While driving on the freeway in the fast lane at 75 MPH I80W, the car began a loud humming sound that came from the rear. My passenger and I noted that it was significant sound that we took a video to record the sound. I was the driver and I noticed that the sound increased as I accelerated. Then without any warning the car began to decelerate and no longer accelerated. The car slowed considerably
... to 35 MPH and it became a major safety issue because I had to cross all lanes of traffic at this slow speed. It was only after the car had completely shut down that the warning messages began to display. I reached the side of the road and we waited for the tow truck driver for about an hour. Subsequently, the car was towed to a Tesla dealer near my home. I was informed that the drive unit needed to be replaced with an explanation of internal failure, costing over $7000. However, the root cause of the internal failure was never explained or investigated even at my request stating that they do not take apart the drive unit. This incident occurred after driving 7hrs of heavy rainfall for 2 days. A quick google search of "2016 Tesla model S drive unit failure" reveals plenty of Tesla owners that had similar issues.Read more

To: Administrator National Highway Traffic Safety Administration (NHTSA) 1200 New Jersey Avenue SE Washington, DC 20590 USA Subject: Formal Safety Defect Claim Regarding Unaddressed Recalls — 2012 Kia Optima Hybrid (VIN: [XXX] ) From: [XXX] Email: [XXX] Phone: [XXX] Vehicle Information: - VIN: * [XXX] ** - Make / Model: 2012 Kia Optima Hybrid - Mileage: approx. **
... 250,000 miles** - Current Location: Moldova (vehicle manufactured in Korea for the US market) --- ## Summary of the Problem I am writing to file a **formal safety defect claim** regarding my 2012 Kia Optima Hybrid, VIN * [XXX] **, which, despite having documented recall campaigns, has never had critical parts replaced. As a result: 1. **Internal scoring and scratches (zadir)** have been detected within the engine, consistent with **metal shavings / debris** circulating in the oil system. 2. There is **metallic residue (shavings)** found in the oil pump, which strongly suggests that worn or damaged components (likely from a manufacturing defect) were never replaced during the recall. 3. Given the extraordinarily high mileage (≈ 250,000 miles), the continued use without the required recall repairs poses **significant safety risks** and potential for further engine failure or fire. 4. I was not provided with a proper remedy when recall notices were issued — in my case, the relevant parts were not replaced in a timely fashion (or at all), despite the recall campaigns being active, leading to the deterioration of the engine. --- ## Legal and Technical Basis ### Recalls Applicable to My Vehicle - NHTSA Recall **20V750** for the 2012 Kia Optima Hybrid: This is a recall related to engine defects, including risk of engine fire. The engine of our 2017 Hyundai Santa Fe Sport has been significantly problematic since we first purchased the vehicle in 2017. These issues have been well documented by other Hyundai owners and have resulted in class action lawsuits. After purchasing our vehicle new from dealer, it began experiencing difficulties, consuming large amounts of oil, spewing smoke from the tailpipe, leaving metal sha
... vings in the engine oil, failing to start, and at times going into a protection mode to keep the engine from further damage and restrict speed above 15 miles per hour. The vehicle had to be towed multiple times at our own expense. Hyundai delayed replacement of the engine for months resulting in thousands of additional miles being put on the vehicle under unsafe operating conditions. Hyundai reluctantly replaced the engine in February of 2024. What appears to not be well-documented in NHTSA complaints are the impacts of these engine issues to other Hyundai components and parts. Hyundai replaced our original engine but did not replace the engine’s catalytic converter. 20 months later, the check engine light has illuminated again as a direct result of oil sludge and buildup from accelerated oil consumption. In November 2025, the dealership identified the fault code P0420 for catalyst efficiency system below threshold. We have experienced a loss of acceleration, increased fumes, and decreased fuel efficiency as a result of the catalytic converter failure, and believe the vehicle is no longer safe to drive at high speeds on freeways. The dealership has confirmed damage to the catalytic converter and has quoted a replacement fee that is $5000, almost $1000 higher than other dealerships in our area. We asked for the dealership to submit a request to HMA for goodwill assistance and were denied on November 24th. We strongly believe that Hyundai is responsible for both the damage to the catalytic converter as well as the safety concerns caused by this issue.Read more

On the day of the incident, I drove my vehicle into the charging station and activated the Tesla FSD autonomous driving and automatic parking functions. During the automatic parking attempt by the vehicle, the system exhibited the following severe loss of control behaviors: 1. The vehicle automatically collided with the vehicle in front without any human operation. 2. It paused for about 1 second
... after the collision. 3. Then, the vehicle suddenly accelerated in reverse at high speed, again without any driver operation. 4. It directly hit another Tesla that was charging behind my vehicle. The entire process is fully executed automatically by the vehicle system. I cannot intervene or stop the system through the steering wheel or brakes. II. Severe Security Risks When the accident happened, my friend was sitting in the passenger seat. If the vehicle had moved a few more inches, it could have caused serious injury or even a life-threatening situation. This system failure is a combination of Sudden Unintended Acceleration (SUA) and the loss of control in autonomous driving decision-making, posing a significant systemic risk to public traffic safety. I immediately contacted Tesla and sent a complaint letter to the North Carolina Consumer Center. They accepted my complaint and sent an email to Tesla, but it has been over twenty days and I have not received a response from Tesla.Read more

Hi, its not really an incident but I found what have happened suspicious. I got my tesla from the delivery zone at the Giga factory in Austin, TX last Wednesday. Right after the delivery i found an issue with steering wheel alignment. Steering wheel was tilted right and car was pulling right in the road. I addressed this issue to Tesla and visited service center last Saturday Nov-22nd. Technician
... confirmed an issue. Wheels alignment was performed on the vehicle and it was returned to me on the same day. As soon as I got the the road I found that issue wasn't fixed. It went better but not fully fixed. I came to the service center again. Senior technician took a ride with me and confirmed the issue again. She also told me that they will probably will have to look into suspension because alignment didnt help last time and it could be more serious issue. The car was taken care of and returned to me in 90 min or so. Service advisor told me that suspension and steering were torqued and another alignment performed. When I took it to the road and I found that issue was fixed and I'm happy now but ifter that i recieved a message from the Service Center that I found suspicious. Despite the communication in person in the message afterwards was stating that no issues were found (See attach). I found this suspicious and decided to let you know as that may look like an attempt to hide some serious safety concern with steering. Please find screenshot in the attachmentRead more

After intermittently experiencing sticking of my steering wheel and creaking/grinding when turning the wheel, in April 2024 my vehicle’s rack & pinion recall was closed at Northside Honda (28,676 miles). In May 2024, I was provided a free oil change at the same dealership. I wasn’t informed of any failed recommendations or provided a copy of the multipoint inspection (29,328 miles). The sticky
... steering continued to be intermittent and I was unable to replicate it, specifically as cooler weather began. In February 2025, my vehicle’s steering gearbox recall was closed and corrective action was performed at Hill Country Honda due to my sticky steering noticeably occurring at highway speeds, making it difficult to return to center, specifically during a drive home from work that required significant effort to stay in my lane (38,957 miles). In October 2025, I scheduled another inspection, as the sticky steering had noticeably returned once more at highway speeds. It was then diagnosed by Hill Country Honda’s technician to be an internal failure of the gearbox with a recommendation of electric steering gearbox replacement (51,073 miles). After numerous attempts to inform Honda Corporate of my recent finding (fail recommendation “incorrectly assembled steering rack”) which Northside Honda never mentioned to me in 2024, neither Honda Corporate nor the dealership will take accountability. I’ve also noted that no comments were listed in the rack and pinion recall to indicate a concern/complaint, despite my description stated above. The dealership is continuing to state my car was not included in the recall though listed on my oil change visit after the recall was closed and they won’t cover 100% replacement regardless of this being incomplete warranty work from 2024. A new investigation NEEDS to be opened on this steering recall. Honda Corporate’s corrective action has not helped numerous people from what I’ve researched even with the replacement of a steering rack.Read more

I received a safety alert on my car screen: Front passenger safety restraint system issue. Service is required. The front passenger safety restraint system fault typically involves a malfunctioning occupant weight sensor in the passenger seat, which controls whether the airbag activates or deactivates EVGOER. This is a serious safety concern because the airbag may be disabled at that seat locatio
... n in the event of a crash. The issue is often caused by loose, broken, or unplugged harnesses or connectors under the seat, particularly related to the Occupant Classification System (OCS) Tesla Owners OnlineDigital EV Doctor. Many owners report this happens because wires under the seat get crimped over time due to Tesla's "easy entry" feature that automatically moves seats. Since this a widespread Tesla Model 3 problem, I believe this should be a recall.Read more

Upon purchasing the vehicle, I discovered multiple critical safety defects that were not disclosed at the time of sale and were confirmed by an independent EV specialist (Electrified Garage). These defects include worn and failing front and rear suspension components (control arms, thrust arms, and cracked/damaged bushings), a failing sway bar link boot, and a malfunctioning window that makes cont
... act with the internal wiring harness. The suspension defects create instability over bumps and uneven roads, producing clunks, shifting, and vibration that affect steering control and overall stability. These issues present a risk of suspension failure while driving, which could cause loss of control. The window/wiring contact also poses a potential electrical hazard if the harness becomes damaged. The independent inspection documented that these defects existed before I took possession of the vehicle. The dealer did not disclose any of these issues, and the defects were not visible during a standard test drive. I am reporting this because the suspension defects directly affect vehicle handling, safety, and crash-avoidance capability. I want NHTSA to be aware in case similar defects appear in other vehicles of the same model/year.Read more

23S64-Engine Oil Pump RECALL 8/19/2025 driving from Hillsboro Beach, Florida to St. Petersburg, Florida. We had a full engine seizure, we lost oil pressure, and also lost use of the transmission. We overheated and the engine seized. With a loss of power, we had to guide the car off the road. The engine was smoking heavily. The back of the car was completely covered with motor oil. The Florida Hi
... ghway Patrol called us a tow truck that took us to DeSoto Ford, 3039 Southeast Highway 70, Arcadia, Florida, 34266, 863-494-4848. DeSoto Ford ended up doing multiple repairs on the car. They did the replacement of the 23S64 recall for the oil pump belt and tensioner. The car was there for 24 days. We drove down on 9/12/25 and picked up the car. When they pulled the car out, the car was still covered in oil in the back. We found out within days that they neglected to fix the radiator. Once we got into the car and started driving, we had yet another very loud noise, forcing us to bring it to AutoNation, St. Pete, Florida. At AutoNation, we had a multitude of problems with it. We had severe vibration with the car when we brought it there 9/18/25. This was only a few days later. They changed out the radiator and replacing the full radiator because they said there was a full leak that was not fixed by DeSoto. They fixed the car and gave it back to us 9/30/25. On 10/22/25, I had the exact same experience with the car that we originally had on the highway in Arcadia. The engine fully seized in the intersection of 3rd Street South and 6th Ave south in St Petersburg just as it had done before. The car overheated and had full loss of power/motive power. I was able to restart it briefly to get out of the cross section. The car stalled two more times before I was able to bring it to a safe place. We ended up having it towed from 15th Ave S at 3rd Street S and brought to AutoNation. Vehicle broke down again 11/7 and has been in shop ever since. Much more details available.Read more

My vehicle experienced multiple safety system malfunctions before a minor collision occurred. These failures happened before any impact. At the same time, the vehicle’s cameras were not functioning correctly, and
... the vehicle did not provide any forward collision warning. The automatic emergency braking system also did not activate. Because these systems failed, the vehicle did not warn, slow down, or brake to avoid a collision. After the collision, the vehicle showed several additional safety system failures. These included automatic emergency braking unavailable, and repeated safety restraint system faults affecting both the front left and front right restraint systems. The airbags did not deploy even though conditions suggested they should have, and afterwards the vehicle displayed persistent airbag and seatbelt system error icons. The restraint system appears to have gone offline. The vehicle is currently unsafe to drive due to failures in braking, traction control, perception/camera systems, forward collision warning, automatic emergency braking, and the airbag/restraint systems. These failures occurred before the collision, contributed to the collision, and continued afterward. Having had my foot on the brake at the time I decided to test the ignition switch to determine why it was lit while the engine was off. When I pressed the start button there was
... a loud, whining starter like sound, grinding like metal on metal. The noise was so alarming that I immediately took my hand off the ignition. The engine was still off. After waiting a significant amount of time I tried to restart the engine. Exactly the same thing occurred. On the final attempt to start the engine, 15-20 minutes later, it started and ran quietly like nothing had ever happened. On returning to the vehicle my wife asked what all the loud grinding noise was about. She had heard it while she was inside the store. I advised her it was our truck! The incident was shocking to us both and because of its bizarre nature I was afraid to drive the vehicle back home. As I already had an appointment on November 6 at DeLuz Chevy for another incident, I called them on November 3 to add this incident to the existing appointment. This incident was added to the November 6 appointment. Unfortunately the service department did not assess the previous incident and only addressed this incident.Read more

When using cruise control (traffic aware cruise control and autosteer) the car acts dangerously and stops for no reason. I've been driving on open roads with no traffic in front of me on regular roads, the car will slam on the brakes and it causes the people behind me to need to slam on their brakes as well. I feel like I'm going to get into an accident constantly... I was on the freeway and no o
... ne around in front of me and the car slammed on the brakes to almost a stop on a 70 mph speed limit road. Today the same thing occurred in high traffic on a 55mph road, luckily the driver behind me wasn't to close and was able to stop in time ... There is no option to use regular cruise control or disable enough features to prevent this from occurring... It's incredibly terrifying. If they can't do traffic aware cruise, there should be the option to do regular cruise... And it shouldn't require passing Tesla $8000 for full self driving to be able to drive your car at a speed without the that of being rear ended. This feels like extortion... I've submitted feature requests to Tesla, talked to their bot about fixing this and I've currently been on hold for almost an hour without any response. This dangerous tech is unacceptable!Read more

While on a road trip our vehicle began to run louder, same shifting, same gas mileage but louder especially under acceleration. There was no warning and luckily we made it home only to find out it was the High Pressure Fuel Pump in the engine bay that was failing. We had the previous Fuel Pump Recall completed but now at ~82,000 miles the HPFP goes out, these usually last much longer. Took to a
... n independent shop who diagnosed and fixed problem resolved but said we could have easily been stranded or worse the hose with fuel that was violently pulsating in the engine bay could have come unattached and fuel could leak on the hot engine.Read more

My 2024 Jeep Wrangler 4xe is currently subject to an active fire-risk battery recall with instructions to “park outside” and “do not charge.” There is no remedy available for my VIN at this time. The manufacturer states there is a risk of the high-voltage battery catching fire. In addition to the recall, the vehicle has exhibited violent, unsafe jolting movements during acceleration and n
... ormal driving, which is consistent with high-voltage battery or hybrid drivetrain malfunction. These unexpected surges and jolts make the vehicle feel unstable and dangerous to operate for me and my family of four. Because of the fire risk AND these sudden jolts, I do not feel safe driving this vehicle. I have attempted multiple times to work with Jeep Customer Care to initiate a Safety Recall Compensation case, but Stellantis/Jeep has repeatedly refused to open a case and continues redirecting me between departments. On 11/17 at approximately 3:20 PM, a Customer Care agent named Louie transferred me to a local dealership when I requested a supervisor, even though the dealership confirmed they cannot initiate compensation or buyback requests. Jeep Customer Care has failed to: Open a recall compensation case Escalate the safety concerns Address the violent jolting behavior Provide written communication Follow proper recall procedures At this time, I have a vehicle that is unsafe to drive, unsafe to charge, and exhibiting dangerous mechanical behavior with no available repair. I am requesting NHTSA assistance to compel Stellantis/Jeep to follow recall safety protocols and provide a remedy, compensation, or replacement for this unresolved safety defect.Read more

I am reporting a major safety defect that began immediately after Tesla installed the Full Self-Driving (FSD) hardware upgrade on my vehicle. After paying $2,000 for FSD and receiving the hardware retrofit, the car now instantly veers left toward oncoming traffic whenever FSD is engaged. This began immediately after Tesla’s installation. Before this service, my vehicle operated safely. I have
... contacted Tesla Cherry Hill Service Center in New Jersey multiple times. The advisors there repeatedly told me to “just recalibrate the cameras from the touchscreen.” I’ve done this numerous times, following every instruction carefully, but the issue persists. When I called the Service Center, I was told that “everything looks fine remotely,” even though the problem continues and is clearly dangerous. I was also told they could take it for a test drive, but that if they couldn’t find anything, I would have to “wait for a future update” to make FSD usable. This is unacceptable and unsafe. Tesla delivered this car to me in a dangerous condition that could easily cause a serious crash. A car that steers itself into oncoming traffic under FSD control should never have been returned to a customer — especially one new to using FSD. I should not be responsible for any repairs, recalibrations, or diagnostics related to a defect introduced by Tesla. This issue appears to be the direct result of improper installation, misalignment, or faulty calibration following the FSD hardware upgrade performed by Tesla Cherry Hill. This poses a severe risk of collision and injury, and Tesla’s refusal to take responsibility or correct it under warranty demonstrates clear negligence and disregard for safety. Tesla Cherry Hill is also known for poor customer service and repeated dismissal of safety complaints from other owners, which further concerns me.Read more

I would like to report a significant incident involving the auto-corrective steering in my vehicle. While driving on the highway, the steering wheel experienced a strong jolt, which I believe was caused by the auto-corrective steering system. This resulted in the vehicle spinning three times and ultimately coming to rest on the grassy center divider. Fortunately, no damage occurred at that time, a
... nd I initially attributed the incident to road conditions. I continued driving for approximately 30 minutes in rainy weather without further incident. However, a few hours later, during my return journey, the same forceful jolt to the steering wheel recurred about 20 minutes into the drive. On this occasion, the car spun 1.5 times and subsequently collided backward with the concrete center divider. It is fortunate that no other vehicles were involved, given the heavy traffic and rainy conditions at the time. During both of the incidents, I had both hands on the steering wheel and was not able to correct steering due to the strong force from the steering wheel jolt. I did have a Police Report filed, but have to get number when I return home.Read more

November 20, 2025 To the National Highway Traffic Safety Administration (NHTSA) Office of Defects Investigation Subject: Safety Defect Complaint – Tesla Model S I am submitting this complaint regarding a serious and dangerous safety defect involving my Tesla Model S. Safety Defect Summary: • After the high-voltage battery failed, the vehicle would not enter Neutral. •
... I had to use Tow Mode in the middle of a street on a hill to avoid blocking traffic. • Once in Tow Mode, the car could not enter Park and the emergency/parking brake would not engage. • The car became completely free-rolling, posing danger to myself, other drivers, and pedestrians. • When Tesla returned the car to my home, their technicians also could not get the vehicle into Park or activate any emergency braking system. • The car is currently stuck in Tow Mode in my driveway with physical objects placed behind the wheels to prevent it from rolling. Why This Is a Safety Hazard: A vehicle that cannot shift into Park, cannot use its emergency brake, and remains free-moving after a failure represents a serious safety defect. This occurred without warning and left the vehicle unsafe to operate or even store. Request: I respectfully request that NHTSA investigate this as a potential safety defect affecting the Tesla Model S when the high-voltage battery fails. A car should never lose all ability to secure itself.Read more
