The instrument cluster on my vehicle has failed intermittently and/or completely, resulting in loss of speedometer visibility and loss of critical warning indicator information. This includes safety-related alerts necessary for driver awareness, such as warning lights and vehicle status indicators. Due to this failure, the vehicle is unsafe to operate because the driver cannot reliably monitor spe
... ed or receive safety warnings required for safe driving. A repair order was opened at a Ford dealership to address this issue. The vehicle is covered by an extended warranty provider, which approved a repair request related to the instrument cluster failure on December 16, 2024. The warranty provider further confirmed approval for instrument cluster replacement on December 22, 2024. These approvals confirm the defect was acknowledged and authorized for repair. Despite the safety defect remaining unresolved, the dealership closed the repair order. Ford corporate customer service has confirmed that the repair order shows as closed on their end, even though the instrument cluster failure has not been corrected and the vehicle remains unsafe to operate. Closing the repair order disrupted manufacturer responsibility tracking and reimbursement eligibility while the safety defect persists. As a result, I have incurred out-of-pocket expenses including towing, rental vehicle costs, and related transportation expenses. This instrument cluster failure constitutes a safety-related defect under DOT and NHTSA guidelines because it prevents safe operation of the vehicle. I am requesting manufacturer accountability and corrective action to ensure this safety defect is properly addressed and tracked.Read more

All in the same day: 1) Odd sound from behind the pedal area of driver side front quarter (sounded like a styrofoam cooler top rubbing against the cooler is the best way to describe - faint though). 2)BANG when in a parking garage turning full right but still able to control without difficulty. Made it home without incident. 3)Later that day, just after getting off of I4 in Orlando I pulled into
... grocery parking lot and parked. When I returned to my car I again turned right and a loud BANG and the car jerked to a sudden halt. I was able to back into a parking spot but there were 2 large screws in the driving land and the left wheel was toed slightly to the left. Ultimately it was detached when we tried to tow it we had to remove tire and put on a device to allow us to move it. Per Tesla “I have updated that estimate! The vehicle has very heavy damage, and this is the start. Once we replace these components their (sic) could be more damage, and it also could potentially need to go to a body shop!” Thankfully this didn’t happen at speed with my daughter in the car on I4! This is unsafe and a simple internet search revealed it is not an uncommon parts failure at this mileage! This is dangerous. All of this at just 54,000 miles. (4k out of warranty)Read more

Incident date: Dec 9, 2025
Description of Incident: The vehicle exhibits a severe and unpredictable transmission "shudder," "stutter," and hesitation during gear synchronization, specifically during 1st–3rd and 8th–10th gear transitions. This manifest as a violent "shaking" sensation and a loss of momentary acceleration while merging into high-speed traffic. This issue was first reported to an authorized Ford dealershi
... p on December 17, 2024. At that time, the vehicle exhibited harsh shifting and "shuddering" consistent with Ford Technical Service Bulletin (TSB) 22-2428. Despite being under a 150,000-mile service contract, the dealer misdiagnosed the mechanical transmission defect as an engine misfire, resulting in ineffective repairs. The defect has persisted and worsened. The internal failure has been identified as a defective CDF Clutch Drum (Cylinder) where an internal bushing moves out of place, obstructing fluid flow and causing mechanical "slip." This is a known manufacturing defect documented in multiple Ford TSBs (22-2428, 23-2250, and 24-2101). Safety Risk: The failure causes unpredictable power delivery. On multiple occasions, the vehicle has hesitated or "stuttered" while attempting to merge onto highways or cross intersections, leaving the vehicle vulnerable to side-impact or rear-end collisions. The lack of consistent throttle response and the violent nature of the shifting creates a significant safety hazard for the driver and surrounding motorists. Verification/Inspection: The symptoms were reported to a Ford dealership multiple times. As of December 2025, the dealership has opened a formal case with the manufacturer to verify the internal "cause of failure" by the Shop Foreman. No warning lamps or Diagnostic Trouble Codes (DTCs) appeared on the dashboard, as the failure is a mechanical internal bushing slip that does not always trigger a software sensor until total failure occurs. The vehicle is available for inspection.Read more

The brake booster vacuum hose attached to the intake manifold fitting swelled over time and was close to detaching from the intake manifold fitting. This condition could allow the hose to detach from the fitting during an engine backfire, resulting in loss of power brake assist and increased brake pedal effort. As a result, stopping distance may increase and potentially result in a crash.

On [XXX], I purchased a new Toyota Sienna 2025 from Toyota Pasadena. The car has had frequent “check rear seat” and “check 3rd row seat” alerts everyday I drive. I have contacted Toyota Motors a few times since May, 2025, and visited Toyota Pasadena countless times to solve this error since April, 2025. After their “alarm specialist” checked my vehicle, they started to intentionally ig
... nore my repair request. It seems Toyota knows the problem but couldn’t fix it in an easy and cheap way. Toyota Pasadena always insisted on me seeing their alarm specialist OFF THE RECORD, rather than accepting my vehicle for repair, so they can avoid Lemon Law punishment. When I contacted Toyota Motor a few times, they always pushed me back to Toyota Pasadena and ignored my request to have other dealers’ or the engineers fix my Sienna. I have been trapped in this endless fraud loop wasting time for a year. I have a [XXX] daughter. With this error not being fixed, I can’t risk putting my daughter and her seat on the rear seat. I was forced to buy another SUV, which created significant financial stress along with the stress of being pushed away between Toyota Motor and Toyota Pasadena over and over again. The bottomline is I need to make sure the rear seat is safe because my family’s lives are on the line. They shouldn’t intentionally hide the safety issue and forced me to accept the risk. It’s a new car in the original condition that Toyota Pasadena handed me. Everything on this vehicle was installed by Toyota Motor and Toyota Pasadena before it was handed to me. I already requested that if it's alarm bugs they cannot fix, they can remove the faulty alarm with a refund. I only want a safe vehicle. But they rejected me. The vehicle still makes random alarms and shows the “check rear seat” today and everyday. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

While operating my 2023 Tesla Model S Long range using Full Self Driving Supervised and software version 14.2.1.25, the speed control no longer is available and has been replaced by speed profiles. This is highly unsafe and I had to repeatedly move back to Chill or Sloth mode at which time the vehicle strongly and unsafely braked causing cars behind me to close on me quickly and have to make strong and unplanned braking maneuvers to avoid hitting me. This was not the case when I bought the car and FSD when operating under 13.2.9. The removal of max speed control is extremely unpredictable and unsafe for me and those around me.Read more

The issue was confirmed by an authorized GMC dealership, which diagnosed the failed upper radiator hose and performed the replacement. This issue has also been reported by multiple owners of the same model year, indicating a recurring problem. The vehicle and failed component were inspected by an authorized GMC dealership. No inspection by police or insurance representatives was required, as this was a mechanical failure and not the result of an accident. Prior to the failure, symptoms included coolant loss and cooling system concerns, which ultimately required dealership diagnosis. Any warning indicators appeared shortly before the issue was identified, with little to no advance warning before the component required replacement.Read more

Shortly after purchase, my 2025 Ford Maverick Hybrid AWD experienced a sudden and severe electrical burning or melting odor while driving at freeway speed (approximately 75 mph). The odor was extremely strong and clearly coming from my vehicle. At the same time, the vehicle produced a loud, continuous whirring or growling noise and experienced a sudden loss of power, as if under heavy load or brak
... ing. I exited the freeway as soon as safely possible. No visible smoke or fire was observed, but the odor raised serious concern for overheating electrical or hybrid drivetrain components Since the initial incident, the vehicle has experienced multiple similar but less severe events, including intermittent electrical odors, abnormal hybrid noises, hesitation or lag when switching between electric and gasoline operation, and a significant and permanent reduction in fuel economy. The vehicle has been taken to a Ford dealership multiple times for inspection. I requested inspection for signs of overheating or electrical damage. No diagnostic trouble codes were found and the vehicle was returned without repair. I am concerned this condition may involve overheating or failure of hybrid or electrical drivetrain components, such as the inverter, electric motor, or wiring, which may not immediately trigger diagnostic trouble codes but could pose a safety risk. I am also aware of other 2025 Ford Maverick Hybrid owners reporting similar electrical odors, abnormal hybrid noises, and power loss symptoms in public owner forums, suggesting this may not be an isolated incident. I am submitting this complaint to document a potential safety-related defect involving overheating or electrical failure in the hybrid drivetrain system.Read more

While operating my Tesla using the Auto Park function (on 12/8/2025 between 8:45 am and 9:00 am) , which is part of the Full Self-Driving (FSD) system, the vehicle initiated an automated parking maneuver. During the maneuver, the vehicle was reversing into a parking space when it failed to detect a stationary wall located behind the vehicle. The vehicle continued reversing without issuing any vis
... ual or audible collision warnings and without performing automatic braking. The Auto Park system did not disengage or request driver intervention prior to impact. The vehicle subsequently collided with the wall, resulting in damage to the vehicle. Based on the vehicle behavior, it appears that the object detection and/or obstacle avoidance functions—relying on onboard cameras and proximity sensing—did not properly recognize the stationary obstacle during the automated maneuver. Following the incident, I contacted Tesla to request a review of vehicle telemetry and system logs to determine whether a fault existed within the Auto Park software, perception system, or associated sensors. Tesla advised that a service diagnostic fee would be required solely to access and review the logs, despite the incident occurring during use of an automated driving feature.Read more

I own a 2022 Nissan LEAF (VIN [XXX] ) that is subject to a Nissan safety recall advising owners not to use DC fast charging due to risk of battery overheating and fire. DC fast charging is a core function of an electric vehicle. Nissan’s instruction to avoid fast charging effectively removes this capability. In addition, some public charging networks have restricted or blocked Nissan LEAF fast
... charging, further limiting safe operation. Nissan has not provided a permanent repair. A proposed software update does not address the underlying physical battery risk and does not restore full charging functionality. As a result, the vehicle cannot be used as intended, and owners are left with reduced charging capability and increased safety uncertainty. I believe this defect presents an ongoing safety risk and warrants regulatory review. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Vehicle: 2018 Tesla Model X Recall Reference: Tesla eMMC Recall – NHTSA Campaign Number 21V-035 (Recall 21V035000) Defect Description: In March 2021, Tesla notified me by email of a recall involving failure of the eMMC memory chip in the Media Control Unit (MCU). This defect is known to cause loss of vehicle system functionality and safety-related features. Despite this notification, Tesla has r
... efused to honor the recall and replace the eMMC when I brought my vehicle in for service. Current Vehicle Condition: My vehicle exhibits severe and worsening symptoms consistent with eMMC failure, including: System freezing and frequent unresponsiveness GPS extremely slow or nonfunctional Turn signal indicators intermittently fail Horn fails to operate approximately 95% of the time Vehicle reset/reboot does not work Touchscreen and vehicle controls are very slow or nonresponsive These issues occur regularly and without warning. Safety Risk: This defect creates a significant safety hazard. The failure of turn signals, horn, and system responsiveness increases the risk of accidents. The inability to reset or restore the system while driving further endangers occupants and other road users. Due to these failures, the vehicle is unsafe to operate. Manufacturer Response: Although Tesla issued a recall notice in March 2021, they have declined to perform the recall repair on my vehicle. No effective remedy has been provided, and the vehicle’s condition continues to deteriorate. Request for NHTSA Action: I am requesting that NHTSA investigate Tesla’s failure to honor Recall 21V-035 and require Tesla to provide the mandated repair. This unresolved defect affects safety-critical systems and presents an ongoing risk. When I went there last Dec 4 to get this repaired, they wanted me to upgrade and pay almost $2000. I am willing to pay half of the cost but not the entire amount.Read more

I brought my 2024 Sierra in for its L87 recall. It passed. Two weeks later I had to bring it in for check engine light. They reset the system under a TSB that is known to occur with the l87 recall. Not a total of 300 miles of passing the recall I was driving and the engine started making a horrible noise and I started experiencing loss of power. I managed to get the vehicle safely parked. The deal
... er pulled the oil filter and said it had 100% metal contamination in the oil. The filter looked like a glitter bomb went off. It took 3 weeks, they replaced the engine with a remanufactured engine even though I told them new only. They also had to replace the radiator as it was polluted with metal.Read more

A structural chassis component (V-brace) was permanently removed from my vehicle by Tesla Service during a service visit. The removal was performed under a Tesla-described engineering retrofit (Tesla Service Article 9188600). The component was not reinstalled or replaced, and Tesla stated that once removed it cannot be reinstalled. The service visit was originally initiated due to a persistent rat
... tle/noise from the vehicle The safety concern arises from: •A permanent modification to a structural chassis component, performed without my authorization. •Removal of a component originally installed for structural reinforcement and stability, creating uncertainty regarding vehicle rigidity, crash behavior, and handling. •The continued presence of an unresolved structural noise/rattle, which can be distracting to the driver and may indicate an underlying structural or mechanical issue. The vehicle was sold and delivered with this component installed, and its removal was not disclosed or approved prior to service. The noise/rattle was present prior to the service visit and prompted the service appointment. The structural component removal occurred during the service visit. The problem was reproduced and confirmed by Tesla Service and they: •Acknowledged the noise/rattle concern. •Performed a structural retrofit involving removal of the V-brace. •Marked the concern as “resolved”, despite the noise still being present. •Later stated the noise is considered “normal”, even though it is not present in all Model Y vehicles, including the Juniper variant. No warning lights or system messages appeared. The only symptom was a persistent rattle/noise originating from the vehicle chassis The vehicle was inspected and modified by Tesla Service, acting on behalf of the manufacturer. The modification was performed under a Tesla engineering directive. No independent service center, insurance representative, or law enforcement agency has inspected the vehicle.Read more
