My family and I were in my Tesla Model 3 2022 and heading from Charlotte to Orlando on [XXX]. The car was in Full Self-Drive (FSD) mode while commuting through Daytona Beach. The rain was heavy in the area. While driving in Daytona Beach, FSD caused the car to change lanes and go to a lane that had standing rain puddles on [XXX]. After FSD caused the lane change, the car immediately hydroplaned
... out of control. The car's short clearance from it's bottom and the ground which traps water that exacerbates hydroplaning, along with its generative breaking feature made matters worse and uncontrollable. The car spun into the median three times and settled in the middle of danger interstate. The car is totalled. My family and I require medical attention and counseling due to the trauma. There was also a delayed airbag deployment from the back of the driver seat that only ripped the seat but never deployed fully. The smell of fire / burning and debris was all that happened from the driver seat. I requested a crash report from Tesla which outlines the speed and airbag deployment. The report says that knee level airbags deployed but they never did. I crossed referenced crash test speed data and various complaints about Tesla airbags. The data reviewed seems to indicate that I experienced failed equipment which is currently under review. I’ve also reviewed various sources of information about 7 2024 Model 3 driver side airbag known issues, where Tesla only recalled one vehicle. Both B-pillar camera housings on my 2023 Tesla Model Y Performance become extremely hot even when the vehicle is parked in a closed garage and not in use. On the morning of the incident, the garage temperature was approximately 78°F. The car had been parked overnight for over 12 hours, plugged in, and charging to 80%. While the vehicle was preconditioning, no defrost features were active. Despite
... this, both pillar surfaces measured 156°F with an infrared thermometer—hot enough to cause skin burns within seconds according to standard burn-threshold data for human skin (~155°F). Both sides heating to this temperature simultaneously suggests an electrical or control malfunction keeping the pillar camera assemblies energized. These components sit near the seatbelt anchor and side-curtain airbag area, raising concern of fire, electrical, or occupant-safety risk if the condition persists. Continuous heating while parked could also degrade wiring insulation or camera electronics used by the vehicle’s driver-assistance systems. The vehicle and affected components are available for inspection upon request. The issue has not yet been inspected or confirmed by Tesla, but a service appointment request was submitted immediately after the event. Tesla repeatedly rescheduled the appointment, delaying it by more than six weeks despite the described safety concern. No diagnostic alerts, warning messages, or notifications appeared in the vehicle or Tesla app. This abnormal heating has occurred more than once under similar conditions and does not appear to be caused by ambient temperature or sunlight exposure.Read more

I am writing to formally report a critical safety incident involving my 2020 Tesla Model 3, where the bumper detached while driving through a puddle of water on Oct 9th, 2025. I have since learned that this issue is recognized internally by Tesla, as they have created a service incident regarding it, but have failed to communicate this to customers effectively. Tesla's lack of accountability is concerning. Despite repeated attempts to address this with their support and service teams, they have deflected responsibility and continue to downplay the seriousness of this matter, insisting I must rely on insurance to cover the costs. This situation raises significant safety concerns, not just for myself but for all Tesla Model 3 drivers. I urge NHTSA to take the following actions: 1. Investigate this safety issue regarding the bumper detachment on Tesla Model 3 vehicles. 2. Pressure Tesla to publicly acknowledge this known issue and take immediate corrective action to prevent future occurrences. 3. Ensure that affected customers are compensated for damages and repairs due to this defect. I appreciate your attention to this serious safety issue and look forward to seeing NHTSA hold Tesla accountable to protect all consumers and ensure vehicle safety. I would like to formally report several incidents I have experienced with my newly purchased Tesla Model Y. I acquired the vehicle on [XXX], collected it from Washington D.C., and drove it back to Houston between September 20 and 21. Prior to my return journey, I conducted a test drive. ... ic light; however, once the light turned green, it moved forward but veered into the yellow safety buffer zone located between the lanes of opposing traffic. I intervened by manually steering the car back into the correct lane. A similar occurrence happened in Washington D.C. while attempting a left turn under a highway overpass—the vehicle again crossed into the yellow lines. These incidents suggest that the FSD system did not reliably detect the yellow lane markers. On the weekend of [XXX], during my drive to Houston, I primarily used FSD. While navigating a road construction zone, the system failed to recognize large safety cones (yellow columns) and nearly collided with them. Fortunately, I was attentive and promptly took control, braked, and changed lanes to avoid an accident. This demonstrated the FSD’s inability to identify these safety markers. On Tuesday, [XXX], I drove the vehicle to work and returned home using FSD. As I exited the highway onto a ramp merging with frontage lanes, I observed that FSD did not reduce speed appropriately and nearly made contact with vehicles on the frontage road. This indicates that the system was unable to interpret the ramp as a short section intended for deceleration and safe merging. On [XXX], while returning home in the evening after work, I used FSD due to light traffic. The vehicle navigated turns and stops satisfactorily and paused roadside before my house. When I resumed manual control to park, the vehicle suddenly became unresponsive, accelerated onto my front yard, struck the flower bed stones, INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5Read more

When accelerating from a complete stop, the vehicle intermittently fails to respond when pressing the gas pedal. The duration of this lag varies, and it has been increasing over time. Most recently, the hesitation lasted long enough t
... hat I was nearly involved in a collision. I pulled out from a stop sign to make a right-hand turn, but the vehicle failed to accelerate as expected. The car behind me nearly rear-ended me due to my sudden lack of movement in the lane. No warning lights, error messages, or other indicators appear before or during the event. I have been reporting this issue to my dealership for over a year during regular service appointments. The dealership has been unable to reproduce the problem and has attempted a software update, which did not resolve it. The malfunction has not been inspected by the manufacturer, police, or insurance representatives. The issue has occurred multiple times, including but not limited to the following dates: -March 12, 2025 -March 24, 2025 -July 21, 2025 -September 18, 2025 -October 1, 2025 These dates reflect times when I documented the issue via text messages to my husband; it has also occurred on many additional occasions not recorded above. Online owner forums and NHTSA complaints indicate that other Toyota Grand Highlander owners are experiencing the same issue, suggesting it may be a widespread defect affecting vehicle safety.Read more
