Date: December 28, 2025 Time: Approximately 3:25 PM Vehicle: 2026 Tesla Model Y Incident Location: Southbound on the I-35N feeder road, just north of Highway 114. Safe Harbor Location: Burger King (15925 N Hwy 114, Northlake, TX) Statement of Facts: I was driving my 2026 Tesla Model Y southbound on the I-35N feeder road at approximately 3:25 PM today. I was traveling at a steady speed of 55 MPH. W
... ithout any warning chime or dashboard notification, the front hood (frunk) suddenly and violently flew open. The hood slammed into the windshield, shattering the glass across my entire field of vision and completely obstructing the road ahead. I was forced to perform an emergency "blind" stop on the feeder road. Once the vehicle was under control, I drove at approximately 10 MPH with zero forward visibility to reach a safe location off the main road. I pulled into the Burger King parking lot located at the intersection of I-35 and Highway 114 to inspect the damage and call for assistance. Key Safety Failures: • Software Failure: At no point before the hood flew open did the vehicle alert me that the frunk was unlatched. • Mechanical Failure: The latching mechanism failed to hold at standard highway speeds, creating a life-threatening visibility hazard. • Structural Damage: The windshield is fully shattered, and the hood and hinges are visibly warped from the force of the impact. Conclusion: This incident was a catastrophic safety failure. Given Tesla's history of recalls for this exact issue (NHTSA Recall 24V-554), it is clear that this 2026 model either suffers from the same defect or the previous "software fix" is insufficient to protect drivers.Read more

At approximately 10:30 a.m. on 12/28/2025, while traveling on the highway, the panoramic sunroof of my 2019 Ford Edge SEL spontaneously exploded without warning. The failure was accompanied by a sound similar to a gunshot. The weather was clear and sunny, with an ambient outdoor temperature of approximately 20°F. The vehicle had been pre-warmed, and the interior climate control was set to 73°F.
... There were no vehicles immediately in front of me, no overhead bridges, and no evidence of road debris or external impact. Upon the failure, the tempered glass shattered completely, the majority of the glass was forced into the passenger cabin, landing primarily in the backseat. This created an immediate safety hazard, distraction, and risk of injury from glass debris while maintaining vehicle control at high speeds. There is no "impact crater" or point of origin suggesting a rock chip as the front portion of the roof shattered completely; the glass appears to have failed due to internal tension or thermal stress. I am reporting this as a structural defect of the Vista Roof glass assembly.Read more

Date & Time: December 25, approximately 6:00 PM Location: Northbound I-880 between the Mowry Avenue and Thornton Avenue exits, Fremont, CA Weather & Road Conditions: Heavy rain at the time of the incident. Road surface was wet with reduced visibility. Vehicle & System State: Tesla Model Y. Driver assistance / self-driving functionality was active at the time of the incident (traffic-aware cruis
... e control / Autopilot functionality). Description of the Incident: While driving northbound on I-880 in heavy rain, the vehicle had been operating normally. Without warning, the vehicle appeared to incorrectly detect that it was drifting out of its lane and attempted to correct this perceived drift. Instead of a minor lane correction, the vehicle abruptly applied heavy braking and initiated a sudden and aggressive left steering maneuver. The car crossed approximately four lanes of traffic in the same direction of travel and continued rotating, ultimately performing an unintended U-turn while remaining on the northbound side of the highway. As a result, the vehicle came to a stop oriented against the direction of travel, facing oncoming northbound traffic. Immediately afterward, the vehicle unexpectedly began reversing at high speed without driver input, creating an additional imminent risk of collision with the outer highway wall (noise barrier separating the highway from adjacent residential areas). During this entire sequence of events, I was unable to regain control of the vehicle despite attempting to intervene. There were no vehicles immediately surrounding us at that moment. Had there been nearby traffic, this incident would likely have resulted in a severe or fatal multi-vehicle collision. Occupants: My wife and two young children were in the vehicle. The children were asleep in the back seat, secured in car seats. Outcome: No physical collision occurred. However, the incident caused extreme fear and distress, and we remain shaken by the event.Read more

My brakes went out completely while driving my 2015 kia soul with my daughter in the car. It was traumatic and stressful for us both. I had just received a recall notice for break lines/ undercarriage spray. I called Fairfield Kia and I had my car towed to the dealership. They inspected my car and said they would replace the rear brake lines under the recall. A couple weeks went by and I was told
... they couldn't find the parts to fix it and they are going to cover the front break lines as well under the recall. I didn't receive a loaner car and transportation was a daily struggle. Altogether my car was at the dealership for around 35 days. I received an invoice through email and didn't get a call or any notification that my car was ready for pickup. I called the dealership 2 days later and the person said it was ready. I get to my car and it was dead and I had to jump my car in the parking lot. The cover for the under carriage was not on my car where is goes it was in my back seat and it was dirty. My gas light was on and I had a quarter of a tank when I paid to tow it to the dealership. My receipt said my car was fixed and in working order and they completed a check point inspection and besides the battery they said it was all good. I drove my car to my house and by the time I got to my house I noticed my breaks were going all the way to the floor. My car still has no breaks just as it did when I had it towed to the dealership. It feels like no work has been done. I've already been without my car for over a month to fix this problem under a recall. The dealership was rude and not professional and I notified Kia America through email but because the Christmas holiday I haven't heard back yet. I'm scared to drive it now and it has created a fear I can't shake. Im not sure what to do next but I'm going to call Kia corporate in the morning.Read more

While driving approximately 5–10 mph and making a right turn on a local street, the vehicle suddenly produced a metallic pulling sound followed by a loud impact. The vehicle abruptly stopped and became undrivable. Upon exiting the vehicle, the left front wheel was visibly displaced from normal alignment and appeared to have shifted rearward into the wheel well. No collision, curb strike, pothole
... , or road debris was involved. The vehicle had exhibited a wire-tugging or metallic noise during turning in the days prior. The failure occurred without warning and resulted in a sudden loss of vehicle control at low speed. The vehicle was towed to Tesla, where the left front control arm was found to be disconnected/broken. Tesla has not yet provided a determination and is attempting to route the vehicle to a collision center despite no evidence of impact. This appears to be a suspension component failure that could have resulted in a serious accident at higher speeds. Safety Impact: Loss of steering control, risk of crash, inability to drive vehicle.Read more

Vehicle Breaking on Its Own While Driving I am writing to file a formal safety complaint and to demand immediate corporate intervention regarding an extremely dangerous mechanical/ADAS defect. My Nissan Altima has serious, ongoing safety issues where the vehicle will unexpectedly and aggressively apply the brakes on its own while I am driving, including on highways and regular roads. This defect h
... appens without my input, without warning, and creates a major risk of being rear-ended, losing control of the vehicle, or causing a serious accident. This issue is Not minor. It is a life-threatening malfunction that makes the vehicle unsafe to operate. I have taken the vehicle to authorized Nissan dealerships for diagnosis and repair on multiple occasions & originally they told me it should be under warranty & when they spoke to the dealership they said it did not qualify under the warranty nor the extra insurance purchased & i'd have to come out of pocket for repairs. A vehicle that breaks on its own, especially at highway speeds – is not safe, not reliable, and not roadworthy. This situation has gone on for years and the vehicle is now a clear danger to me and the public and my three minor children who ride in the car with me. I am requesting immediate action and a good-faith resolution.Read more

Complaint Type: Vehicle Safety / Potential Defect / Dealer Negligence Description of Complaint: I am reporting a serious safety issue resulting from negligent service performed by Kocourek Wausau Imports. Only after my insistence did they admit a mechanical issue involving the cam/valve area, stating a plug required replacement. The part was on backorder, and I was not warned of any risk. Shortly thereafter, I discovered the engine was completely out of oil, despite recent dealer service. A vehicle of this model and age should never lose its entire oil supply immediately after a service, creating a serious risk of engine failure, fire, and exposure to toxic fumes for the driver and passengers. This incident represents: -Grossly negligent maintenance -Returning a vehicle unsafe to operate -Exposure to harmful fumes inside the cabin -Potential permanent engine damage due to oil starvation Requested Action / Resolution: -Investigation into dealership practices and potential vehicle defects -Determination of whether this constitutes a safety-related defect -Notification of corrective action to prevent recurrence -Documentation that the vehicle is safe to operate before further use I am submitting this complaint to formally document the endangerment and unsafe condition, which may affect other vehicles serviced by this dealership. I am writing to formally report a serious safety incident involving a Tesla vehicle and to document multiple critical system failures that occurred during an accident, resulting in physical injuries to me. On the 12/24/2025, while operating Tesla under rainy conditions, I experienced a sudden and dangerous malfunction of the vehicle. During the event, the steering system became unresponsive and

These failures significantly impaired my ability to avoid or mitigate the collision. Most concerning, the vehicle’s airbag system did not deploy during the impact. Given the severity of the collision and the resulting injuries, the lack of airbag deployment represents a serious failure of a critical safety system designed to protect occupants in such events. As a direct result of this incident and the apparent malfunction of multiple safety systems, I sustained physical injuries that required medical evaluation and treatment. Beyond the physical harm, the incident caused significant emotional distress and raised serious concerns about the safety and reliability of the vehicle. I am submitting this complaint to formally document these safety failures and to request a thorough investigation into the performance of the steering, braking, and airbag systems involved in this incident. I believe this matter presents a significant safety concern not only for myself but also for other drivers and passengers who rely on these systems to function properly in emergency situations. On the 12/24/2025, while operating Tesla under rainy conditions, I experienced a sudden and dangerous malfunction of the vehicle. During the event, the steering system became unresponsive and
... appeared to lock, preventing me from maintaining control of the vehicle. Simultaneously, the braking system failed to respond as expected despite my attempt to apply the brakes. Please confirm receipt of this complaint and advise me of the next steps in your investigation process. I am prepared to provide additional documentation, including medical records, photographs, vehicle data, and incident details, upon request. Thank you for your attention to this serious matterRead more

Hello, my 2019 Hyundai Tuscan I have owned for 3 years-4 years. Unfortunately it is a GDI Motor, it uses oil so bad, even though I do change the oil every 3,000 miles, and do every maintenance on the vehicle that is told to me done, ever time I change the oil it is low, even though I top it off through out the 3,000 miles. I have no leaks at all and it is just evaporating, I do not think this is r

Was driving on the highway, all of a sudden showed a sign of transmission malfunction and started decelerating which I could safely navigate the vehicle to the side of the road, got the vehicle towed to repair shop which they reported a transmission code P17F900- parking lock mechanical malfunction with codes P074700 and p077700 I am just out of warranty for this vehicle as I crossed 48000 miles.
... They have issued bulletins regarding this problem but no recall or extended warranty on that part this I deem as dangerousRead more

While operating my Tesla Model Y in a commercial parking lot at a T-intersection controlled by a stop sign, the vehicle unexpectedly and automatically accelerated without driver intent. The incident occurred during a low-speed maneuver. Upon the unexpected acceleration, I immediately attempted to regain control by applying the brake pedal repeatedly and firmly. Despite clear manual brake applicat
... ion, the vehicle did not decelerate as expected and continued to move forward. I also attempted to stop the vehicle by pressing the Park button, but this action did not stop the vehicle. The vehicle continued uncontrollably for approximately 150+ yards, traveling over curbs, striking a fence, and ultimately colliding with the garage structure of a residential home. Throughout this sequence, the vehicle remained unresponsive to braking and stop inputs. Vehicle data later confirmed that brake pedal application was detected prior to impact, yet braking response was insufficient to prevent continued motion. At the time of the event, the vehicle recorded system fault states, and collision avoidance systems did not activate. Despite significant structural impact and property damage, airbags did not deploy. The incident raises serious safety concerns regarding: •Brake-by-wire system execution during fault conditions •Pedal input arbitration between accelerator and brake •Failure of collision avoidance systems to intervene •Failure of restraint systems to deploy during a severe impact •Vehicle behavior when control systems are in a fault state This event occurred without warning and despite appropriate driver response. Based on vehicle data and system behavior, this appears to be a system-level failure rather than driver error.Read more

What component or system failed or malfunctioned, and is it available for inspection upon request? The engine and/or powertrain system catastrophically failed while the vehicle was in motion on the freeway. The vehicle lost power, shut down, and became disabled. The vehicle is currently in the possession of the Buick dealership and is available for inspection upon request. How was your safety or
... the safety of others put at risk? The failure occurred while driving my husband and children in fast-moving freeway traffic. The vehicle suddenly lost power and shut down, forcing us to coast to the shoulder while traffic was moving around us. This created an extremely dangerous situation that could have resulted in a serious crash, injuries, or fatalities to my family and other motorists. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. The Buick dealership confirmed the engine failure and advised that the vehicle needs a replacement engine. The vehicle was towed directly to the dealership following the incident and was declared inoperable. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? The vehicle has been inspected by an authorized Buick dealership service department. The manufacturer (GM/Buick) has been notified, and a corporate case has been opened. Were there any warning lamps, messages, or other symptoms of the problem prior to the failure, and when did they first appear? Yes. Multiple warning alerts appeared immediately before the shutdown, including critical warnings such as low oil pressure and instructions to turn the vehicle off. Moments later, the vehicle lost power completely and shut down.Read more
