I have experienced a catastrophic engine failure due to the faulty GM DFM lifters and oil pump. I was advised that my vehicle will need a new engine. I know there is Bulletin No.: 24-NA-173 for my vehicle. I have contacted GM, and they advised that there is nothing they can do since the truck is out of warranty. I know they are under investigation for the 6.2 L engine failure issues, and I feel as

The steering angle sensor failed. My mechanic replaced it. I do not know if the part is still available for inspection. The safety of those in the vehicle as well as any around it were in jeopardy when the incident occurred. Thankfully no one was injured nor was there any collision for further vehicle damage. While the vehicle was running, suddenly the steering wheel whipped to the left. The v
... ehicle was in motion when this occurred, causing the vehicle to turn further than intended, and requiring substantial strength to re-direct the vehicle under the driver's (and passenger's) guidance. As long as the vehicle was running, the power steering pulled the wheel to the most extreme left. The mechanic confirmed that the steering angle sensor was the issue and replaced it. I was able to reproduce the steering moving itself (the steering wheel, and front tire) left, when the engine was engaged. With the vehicle off, I was able to maneuver the tires left and right. But when I turned on the vehicle, the wheel would turn itself left. The mechanic assessed the vehicle, replaced the part, and reprogrammed, I presume the part, for this issue. Police officers did block traffic, in order to add safety while we pushed the vehicle out of the roadway. However they did not personally assess the vehicle, or file a traffic report that I am aware of. A friend of my daughter's did come by and feel the stiffness with steering wheel tension, also I believe they saw it turn itself left when the engine was re-engaged. After we purchased the vehicle, we found a post from a previous owner of the vehicle mentioning an issue with the steering. The cause, degree, etc. was vague in the ad's post. I do not recollect if the dealer mentioned the issue to me. Also, we did not see any related dash lights prior to the moment, nor did we notice any symptoms of this, prior to the incident suddenly occurring. I did contact a dealership, and the manufacturer with regards to this.Read more

My 2017 Hyundai Santa Fe began consuming oil at an excessive rate—approximately one quart every 250 miles. The vehicle had around 97,000 miles when I first became concerned. I brought it to the Hyundai dealership for evaluation, and they instructed me to continue driving the vehicle and return after 1,000 miles of driving so they could document the consumption. I returned after only 500 miles b
... ecause of the rapid loss of oil and my concern for driving the vehicle in that condition. While following the dealership’s required oil‑consumption test procedure, the engine performance deteriorated. The vehicle eventually began stalling and had to be towed to the dealership. Because I was required to keep driving the vehicle as part of Hyundai’s diagnostic process, I accumulated an additional 4,000 miles, which pushed the odometer just over 100,000 miles. After completing the test, the dealership submitted a claim for an engine replacement, but Hyundai Corporate denied the claim solely because the mileage exceeded 100,000—despite the fact that the over‑100,000 mileage resulted directly from Hyundai’s own required testing procedure.Read more

Vehicle equipped with Tesla Full Self-Driving (Supervised v 14.2) software exhibits unsafe automated following behavior. The system maintains following distances that are too short at steady speeds, including behind stable lead vehicles with no braking or traffic disturbances. Tesla’s own Safety Score system flags this behavior as “following too closely,” indicating elevated collision risk.
... However, the driver has no available control to adjust minimum following distance or impose safer headway while using FSD This represents unsafe autonomous tailgating behavior with loss of driver control authority over safe following distance, creating increased risk of rear-end collision and hazardous automated driving conditions. Tesla offers several controls (Sloth, Chill, Standard, Hurry, and MadMax) modes, none of which solve the issue if the driver in front is driving the speed limit. My car under FSD will crowd (or tailgate) the front car regardless of the mode. In one instance, I was following a car under FSD in sloth mode and it made the car in front of me pull over just to get me off their tail.... this is certainly now how I wish to drive, and don't want my car driving this way. Failure mode: automated headway policy dominance, lack of driver override, unsafe autonomous following distance.Read more

Vehicle equipped with Tesla Full Self-Driving (Supervised v 14.2) software exhibits unsafe automated behavior due to removal of driver speed control. The system infers speed limits and driving speed without allowing the driver to set a safe maximum speed. In residential neighborhoods with children, pedestrians, and shared social spaces, the vehicle drives at model-inferred speeds that are sociall
... y and physically unsafe. Driver is unable to impose a lower safe speed limit without disengaging. Tesla removed previous option for driver to adjust speed. In a state park campground, the system failed to detect a posted 15 mph speed limit and inferred a 55 mph limit. The vehicle accelerated to unsafe speeds on narrow, pedestrian-heavy roads, with no driver ability to cap speed while under FSD. This represents a loss of human override authority and unsafe autonomous system behavior, creating pedestrian hazard and safety risk. I am filing this complaint regarding the ongoing safety risk and lack of remedy for NHTSA Recall 24V-700, which affects my 2019 Nissan Leaf. This recall was officially initiated in September/October 2024 due to a risk of high-voltage battery fires during Level 3 (DC Fast) charging caused by lithium deposits in the battery cells. It has been approximately 16 months since Nissan issued the interim
... notification advising owners to stop using Level 3 quick charging. To date, no functional remedy has been provided. This delay has resulted in: 1. Unaddressed Fire Risk: The vehicle contains a known defect that can lead to a thermal incident. While Nissan’s interim "solution" is to avoid fast charging, the underlying hardware defect remains in the vehicle. 2. Loss of Essential Functionality: I am unable to use the advertised DC fast-charging capability of my vehicle. This severely limits the vehicle's utility for trips beyond its immediate range and forces me to rely exclusively on slower Level 1 or Level 2 charging, which is not always feasible. 3. Unreasonable Delay: Under 49 U.S.C. Chapter 301, manufacturers are required to remedy safety defects within a reasonable time. A delay of over a year for a software-based fix (which Nissan has stated is the intended remedy) is unreasonable and leaves the consumer with a diminished and potentially hazardous product. I am requesting that the NHTSA investigate the cause of this extensive delay and compel Nissan to either provide an immediate repair, offer a vehicle buyback, or replace the defective battery modules, as the proposed software "patch" appears insufficient to address the physical cell degradation.Read more

I discovered a propane leak and in tracing it down, found that Dynamax had run their main propane delivery over a flexible hose. This hose was improperly protected from being damaged by poor installation process in that the hose was strapped to a cross member under the unit without consideration to protecting the s
... heath. I have just over 20K miles on this and the main sheath has a pinhole caused by it rubbing against a sharp edged metal cross-member. The cross-member is directly under the passenger compartment exposing us to fire and gas exposure. This danger could have easily been avoided by either putting a protective sheath between the hose an the cross-member, or plumbing the entire length with black pipe. I consider myself fortunate that this leak did not present itself while driving down the road. I have attached pictures of both the cross-member where the gas line is strapped down and the pinhole in the gas line (verified with a propane tester).Read more

The vehicle has been seen by Tesla and I have been told that the warranty has expired and the computer must be replaced at my cost. Tesla has replaced other computers under a recall for the same issue reported. This is a serious safety issue as without a functional computer, features such as software updates, navigation, bilnd spot monitoring, lane keeping, cruise control, all cameras (rear, side, front), autopilot, autobraking etc. are nonfunctional.Read more

On [XXX], at approximately [XXX], I was driving my 2013 Tesla Model S (VIN: [XXX] ) on [XXX] in Barrow County, Georgia when the instrument cluster/speedometer display went completely blank while driving. This is an intermittent issue that occurs without warning - the display turns off and sometimes freezes. Because the speedometer was not displaying, I had no way to know my actual speed. I was pul
... led over by Barrow County Sheriff’s Office and cited for speeding. I explained the instrument cluster malfunction to the officer at the scene and showed him the blank display. I took my vehicle to Tesla Service Center in Duluth, GA (3380 Satellite Blvd) on November 4, 2025 - as soon as I could get an appointment after the incident. Tesla cleared the filesystem caches but did NOT inform me about the existing eMMC recall (SB-21-21-001) that affects my vehicle, nor did they perform the recall repair. My 2013 Model S is equipped with the MCU1 with the 8GB eMMC that is subject to NHTSA Recall 21V-119. This is a known defect that NHTSA determined to be a safety issue because it affects critical displays including the speedometer, backup camera, and turn signal indicators. I have video evidence of the malfunction occurring and the Tesla service receipt documenting the issue. The fact that Tesla’s service center did not notify me of the applicable recall or perform the free recall repair. This defect directly resulted in me receiving a traffic citation because I could not see my speedometer while driving INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more
