“The affected system was Tesla Full Self-Driving (FSD) (Supervised). FSD correctly selected the northbound left-turn lane but, while executing the left turn to westbound Pecan Street, intermittently directed the vehicle into the eastbound lanes carrying opposing traffic.”

I recently purchased this vehicle. I love it. It has a trial of full self driving, which I love almost all of the time. However, I noticed there is not any of the 5 settings for FSD that do not speed in school zones. I want to use FSD as much as possible, but I will not speed in a school zone. I am amazed that Tesla has been allowed to program the FSD in a way that will allow speeding in a school
... zone. I have to turn off FSD to maintain safety and avoid a ticket. Please require TESLA to have at least one of the 5 settings that will obey school zone speed limits.Read more

My 2022 Tesla Model 3 Long Range has developed a failure of the Autopilot/ADAS computer. The AP computer remains installed in the vehicle and is available for inspection. The vehicle operated normally before software update 2026.26.6. Following the update, Driving Visualization and Autopilot began going offline, along with automatic headlights, automatic turn signals, emergency braking, collision
... and traffic warnings. The failures have continued despite software/firmware reinstall, DAS reset, camera calibration, and hard reset. Tesla Service inspected the vehicle and confirmed that the AP computer is defective and recommended replacement at approximately $2,600. I paid $120 for Tesla's diagnostic assessment. Service Mode has recorded repeated AP watchdog/kernel-panic events, including APS_w169_TurboA_SCS_LKUP, APS_w134_appMIA, APS_w135_appCritical, and PM_a041_dasMIA. The vehicle has also recorded simultaneous exits of multiple camera streams, sometimes causing camera feeds to go black while driving. Service Mode has shown the AP ECU in Coreboot/INIT, intermittent loss of communication between the AP ECU and infotainment ECU, missing GNSS Ethernet communication, and unavailable AP temperature telemetry. This creates a safety concern because multiple safety-related driver-assistance and camera-dependent systems can unexpectedly become unavailable while driving. The vehicle remains manually drivable, but the driver must unexpectedly operate without previously available functions. The failure can also affect automatic headlights and turn signals, requiring manual operation. I am reporting this as a potential safety defect because multiple ADAS/camera systems fail simultaneously and the AP computer repeatedly crashes/reboots. The failure began after a software update, although Tesla has stated that some alerts predated the update. Due to the GPS board contacting the autopilot first before the infotainment, precise location is off and navigation is unavailable.Read more

On approximately August 15th,2026, I experienced a serious near miss while driving my 2026 Tesla Model Y at approximately 70 mph on an interstate. I activated my turn signal and began moving toward another lane while another vehicle was in or approaching my blind spot. I did not receive an audible blind spot warning early enough to alert me before beginning the lane change. The audible warning occ
... urred only after I was already moving toward the occupied lane and the situation had become dangerous. My previous vehicle, a 2015 Hyundai Santa Fe, provided an audible warning when the turn signal was activated and another vehicle was detected in or near the blind spot. That earlier warning gave an opportunity to cancel a lane change before a dangerous situation developed. I believe the Tesla Model Y should provide an optional early audible blind spot warning when the turn signal is activated and another vehicle is detected in or rapidly approaching the blind spot. This would supplement the visual warnings and could help prevent high-speed lane-change collisions without requiring the driver to look toward a display. I respectfully request that NHTSA evaluate whether the timing and design of audible blind spot warnings in the Tesla Model Y provide drivers with adequate advance warning and whether an earlier audible alert could improve safety and save lives.Read more

I had brought my Tesla Model Y to a stop. Given that the car has an auto-park feature, I did not put the car in park. My daughter got out of the car and opened the rear passenger door. I moved towards the Tesla screen to open the rear trunk for her. As I moved, my foot accidentally hit the accelerator running the car over her foot and causing a possible fracture. She was in agonizing pain and I ha

This situation can be easily avoided by Tesla by immobilizing the car (placing it in Park) when the car has been brought to a stop or has been brought to a very low speed and ANY door is opened. Right now, immobilizing the car happens only if the driver door is opened when the car has been brought to a stop or the speed is very low.Read more

FSD routing Issue: it’s a repeatable FSD lane selection safety issue, France Avenue South to I-494. Description: when using FSD from France Avenue South onto I-494, the system turns into the westbound only lane instead of staying in eastbound lane. At the end of westbound lane, FSD performs a sudden, last-second swerve into the eastbound only lane, cutting off vehicles behind already existing
... n the eastbound lane. This has happened multiple times, including both before and after the latest update v14.3.8. Expected behavior: FSD should identify the correct east only lane well in advance and stay in it when the rout is set for east 494. Concern: the late lane change cuts off traffic and creates a collision risk.Read more

With its latest upgrade, 2026.20.6.11 with full self-driving V14 Light, Tesla no longer allows users to set a max speed. This is very dangerous, especially in school zones where the vehicle, with Full Self-Driving, consistently misreads the speed limit. There is no way to revert this "upgrade." Many people have posted about this concern, but Tesla is ignoring it.

Child entered our vehicle and was UNABLE to exit/open his door (driver's side passenger door) from inside. The child locks were not enabled. The door would also NOT open from the outside. When entering service mode, a VCLEFT_a187latchReleaseFailedR error was present. I looked up on Tesla service how to manual release the door. It states to use a fingernail or flat tool to open the plastic flap i
... n the pocket of the door handle. I was unable to open with a my fingernail and using a small screwdriver took me multiple attempts. It then took me multiple attempts (~5) of pulling the release cable for the latch to unlock. I believe I finally got it to unlock while holding the electronic latch while pulling --this is not mentioned in the service manual (unsure if this step is missing or it just was extremely stuck/luck that it finally unlatched when I did this). Once the latch was released, this error and all history of it was not even logged in the service alerts section. Made a service appointment, however, first available is not for >20 days. I also have a video of us trying to open the door multiple times but I can not upload it here.Read more

I am submitting this safety complaint regarding premature suspension failure on my 2021 Tesla Model Y (VIN: [XXX]), specifically concerning the front lower suspension links.During a dealer inspection at a Tesla Service Center on XXX, with 72,758 miles on the odometer, technicians diagnosed that the vehicle’s front lower lateral links and front lower compliance links suffered from severely torn f
... actory air-gap rubber bushings. The degradation of these critical structural components required immediate, full replacement of the entire arm assemblies to ensure safe operational handling, resulting in an out-of-pocket repair quote of $1,976.57 (Tesla Invoice #XXX). A compromised or torn bushing in the lower lateral link assembly compromises structural integrity, increasing the risk of mechanical detachment or fracture while in motion, which poses a severe loss-of-control safety risk at highway speeds. This issue directly correlates to the high failure rates and premature degradation profiles currently being investigated by the Office of Defects Investigation under active NHTSA Probe PE26006. I request that this case data point be reviewed as further evidence to transition this active preliminary evaluation into a mandatory, formal safety recall for the protection of all affected owners. For over the past 18 months, Tesla FSD software deliberately removed the driver’s ability to control following distance, instead replacing it with fuzzy descriptions of “mad max, hurry, standard, chill, and sloth.” As a result the car cannot maintain a safe highway following distance as determined by the driver. When in Hurry and or Standard mode, the car will routinely reach speeds of 75
... to 80 mph and decrease following distance to about 3 car lengths, with no car for more than 20 car lengths behind. When a sudden slowdown occurs, the tesla reacts slowly, often decreasing the following distance to 1 car length, as the car behind closes and misjudges the deceleration of the Tesla, resulting in a potential 3 car collision. Adjusting from hurry to standard, or even down to chill or sloth mode does not bring the following distance back to the driver’s education rule of thumb - 1 car length per 10 mph. About 3-10 seconds after the mode change, the software forces an unexpected lane change into a slower lane. The only solution is to disable all driver assistance measures, coast to the necessary following distance and then re-engage. The cycle then repeats. Sometimes when there are nearby cars blocking lane changes, the car remains at 1 car length following distance (for up to 7 seconds after changing from hurry to sloth mode), with the car behind at 3 car lengths while remaining at 70 to 75 mph. Software versions include the latest: 2026.21.6, with the HW4 autopilot computer. This is completely a vendor chosen software defect which could easily be addressed by new defaults, or with simple software changes. The elegant solution is to use the autopilot control scroll wheel. Presently, it takes 2 “detents” to change between modes, if turning one detent downwards would allow driver to increase following distance, that would be sufficient. Alternatively within the Byzantine onscreen menus, an option could be added, but that would be unsafe to adjust while driving.Read more

My 2023 Tesla Model S has a horn activated by a small button on the right side of the steering wheel rather than by pressing the center of the steering wheel. During a recent dangerous traffic situation, I needed to use the horn immediately to warn another driver, but I could not locate and activate the horn button quickly enough while concentrating on avoiding the hazard. No collision occurred.
... The incident occurred approximately on August 23–24, 2026; I do not recall the exact date. I reported this to Tesla as a safety concern. Tesla confirmed that my vehicle can be converted to the newer steering wheel configuration with a mechanical center horn. Tesla initially quoted $1,177.34 for the replacement. After further diagnostics, Tesla determined that the conversion also requires replacement of the driver airbag and revised the customer-pay estimate to $1,748.50. Tesla declined to cover the conversion under warranty or goodwill. After I escalated the matter through Tesla Customer Support, a Tesla Service Manager also declined the request and advised me that if I believe this presents a safety issue, I should report it to NHTSA. The existing horn is operational. My safety concern is that the location and method of horn activation can make this essential warning device difficult to access instinctively and immediately during an emergency.Read more

On August 27, 2026, the 12V/low-voltage battery in my Tesla completely failed without any prior low-voltage warning displayed on the screen or sent to the mobile app. After placing my infant/toddler in their car seat in the rear and closing the door, the vehicle's electronic systems shut down entirely. The electronic door handles and key access became completely inoperative from the outside, locki
... ng my child inside the vehicle with no way for me to open the doors or retrieve them. The manual override releases are inaccessible to a young child buckled in a car seat, and the vehicle provided zero advance warning that the low-voltage battery was at critical failure. This severe design flaw creates an immediate, life-threatening entrapment hazard.Read more

Screen provides alert that "center brake light condition deteceted". Shows up as VCRIGHT_a487. Tesla has said it is a software condition that will be fixed in an update, yet several updates later, the condition persists with no fix in sight. This error has appeared since the time of new vehicle delivery in early May 2026 to the present.

On XXX at approximately 12:56 PM Pacific on [XXX], my 2024 Tesla Model 3 VIN [XXX] was operating in Full Self-Driving (Supervised). The system detected oncoming traffic, stopped, then reversed to yield and backed into a parked 2015 Ford Transit van that had an occupant inside. I am the registered owner. [XXX] was the driver. TeslaCam saved events at 12:56:33 and 12:56:43. There was little visible
... damage, no injuries logged, and no police report. Tesla's consumer privacy form does not accept a crash narrative. I requested a Vehicle Data Report / copy of my data on XXX. USAA/[XXX] claim XXX later assigned 100 percent fault to the Tesla driver for backing, treating this as a conventional reverse collision rather than an FSD commanded maneuver. I am reporting this as a safety defect in FSD (Supervised) because the vehicle reversed into a parked occupied vehicle while the feature was engaged. 2018 Tesla Model 3, VIN [XXX], approximately 58,736 miles. Front lower suspension lateral link (fore link) separated from the subframe on the right (passenger) front. The fastener that attaches the link to the subframe backed out and is missing. The link is hanging down and the right front wheel sits at a visibly different angle compared to the left, indicating the alignment has shifted. I discove
... red the hanging component while the vehicle was parked; there was no impact or off-road use. The vehicle is not being driven and is being towed to service. This is the same failure mode addressed in NHTSA recalls [XXX] and [XXX] (Tesla [XXX]) and currently under investigation in [XXX]. My VIN is not included in either recall per the NHTSA VIN lookup. Photos of the separated link and wheel angle are attached. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

On XXX at around 3:30 PM EST, I was driving my 2023 Tesla Model 3 on [XXX] about 10 miles outside of [XXX]. I had the adaptive cruise control set at 75 mph. I was driving in the right lane of the two available lanes of travel in that direction. I noticed an unattended car on the right shoulder about a 1/4 mile up the road and continued my drive as normal. The car was at least 5' to 6' off the
... highway. As i got about 30 yards from the car, my car suddenly slammed on the brakes automatically, which forced me to react in a corrective manner. Thankfully there was nobody behind me or the severity of the braking would have likely caused a rear end collision. To my understanding, this phantom braking is a known issue with Teslas, so I wanted to document my experience to add to the record.
