Vehicle dash intermittently goes out. Flickers, goes dark, or shows every warning that exists on the panel. Could be unsafe as speed, gas gage, or other warnings that provide safety are going dark. Took it to dealer in Roseville, Ca. They tried to repair it, but problem began to occur again a week later. I have a video of it, but can’t show here.

The contact owns a 2013 Kia Forte. The contact stated that while driving at 30 MPH, the brake pedal failed to operate as needed and depressed to the floorboard without warning. As a result of the brake failure, the contact crashed into the rear of another vehicle. The contact stated that both front driver and front passenger seat belts failed to retract as both she and her son were violently jerke

The whereabouts of the other driver are unknown. The vehicle was driven to an independent mechanic, where the contact was given an estimate for the repair. Neither the manufacturer nor the dealer had yet been notified of the failure. Upon investigation, the contact discovered that there were two recalls on the vehicle, NHTSA Campaign Number: 17V773000 (Service Brakes, Hydraulics) and NHTSA Campaign Number: 23V652000 (Service Brakes, Hydraulics). The vehicle was not repaired and remained in the possession of the independent mechanic, where it could be salvaged. The failure mileage was approximately 159,000.Read more

I was driving on the CA 405 SB, Long Beach, at about 6 PM 9/19/25. All of a sudden this loud screeching noise started. It was so loud that I pulled off on the left shoulder instead of driving to the first exit. I assumed I was going to find something wrapped around an axle or wheel or dragging from the ground. There was nothing hanging from the car, so I got back onto the freeway and parking the c
... ar ASAP did a more thorough inspection of the car. I got on my hands and knees looked with a the flashlight And saw nothing. I got online and found Hyundai TSB 25-01-056H explaining that IONIQ 5 built between 12.03.24 - 3.25.25 are missing a wave washer that can cause the MGDU housing to make noise. And the MGDU needs to be replaced. I should take it to a dealer to get it fixed. I left a message with the service dept at Kearny Mesa Hyundai, where I'd leased the car on June 2, 2025. I left a VM with the Service Dept requesting an appointment and the screeching noise is my car. I made a number of recordings of the noise so I could easily provide it later. The noise stopped when I got off the freeway and hadnt started again. When I took it in on 9.22.25. They said I would have to leave the car for at least two weeks before they could look at it. They wouldn't provide a loaner because my car was built in APR/2025 after 3.25.25. The SA suggested I try another dealer for faster service. I took it to Pedder Hyundai on 9.24.25. They kept it all day and "verified the complaint." My window was not fully up. I told them that this is absurd. I was told the dealer needs to verify the noise to get it fixed under warranty. On 10.19 the noise started I made more recordings & drove it straight to the dealer. They couldnt get someone to go for a ride and had to get appnt and bring it back. I took it in on 10.21 while driving there, and the noise returned. I called the Serv Rep. He heard it clearly How can I send you the recordings of the noise?Read more

Following a day trip involving sub-freezing temperatures and full depletion of the traction battery, the vehicle was plugged in at home and appeared to be charging. The following morning, I enable the preheat via the remote before leaving the house for work, and notice the CEL/MIL before starting the car. Car turns on but seems to only be accessories; no power steering. Seems like the state of charge indication kept transitioning from empty to full. I unplugged the car to see if it could still drive, but no luck. getting "Shift to Park" when I put it in drive. Switching to HOLD does not engage the engine. After plugging it in again, the car no longer receives charge and simply holds the orange charge indicator light. In an attempt to get the car into limp mode, I tried clearing the codes, but no luck. After reading the codes again in the afternoon, the following codes posted: P1E00, U2622, U2623, P1FD4, P0AFA. These codes and behavior are consistent with the following NTSB service documents, which call for the replacement of the BECM: Special Coverage N232432680 Technical Service Bulletin 18-NA-261 However, the vehicle in question does not appear to be one of the 19,000 vehicles covered under the special coverage, per GM’s warranty look-up. The vehicle has not yet been serviced for the above events, but it is assumed the BECM is failing, which is available for inspection. My safety was not put at risk this time, but it is possible for the BECM to fail in an unsafe manner. Additionally, the associated repairs are costly since the BECM is not easily accessible, requiring lifting the car and removing the entire traction battery.Read more

Incident Description (in my own words): This incident occurred in the parking lot of the [XXX] in Bretton Woods, New Hampshire. It was evening after sunset. The parking lot was dark but illuminated by overhead parking lights. My Tesla Model 3 headlights and vehicle lights were fully on. I was reversing slowly out of a tight parking space, traveling approximately 2–3 miles per hour. I was inten
... tionally moving cautiously. My attention at the time was focused on the front right corner of the vehicle, as clearance there was tight. While reversing, the rear of my vehicle struck a parked truck behind me. The truck was relatively high (its headlights were positioned high on the vehicle). Neither I nor my passenger heard any rear collision warning, cross-traffic warning, or parking alert prior to impact. The vehicle also did not apply automatic braking. The contact occurred at very low speed and resulted in only minor cosmetic damage (a small dimple/deformation), but I was surprised that no warning or braking intervention occurred. Based on prior experience driving this vehicle, the collision and parking warning systems are typically configured to warn early, and I had confidence that a warning or automatic braking would activate in this scenario. Following the incident, I conducted a simple test by placing cones in front of the vehicle to see whether forward collision warnings would activate. They did not. While I understand cones may not trigger frontal collision alerts, this further contributed to my concern about object detection reliability in low-speed scenarios. The vehicle’s cameras were clean and unobstructed. I regularly clean the camera lenses. I am filing this complaint because my confidence in the vehicle’s collision warning and automatic braking systems has been shaken. I recently submitted a similar complaint regarding a collision involving our Tesla Model Y, and these two incidents—on two different Tesla vehicles—raise concerns for me INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACTRead more

Incident Description (in my own words): We were driving southbound on [XXX] from New Hampshire back to Boston in our Tesla Model Y. Traffic became increasingly congested and slowed to a crawl. My wife was driving, and I was in the front passenger seat working on my laptop. As traffic built up, my wife became momentarily distracted and did not apply the brakes in time. I noticed, in my peripheral
... vision, the vehicle ahead of us—a truck—suddenly filling the windshield. At that point, I heard the forward collision warning alert, but it activated extremely late, essentially at the moment of impact. The vehicle did not automatically brake or stop, despite the low speed, close proximity, and clear visibility of the truck ahead. We collided with the truck in front of us. Damage was minimal (license plate damage and a small dimple on the front of the vehicle), but the safety concern is significant. Both of our Teslas are configured with forward collision warning set to “Early.” Under normal circumstances, this alert triggers frequently, sometimes conservatively. In this incident, however, the warning activated far later than expected, and there was no automatic braking intervention prior to impact. The front camera appeared clean and unobstructed. I am filing this complaint because this behavior undermines my confidence in the vehicle’s collision avoidance and automatic emergency braking systems. I am also filing a similar complaint for our Tesla Model 3. I am now anxious about whether the software will perform as expected in situations where driver attention lapses briefly—precisely the scenario these safety systems are designed to mitigate. I am requesting that NHTSA review this issue, as the system did not behave in a manner consistent with its intended safety function. The vehicle underwent a major warranty repair where the air suspension compressor was replaced due to a stuck exhaust valve and damaged air line, followed by recalibration. Despite this, suspension overheating warnings and intermittent system failure notifications continued to appear. On December 23, 2025, while driving at moderate speed in clear conditions, the vehicle suddenly and uncorrectably migrated off the roadway. Adaptive steering correction, braking intervention, and lane assist failed to engage. At the time of the incident, the display actively showed service notifications indicating suspension system failure and overheating. The vehicle veered into a ditch, resulting in a significant drop and requiring specialized heavy-duty extraction. This defect has impaired the vehicle’s handling, stability, and safety from delivery onward, with no resolution after attempted repair. I am submitting this complaint regarding a serious safety defect involving a 2017 Chevrolet Equinox that experienced a sudden and complete loss of engine power while the vehicle was in motion. While traveling approximately 50 mph in active traffic, the engine abruptly shut off and the vehicle immediately lost all ability to accelerate. The loss of propulsion was total and occurred without warning
... . There were no warning lights, alerts, or indicators of a low oil or engine condition prior to the failure. Moments before the shutdown, the engine began making an abnormal mechanical noise. A video was recorded during the incident while the vehicle was still operating in traffic. The recording captures the engine failure in real time, including the vehicle decelerating from approximately 50 mph to a complete stop as the engine shut down. I was able to coast into a breakdown lane and then into a nearby parking lot, narrowly avoiding a collision. A sudden loss of propulsion at this speed and in these conditions presents a severe and immediate safety risk to vehicle occupants and surrounding traffic. This vehicle has been regularly serviced by authorized Chevrolet dealerships and maintained according to the manufacturer’s recommended schedule. At no point was I advised that the vehicle required frequent manual oil checks between scheduled oil changes, nor did the vehicle provide any warning prior to engine shutdown. I routinely operate this vehicle at highway speeds and regularly transport my child. Had this failure occurred on a highway or in heavier traffic, the risk of serious injury or fatality would have been significant. The exact cause is UNKNOWN at this time. The vehicle is available for inspection, and I can provide maintenance records and video documentation upon request.Read more

This is for a 2022 Holiday Rambler Admiral 28a motorhome. While driving my dashboard started to move and make a lot of noise. After I returned home I took it to have someone look at it. They informed me that the welds that hold the motorhome to the frame were all breaking and the steel was musrooming from the weight. I talked to REV which is the manufacturer and they told me they decline to parti
... cipate in fixing this very serious issue. Basically the house part of the motorhome could detach from the frame causing serious injury or death to me and my family. I feel this is probably not an isolated incident to my motorhome.Read more

One to two times per month upon starting my Cybertruck I receive multiple notifications for systems not working including "Driver Assistance - Automatic Emergency Braking is unavailable" and "Steering & Braking - Adaptive ride control degraded/Ride comfort may be reduced." These alerts cause the disablement of the regenerative braking, auto-hold (braking), emergency braking, adaptive ride control
... , traction control, and the self drive features. While the vehicle is still drivable, my ability to control the vehicle is greatly diminished, as is the safety of the vehicle without any of the accident avoidance and stability systems working. After driving in this condition, if I park the vehicle, exit it, and let it sit for at least 10 minutes, all systems are normal upon reentry. On 1/5 I experienced this issue and opened a service request for warranty work and Tesla demanded that I agree to a $200 charge before they would even look at it and said if they were unable to see the issue then it would not be considered as warranty. Since I cannot reproduce this issue on demand I cancelled the service request as I do not want to pay them $200 to say they couldn't reproduce the issue. Internet searches have revealed that this is a common issue among Cybertrucks and despite multiple software updates since owning my vehicle, it has not gotten any better.Read more

Fuel injectors are bad. The car misfires, I smell gas, runs rough. They fixed one the second week of December 2025. I had the same symptoms when my light came on again on December 23,2025, they said they couldn’t find anything and gave me the car back. On January 3, my engine light came on again, I had warnings sent to my email and phone app about potential engine failures they said today ( 1/6

We've had this car on lease for 23 months, and it has had an escalating pattern of electronic, mechanical and brake issues. Earlier today, on January 5, the car started to slow down and started shaking on the highway today and when I used the brakes the full body of the car started rattling and the brakes were far less responsive than they should have been taking far longer to decelerate than sa
... fe. There was an alarm with a turtle warning me to move over. On January 4, We parked our car in a parking lot and went into a store, when we returned all of the windows were lowered and the moonroof's window was also retracted. The car was beeping and making loud noises and then had issues starting. Prior to both of these issues, the car has taken to Mercedes repeatedly for locking passengers into the vehicle and blaring alarms, not allowing passengers to leave and the seatbelts will not retract. The airbag system has also malfunctioned during these periods. Mercedes has repeatedly been informed of these issues and has claimed that there were bugs in the car's code that they had fixed; however, each time they have fixed the issue a new issue has arisen. In late November 2025, a representative from Mercedes corporate asked for us to take the vehicle back to dealer for further inspection after which the dealership said they had carried out major updates and returned the car in early December. After that the car had been hardly driven until this week. This was the first major drive of over 60 miles.Read more
