While using Tesla's Full Self-Driving (Supervised) software on the Pacific Coast Highway (PCH) and other highways, the vehicle’s automated steering system repeatedly malfunctions by failing to maintain lane centering. Specifically, the car drifts or actively steers across double yellow lines into the lane of oncoming traffic. This issue is a persistent failure of the Advanced Driver Assistance S
... ystem (ADAS) and Lane Keeping Assistance. The vehicle has attempted this maneuver multiple times at [Approximate Speed, e.g., 40-60 mph], requiring immediate and forceful manual steering intervention to avoid head-on collisions with oncoming vehicles. The system provides no warning chimes, error messages, or "Take Over Immediately" alerts prior to crossing the lines; it appears to perceive the oncoming lane as a valid path or fails to recognize the double yellow boundary entirely. This has occurred during [mention conditions: e.g., bright daylight with high glare / night time]. The vehicle is equipped with Hardware 3.0 (HW3). I believe the 1.2-megapixel camera suite or the processing power of the HW3 computer is insufficient to safely navigate the geometry of the PCH or any roads within mountains/hills. I have attempted to "Clear Calibration" and performed system resets, but the life-threatening behavior persists. The vehicle and its internal data logs are available for inspection. I have also sent "Bug Reports" via voice command to the manufacturer immediately following these near-miss events.Read more

I was using the vehicle's Autopark feature to parallel park on a public street in a normal/spacious parking spot. I selected the parking spot on the center screen and started Autopark. During the maneuver, the vehicle reversed into the parked car behind me and caused a dent (low-speed crash). I was supervising the maneuver and did not press the accelerator or brake before impact. After the incide
... nt, I reviewed dashcam footage and vehicle telemetry. The rear vehicle was clearly visible in the rear camera view. Telemetry shows the vehicle had already shifted to Drive before impact, but it continued moving backward for about 1 second and then contacted the parked car behind. Telemetry also shows accelerator pedal position was 0 and brake was not applied during the impact sequence. This appears to be an automated parking/close-range obstacle stopping failure or delayed stopping response. No warning lamp or prior vehicle fault message was observed before the incident. The issue has not been reproduced since the incident. I have dashcam footage and telemetry logs documenting the event.Read more

I am reporting a manufacturing defect involving the dashboard on my 2022 Audi A6. The dashboard has begun bubbling and warping, particularly near the area close to the windshield. This defect appears to be common among Prestige‑trim Audi A6 vehicles, as other owners have reported similar dashboard warping. The warping causes the dashboard material to rise upward, which reflects additional glare
... and partially obstructs visibility through the lower portion of the windshield, especially in certain lighting conditions. This reduces my ability to clearly see hazards low on the road ahead, creating a potential safety risk. Additional Details: - The vehicle has not been modified. -The issue has progressively worsened over time. - The defect appears to affect normal forward visibility and could distract the driver. Audi has not issued a recall for this known problem, despite multiple similar complaints from other owners (Effected Models per online research: ETron, R8, A6, A7, A8, Q7, Q8). I am asking NHTSA to investigate this dashboard defect as a potential manufacturing defect and safety hazard due to obstructed visibility and distracted driving caused by glare/reflection from the warped dashboard surface.Read more

Dear NHTSA Representative, I am filing a complaint against Infiniti regarding refusal to perform a recall-related engine replacement on my vehicle. The dealership refused to perform the recall-related engine replacement.

However, since I have owned the vehicle, all oil change maintenance has been completed on time and properly maintained. On February 10, before repairs were completed, multiple safety systems suddenly disabled while driving, including regenerative braking behavior, traction and stability control, lane safety features, and automatic emergency braking, creating a dangerous situation. I returned the vehicle immediately and was provided a loaner. I picked up the vehicle on February 13 and was told the suspension issue was resolved, but the noise remained. I then sought a second opinion at Tesla Peabody, where a master technician conducted a ride-along and confirmed the condition. When the system does activate, the image is sometimes mirrored or reversed, making it difficult or unsafe to interpret the surroundings behind the vehicle. Overall, the cam

This failure is unpredictable and occurs without warning, creating a safety risk due to the inability to consistently see pedestrians, children, or obstacles behind the vehicle. The vehicle was inspected by a Ford dealership for this issue; however, no remedy was provided because the vehicle was outside the warranty period. I am aware that Ford has issued recalls and extended coverage for rearview camera failures on later model year Ford Fusion vehicles (2015 and newer) exhibiting similar symptoms. My vehicle demonstrates the same defect behavior, despite being excluded based solely on model year. Although rearview cameras were optional equipment in 2014, this vehicle was factory-equipped with a camera, and the failure constitutes a safety-related defect rather than normal wear. This system failure directly resulted in a high-speed collision with a road divider, causing significant vehicle damage and physical injuries to the driver that required medical treatment. Inspection/Confirmation: The vehicle is currently subject to an open insurance claim and has been evaluated for repair. Warnings: There were no warning lamps, messages, or requests for the driver to take over prior to the immediate 1-second window before the crash. Evidence: I have downloaded and preserved the complete Event Data Record (EDR) and the multi-angle dashcam video files. My concern is the holding tank fell down . It’s brand new never water in it . And I was 500 miles from home when I noticed it . This could have caused a wreck for me or others had it fallen all the way out . At highway speeds. The dealer told me they the manufacturer told them to strap it in . I think this repair may work but I feel I need to get a second opinion. The framework and the trailer f
... rame is very flimsy where the tank sits in is the issue of it falling out and in my opinion they need to recall these and let others know this can happen before it does happen and hurts someone. Most people would not look like I did . And if the tank is full of water and you hit a bump it would fall out without further bracing. There seems to be a firewall between me and the manufacturer to get answers , hopefully no body gets hurt . On February 17 at approximately 9:30 AM, while my wife was driving uphill, the vehicle abruptly shut down and became completely disabled. It began rolling backward despite her attempting to accelerate. Multiple warning alerts activated simultaneously, and the vehicle would not respond to forward input. Our [XXX] child was in the vehicle at the time. A total loss of power while in motion, combin
... ed with unintended rollback on an incline, represents a catastrophic safety defect. This was not a minor malfunction — it created a direct and immediate risk of collision and serious injury. Compounding this concern, on the morning following the incident at approximately 6:00 AM, I discovered one of the vehicle doors open without any user interaction. The vehicle had been secured the night before. This raises serious questions regarding electronic reliability and system integrity. Given the severity of these failures, I have lost confidence in the safety and dependability of this vehicle. My family’s safety cannot be subjected to further uncertainty. If this matter is not resolved promptly and satisfactorily, I am prepared to pursue all available legal and regulatory remedies, including formal complaints to appropriate federal safety authorities. Please confirm receipt of this letter and provide a written response outlining next steps within five business days. This situation placed my wife and child in direct danger. I expect a resolution that reflects the seriousness of that fact. Sincerely, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more
