While my wife was driving, the high-voltage battery failed and the car went into reduced power/limp mode. Tesla later determined that coolant had gotten into the high-voltage battery and caused it to fail. My main concern is that we had no warning that there was a coolant leak or that coolant was getting into the battery. There was no low-coolant warning or anything telling my wife that there was
... a serious problem developing. By the time the car started giving high-voltage and reduced-power warnings, the damage had already occurred and Tesla said the battery needed to be replaced. What concerns me even more was what happened after Tesla received the car. Tesla removed the battery and quarantined it in the parking lot. They told us in writing that it was removed because of coolant ingress as a safety precaution and that a specialized technician had to determine whether the battery was safe to discharge. During a phone call, a Tesla employee also told me they quarantined it because there was a possibility the battery could explode and they did not want anyone injured or anything damaged if that happened. That was alarming because my wife had just been driving this car, and afterward it sat in our driveway while we waited for it to be taken to Tesla. At no point were we told that the battery could be dangerous or that the vehicle should be kept away from people or property. We had no idea Tesla would consider the condition serious enough to quarantine the battery once they received it. Tesla later confirmed in writing that coolant ingress was the root cause of the battery failure. I would like NHTSA to look into why this condition was able to get this far without warning the driver and whether other vehicles could have the same issue.Read more

Vehicle: 2026 Tesla Model Y System involved: Full Self-Driving (Supervised) Date of incident: September 2, 2026 Approximate time: 5:20 PM Location: I-10 area near Tucson, Arizona Crash: No Injury: No Near-miss / safety-critical event: Yes I am reporting a serious safety incident involving the Full Self-Driving (Supervised) system in my 2026 Tesla Model Y. On September 2, 2026, at approximately 5
... :20 PM near Tucson, Arizona, I was using Full Self-Driving (Supervised) while traveling toward I-10. The vehicle was expected to enter I-10 but did not enter the highway as intended. Instead, the vehicle proceeded onto a service road, then entered an abandoned parking lot. Rather than safely correcting the route it started towards highway ramp against the normal direction of traffic. I immediately took manual control, stopped in an empty emergency area between the ramp and the service road and activated my emergency hazard lights. I remained stopped while approaching traffic slowed after observing my vehicle. Once it was safe to proceed, I manually and slowly repositioned the vehicle in the correct direction and entered I-10 using the ramp. There was no collision and no injury. However, I believe this event created a serious and potentially fatal risk because the vehicle’s automated driving system directed the vehicle toward a highway ramp in the wrong direction, creating the possibility of a wrong-way conflict with incoming traffic. I am also notifying Tesla for evaluation. I wonder if more similar incidents were reported recently (I learned the fatal collision killing passenger on Oct 31st in AZ due to FSD malfunction).Read more

Whenever I disengage full self driving the car forces me to tell it why I did it via an on screen selection. It does not go away after a period of time. I used to be able to dismiss it by hitting the voice button twice but they've removed that loop hole. It's very distracting and forces me to interact with the screen while driving. The selection screen also blocks the blind spot cameras. It comes

My Tesla was operating under FSD at 9pm on a dark country road. I was traveling at 45mph and vehicle randomly applied hard emergency braking and I had to take over. It appeared that the vehicle may have been responding to a dark oil stain or possibly a mailbox. Had another vehicle been closely behind me, there would’ve been a collision.

The electronic power steering (EPAS) system/steering gear malfunctioned. Tesla diagnosed fault code EPAS3P_A072_mot and recommended replacement of the steering gear/rack, part number 1188832-00-B. The vehicle is currently available for inspection upon request. ... le.The vehicle displayed a warning stating “Steering Assist Reduced Steering May Require Increased Effort.” A reduction or loss of electronic power steering assistance could require significantly greater steering effort and could affect the driver's ability to safely maneuver the vehicle, particularly during turns, low-speed maneuvers, or an emergency maneuver. I was concerned that the steering assistance could further deteriorate or fail unexpectedly while driving, putting the driver, passengers, and other road users at risk. The problem was confirmed by a Tesla Service Center. Tesla diagnosed fault code EPAS3P_A072_mot and determined that the steering gear/rack requires replacement. Tesla recommended the replacement of the steering gear part number 1188832-00-B and provided an estimate of approximately $3200.The vehicle was inspected by Tesla technicians at the Tesla Service Center. I specifically asked Tesla to determine whether a pothole impact could have caused the steering rack failure. After inspecting the vehicle, Tesla informed me in writing that its technicians did not observe any signs of impact damage and that Tesla does not believe the pothole incident caused the current steering rack failure. The vehicle has not been inspected by police or insurance representatives. The vehicle displayed the warning message “Steering Assist Reduced / Steering May Require Increased Effort.” The warning first appeared on or about 8-25-26 at approximately 136K miles. I am submitting this complaint regarding a safety concern involving the panoramic glass roof of my 2024 Tesla Model Y. My toddler lightly bumped their head against the inside of the roof glass, after which the glass developed a significant starburst crack. The contact was very minor, and the resulting damage raised serious concerns about the safety and durability of the roof glass, particularly b
... ecause I regularly transport my children in this vehicle. On XXX, I brought the vehicle to a Tesla Service Center in Orland Park, Illinois. Tesla documented my report as a child/passenger safety concern. The technician verified an impact zone, documented that the top/exterior portion of the roof glass was cracked, that the interior section did not crack, and that a portion of the roof glass was pushed inward at the impact zone. Tesla recommended replacement of the roof glass. I requested written confirmation that the vehicle was safe to continue driving with the damaged roof. After escalation, Tesla’s Regional Manager responded that whether it was “safe” was a decision I would need to make at my own discretion and suggested replacement if I did not feel safe. He also stated that Tesla’s glass is deemed safe for its intended use based on testing and government regulations. My concern is not simply repair cost or coverage. I am concerned that very minor interior contact resulted in significant damage to a large glass panel directly above occupants, including children. I am requesting that NHTSA document and review this incident as a potential vehicle safety issue.Read more

I own a 2021 Tesla Model Y with approximately 51,300 miles. Beginning in June 2026 I noticed a persistent creaking noise from the front suspension, most audible when accelerating and when turning at low speed. The vehicle had been rear-ended in June 2026 and repaired at a body shop; the noise was reported to the shop at that time and persisted after the repair was completed. On September 2, 2026
... I brought the vehicle to a Tesla Service Center for diagnosis. The technician test drove the vehicle, verified the noise, and performed an inspection on a lift. The written findings were that both front upper control arm ball joints were partially seized, both front lateral link bushings were torn, and both front compliance link bushings were torn. Tesla recommended replacement of the front upper control arms, front lower lateral links, and front lower compliance links, quoted at approximately $2,610 at my own expense. Tesla further stated that a four-wheel alignment could not be accurately performed in the vehicle's current condition because of the torn bushings. No crash, fire, or injury has resulted. The vehicle remains in service. I am filing this complaint because these are the same front suspension components identified in NHTSA's open investigation into front suspension link separation in Model 3 and Model Y vehicles, and my vehicle falls within the model years under review. The failure is bilateral and occurred with no impact to the front of the vehicle and no off-road or track use. I am concerned that degraded bushings and seized ball joints across all three component types create a risk of link separation and loss of steering control without meaningful warning beyond the creaking noise.Read more

Tesla only backed down and waived the fee to $0.00 after being confronted with my personal repair invoice as leverage. This proves Tesla is actively executing predatory billing and hiding a systemic batch safety hazard from uninformed consumers. Regulatory investigators can verify this pattern by auditing the Littleton Service Center's internal torque check logs and warranty waiver histories for 2023 Model Y vehicles. Furthermore, after touchless app-pickup on XXX, Tesla staff omitted any key card communication. Two weeks later, after discovering my physical key cards were missing, I initiated a brand-new service request to inquire, and agent Tony admitted they were left at the front desk, exposing that Advisor Luis Rodriguez falsified records on XXX by logging that the vehicle was retrieved on XXX. My Model Y has an intermittent HVAC issue that occurs only when there are more than 4 occupants in the vehicle. I first reported this while the car was still under warranty, at just over 46,000 miles. We had changed the filter as we thought it was resolved. The issue reappeared, and I scheduled a service call within 10 days. Because the fault is sporadic and couldn't be reproduced on demand, the
... service call was canceled. The problem persisted, and I brought the car back in. This time, at 50,750 miles, your technician diagnosed the root cause and confirmed it is a system fault due to defective parts, and that it has been happening even when the car was under warranty—not something I caused or could have prevented. He also told me directly that if left unaddressed, this will likely lead to compressor failure down the road. Despite the issue being reported and documented while the car was under warranty, I've now been quoted $3,200 to fix it, entirely out of pocket, simply because the diagnosis happened to land a few hundred miles past the warranty cutoff. Had this surfaced further down the road as normal wear and tear, I'd have accepted it as a cost of ownership. But this is a known defect that was reported in-warranty and only went undiagnosed because of its intermittent nature — that shouldn't be the customer's burden to absorb. On top of that, I found an uninstalled HEPA filter assembly sitting loose in my trunk after this service visit — not installed, not packaged, just left there. It had nothing to do with the nature of the visit. That's not the kind of quality control I'd expect, especially given the rest of this experience. This raises real concerns about the quality control on this visit & customer experience from the service team (manager).Read more

Driver-side front suspension is creaking and popping when turning the steering wheel, particularly at low speeds. Noise appears to be coming from the left-front suspension. Vehicle needs inspection of the driver-side front upper control arm/ball joint, lateral and compliance links, bushings and associated suspension components for wear or excessive play.”

On August 26, 2026, at approximately 8:56 AM, I entered a public parking garage in Washington, DC. Full Self-Driving (Supervised) was engaged and controlling the vehicle during a reversing maneuver. I remained attentive to the vehicle's surroundings, including pedestrians and surrounding traffic. While reversing under FSD (Supervised), the vehicle's right rear corner collided with the corner of a
... stationary structural pillar in the parking garage. I did not command the vehicle to steer into the pillar. Upon becoming aware of the impact, I immediately applied the brake. There were no injuries. The collision resulted in damage to the right rear area of the vehicle. After the incident, I photographed and recorded the scene and vehicle damage and saved the available Tesla Dashcam footage. I also requested the vehicle data from Tesla. I am reporting this incident because the vehicle collided with a stationary structure while FSD (Supervised) was engaged and controlling the reversing maneuver. I do not know the specific technical cause of the incident. The vehicle and available recorded data can be made available for inspection upon request.Read more

Tesla replaced the windshield on my 2021 Model 3 at the Tesla Service Center in Schaumburg, IL. Days after the installation, following a light rain, a film appeared on the exterior surface of the glass. In the presence of moisture the film hazes over and substantially obscures forward visibility from the driver's seat. The first occurrence was on 8/22/2026 while leaving a venue in heavy pedestria
... n traffic. Forward visibility was significantly degraded. During that drive the vehicle's driver-assist system issued a take-control prompt and returned control to the driver. The system did not display a reason for the prompt, so I cannot attribute it; I am reporting that it occurred while visibility through the windshield was degraded. The condition is not removable by normal means. Windshield wipers and washer fluid do not clear it. The defroster/defogger does not clear it, which rules out interior condensation. It required manual scrubbing to reduce, and it has recurred repeatedly since. It presents only in high humidity or after precipitation; in dry conditions the glass appears normal. The condition did not exist prior to the windshield replacement. I have photographs taken from the driver's seat showing the obscured view. I reported this to Tesla on 8/22/2026 through two in-app channels; both were closed without a response. I escalated by email on 8/24/2026. The service manager responded and scheduled an inspection. As of this filing the substance has not been identified, no cause has been determined, and no correction has been made. The condition remains active. I am filing because a windshield that intermittently obscures forward vision under ordinary weather conditions is a driving hazard, and because the vehicle has been operated in that condition while awaiting resolution. No crash, injury, or fire has occurred.Read more
