Component or system that failed or malfunctioned, and availability for inspection The torque converter and transmission system in my 2020 Ford Edge have malfunctioned. The vehicle experiences transmission shuddering/stuttering during normal driving, particularly during low-speed acceleration and gear changes. The vehicle remains in my possession and is available for inspection upon request. Safe
... ty risk The transmission stuttering causes hesitation, jerking, and unpredictable power delivery while driving. This creates a safety risk, especially when accelerating from a stop, merging into traffic, or driving at low speeds in congested areas. The lack of smooth and reliable acceleration increases the risk of rear-end collisions or loss of control, putting both myself and other drivers at risk. Confirmation or reproduction of the problem The issue has been consistently reproducible during normal operation of the vehicle. The symptoms have been observed repeatedly and were confirmed by a dealer/independent service center, which identified the torque converter and transmission as the source of the problem and advised that significant repairs or replacement would be required. Inspections performed The vehicle has been inspected by a dealership and/or independent repair facility. The manufacturer has not yet directly inspected the vehicle beyond dealer diagnostics, but the vehicle is available for further inspection if requested. Warning lamps, messages, or symptoms prior to failure There were no immediate warning lamps at the onset of the issue. The primary symptoms were transmission shuddering, stuttering, and rough shifting, which gradually worsened over time. These symptoms first appeared during regular driving and became more frequent and severe, indicating progressive failure of the torque converter and transmission system.Read more

The electronic power steering rack failed. We have had consistent temperatures near 0°f, with highs rarely above freezing for the past couple weeks. The motor and circuit panel for the steering rack built up enough water to freeze solid and completely lock the steering, luckily in the yard so I didn't have to go get my wife from work. But I've had to sideline my work both to work on it, and so my
... wife can take my truck to her job as this is her truck. I thawed the unit and re sealed the seam by the motor, as well as the cover over the circuit panel. Some part of it appears to have shorted out as it won't work now that it's been thawed, and is giving a message on the dash to service power steering. No prior issues, or warnings that something was failing. Glad it didn't fail last week when my wife and one year old daughter were on the road visiting family hours away. I can barely afford to buy a vehicle any more, never mind take one to a dealer or other shop for repairs. Plus I'm an hour from any of them, and at least 2 weeks wait to get in. I did try to get an answer from a couple of them. I do all my own repairs. I just can't fathom how putting sensitive electrical components underneath a 4wd truck and relying on a half hearted dab of silicone to seal them against the elements is a good idea. What was wrong with good old hydraulic power steering. I'm waiting for a cost and timeframe to get a replacement currently.Read more

After loading our two children ages 2 and 5 in to vehicle and securing in car seats, the car doors were closed and would not reopen. The children were unable to open the doors themselves. The trunk and other entries did not open. There was no obvious way to open the doors or the issue the car was experiencing. The battery cover would not open. Eventually after I searched the Internet I determined
... that the 12V battery died (no warning whatsoever, doors worked before loading children) and this caused all occupants to be detained in vehicle as the emergency release is the mechanical means of opening the door and this emergency release is not intuitive or obvious. Within 20 minutes I was able to use an arcane hot wiring method to unlatch a door with a spare 12 V source and a hidden 12 V port not immediately obvious either. Thankfully not in setting of an emergency, not a sunny or excessively warm day and had internet access to research this event. HI, Vehicle is under a second recall for the High Voltage Battery potential fire hazard. The first recall lasted 7 months before a potential fix. It is now down to around a range of 20 on full charge with an actual driveable of about 16. I have asked both Ford and delivering dealership to replace the High Voltage Battery pack as it has seriously deteriorated. Ford refuses to allow dealer to replace the Battery Pack. The vehicle is basically unusable for it's original intended purpose as a partial electric vehicle. I asked Ford, what am I supposed to do if I am transporting my two grandchildren in the vehicle and it starts on fire. How do I choose which child to get first? No response. These vehicles should be removed from use by Ford and loaner vehicles provided or a full buy back should take place. I only have 11,000 miles on this vehicle and it has been in the shop for more than 30 days for various warranty repairs and multiple recalls. These vehicles are not safe. In the months prior to the failure, traction control warning indicators intermittently appeared on the dashboard. I did not recognize their significance at the time a
... nd did not document them, but they may have been early warning signs of the current operational failure. Approximately 22 days prior to the vehicle becoming completely inoperable, it underwent a recall update, oil change, and tire rotation at Joe Machens Nissan. Staff reported no concerns at the time. Shortly afterward, the vehicle began exhibiting reduced driving function and erratic power, which eventually led to complete inoperability. On [XXX], while commuting approximately 40 miles at highway speeds (up to 70 mph), the vehicle exhibited unpredictable engine response. The RPMs fluctuated erratically, and the vehicle would not accelerate beyond 70 mph. There were no dashboard warning indicators at the time. The vehicle is currently in the care of Joe Machens Nissan pending evaluation by Nissan Consumer Affairs. I am submitting this complaint to document potential safety concerns and to contribute to investigations regarding this vehicle model, including similar complaints reported by other 2021 Nissan Versa owners. Hyundai - DAW - Driver Attention Warning / Forward Attention Warning System This issue warrants immediate consideration for a recall. Despite wearing glasses, sunglasses, or no glasses, the problem persists. I occasionally remove my glasses while driving, and the warning continues to sound. This occurs regardless of the time of day, weather conditions, or whether I am sitting upright or reclining
... . The warning randomly activates and deactivates, sometimes without any intervention. Occasionally, I simply allow the warning to sound without taking any action, and it remains active for a few seconds or until it triggers a warning that the system will be turned off, at which point it deactivates. I have attempted various troubleshooting methods, such as keeping my eyes open, adjusting my seating position, wearing and removing my glasses, and tilting them to minimize glare. None of these methods have proven effective in preventing the warning from activating or in making it deactivate. This is a persistent problem. Hyundai dealerships initially informed me that the warning might be an annoyance and their response was to simply refrain from using it. However, if a basic feature like cruise control in a 2024 & 2025 model is rendered unusable due to a design flaw (which this certainly is), it constitutes a clear violation of safety standards and should be addressed through a recall. The incessant alerts are highly disruptive and can be quite frustrating. For instance, driving with an arm in the 12 o’clock position triggers a warning, while wearing polarized sunglasses also results in an alert. The presence of a setting to disable the warning is a welcome feature, but the system’s persistent alerts can be overwhelming. The alerts continue even after taking a break. It is distracting and a safety hazard. Drivers should be able to adjust the sensitivity of the feature to turn off this feature. Currently, there is no way to disable or temporarily disable this feature.Read more

To Whom It May Concern, I am submitting this complaint to report a serious and potentially dangerous safety defect affecting my 2021 Tesla Model 3 Performance. After exposure to heavy rain or wet conditions, the vehicle’s horn completely stops functioning. The failure occurs regardless of the activation method—neither pressing the steering wheel horn nor using the horn via the mobile app pro
... duces any sound. The horn remains nonfunctional even though the vehicle is otherwise operable. This issue has occurred after the vehicle becomes wet due to rain and appears to be related to moisture intrusion or an electrical/design flaw. Once the failure occurs, the driver fully loses the ability to use the horn for warnings or emergency signaling. This is a critical safety concern. The horn is a federally required safety device and is essential for alerting pedestrians, cyclists, and other drivers to avoid collisions. Losing horn functionality significantly increases the risk of accidents, especially in urban driving, highway merging, and emergency situations where immediate warning signals are necessary. This issue is not isolated to my vehicle. Numerous Tesla owners have reported the same horn failure after rain or wet conditions on online forums and owner communities, indicating a widespread and systemic defect rather than isolated wear or misuse. Given the safety implications and the volume of similar reports, I respectfully request that NHTSA investigate this issue to determine whether a design or manufacturing defect exists and whether corrective action or a recall is warranted. Location: Intersection of [XXX] and [XXX] Lakeville, MN Date/Time: [XXX], at approximately [XXX] Vehicle: 2025 Kia K4 (911 call placed at 10:08 am) On [XXX], while traveling toward the roundabout at [XXX] and [XXX], my vehicle experienced a sudden and violent unintended acceleration. Despite having my foot completely off the gas pedal, the engine began to accelerate the vehicle rapidly on its ow

As the speed increased, the vehicle began to swerve, and I realized I was losing control while rapidly closing the distance between myself, the roundabout, and the vehicles ahead of me. Fortunately, the right lane was clear. Because the vehicle was already veering in that direction and the brakes were failing to slow the car, I made a split-second decision to steer into a large snowbank to avoid a high-speed rear-end collision. The force of the impact with the snowbank was substantial, resulting in the front body panel and a lower front component being torn off the vehicle. The Lakeville Police and Fire Departments responded to the scene and were required to dig the vehicle out of the snow pile. I have attached the official police report to this statement. Had I not maintained a safe following distance or made the decision to steer off the road, the result would have been a catastrophic multi-vehicle collision. ... of this failure in scheduling and coordination: –I was required to rent a vehicle unnecessarily. –No loaner vehicle was available, despite recall instructions advising customers to ask dealers about alternate transportation. –I incurred rental charges, mandatory fees, fuel costs, and extra travel time that would not have occurred absent the dealer’s error. –I also experienced additional personal hardship, including having to transfer a child car seat between vehicles and adjust family obligations, such as waiting at a medical appointment, due to the unexpected need to return and manage the rental vehicle. The manufacturer’s customer care has limited reimbursement strictly to a standard policy cap and has declined to consider dealer-caused hardship or goodwill review, despite conflicting written guidance regarding reimbursement limits. This complaint is about whether recall compliance processes adequately protect consumers from avoidable hardship caused by dealer scheduling failures during mandatory safety recall repairs.Read more

I own a 2016 Audi A6 3.0T. After filling the fuel tank completely, the vehicle only traveled approximately 190–200 miles before the fuel gauge indicated empty. This represents a sudden and significant loss of fuel range compared to normal operation. There are no warning lights, check engine lights, fuel smells, visible leaks, rough idle, or abnormal noises. The vehicle operates normally otherwi
... se. The issue appears to involve excessive fuel consumption or incorrect fuel management without any driver warning. This condition poses a potential safety risk because the vehicle may unexpectedly run out of fuel while driving, which could result in loss of power in traffic or on highways, increasing the risk of an accident. The issue has been observed repeatedly after refueling. At the time of this report, the vehicle has not yet been inspected by a dealer or independent repair facility, but diagnostic evaluation is planned. Based on owner research and symptoms, the issue may be related to a fuel system, engine management, or emissions-related component malfunction that does not trigger a warning indicator. No prior warning messages or lamps were displayed before or during the occurrence.Read more

The vehicle makes clicking/popping noises when steering. It starts only when turning the wheel hard one way or the other. As the problem gets worse, turning slightly is enough to make clicking noise. CV Joints/Axles have been replaced every 3,000 miles on my vehicle; around 3,000 and 6,000 miles. Honda seems to be aware of this issue, but is using the same parts to replace the bad joints every t
... ime. So it seems the problem is destined to repeat itself over and over. I am wondering when this will become a safety issue. Especially once my warranty runs out and it becomes exorbitant to replace the joints for myself.Read more

Transmission failure at 80,000 miles. This a known issue with 8F35 transmissions in these model years. Left me stranded and in danger on the roadway. The transmission has other recalls, and a TSB from Ford acknowledging the issues without recall for known failures. There were no lamps, indicators, or codes before failure.
