This 2021 Acura TLX Type-S has experienced a complete failure of the transfer case after approximately 40,000 miles of use. The issue is due to weak splines within the transfer case that shear off and thus damage the 10-speed automatic transmission in the process. Acura has a 6 year 70,000 mile warranty, so a complete replacement is covered, however my concern is for whether the new transfer case part # 29000-5YK-060 which replaces the old 29000-5YK-050 part will do the same. This is a very expensive fix into the 5 figures (over $10,000) and really needs to have a recall as there are dozens and dozens of owners online who have experienced this failure.Read more

Tesla provided a CSV dataset; however, the FSD engagement and autonomy decision-layer data were almost entirely missing, despite the vehicle being in motion and presumably under FSD control. On follow-up requests, Tesla stated they are unable to provide additional autonomy data and that “Tesla does not collect all your vehicle data,” despite marketing the system as Full Self-Driving and collecting extensive telemetry. This raises a safety concern because: 1.A collision occurred during FSD operation. 2.Tesla is not providing complete autonomy data for safety assessment. 3.There appears to be no transparency into FSD decision-making, object detection, or control authority at the time of impact. 4.Owners, insurers, and potentially regulators cannot review how FSD behaved during a safety-related event. I had notified Telsa twice of this issue, the car left the lane and did not notice a concrete barrier on the left side going north on I-95 in or around Broward County. To my knowledge no one has inspected the car. I was not warned that this car would cause an accident, the problem first appeared when I reported the issue using the voice reporting feature around November 2025. The issue was caused by missing hardware on the upper control arm, leading to further damage.” Tesla did not attribute the failure to impact, road debris, wear, or misuse. The missing hardware was at a critical suspension attachment point. As a result, Tesla replaced multiple major safety-critical components under Goodwill / Internal Service, including the front left active damper/air spring, upper control arm bracket, halfshaft, wheel speed sensor, and multiple structural fasteners. A wheel-off event caused by missing suspension hardware represents a severe safety defect with high risk of catastrophic injury or fatality. This incident raises serious concerns about suspension assembly, post-service quality control, and whether other vehicles may be operating with missing or unsecured hardware. I respectfully request NHTSA review this as a suspension/wheel retention safety defect and evaluate whether additional investigation or corrective action is warranted.Read more

January 2026, NY, in normal winter conditions with windshield defroster on, fan speed high, temperature set at 76, the vehicle is unable to prevent the wipers from freezing. The wipers are unable to clear windshield even after 30 minutes or one hour of vehicle operation. I have never, ever, had a vehicle that was unable to deal with a normal Northeastern winter. This is unsafe in the extreme.

While driving at highway speeds my vehicle lost steering the radio and the hvac turned off. I had abs and brake lights on the dash as well as a service stabilitrac message. The vehicle completely shutoff and I almost had an auto accident. I have not taken it anywhere as I’m to afraid to drive it for fear of having an accident.

I have started receiving airbag & seat safety issue alert but recieved appointment after a month even considering major safety concern. Some of 2021 vehicles are serviced under the recall Tesla sent. But told me mine doesn’t qualify for same issue for same year, same model cars. It seems like issue is with all Tesla Model S 2021 but Tesla charging big dollar amount to repair the safety concern.

The problem has not been confirmed objectively by Tesla; during at least one evaluation, a technician drove only at speeds below complaint threshold (despite requests to test at the speeds where symptoms occur), stated he did not feel the issue, and performed no further diagnosis. Multiple appointments were canceled or rescheduled by Tesla (including one after I arrived). The vehicle was repeatedly returned without documented objective testing or repair addressing the complaint. During one visit, the Tesla app indicated an active “visual quality check” while the vehicle remained parked outside and not being worked on for an extended period (documented). The Brakes were not working within driving this vehicle for less than 3 weeks. We have made a formal complaint to the dealership. Taken it to a certified Nissan Dealer. The dealership we purchased this truck from has ignored every request we have made. We hired an attorney here in St. George, UT. The dealership has not replied to our Attorney. This level of tire wear is well below reasonable tire life expectations and raises safety concerns, including reduced traction, increased stopping distance, and the risk of tire failure. After replacing the prematurely worn tires at my own expense, I reported the issue to Tesla Service and requested that the vehicle be inspected to determine whether alignment, camber, or suspension issues contributed to the abnormal wear. Tesla indicated that an evaluation would only be performed if I agreed to pay for diagnostic services, rather than proactively inspecting the vehicle for a potential underlying defect. As a result, no documented root-cause analysis or corrective action was provided at that time. During a later Tesla service visit for a separate suspension noise concern, Tesla replaced multiple front suspension components (including control arms and lateral/compliance links) and performed a four-wheel alignment under warranty. These repairs occurred after the tires had already been replaced, which created doubt as to whether the suspension condition at the time may have contributed to the earlier premature tire wear. Because the suspension was not evaluated when the abnormal wear occurred, the underlying cause remains unresolved. Replacing tires alone without timely evaluation of possible contributing suspension or alignment issues does not adequately address the associated safety risk.Read more

Multiple suspension components were replaced under warranty, confirming a manufacturing defect, not customer-caused damage. This is a serious safety-critical failure. At driving or highway speed, it could have caused loss of steering, severe injury, or death. The day before, my spouse drove the vehicle on the highway with our younger child — by luck, a catastrophic event did not occur. Before and after the wheel detached, the Tesla monitor did not display any warnings. Even after the lateral link and wheel partially detached, the vehicle remained in driving mode and ready to operate, despite a wheel being unsecured — a basic condition that should trigger an immediate alert. This demonstrates a critical failure in Tesla’s safety systems. There were no recalls listed under my VIN. However, NHTSA records show similar failures, with recall notifications issued under NHTSA Recall 21V-835 in 2021 for 2021 and earlier Tesla Model Y and Model 3 vehicles. The same failure in my 2023 Model Y at 23,641 miles shows the defect was not fully corrected and continues to pose a public safety risk.
