websites for GMC and your agency. However, based on published recalls for my vehicle and a repair that is currently underway, I have reason to believe that my vehicle should have been subject t
... o three prior recalls for which it was not [NHTSA ID Numbers: 20V735000 (Front Seat Bolts), 25V274000 (V8 Engine Failure, and 21V739000 (Fuel Power Control Module] Currently, the fuel pump control module is being replaced at a local GMC dealership because it failed last week and left my stranded at home. GMC issued a recall for this in 2021, but my vehicle was not included. Aside from risk and inconvenience, this is also costing my over $300 for a rental. Secondly, though my vehicle's make/model/year is cited in GMC's ongoing engine failure recall, which you are investigating, I have not received a recall notice. I note GMC's financial interest in minimizing vehicle's it recalls for such a costly repair. I have inquired with GMC and two of its dealers and have only received reiterations that my vehicle has not been subject to GMC's recall, which I already knew. I have not been given any explanation for why my Yukon has not been recalled despite my inquiries. Lastly, GMC issued a 2021 recall for front seat bolts; however, I did not receive a notice for my Yukon. I have occasionally felt slight movement in the driver's seat since buying the vehicle. Based on reason and facts, I am alleging that GMC appears to be artificially minimizing recalls and notifications for vehicles that are in fact impacted by defects in their vehicles.Read more

[XXX] , my [XXX] son, was strapped in his car seat, wide-eyed and confused, as we sat trapped in our Tesla for thirty agonizing minutes. The battery had died without warning, leaving us locked inside our own garage. No power, no way to open the doors. I tried to keep [XXX] calm, singing his favorite songs, but my heart raced as I called for help. A tow truck finally arrived and jumped the car—
... not enough juice to start the engine, but just enough to unlock the doors. Freedom at last. Tesla’s response? No roadside service; they insisted the car be towed to their service center. Easier said than done—the tow truck couldn’t maneuver it out of the garage. Frustration mounted as I realized I’d have to miss work. Days later, I drove to the service center, where they replaced the battery. But the ordeal wasn’t over. The trim was damaged during the chaos, and now every drive is accompanied by relentless wind noise, a constant reminder of that day. [XXX] still talks about being “stuck in the car,” and I can’t help but feel let down by a system that left us stranded. Summary of Complaint: My 2023 Tesla Model Y has had two roof glass failures in three months (two glass roof replacements). Both times, the glass cracked from small road debris while driving under normal city conditions. Safety Concern The roof glass appears unusually fragile and prone to cracking, raising concerns about whether it meets industry standards for impact resistance and occupant safet
... y. The rapid failure rate (two roof glass panels in three months) suggests a potential design or materials defect rather than isolated incidents of road damage. If roof glass panels cannot withstand normal road debris, this presents a safety risk for drivers and passengers, as well as for other motorists if large shards detach while driving. The roof glass seems unusually fragile. Two failures in such a short time suggest a design or material defect. If the glass cannot withstand normal road debris, it may not meet durability or safety standards. There is also risk to occupants and other drivers if roof glass fails while in motion. Request: Please investigate whether the 2023 Tesla Model Y roof glass meets required safety standards, and whether this issue is affecting other owners.Read more

The auto headlights do not come on and if they do they almost always turn off immediately. The will not come on at speeds under 25 and this is too slow. To cap all of this they randomly come on when cars are driving towards you even though I have tried to disable this feature totally. They are a safety hazard and used to work on older models flawlessly.

The contact owns a 2019 Mazda CX-5. The contact stated that while driving 30 MPH with a toddler occupying the rear seat, the contact noticed smoke and an abnormal burning rubber odor coming through the vents. The contact pulled into a gas station and then pulled into a nail salon parking lot. There was no warning light illuminated. The contact was able to drive to the residence, where an independe

The dealer informed the contact that the repair could not be covered under an unknown Extended Warranty because of the type of engine. The manufacturer was notified of the failure and informed the contact that the repair could not be covered under Goodwill assistance because the oil changes had not been mainly performed by the dealer. The vehicle was not repaired. The failure mileage was approximately 83,000.Read more

I saw that the US National Highway Traffic Safety Administration is investigating the Tesla Model Y (2021 and over) due to the electronic door handles becoming inoperative. I am happy and relieved to see this is being looked into since this recently happened to my vehicle a couple of months ago. The rear passenger side door unlocked initially for me to put by [XXX] old into the car seat but when
... I went to open the driver's side door, the handles would not open. I tried all other door handles and all were locked. Luckily, I had not shut the door next to my child's car seat yet, otherwise it would've locked my child inside the vehicle. I opened a ticket with Tesla and they sent a technician out. He said the low voltage battery needed to be replaced and when this happens all electric door handles will be unresponsive and locked. However, the vehicle never showed any warning or notification leading up to this. He said when the 12V battery fails, the only way to unlock the unresponsive doors is to use the manual release inside the car or jump the 12V battery under the frunk (assuming you have the right jumping cables outside of the car). The Tesla technician said it is a flaw in the Model Y design and he said I would've had to break a window to get my child out if they are not old enough to locate and release the manual door release inside the vehicle. This poses a huge safety risk to parents with young children. I really hope Tesla implements another emergency way to release the doors from outside the vehicle to avoid young children from being locked inside. This started at about 65000 miles in my 2023 Model Y. The rear camera first lost picture and I received a warning that it may be intermittent. It has progressed now to the point that autopilot, navigation, all cameras, cruise control, auto headlight, and auto wipers are all nonfunctional. The car does not know its location. All safety features such as lanekeeping, cross traffic alert, auto bra
... king are disabled. The rearview camera no longer functions. Software updates no longer complete downloading. While there are numerous reports of computer short circuits in hardware 4 cars, the only solution I am offered is to have the computer replaced at my own cost of just under 3000 dollars. And that would be warranted only for one year or 12500 miles. Reviewing the submissions on the NHTSA website as well as searching online, this problem is not infrequent. I do not feel safe letting my wife or daughter drive the car. Certain 2024 and 2025 models with this problem have been recalled for new computers. This must be expanded to include 2023 models as well.Read more

The shoulder straps will not stay. They are supposed to lock in place once you adjust it and buckle the child in, but unfortunately, they will play back-and-forth and they will not lock in place. It is the same problem that Nuna is having with other versions of their car seat. They will not tell us what they think is broken, they will not tell us how to potentially fix it, nor will they admit that there is a problem, even though we have shown them video evidence. Instead, they want us to re-purchase a replacement item for half price. This issue allows the child to climb out of the car seat because the straps will not restrain them. The child can simply push on the straps and climb out.Read more

Formal Complaint to NHTSA – Summary I am submitting this formal complaint regarding repeated mechanical and electrical failures involving my 2017 Toyota Highlander, purchased as a Toyota Gold-Certified vehicle with an extended Platinum Warranty. Despite strictly following Toyota’s maintenance guidelines and using only certified dealerships for all services, the vehicle has become inoperable m
... ultiple times, requiring five separate tows and resulting in repair costs exceeding $10,000. In June 2025, a Toyota dealership replaced the engine after diagnosing a warped head due to water pump failure. Following this, the vehicle began experiencing repeated charging system failures and electrical malfunctions. Multiple components—including the alternator, battery, and sensors—were replaced at another Toyota dealership based on ongoing issues, but the problems persisted. On September 2, 2025, a Toyota technician identified a missing ground wire nut as the likely root cause of these electrical failures. He confirmed the issue originated during the engine installation and that the unsecured ground caused system malfunctions. After repairs, the vehicle was again returned with a loud engine knocking noise, later traced to an improperly secured component from a prior repair. These events have raised significant concerns for the safety of myself and my family. Improper installation of major components and misdiagnosis by certified service providers have led to hazardous driving conditions, unexpected vehicle shutdowns, and unnecessary costs. I am requesting that the National Highway Traffic Safety Administration investigate this matter for potential safety defects related to improper engine installation, grounding failures, and faulty electrical diagnosis by certified Toyota service centers. During a routine service, the dealership failed to ad
... d engine oil, but recorded in the service invoice that it was added. I unknowingly drove the vehicle 3–5 miles without oil, and multiple DTC error codes (P052400, P001100, P001400) appeared. The oil warning light, which should have alerted me, did not activate. Although a later borescope inspection stated there was “no immediate damage,” since then, the vehicle has shown: Significant loss of power and acceleration Abnormally high RPMs, even when the car is stopped Louder engine noises RPM is increasing suddenly while driving downhill without acceleration This vehicle is less than one year old and costs over $40,000. I should not be forced to drive a new car that may have hidden or delayed engine damage. Safety Concern: I have a [XXX], and I cannot risk sudden engine failure, stalling, or fire while driving. This is not only a reliability issue, but a serious safety hazard for my family and others on the road. Attempts to Resolve: Reported to Hyundai corporate, case opened. The dealership admitted the mistake but denied responsibility after the inspection. Hyundai corporate stated “no action until further inspection,” despite clear evidence of negligence. I continue to experience performance and safety issues. Requested Action: I urge NHTSA to investigate this incident and hold Hyundai accountable for both the dealership’s negligence and the potential safety defects (oil light failure, possible hidden engine damage). Consumers should not be left to drive unsafe vehicles, especially when children’s safety is at stake.
