... ber 15, 2025, while driving my Tesla Model S southbound on Chambers Road at around 35 mph, the driver-side front tire suddenly exploded and detached completely from the vehicle. I heard a loud thump, then saw a tire rolling ahead of me on the road. The car’s display immediately showed “tire pressure 0.” When I stopped and exited the vehicle, I found that the tire had either blown apart or peeled off the rim and separated entirely from the car. Weather was clear and around 70°F, road conditions were normal, and traffic was light. My tire pressures had been checked earlier that day and were all around 42 PSI. I was unable to retrieve the tire because I have limited mobility and use a scooter. My daughter arrived soon after but could not locate the missing tire either. The vehicle was towed by AAA to a tire shop. The technician stated that the tire appeared to have peeled off the wheel, but could not explain why. The tire was less than one year old and had very low mileage. I drive short local trips daily (around 10–15 miles) and had only taken one 700-mile trip earlier this year. Goodyear was contacted, but they declined to investigate because the tire could not be recovered. This incident could have caused a serious crash had it occurred minutes later on I-25, where I planned to drive at 75 mph. I am reporting this because I believe the tire may have suffered a manufacturing defect, and I am concerned other drivers could experience the same failure.Read more

I then tried resetting and reconnecting my phone key through the Tesla app, but nothing worked. There were no 12-V or low-voltage warnings on the display or app. I could not find a phone number or live help in the Tesla app; my son and I searched YouTube for guidance but could not fix it. My dog was trapped inside in dangerous heat, so I called 911. Police first tried a slim-jim tool without success and then broke the driver’s window to reach in and open the front door, but the rear doors and windows remained inoperative. The dog was in the car for 30 to 45 minutes before the window was broken. Once inside, I found that the front seats would not move forward or backward, none of the windows would not roll down, and the rear doors would not open from inside. It took another 30 to clear enough glass to evacuate my 75-lb dog through the front seats; we both sustained cuts, and I sprained my ankle. The event was traumatic and posed a serious heat-injury risk (~85 °F ambient, interior >110 °F within minutes). Police documented the incident. Tesla Service Center (Naperville IL) later confirmed a 12-V failure as the cause, replaced the 12-V battery with an upgraded model under warranty, and repaired the driver’s window and door trim. Tesla Insurance was notified the next morning but closed the claim and covered repairs under warranty. As it neared the end of the braking event (while the car was still moving), the FSD system disengaged itself without any input from me (neither by manually applying the brake pedal, applying torque to the steering wheel, or pressing the FSD button). When the FSD system disengaged itself, my vehicle continued forward. Had I not been paying attention, my vehicle would have collided with the rear end of the vehicle in front of me. Since I was paying close attention, I immediately slammed on the brakes after noticing that FSD had disengaged itself. I have dashcam footage of this event from the TeslaCam system; however, it does not include audio or telemetry data. Even without that, you can see in the video that my car noses down while FSD applies the brakes, then noses up when FSD disengages itself, and then noses down again when I apply the brakes manually. (The dashcam footage exceeds the 20MB limit; however, I can provide it via other means upon request.) They indicated that the hardware was all working and adjusted properly and that the issue is a software issue. They told me to not use it and that eventually maybe Tesla will put out an update that may fix the problem. Tesla has sold thousands of cars with FSD and continues to call the system in BETA. They have since moved on to 2 more versions of the software and not offered an upgrade to the thousands of early adopters with the original hardware and software. The system is dangerous to drivers and everyone on or near the roadways when these are activated. Tesla said that can upgrade these “older” 2015 and newer models but haven’t because it will cost too much for them. They charged over $8000 for the system and it has yet to work. They need to be forced recall the system and fix these cars for everyone’s safety. The service manager told me he has a similar issue with car and he just opts to not use it. This issue has made the vehicle unsafe to drive because it can unexpectedly drop into Neutral at critical moments for example, while crossing lanes, merging, or pulling into intersections. There are no warning lights or alerts before this occurs. Sometimes, the truck will lunge forward or move forward unexpectedly when slowing down. Other times, it just rolls even though we are pressing the gas because it’s actually in Neutral. The problem has been repeatedly reported to the local dealership, which has replaced several parts -including the transmission and performed multiple repair attempts, but the issue continues. No other independent mechanic or third party has inspected the vehicle only the local dealership. Due to the ongoing safety hazard and repeated failure to repair, I am requesting that NHTSA investigate this issue. Because the Service Center was closed that evening, I couldn’t contact anyone, so I drove back first thing the next morning (July 23) to report the issue. I was initially told the next available appointment was two weeks later, but I had to beg to be seen the same day because the car was unsafe to drive. Tesla redid the alignment and told me the car was fine. After reviewing the alignment reports, both the July 22 and July 23 data are mechanically impossible. The reports show rear camber changes from −1.8° / −2.3° to −1.5° / −1.6°, then to −2.0° / −1.8°, even though rear camber on a stock Model Y is not adjustable. This means the alignment equipment or calibration was inaccurate, and therefore the toe settings were also unreliable. The car was almost certainly released with incorrect rear toe, which explains the immediate handling instability and safety system shutdowns. A later independent alignment on October 13, 2025 (America’s Tire, Roseville) confirmed the correct geometry (−1.9° / −2.0°) and proved Tesla’s readings were wrong. Since the faulty alignments, my car developed severe inner-edge tire wear, and I had to replace all four tires prematurely. Along with the current unable to be fixed PCM recall, the customer service satisfaction program regarding the drivers side buckle replacement and the replacement of high-pressure fuel injectors that I was told they will only replace if there is an issue because it is not considered a recall. ... first year the bushings were replaced to ease the loud noise when backing up. At 36,0000 miles I had no heat. The water pump failed and was fixed under powertrain warranty. At 41000 miles (just over warranty) the AC stopped working. The condenser failed and was my responsibility to pay. Ford did work out a deal with me on that repair due to all of the other issues I had previously. Currently, my drivers side back seatbelt does not work, the car shudders when driving and recently my forward collision indicator goes off for no reason. I have not reached out to have these items checked yet as I have spent far to much time dealing with Ford. I bought this car to feel safe and confident on the road and I have never felt that. There was a software update issued the previous Sunday. The warning came on the display that the lane changing was not valid. At that point the car took over and accelerated. I as able to get off at the [XXX] exit for it is a bit of a "hill" up in Florida, but knew I could coast into a lane hopefully no one would be in. I was able to at least coast into a parking lot, and bump the car into a concrete barrier, and a tree infront of the barrier. The car would not respond to park, neutral or the emergency brake. I tried re-booting the car 3 times and nothing. I had to call a tow truck to tow to the Tesla Service Center in Clermont, Florida. They could not get it in park either and had to drag the Tesla onto the flatbed. After 5 days of not responding to my text, I walked into another Tesla service center to plead my problem. Long story short, there is an unexplained anomaly that is in the CANBUS and cannot duplicate the error. More $ to have to tested for another week. They gave me a loaner to use, a Model S. After a few days driving it the loaner screen went blank and I was stranded. I had their loaner towed back to the service center. They will not 100% address the problem. They know it is in the CANBUS system, but now I definitely am scared to drive the car or get it back. How do I know if they will really tell me it is fixed? I fear if I get back in the car.
