November 20, 2025 To the National Highway Traffic Safety Administration (NHTSA) Office of Defects Investigation Subject: Safety Defect Complaint – Tesla Model S I am submitting this complaint regarding a serious and dangerous safety defect involving my Tesla Model S. Safety Defect Summary: • After the high-voltage battery failed, the vehicle would not enter Neutral. •

I had to use Tow Mode in the middle of a street on a hill to avoid blocking traffic. • Once in Tow Mode, the car could not enter Park and the emergency/parking brake would not engage. • The car became completely free-rolling, posing danger to myself, other drivers, and pedestrians. • When Tesla returned the car to my home, their technicians also could not get the vehicle into Park or activate any emergency braking system. • The car is currently stuck in Tow Mode in my driveway with physical objects placed behind the wheels to prevent it from rolling. Why This Is a Safety Hazard: A vehicle that cannot shift into Park, cannot use its emergency brake, and remains free-moving after a failure represents a serious safety defect. This occurred without warning and left the vehicle unsafe to operate or even store. Request: I respectfully request that NHTSA investigate this as a potential safety defect affecting the Tesla Model S when the high-voltage battery fails. I was never notified of this program. This short time frame under the program leaves MOST customers with these vehicles at risk. A safety RECALL should've been done as this is a big safety issue. It is not safe to drive with a bad brake booster because it significantly compromises your vehicle's braking power, making it dangerous. A failing brake booster can cause a vacuum leak, which can lead to the engine stalling when you apply the brakes. Unreliable and worsening problem: The issue is likely to get worse over time, and you could be caught off guard by a complete failure, leaving you with dangerously unreliable brakes. Vehicle has been inspected by Lexus and Toyota, both quoting astronomical fees associated with Brake booster replacement due to malfunction. Lexus Customer Brand Support was of no help, just repeatedly stating that the program expired and that it is up to me if i want to pursue legal action because this needs to be resolved by lexus. There have been class action suits and accidents due to this malfunction and I will pursue legal action as this is complete negligence by Lexus.Read more

The latest version of Tesla FSD software does not let me fully control my speed. You can switch profiles or stop using Full Self-Driving, but you cannot set the speed to what you want. In the last version, you could use the right scroll wheel to increase or decrease the speed. Now that shifts profiles but even that does not give you control over what exact speed you are driving

It is swerving and using the breaks so hard it actually affects the steering wheel and self disengages it's self (without the big-loud safety alert to immediately take over control), it just does the soft disengage audio notification, like if you wanted to cancel FSD

The serious problem is that the car does not stop like it should, instead it just continues it seems like it just neutrally rolls on its own drive and twice in the last week it would swerve directly into oncoming traffic. It is not supposed to be doing this at all, especially now that the "hands off" approval is enacted. I drive about 450 miles a week and this has happened about twice a week for the last two weeks. Thank god i was paying attention all four times, but I am so worried this is going to kill someone. I have tried to call TESLA but they purposely don't staff their phones and no one answers.Read more

Tesla performed a four-wheel alignment and tightened the components but stated the issue is “maintenance” and not covered under warranty, despite the vehicle having only 12,890 miles. Tesla also stated the steering rack had “no issues,” but they did not perform any repair on the steering system beyond tightening the loose tie rod nut and charging me for alignment. They also did not explain how a nearly new vehicle could develop a loose steering component. A loose outer tie rod nut should never occur on a new car unless there is a defect or improper assembly from the factory. This poses a significant safety hazard, especially at highway speeds. Tesla did not treat this as a steering safety defect and only attributed it to alignment. They declined warranty coverage and did not fully address the root cause. I am filing this complaint because steering components should not loosen on a 2025 model vehicle, and the situation could have resulted in loss of control or an accident. Today, I noticed the same issue arising. The battery died again, presumably from the same parasitic drain, and voltage was sitting around 10 amps. I would approximate that, over the past six months, the hybrid system has not been functioning as normal when I first purchased the vehicle. The trucks display notes why/when auto/start stop doesnt work. Over the past few months it displays that the battery is charging so auto/stop start is disabled, no matter how far/long I drive. My complaint is with the OEM lug nuts applied on my 2023 Ford F250. I recently tried to check the tightness on my wheels lug nuts only to find out that the lug wrench provided with the vehicle when purchased new will not fit 90 percent of the lug nuts. Dealership has rotated tires at routeen intervals since purchased new in spring 2023. Service manager stated that his mechanics routeenly use the n
... ext larger socket to loosen and reapply nuts. They are a carbon steel with chrome cap. Evertime you use the wrong size socket you further distort the chrome cap. The caps are loose and swollen so I can not change out a flat without calling and waiting for roadside assistance. My vehicle is still well within the 36 month 36 thousand mile bumper to bumper warranty. Local service manager states that Ford will not warranty faulty lug nuts. It's a safety issue when the nuts are improperly torqued or over torqued because they are using the wrong size impact socket on an impact wrench. It's a safety issue if I get a flat in an area with no cell service, or stuck on the side of the highway and get rearended. Ford and it's dealerships are aware of the failure of these two piece lugnuts and recommend that I go buy single piece lug nuts from an after market manufacturer like Dorman. This is the second time I have been gooned by Ford on lug nuts, Walmart will refuse service if the required size socket cannot be applied to the lug nuts. Back in 2014 I needed two new tires on my 2010 F150 , local auto zone and they only had 8 nuts in stock but enough to do one flat tire, we were on vacation in Kentucky. Drove home with spare on, we live in Belvidere Illinois. These two piece lug nuts should be outlawed, and using the wrong size sockets should be some kind of code violation, and the correct final torque should be checked with an actual torque wrench, not a best guess setting with your battery impact wrench. I own a 2019 Model X which I have been driving for six years without any driving issues. I did have a few incidents in which the screen went black while driving. The only reason it finally stopped was because the back tires got stuck on the ledge of the wall. The Tesla went through the metal gates and walls of the building, but the safety airbag never deployed. The car has been towed away and reported to the insurance immediately after the police report. This is the second time my vehicle has experienced a critical malfunction. The first major incident occurred on July 28, 2025. A complaint has been filed with the NHTSA under tracking number 11676662. Both failures involved sudden loss of essential functions, raising serious concerns about electrical, cooling system, or display system defects. I am reporting this because it is a serious safety risk and could lead to vehicle loss of control, inability to signal, or inability to see speed or gear position. The problem has been reported to Tesla and is pending evaluation at a Tesla service center. Warning messages and symptoms appeared at the moment of failure with no prior signs.Read more

The contact owns a 2021 Grand Design Momentum RV. The contact stated that while inspecting the RV, the contact became aware that the outrigger brackets were bent and fractured. The contact stated that the failure was visible over the axles. In addition, the welding on the cross-member beams was fractured, causing the floor to crack, and the exterior fiberglass was cracked by the bedroom slide-out.
... Additionally, there was a four-inch-long crack on the frame of the upper deck. Most recently, the contact found movement of the cabinets in the upper deck, and a portion of the fiberglass on the front cap had separated from the roof. The RV was taken to a welding shop to be diagnosed, and it was determined that the metal used to support the walls in the unit was not strong enough. The contact was informed that the weight on top caused it to bend and crack. The outrigger brackets were replaced, the cross members were replaced, and the other cracks were repaired. The manufacturer was notified of the failure and denied the warranty claim. However, the manufacturer failed to acknowledge the failure and advised the contact to call the insurance company since the manufacturer believed that the RV had been involved in an accident. The contact denied that the RV had been in an accident. The contact noticed the issues since the RV was purchased. The failure mileage was unknown.Read more

We noticed the passenger side taillight was out. It’s not a simple bulb replacement but an entire assembly. It was put back in place and it lit up again. A few weeks later it wasn’t working.
