I am reporting ongoing safety concerns with my 2018 RAM 2500 (6.7L Cummins) related to repeated emissions/aftertreatment failures and the lack of timely repair options. These failures directly affect drivability and can create hazardous situations if the vehicle enters derate mode while towing or driving at highway speeds. Over the past year, my truck has suffered multiple emissions-related compo
... nent failures, including DEF dosing issues, SCR/DPF system faults, repeated warning messages, and the threat of speed-limiting derates. The vehicle is used for normal personal use, is well-maintained, and has not been modified. Despite proper maintenance, the same types of failures continue to recur. A major concern is that local RAM dealers have extremely limited ability to service these issues in a timely manner. Appointments are often weeks out, and when the truck is already in a failure state it is unsafe to continue driving. I have previously reported safety-related defects to NHTSA (ABS/HCU), and those issues later resulted in recalls—so I know that reporting matters. The combination of emissions failures, derate risks, and lack of dealer support creates a situation where the vehicle may suddenly lose power or become unsafe to operate, especially when towing or traveling in rural areas with limited service availability. I am requesting that NHTSA review whether there is a broader pattern of emissions-system failures on RAM heavy-duty trucks with the 6.7 Cummins and whether these failures pose a safety risk due to the forced-derate logic and inability to obtain timely repairs.Read more

Dealerships are not willing to give fair value for a trade-in because t

I was driving our 2017 Kia Sorento 2.4 liter, on I-94 in Detroit, MI when suddenly, the vehicle would not go over 50 mph when the engine suddenly lost power on Friday 11/28/2025 ..We towed the vehicle to the Auto Repair Shop a few days later. On Tuesday 12/9/2025 the Auto Repair shop contacted us and said that their was plenty of oil in the vehicle but Cylinder 3 misfired and there is a loss of
... engine compression. Auto shop says cylinder 3 failed and now only 3 cylinders are now working on the 2017 Kia Sorento now mileage is now 94, 971 miles. I have not taken it to the dealer yet because even though there was a class action lawsuit, the Auto Repair shop has informed us, as well as other information online, that the dealer will sometimes try to avoid replacing the engine. I did contact the dealer on 12/10/2025 and informed that we do have maintenance records and have the vehicle towed there this weekend, however, my initial conversation with the Service rep did not sound promising to date.Read more

Date of Incident: December 8, 2025 Mileage at Failure: Approx. 580 miles Vehicle Status: At Tesla Service Center since December 9, 2025 (no repairs started; status shows “Preparing”) Failure Summary While driving 40–45 mph on a busy three-lane road in California, approaching a signalized intersection, the vehicle suddenly displayed multiple red warnings, a “Reduced Speed” alert, and num
... erous fault messages. Within 3–5 seconds, propulsion was completely lost. Incident Details Using residual momentum, I steered across two lanes into a nearby retail parking lot. The vehicle stopped at an angle, partially obstructing traffic. Towing Delay & Safety Risk The vehicle remained stranded for ~4 hours. A flatbed tow arrived ~2 hours later but could not load the vehicle due to inoperative Tow Mode. A second dolly-equipped tow truck was required, adding ~90 minutes. During this time, the vehicle continued to block traffic and posed a safety hazard. Component/System Failure Complete propulsion loss and failure of major electrical/control systems, including doors, frunk, trunk, Tow Mode, and non-responsive key card and phone key. Vehicle remains inoperable. Safety Hazard Sudden shutdown in a center lane near a major intersection left almost no opportunity to maneuver safely. Extended roadside immobilization and inoperative Tow Mode further increased risk. Warning Prior to Failure None, except 3–5 seconds of simultaneous red warnings and fault messages. Problem Status Ongoing and unresolved. Tesla has not diagnosed or repaired the vehicle and continues to delay the estimated repair date. Inspections Vehicle is under evaluation at Tesla Service Center. No repairs, police, or insurance inspections have been conducted.Read more

My vehicle has a hand brake built into the vehicle and at 25,000 miles my hand brake started to fail and wouldn’t hold the car in place. I had it repaired under warranty and the whole emergency/handbrake assembly was replaced, I looked at it and tested it before I left the dealership and noticed the same issue, I brought it up to the dealership and they noticed the same issue also after testing
... it again themselves, and they also stated there were other vehicles of the same model with less miles having the same issue and also said it could possibly be a recall. I drive a car with a manual transmission so the hand/emergency brake is what parks and holds the vehicle in place. I originally took it there because my handbrake was no longer holding my car in place to avoid hitting other vehicles especially being parked on an incline or decline.Read more

In early Nov 2025 my Tesla Model Y began intermittently showing warnings about the forward collision warning system and multiple sensors being offline. Tesla service replaced a faulty connector on Nov 3. On Nov 15, 2025, about two weeks later, the car had a sudden drivetrain failure while I was parking on a steep hill. I heard a loud bang from the front of the car and the vehicle immediately lost
... drive power and had to be towed. Tesla diagnosed a failed front motor inverter that blew the high-voltage pyrofuse and damaged the 12V battery; they replaced those parts. The vehicle is available for inspection and the repaired components and records are with Tesla. After that repair the car needed another visit for alignment and has continued to show worrying behavior: “VCFRONT offline/unable to contact VCFRONT” in the service menu, repeated “autopilot computer overheating” messages, intermittent loss or reduction of regenerative braking (braking feel changes unpredictably), a rear right seatbelt malfunction warning (later inspected and cleared by Tesla), and what appears to be significantly increased battery drain compared to before the inverter failure. This has now resulted in four service visits within about a month, all at Tesla service centers (no police, insurance, or independent shops involved). Tesla has confirmed the inverter/pyrofuse/12V failure and says the overheating was due to an air pocket in the cooling system, but the VCFRONT messages, inconsistent regen braking, and increased drain have not been fully resolved or clearly explained. I am concerned that a sudden loss of power, loss of regen, or failure of VCFRONT/autopilot systems could recur at speed or in traffic, creating a serious safety risk. Please note that I'm only able to view partially-complete paperwork from the latest service appointment in the Tesla app. I have requested this documentation from Tesla and will be able to provide all the service reports and communication as a follow-up.Read more

1) The Power Take-Off Unit (PTU) in the drivetrain failed suddenly while driving at approximately 60 mph. This is a critical drivetrain component responsible for transferring power. The PTU is currently at the Jeep dealership where the repair order is open, and it is available for inspection upon request. 2) The PTU failure caused the vehicle to lose the ability to operate safely and became inoper
... able in the middle of travel on the highway. This placed my husband and son at risk of being stranded on the side of the road at highway speeds, vulnerable to collisions, limited ability to maneuver the vehicle, and potential loss of control. It also created risk to other drivers due to sudden deceleration and inability to move the vehicle to safety under its own power. 3) Yes. An independent mechanic first confirmed that the issue was related to the drivetrain. The Jeep dealership later confirmed that the failed component is the Power Take-Off Unit and that it is covered under the powertrain warranty. A repair order is open, and the part is currently on backorder. 4) Yes. The vehicle was inspected by a local independent mechanic immediately after the breakdown, and then by the authorized Jeep dealership after it was towed for warranty repair. No police or insurance inspection occurred because the incident was a mechanical failure without a collision. The manufacturer’s dealership has acknowledged the drivetrain failure. 5) No, there were no warning lights, messages, or symptoms prior to the sudden failure. The vehicle was operating normally up until the moment the PTU failed on November 27, 2024 at approximately 11:40 AM. The failure was immediate and unexpected.Read more

My 2015 Tesla Model S has a serious safety issue where the vehicle suddenly reverts to Park while driving and will not stay in Drive. My 2022 Ram 1500 TRX has recurring safety-related failures of the infotainment system affecting both Uconnect 5 (12” screen) and earlier Uconnect 4 units. These issues are widespread in 2021–2024 Ram TRX and Ram 1500 models and began or worsened after forced over-the-air updates. The primary defect is black-screen failures while driving. When the screen goes black or freezes, the truck loses
... all functions controlled through the display, including HVAC/defrost, backup camera, vehicle settings, seat controls, and performance gauges. This creates an immediate safety hazard: I cannot adjust climate/defrost, cannot see the required rear-view camera when shifting to reverse, and cannot access key vehicle information. Owners across multiple years consistently report: • Black screens, freezes, and reboots • Loss of HVAC control and dual-zone functions • Backup camera not displaying • Vehicle/Apps tabs failing to open • Uconnect error messages • Smaller, harder-to-read gauges after updates • Wireless CarPlay/Android Auto failures • No option to decline or roll back OTA updates These failures distract the driver, remove access to federally required equipment (rear camera), and eliminate visibility and climate controls that directly affect safe operation. The system becomes unpredictable and unstable despite the vehicle being in motion. My vehicle experiences the same issues, including black screen while driving, loss of HVAC, camera not available, system rebooting, etc. Because this affects essential safety-related controls and visibility, I believe this is a safety defect in Ram/Stellantis Uconnect 4 and Uconnect 5 systems. I request NHTSA open a defect investigation and require Ram to correct these failures.Read more

They have stated that failure is internal, and is not caused by anything external The failure was not caused by an accident, environmental exposure, or improper use. This appears to be a component-level defect within the 2021 Tesla Model Y battery packs Considering Tesla has issued prior service bulletins for Model Y high-voltage battery quality issues in adjacent model years, this may indicate a broader pattern of battery defects. They are trying to charge people over $12,000 for battery replacements that should be covered by tesla due to poor workmanship and materials used. My Tesla Model 3 is experiencing premature suspension failures involving the lower lateral links and stabilizer links. These components were already replaced once, and they are failing again in less than 30,000 miles. This is not normal wear for suspension parts on a modern vehicle. These failures cause clunking, instability, and looseness in the front end—conditions that affect the vehicle’s
... ability to track straight and maintain control, especially over uneven pavement or during emergency maneuvers. This isn’t an isolated case. There are extensive reports from other Tesla owners across multiple models (Model 3 and Model Y in particular) showing the same premature wear on these suspension links. Many owners have parts failing even earlier. Once the car is out of warranty, Tesla charges thousands of dollars to replace these components, despite clear indications that the suspension was under-engineered for the weight of the vehicle. Tesla knows this is a recurring issue but refuses to issue a recall or goodwill repairs. A repeated failure of critical suspension links in such low mileage suggests a defect. These parts should not be wearing out this fast, and the resulting instability poses a real safety risk. I’m requesting NHTSA investigate these premature failures.Read more

I received recall notice December 2023 for defect in front passenger seat airbag that could cause the airbag to fail to deploy in the event of a crash. I have contacted the dealer several times and have been told each time that there is no remedy (yet). It has been two years and the risk to passengers in my vehicle has not been remedied. Fortunately, I have not had any collisions but the danger
... and safety concerns remain, and there has been NO ACTION BY TOYOTA ON THIS SERIOUS RECALL NOTICE The Recall Notice stills says “no remedy” and it has not been updated in the past year. This is a serious, potentially dangerous and life-threatening defect, a recall has been issued, and yet NOTHING has been done to remedy the defect FOR TWO YEARS. What remedies are available to me as a customer? Will action be taken on this complaint? Please let me know when I can expect a response.Read more

On several occasions, my Tesla Model Y has braked for no reason while using Traffic Aware Cruise Control as well as when using Autopilot. I can re-create this situation on the same parts of the highway. The sudden, uncommanded stopping creates a hazardous situation with the cars being me as I may get rear ended. At this point, I do not feel safe using Autopilot or TACC. I submitted a ticket to

My 2022 Tesla Model X has two recurring safety-critical defects that remain unresolved despite multiple repair attempts. 1. Airbag / Restraint System Failure (RCM2 / OCS Defect): The vehicle frequently misclassifies the front passenger seat. It shows a passenger when the seat is empty and activates passenger seatbelt warnings incorrectly. Recently the vehicle displayed: “Front passenger safety
... restraint system issue: Service required (RCM2_a064).” Tesla attempted to repair an electrical harness in the seat, but the problem returned. A Tesla technician documented that the seat may still need a new occupancy sensor. Because this affects the Occupant Classification System and airbag deployment logic, it raises potential FMVSS 208 compliance concerns. 2. FSD / GPS Safety Hazard (5 Unsuccessful Repair Attempts): Across five service visits, Tesla attempted to fix GPS drift and unsafe FSD routing. The vehicle still attempts incorrect turns, performs premature lane changes, and misinterprets intersections. One recent incident involved FSD attempting a left turn one intersection early, nearly causing a collision. Tesla replaced the GPS antenna, recalibrated cameras, and replaced the computer, but the unsafe behavior continues. These two defects—incorrect airbag/OCS behavior and hazardous automated-driving/navigation behavior—create a significant safety risk to occupants and other drivers. Both defects persist after repair attempts. I can provide service records, videos, and documentation upon request.Read more

I purchased this 2024 Volkswagen ID.4 as a Certified Pre-Owned vehicle, and the electrical issues began almost immediately after taking ownership. While driving, the main infotainment display will suddenly reset, freeze, or go completely blank. Sometimes the instrument cluster also flickers or freezes during these events. At the same time, I received a “12V battery limited, please drive with car
... e” warning. When the center screen resets while the vehicle is in motion, I temporarily lose access to important driving information such as the backup camera, vehicle status, sound alerts, and sometimes the speedometer. These failures create a safety concern because they remove visibility of warning indicators and essential driving data. The 12-volt system warnings also raise concern that the vehicle may lose power to critical control modules or experience a sudden electrical shutdown. The issues are intermittent but recurring, and started immediately after purchasing the vehicle CPO, which suggests the defect existed before I acquired the car. This appears consistent with other reported electrical and 12-volt system failures on the ID.4. I am reporting this because I believe the issue may be part of a broader safety defect involving the vehicle’s low-voltage electrical system, displays, or charging circuitry.Read more

Central computer (motherboard), cameras (front/rear/side), autopilot/auto steer system. Error code: APS_w132. ... ar emitted constant distracting beeping alerts due to a critical motherboard failure. This made driving hazardous—impaired visibility for parking/reversing, loss of collision avoidance features, and constant audio distractions that could lead to accidents. I had to drive in this unsafe condition for weeks while waiting for service. Tesla attempted a remote fix but it failed. I scheduled service on November 19 for December 3 (19-day delay). Diagnostics confirmed the update induced the hardware failure, a known issue reported by other 2021 Model 3 owners on forums like Reddit (e.g., software bricking motherboards post-update). Despite this, Tesla refused warranty/goodwill coverage, quoting $2,576.83 for repairs. Service advisors Josh and Andy were unprofessional: Josh was rude/condescending; Andy was aggressive, sarcastic, and retaliatory. After I mentioned filing complaints with the AZ Attorney General and News Channel 3, Andy threatened $100/day storage fees with an impossible deadline (called at 4 PM Friday, close at 5 PM, demanded pickup by morning, then noon). He closed my ticket without authorization, delaying fixes and forcing me to contact another location—prolonging my exposure to the unsafe vehicle. This defect poses serious safety risks: sudden loss of cameras/autopilot could cause crashes. Tesla's awareness (from prior reports) and poor handling (intimidation delaying repairs) exacerbates the danger. No crash/injury yet, but potential is high. Please investigate this update-induced failure as a widespread defect. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more
