I am reporting a safety incident involving my Tesla while Autopilot/Full Self-Driving was engaged. On February 11, 2025 at approximately 9:22 am, on Waccamaw Medical Park Ct Conway, SC 29526, I was driving on a normal city street with the system actively controlling steering and speed. Without any warning or alert, the vehicle suddenly steered to the right toward the curb and struck it. I was ve
... ry attentive, but the steering movement happened too quickly for me to safely prevent the impact. No forward collision warning, lane departure alert, or disengagement occurred prior to the incident. The impact caused damage to the wheel and tire (see attached photo). This behavior appears to be an incorrect steering decision by the automated driving system after misinterpreting the roadway edge or lane boundary. I am requesting review of the vehicle logs and investigation of the system behavior during this event.Read more

I am reporting an emissions-related malfunction that occurred a very short time after BMW performed the EGR cooler recall and updated the DDE software on my vehicle. Shortly after the recall work, the vehicle began showing DDE fault code 2BB300 – Particulate Sensor Measuring Electrode, Self-Diagnosis, which triggered the MIL. This issue did not exist at any point before the recall. Although my
... vehicle is now older than 10 years, the critical concern is that this emissions-related failure appeared soon after the recall intervention, indicating a potential recall-associated defect rather than normal age-related wear. The fault was diagnosed at BMW West Houston, where it was confirmed; however, I was told no warranty or goodwill support could be provided due to vehicle age, despite the timing suggesting a recall-related cause. BMW’s emissions warranty extension bulletin (NHTSA TSB ID: MC-10160282-9999) lists particulate sensor failures (2BB700–2BA200) as known issues. Many BMW diesel owners have reported similar particulate sensor faults shortly after the EGR cooler recall and related software changes, suggesting a broader pattern. If the EGR recall or subsequent software calibration is leading to particulate sensor failures and MIL activation, this may represent a post-recall emissions defect affecting compliance. I request that NHTSA evaluate whether the EGR recall may be contributing to widespread particulate sensor failures. Vehicle Information: – 2014 BMW Diesel – Fault Code: 2BB300 – Failure occurred a short time after the EGR cooler recall & software update – Diagnosed at: BMW West Houston Thank you for your attention.Read more

After purchasing a used 2023 Dodge Durango GT, issues arose almost immediately following delivery. I took the vehicle to a Chrysler/Dodge/Jeep/Ram (CJDR) dealership for inspection and service. Diagnostic scans revealed multiple stored and active fault codes, including but not limited to: U0415 / U0401 – Implausible or invalid data received from ABS / ECM / PCM B195F-12 – Power liftgate chim
... e control circuit short to battery B15A1-00 – Active electrical fault Multiple body control, module communication, and liftgate-related fault codes The vehicle experienced intermittent starting failures, creating safety concerns including: Potential inability to move the vehicle when needed, including in traffic Risk of being stranded in unsafe locations Uncertainty regarding whether the vehicle would reliably restart after being shut off The vehicle was removed from service and sent to an authorized Dodge dealership for further diagnosis and repair. The manufacturer/dealer elected to replace a fuel line as part of the repair process to address the starting hesitation. The vehicle has been inspected by an authorized Dodge service department. No police or insurance inspections have occurred. Symptoms occurred intermittently and without consistent warning lights, including: Delayed or hesitant engine starting Requirement to press the brake pedal firmly in order to start the vehicle Intermittent electrical system fault codes retrieved during diagnostics Symptoms first appeared almost immediately after vehicle delivery. The vehicle has a documented history of multiple electrical communication faults, including body control and power liftgate chime control circuits. The selling dealer (Westside Hyundai) was unable to retrieve these fault codes, resulting in the vehicle being out of service and pairs being coordinated at their expense. Thank you for your attention to this matter.Read more

On Saturday morning January 3, 2026 I was driving with my wife, daughter and dog. There was light rain at the time and I had my adaptive cruise set at approximately 72 mph. I was driving our 2024 Tesla Y. At approximately 10:21am the rear of car began to slide right as we entered gradual left curve, possibly because we hit water on road. I tried to slightly correct right and when I did that the st
... eering wheel aggressively corrected back left which put us into a slide. Within a second we hit a tree on the passenger side. The impact was severe and on the passenger side door where my wife was sitting. At that time I theorize that my dog was ejected out of the back window and thrown across the freeway. We then rolled in the other direction (I dont know how many times) down a hill and settled upside down. I immediately smelled smoke and began to try get out of my seatbelt but I couldn’t. I carry a knife and I was able to cut myself out. I then tried to open door and could not so I began punching the glass and eventually got it moving and pushed on it and got it open. I slid myself out, turned around and saw car was on fire. I pulled my daughter out and then began extracting my wife. She was unconscious and had obvious orthopedic injuries. I am 100% confident that had I been driving my other vehicle (non Tesla) that this accident would have NOT occurred. The Tesla and its steering system caused this accident. On top of that, the batteries immediately caught fire, my seatbelt release button did not work, and the electric door button did not work. Had I not been carrying a knife and strong enough to punch door open this story would have ended up differently. The car completely burned. As far as I know, this was the first time this exact issue presented itself. However, when looking online its seems this issue is fairly common with Teslas in wet weather.Read more

It was snowing and I was driving slow. About 8:40am, Feb 6th, 2026, I got a "One pedal driving disabled" alert just for a moment, then tried to stop before an intersection because a signal was turning to red. However, break didn't work and I had to enter the intersection under a red signal. Fortunately, there was no car in front of me, also a car coming from the side didn't notice signal changed.
... No accident, but very very scary. On Dec 6th, I also got an alert "One pedal driving disabled" and even after I released an accelerator pedal, speed didn't slow down. I reached Tesla on Feb 6th, but the service scheduled 4 weeks later. It's not safe to drive at all, thus, I will try to have them to check my vehicle asap.Read more

Component: Engine Vehicle: 2019 Lincoln Nautilus – 2.0L EcoBoost Mileage at failure: ~76550 Incident date: August 2025 Narrative (paste this): My 2019 Lincoln Nautilus equipped with the 2.0L EcoBoost engine experienced a sudden low-coolant warning followed by rapid overheating and complete engine shutdown in August 2025 at approximately 70,000 miles. After adding coolant, the warning immediat
... ely returned and the vehicle entered reduced-power mode and became inoperable within about 20 miles, requiring towing. An independent repair facility performed diagnostic testing confirming coolant intrusion into the engine cylinders, internal engine failure, and coolant pooling in the cylinder head area. Diagnostic trouble codes included P0217, P0316, and P0128. The engine and turbocharger required full replacement at a cost exceeding $6,900 paid out of pocket. This failure matches Ford Technical Service Bulletins TSB 19-2172 and TSB 19-2346, which acknowledge coolant intrusion in 2.0L EcoBoost engines requiring long-block replacement. The failure created a serious safety hazard because the vehicle overheated and shut down during travel in extreme Arizona summer heat, leaving occupants stranded for hours awaiting a tow truck. This appears to be part of a widespread known defect involving coolant intrusion in Ford 2.0L EcoBoost engines. I am reporting this for safety investigation and defect tracking.Read more

ICCU failure at 3889 miles not charging and 12v battery failure
Incident date: Nov 10, 2025
Description of the Defect: While driving with Full Self-Driving (FSD) engaged, the vehicle's primary computer shuts down and reboots, causing a sudden loss of all safety features, including Blind Spot Monitoring, Lane Departure Warning, Forward Collision Warning, and the driving visualization. The cameras go black while the vehicle is in motion. The Trigger: The failure is triggered by high elect
... rical load. If the Cabin Heater (HVAC) and Seat Heaters are active while FSD is fully computing, the 12V power supply to the computer drops below critical voltage, causing a "Turbo A" processor lockup and system crash. Evidence of Defect: Service Center diagnostics confirmed the specific error code APS_w169_TurboA_SCS_LKUP (Computer Crash) occurred at the exact same timestamp as VCLEFT_a302_blowerGeneralFault (HVAC Blower Failure). This proves a common-mode voltage drop affecting multiple critical systems simultaneously. Manufacturer Response: The manufacturer (Tesla) acknowledged the logs but refused to inspect the wiring harness or ground connections, instead recommending a replacement of the computer unit. However, the failure can be mitigated by unplugging external cameras to reduce load, proving the root cause is an insufficient power delivery design (Wiring/Grounding) rather than a component failure. The computer is running at full load with the newest FSD software and it can no longer keep up with additional loads. Safety Risk: This defect causes a sudden, uncommanded loss of driver assistance and situational awareness tools (cameras/visualization) while the vehicle is maneuvering in traffic. The vehicle’s power distribution system appears insufficient to support the electrical load of the software (FSD) and the hardware (HVAC) simultaneously.Read more

Multiple times while driving my 2016 F-150 I would lose all power to my wheels. The RPMs would rise but no power would be transferred to the wheels. This happened one time while on the on ramp at 50+ MPH, a second time while drive down a city street at about 30 MPH, a third time while starting from a stop light, and a fourth time again when driving in the city at about 45 MPH. The check engine lig

After reviewing there are two error codes the vehicle relayed that the mechanic said are related. During these 4 instances there were two time (highway on ramp and city street at 30 MPH when the loss of power to the wheels caused the vehicle directly behind the F-150 to have to swerve around to prevent a collision. My 2012 Hyundai Sonata recently had an engine replacement completed under a Hyundai recall. For about many months now I have had what sounds like a Exhaust Manifold leak. I have been trying to learn more about it. I have a 19' Ram 1500 Laramie and I have found that this is an incredibly common issue amongst Ram owners with a 5.7L V8 Engine. You tube has thousands of videos about how to repair, there are hundreds of message boards online, and even my mechanic said to me "Oh yeah I've fixe
... d hundreds of this issue on Rams". When I remote start my truck I have to roll the windows down when I get in to let the exhaust smell out of my truck. I would assume that the air intake for the cabin climate control is pulling in the fumes from the engine bay. It concerns me to leave my truck run too long while not moving for fear of passing out. When I took it to an exhaust shop he said he could temporarily remedy this with some liquid gasket maker but he was right, it only lasted about 2 weeks. THIS NEEDS TO BE A SAFETY RECALL. I checked with my dealer service dept and he quoted me "at least $1,600" I can buy an entirely new aftermarket system for less than that. I am about 20k miles out of my warranty. This needs addressed.Read more

On [XXX], the vehicle was unable to be started - no lights, no power, unable to open the doors. I called Hyundai Roadside Assist and they sent someone from AAA a few hours later who was able to "jump start" the vehicle. I took it to the dealer, Manly Hyundai, on December 17th, 2025 and they determined nothing to be wrong with either the battery or the electrical system. On [XXX], while driving
... from home to the local grocery store, the car presented a warning light on the dash and the “gas pedal” stopped working. I was able to coast into a parking lot at which point an even larger warning light appeared and the vehicle then lost all electricity completely. I again called Hyundai Roadside Assist and they sent someone from AAA a few hours later who determined that the car was unable to be “jump started” and would have to be towed to the local dealer, Manly Hyundai. The dealer reported back to me that the component that failed is called an “ICCU” and is some kind of controller related to the charging and motors of the electric drivetrain. It is available to be inspected at the dealer currently, Manly Hyundai. My safety was put as risk because the car became essentially inoperable while driving downhill and gave no warning signs ahead of time before the lights lit up on the dash and I lost control. The dealer has confirmed that it is an issue with the “ICCU”. The dealer has inspected the component at their service department. The first warning light that appeared came at the same instant that the vehicle lost drive power. The second warning light that appeared came at the same time that the vehicle lost all electricity entirely. On [XXX], after placing my young child in my Tesla and closing the door, the 12V battery failed suddenly and the vehicle became inaccessible. I was unable to unlock or enter the car. I did not receive any warning or alert that the 12V battery was about to fail. The vehicle was in my garage, and the window automatically cracked, which allowed me to monitor my child while I sought help. I called Te
... sla Roadside Assistance at 8:59 AM. During the call, the representative reassured me that help was being dispatched and at times described the service provider as “on the way” (timing details such as “within ~45 minutes” and the length of the initial call are my best estimates). After continued waiting with my child confined inside the vehicle, I contacted emergency services due to the safety risk. I called 911 at 10:03 AM. I later had the 12V battery replaced at my own expense. This incident involved a sudden electrical failure without warning that resulted in a child being trapped inside a disabled vehicle, and I am also concerned about the accuracy and effectiveness of roadside assistance dispatch communications during the incident. While driving the vehicle warnings alerted me that the battery was low and to start the vehicle ( it was running as I was driving) then the alert changed to pull over when possible. At this time the power steering and power brakes started to fail by the time I pulled into a parking lot the vehicle completely died and I had to push it into a parking spot. I called Land Rover they towed it to a deal
... ership. The dealership in Orlando Florida said that the DC to DC converter failed and that it would need a new one (but the part is on a back order because it’s a common issue and they have no idea how long it will take to get the part) it will take at least 30 days or more to get a new part. Before I managed to pull over in a safe location the steering became very heavy and the brake pedal got very hard and barely stopped the vehicle ( almost had a collision) and steering into the parking lot was extremely difficult.Read more

During a recent incident where a tire issue caused damage to the wiring harness for the parking brake, resulting in a Parking Brake Failure notification on the screen, the vehicle would not stop or turn off. The vehicle displayed N for neutral on the dash, but the vehicle would drive forward unless the brake was depressed. In addition, we were unable to place the vehicle in Park, which meant tha
... t we were unable to shut it off. The only way to stop the vehicle from driving on its own and crashing into something was to place concrete blocks under the tires! Unplugging the 12v battery also had no effect! The vehicle kept driving as if it was in drive, and there as no way to stop it without placing the foot on the brake pedal the entire time or placing the blocks I referenced. The only way I was able to shut it off, was to pull what's known as the Fireman's Loop, which I'm guessing firemen pull in the case of an accident. I wasn't aware of this mechanism, and only learned of it after posting on numerous social media apps regarding this issue. This is a Major issue, as we are talking about a moving and unstoppable car! Tesla needs to build some mechanism that allows the car to be shut off by the driver or to override the screen and place the car in park. Pulling this firemans loop and unplugging the 12v battery, required me being in front of the vehicle, risking my life, as the vehicle could've easily moved if the concrete barriers didn't stop it, and also required me taking 30+ minutes to remove components from the Frunk, just to access these components!!Read more

While dropping to school in my Tesla Model Y on [XXX] , I experienced a severe safety incident. A lower control arm bolt on the driver's side came off, causing the suspension to collapse. Before this, I noticed symptoms consistent with reported Model Y issues: rattling, clunking/popping noises, particularly when driving over bumps or turning, and unstable steering. This appears to be a manufacturi
