Excessive oil consumption leading to catastrophic engine failure
Incident date: Aug 20, 2025
I’m writing to report a serious safety concern with my 2024 Hyundai Palisade (VIN: [XXX] ). The second-row seat belt on the passenger side does not retract or stay secure properly. It often remains loose or gets stuck when pulled, and sometimes won’t tighten back automatically. I brought my vehicle to the dealership, but they said they couldn’t replicate the issue. I recorded a video clearl
... y showing the problem, which I am attaching to this message. This is extremely concerning to me because I have young children who sit in the second row, and I’m worried the seat belt could fail to protect them in an accident. It’s very scary as a parent to see a safety feature not working correctly in such a new vehicle. Additionally, the rear charger port doesn’t work properly — the dealer said it’s due to using a non-Hyundai phone charger, but the issue happens with multiple certified chargers and different devices. After checking online, I learned that Hyundai has an active seat belt recall (#283) for Palisade models (2020-2025), and my vehicle seems to fall within the affected range. I am requesting a case number and for this issue to be investigated under that recall and warranty coverage. Please confirm receipt of this message and let me know how to send or upload the video evidence for your review. I’d also like a follow-up from a regional case manager, as this is a potential safety defect involving children. Thank you for your attention and for helping ensure my family’s safety. Recently Tesla released it's latest version of FSD, v14.1.4. One of the key features is the revamping of the driving modes: sloth, chill, standard, and mad max. Tesla also removed the ability to manually increase or decrease the speed of the car. This is a problem. Let me illustrate it with a real-life example. Yesterday I was driving of a 40 mph road and the car correctly set the speed. I was us
... ing the standard mode so the car allowed an overage of around 10% so the actual speed was 45 mph. In our area this is typical for most drivers. As we entered a village, the posted speed limit was changed to 30 mph. The panel was perfectly visible and readable. The car did not register it and continued at 45 mph. I changed to mode to "sloth" and the car decreased its speed to 40 mph but I was still 10 mph above the legal speed limit. I came across a second 30 mph panel and again the car "ignored" it. Because I did not have the ability to manually reduce the speed of the car, I had to disengage FSD and drive the car by myself. Tesla claims FSD must be supervised. Fine. But removing the ability to manually change the speed of the car should be a requirement not a design preference.Read more

my 2020 High Country Deluxe 6.2 L87 GM Motor: At Every Cold startup, the engine sound is erratic and sounds like metal on metal for internal combustion for approximately 2 seconds until the newly designed two-stage, variable displacement vane-type pump oil sump pump. GM Fix (for 2021 -2024 L87 6.2 motors was to change the viscosity to 5W/30 Supercar Synthetic oil, add a leaf to the owners manual to dictate the oil viscosity change and changing the oil cap from 0W/20 to 5W30. 10-speed Hydra-Matic 10L80: Hard upshifts and downshifts intermittently and confusion of proper gears. There is a TSB regarding this specific Valve Body and torque converter issues. Finally: When deactivating DFM Random cylinders as needed by ECM-TCM-BCM, the lifters are staying in the collapsed state and cant return to full height resulting in bent push rods, rocker arm damage, Camshaft damage, crankshaft damage and Catastrophic engine Failure. I am the original owner of a 2013 RAM 1500 SLT (VIN [XXX] ). I recently discovered, through official Mopar and Mopar México recall databases, that my truck has several open safety recalls — specifically T79 (17V-821), V44 (19V-347), X75 (Fuel Tank Strap Bracket), and U53 (Occupant Restraint Control Module). I was never notified by FCA/Stellantis or my dealership, even though my contact informat
... ion and address have remained the same since purchase in January 2014. This failure to notify is serious: years ago, a similar malfunction in a 2005 RAM caused an unintended reverse movement that almost pinned me between the vehicle and a gate — exactly the defect described in the T79 recall. These recalls have been open for many years with replacement parts available, yet no contact was made. I am requesting NHTSA to review whether FCA/Stellantis failed to notify owners in Mexico and the U.S. of record about the safety recalls still open on this VIN. Attached documents from Mopar and Mopar México confirm the current open recalls. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I purchased my 2019 Hyundai Santa Fe on October 29, 2021. Since then, I’ve experienced a series of mechanical and electrical failures that I believe are far beyond normal for a vehicle of this age. - The remote start has never worked. - On October 20, 2023, the hatch latch failed. The only way to open it now is by climbing into the back and manually pulling the latch. - On October 27, 2023, the
... electric power steering failed. I had to replace the entire steering column at a cost of $4,000. - The paint began deteriorating prematurely and required a full repair. Hyundai covered this under warranty, but the cost was $8,000. - In early spring 2025, all coil packs had to be replaced due to misfires, costing approximately $500. - September 4, 2025, The car stalled while I was driving on a main road and I had to pull over. There were no warning lights and I started home. About a mile from home check engine light came on so contact mechanic. Ended up towing to a dealership and was told the engine must be replaced because the oil had burned off without any warning from the oil light indicator. The turbo also needs replacement. - The combined cost is $14,000. These failures have caused significant financial hardship and stress. I am currently $17,000 underwater on the loan, and the repair costs are overwhelming. The lack of warning before catastrophic engine damage is especially concerning. I believe these issues reflect systemic defects in the vehicle’s design and manufacturing. The repeated failure of critical components—especially the steering system and oil monitoring—make this the worst automotive experience I’ve ever had. I respectfully request that the NHTSA investigate these failures and determine whether Hyundai should be held accountable. Consumers deserve protection from vehicles with such extensive and costly problems.Read more

am filing this complaint regarding a serious safety defect in my 2011 Ford Fusion. I own a 2018 Lexus NX F Sport and have had repeated fuel pump failures since purchasing the vehicle in October 2022. We have replaced the fuel pump twice — once at the dealership where we bought it and again less than a year ago. Despite these repairs, the same issue continues to occur. There were no warning lights before the failure, but once it happens, the check engine light turns on, the tr
... action control light comes on, and the vehicle goes into limp mode. When this happens, the car will not accelerate when pressing the gas pedal. It slows down rapidly and eventually stalls completely. This has happened multiple times, including once in the middle of Dallas traffic and another time while driving to Midland, TX. Each time, my children were in the car with me, putting all of us at serious risk. I have read about the Lexus fuel pump recall affecting similar models and years, and my vehicle is showing the exact same symptoms, yet my VIN is not included in that recall. I believe my vehicle should be reviewed for inclusion in the existing Lexus fuel pump recall, as this ongoing defect poses a major safety hazard to me and others on the road. Recall number: 20LA01 for Lexus (low-pressure fuel pump) — for certain 2018–2019 Nexuses and other Lexus models. Description: “Low-pressure fuel pump inside the fuel tank may fail. If it fails, warning lights and messages may display and the engine may stall while driving.” Additional support: 23LE01 (a Customer Support Program, not a direct recall) covers certain model years with low-pressure fuel pump defects, repairing under this program until July 15 2036 or 150 k miles. I believe my vehicle may fall under recall 20LA01 (Lexus low-pressure fuel pump) or the subsequent support program 23LE01, because the symptoms and repeated fuel pump failures match exactly, yet my VIN returns no open recall. My car currently has 144,400 miles on it. I’d like to get it fixed as swiftly as possible. Diagnostic tests are too costly.Read more

While driving home from a shop, my Tesla Model Y suddenly stopped in middle of busy road and became unable to move forward. I had to press the accelerator hard, which caused a loud noise before the car moved slightly and I was able to pull over to side of road. I narrowly avoided being hit by another car. Upon inspection, it was found that front tire connecting rod and suspension had failed, causi
... ng rod to drop off. The tow truck who assisted me mentioned that this is common issue with Tesla vehicle. This was a terrifying experience and I feel extremely fortunate to have escape an accident. My vehicle is less than 24 months old and I am deeply concerned about the safety and reliability of this component . I strongly urge Tesla to issue a recall and investigate this defect. There is no assurance that this failure won’t happen again. And this is scaryRead more

Component/System Failed: The primary component failing is the Front Overhead Console Assembly (P-R-N-D manual gear selector/shifter and hazard light switch). The assembly detaches, rattles, and fails to remain securely affixed to the headliner. The defect has been present since delivery (Aug 2024, 208 miles). Safety Risk: The overhead console contains the crucial backup gear selector and the haza
... rd light button. Should the vehicle’s primary touchscreen fail (rendering the main gear selector unusable), reliance on this repeatedly detaching or faulty manual shifter compromises the driver's ability to safely operate the vehicle (Park, Reverse, Neutral, Drive) or activate emergency hazard lights. Problem Confirmation/Inspection: The failure is confirmed by the manufacturer (Tesla Service Centers). Tesla attempted to fix the overhead console defect at least four times. The defect was confirmed again on October 22, 2025 (11936 miles), where Tesla noted only a "slight adjustment... as a temporary fix" and stated a "Permanent fix will be performed on the upcoming visit". The vehicle and its components have been inspected repeatedly by Tesla technicians. Warning Lamps/Symptoms: The primary symptom is the physical rattling, looseness, and detachment of the overhead console/gear selector. The center console can come off at ant time that the windshield visor is detached from the console. This has happened to me several times while I was driving and it caused the cybertruck to shift out of gear and has changed the gear to neutral and park on different occasions. The first time this happened while I drove, I was driving down the street and the car came to a hault when the gear shift cover came loose. This could have caused a serious accident, but fortunately it didnt. This problem existed from the time of delivery & we had to make our 1st service apt on the day we took possession of the truck. This happened several times afterwards and tesla was unable to fix this in 7 attempRead more
