Reference my NHTSA complaint # 11681607 dtd 8/19/25 regarding the problem with Autosense wipers that I’ve had since I bought my Aviator 2.5 yrs ago. As I previously reported, problem was finally resolved in April when dealer replaced the Autosense sensor. I have come to learn that the plastic covering dealer found on the sensor (see attached pic) is actually a protective film for part shipment
... & should have been removed by the assembly plant prior to installing the sensor but was NOT. There are other owners that have this same ‘wipers gone wild’ condition with Autosense wiping in which the windshield does not clear enough for safe driving & you need to switch to manual wiper operation to clear the windshield. This plant assembly error should trigger a recall with all manufacturers that use this part, not just Lincoln.Read more

I am writing to formally lodge a complaint against Tesla, Inc. regarding multiple critical safety failures that occurred with my Tesla Model Y (VIN: [XXX] ) during a collision on Saturday, [XXX], while I was traveling eastbound on [XXX] from the [XXX] exit to the [XXX] exit in Pennsylvania. At approximately [XXX], I was driving in the left lane when a BMW SUV abruptly cut into my lane ahead of me,
... accelerated briefly, and then suddenly braked. I immediately applied the brakes multiple times in an effort to stop my car. Even though there was sufficient space to stop safely, the braking system failed to respond, resulting in a collision with the BMW in front of me. Despite my efforts to avoid the impact, the following dangerous and unacceptable failures occurred at the time of the accident: 1.Failure of Regenerative Braking / Deceleration: When I lifted my foot from the accelerator, the vehicle did not decelerate as expected under Tesla’s regenerative braking system. 2.Failure of Braking System: Despite pressing the brake pedal twice—including a heavy application—the braking system failed to prevent collision, contrary to Tesla’s advertised safety performance. 3.Failure of Collision-Avoidance Features: Critical safety systems—including Automatic Emergency Braking, Forward Collision Warning, and Obstacle-Aware Acceleration—did not engage to mitigate or prevent the impact. 4.Failure of Airbag Deployment: Despite the severity of the crash, none of the airbags deployed, exposing me to serious risk of injury and raising grave concerns about the integrity of the Model Y’s occupant protection systems. 5.Post-Collision Failure of Regenerative Braking: Following the accident, when I attempted to drive the vehicle, the regenerative braking system was completely non-functional, creating a continuing safety hazard and rendering the vehicle unsafe to operate. I was provided a Tesla Model Y Juniper demo vehicle for a test drive on August 3, 2026 and the incident took place in it around 6:00 pm (±45 min). Full Self-Driving (FSD) was engaged. I had placed an order for a new Model Y on August 2 and took delivery on August 10 (different VIN). During the demo, FSD initially performed well: it autoparked into an apartment parking spot and exited once correc
... tly. However, the second time, the system made a dangerous maneuver. It activated the left indicator and accelerated rapidly as if entering a roadway, but instead turned left directly into a pole. The pole struck the rear door. I braked, but the system’s sudden move left no time to react. A reasonable human would have exited slowly and cautiously, but FSD acted abruptly and unsafely. My pregnant wife was in the passenger seat, and both of us were frightened. This incident raised serious concerns about FSD’s ability to handle low-speed, high-precision situations. The car should have detected the pole and stopped, yet it did not. If a child or pedestrian had been nearby, the consequences could have been severe. This also makes me question whether FSD could make dangerous high-speed maneuvers. At the Tesla showroom, we were told FSD drives “better than a human,” which misled us as new customers. This was the first car I purchased, and I was unfamiliar with FSD’s limits. Only after the incident did I realize how risky it can be. In my new Tesla, although offered a free FSD trial, I have chosen not to use it. Tesla’s claims administrator, Fleet Response, has since sent me a repair bill of $7,611. I was never asked to sign any waiver, never shown liability terms, and was told demo drives are insured. This incident shows serious safety concerns and misleading sales practices regarding FSD at Tesla.Read more

On Sunday, 9/14/25 driving on a 70 MPH interstate highway, pulling a trailer behind the Newly-purchased 2025 truck (only 700 miles on it), the road narrowed down to one lane because of a traffic obstruction and cones. As I slowed down to about 35 mph and then went to speed up again as the traffic eased, the vehicle provided a ping notification sound and the Electronic Vehicular Instrument Cluster
... (EVIC) display stated something to the effect of "Shift into 'P' to start vehicle". I realized that the vehicle was not responsive as expected, indicating power steering had failed, likely because the engine was off. I'm uncertain whether the steering wheel was locked, which is likely, because at that speed, only a very slight turn of the wheel was needed to move into the closed lane on the straight road. I continued to coast to get the vehicle and trailer over to the right, hugging the cones which were mid-lane in the closed lane, allowing other cars to go around me while I came to a complete stop on the interstate to shift into Park. Once I could get the gearshift into Park, I started the vehicle again, at which point it drove normally. The trailer had been weighed just that morning and both tongue and overall weight were well within the capacity of the vehicle. It was hooked up to the electrical systems of the tow vehicle via the 7-pin connector. I had the EVIC set to show vehicular information - oil pressure, oil temperature, etc. I similarly had information like battery voltage displayed on a widget on the uconnect screen. Everything looked nominal. No error code was logged to my knowledge. I have since seen similar reports from other 2025 RAM owners in NHTSA complaints and on social media. My dealership is unaware of these issues having been reported elsewhere, so they are starting from scratch in finding a diagnosis on a seemingly intermittent issue. It is unknown if the manufacturer has looked at this actual park on this vehicle. But they do know about it on all 2017 Honda Pilots. There is no warning prior to failure and it appeared over a year ago.Read more

There are many complains online about the same issue from other car owners. All I want is for the dealer to repair at no cost to me. The car has less than 30K miles and it is driven only on city streets short distances. Below, the answer to the questions: 1-Steering Rack. Yes it is available for inspection 2-Sticky steering hard to control 3-Yes 4-Yes. Authorized Honda Dealer at Coconut Point, Es
... tero Florida and Honda Corporate office in California knows about it too 5- No warning just a noise We firmly believe many vehicles may have this same issue. Regardless, it is clear that the part in my car is defective and should be replaced by the dealer at no cost to me. The age and mileage of the vehicle do not justify such a serious problem. Either the part was made with poor quality or it is defective. How can a steering rack fail at only 29,248 miles—what is it made of, plastic? This raises serious concerns about Honda Corporate and whether this is part of a broader scheme that should be investigated. My car is practically new and has been serviced exclusively at the same dealer where it was purchased brand new, without missing any of the recommended services ever. note: from the invoice attached you can see how expensive it is this repairRead more

Tesla’s Full Self-Driving (FSD) system has repeatedly demonstrated unsafe behavior in my 2021 Model Y (VIN [XXX] , ~60,184 miles). The issues involve Adaptive Cruise Control, Lane Keeping Assistance, and Forward Collision Warning. FSD frequently follows vehicles too closely, leaving insufficient distance. When I disengage, I am left with no safe option: braking hard risks being rear-ended and tr
... iggers a hard-braking event, while lifting off the accelerator leaves me dangerously close, still flagged as unsafe following. FSD often waits until less than a mile before highway exits to attempt lane changes, causing last-second darting, missed exits, or unsafe maneuvers. It also performs excessive and unnecessary lane changes that add risk without improving efficiency. Merges are frequently unsafe, forcing me to brake hard to let others pass or accelerate aggressively to avoid being cut off. On several occasions, FSD has jerked or swerved into adjacent lanes without signaling, seemingly triggered by shadows or changes in the road surface. These phantom maneuvers are unpredictable and hazardous. On [XXX] at about [XXX], at a T-shaped intersection, FSD failed to yield to a vehicle traveling straight with the right of way while I was making a left turn. I had to slam on the brakes to disengage, narrowly avoiding a collision. This was the second such incident in the same intersection in one week. These repeated failures—unsafe following, late exit attempts, failed merges, phantom swerves, and failure to yield—create significant risks for me, my passengers, and surrounding drivers. The problem has been reported to Tesla multiple times via in-car voice reports and a written service request, and logs from my vehicle should confirm these events. No warning lamps or system alerts appeared before or during the failures. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

In December 2023 I strapped my [XXX] son into his carseat in our Model Y and walked around to the front to get into the car as well. In those few seconds the 12v car battery died (I did not get any notification in my app) and I was unable to open any doors, so my infant son was trapped. We live in Florida and this was an 80 degree day. With no other recourse I was forced to call 911 to have my so

My 2020 Mercedes-Benz A220 experienced the same defect described in NHTSA Recall 21V961000 (fuel injector leakage and potential fire hazard). In March 2023, my vehicle was diagnosed with leaking fuel injectors that were causing fuel to leak back through electrical connections, creating circuit issues and a fire risk. I paid $2,902.65 out-of-pocket at a certified service center to have all four OE
... M fuel injectors replaced (invoice available). Later, I took my car to a Mercedes-Benz dealership, where they performed additional work on the fuel injector system at no cost to me. The service advisor stated that this was covered under a recall/service campaign related to the fuel injector defect. This shows Mercedes-Benz has already acknowledged that my car was affected by the exact same issue covered under Recall 21V961000. However, I was told my VIN [XXX] ) is not officially included in the recall population, and Mercedes-Benz USA has declined to reimburse me for the $2,800+ repair. I believe my vehicle should have been included in the recall population and request that NHTSA review whether the recall scope should be expanded. I also request reimbursement for the repair costs I incurred prior to Mercedes-Benz performing the recall-related fix for free. In January 2021, I purchased a new 2021 GMC Yukon Denali from a GMC dealership, which currently has only about 27,000 miles. To date, several recalls have been assigned and completed for my vehicle. per the websites for GMC and your agency. However, based on published recalls for my vehicle and a repair that is currently underway, I have reason to believe that my vehicle should have been subject t
... o three prior recalls for which it was not [NHTSA ID Numbers: 20V735000 (Front Seat Bolts), 25V274000 (V8 Engine Failure, and 21V739000 (Fuel Power Control Module] Currently, the fuel pump control module is being replaced at a local GMC dealership because it failed last week and left my stranded at home. GMC issued a recall for this in 2021, but my vehicle was not included. Aside from risk and inconvenience, this is also costing my over $300 for a rental. Secondly, though my vehicle's make/model/year is cited in GMC's ongoing engine failure recall, which you are investigating, I have not received a recall notice. I note GMC's financial interest in minimizing vehicle's it recalls for such a costly repair. I have inquired with GMC and two of its dealers and have only received reiterations that my vehicle has not been subject to GMC's recall, which I already knew. I have not been given any explanation for why my Yukon has not been recalled despite my inquiries. Lastly, GMC issued a 2021 recall for front seat bolts; however, I did not receive a notice for my Yukon. I have occasionally felt slight movement in the driver's seat since buying the vehicle. Based on reason and facts, I am alleging that GMC appears to be artificially minimizing recalls and notifications for vehicles that are in fact impacted by defects in their vehicles.Read more
