I am reporting two severe 'phantom braking' incidents involving my Tesla vehicle while driving without autopilot. In both instances, the vehicle initiated hard emergency braking on a high-speed roadway with no obstacles, pedestrians, or vehicles in my path. This sudden, uncommanded deceleration created an immediate risk of a rear-end collision from following traffic. Incident 1: November 2025 a
... t approximately [XXX]. I was traveling at 30 mph on [XXX] . The car suddenly braked to 0 mph. Weather was sunny, and the road was straight. There were several vehicles following behind me. Incident 2: [XXX], at approximately [XXX]. Location: [XXX], Honolulu. The vehicle decelerated from 10mph to 0 mph without warning. In both cases, the car stopped suddenly and was placed in Park. I had to put the car in drive to resume driving. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Vehicle remains grounded. Safety Concern Vehicle telemetry states HV contactors are blocked to protect the vehicle; service staff advised to disregard active alerts. Inconsistent guidance risks unsafe operation. I grounded the vehicle pending engineering review. Troubleshooting and A/B Tests My home EVSEs successfully charge a different Tesla (Model Y). The subject VIN charges at DC fast charging (Supercharger) and once on a third‑party EVSE, but fails on my two home EVSEs thereafter. This localizes the issue to vehicle‑side AC charge path/charge‑port logic rather than the EVSE. Request OEM to provide CP waveform under load + Toolbox logs. Please log this safety‑related defect; aggregate with similar complaints, and, if appropriate, open an investigation into identity/integrity faults causing HV‑enable lockouts and conflicting service guidanceRead more

There was no incident, it is a safety feature that has been removed from Tesla Model Y's since October 2025 and the service center declined to add it in during a service request even at the expense of the owner. One only needs to google or ask an AI about "Tesla Model Y Juniper" front end suspension noises and you will find links to hundreds of complaints and at least two YouTube videos now. Based on reports from recent 2026 Tesla Model Y owners, particularly those with the Juniper refresh, the front-end noises over rough or bumpy roads at very slow speeds sound like a widespread suspension-related issue
... . It's often characterized as a clunk, rattle, or knocking from the front suspension, especially when the suspension compresses and unloads over uneven surfaces like driveways, speed bumps, or minor road imperfections. Youtube link https://www.youtube.com/watch?v=ECerCsSdEeI&list=WL&index=3 For the rubbing-type noises when turning the wheels all the way left/right (full lock), this is another frequently reported quirk in Model Ys, though less tied specifically to 2026 builds. It often presents as a grinding, scraping, or creaking sound. I had scheduled an appointment with Tesla Service, however, there are a lot of cases where the Service Center provides a repair or is supposedly removing the Pencil Braces. Based on all the repairs I've read, 1. The issues still remain. 2. Service Center's don't really seem to know how to provide repairs to fix the issues, and it seems different Service Centers are providing different repairs (they don't really have a fix). 3. There are many cases where supposedly, Tesla Service Technician's are informing customers that Tesla is aware of this widespread issue and their engineers are supposedly working on a permanent solution. I currently don't see any reason to have a Tesla Service Center tear apart my brand-new Tesla, simply to provide a temporary solution. And, if Tesla engineers are indeed working on these issues, I would rather wait. Finally, I am filling this complaint to 1. Notify NHTSA if they're currently unaware of this issue and 2. To have a record of my complaint, if it turns out I need to utilize the Lemon Law or a buy back.Read more

The vehicle’s original engine began producing excessive white smoke during startup. The vehicle was diagnosed by AutoNation Ford, and it was determined that the passenger-side cylinder head was cracked near the exhaust port. This defect allowed coolant to intrude into the passenger-side turbocharger. Due to the extent of the damage, the engine required full replacement. The engine was replaced w
... ith a used motor under a third-party warranty on August 21, 2024. The replacement engine is now experiencing the exact same failure symptoms. Specifically, the vehicle produces white smoke at startup consistent with coolant intrusion. Bozard Ford performed a diagnostic evaluation, completed on February 27, 2026. Their findings determined that the replacement engine has the identical defect: a cracked passenger-side cylinder head near the exhaust port allowing coolant intrusion into the turbocharger. This repeated failure raises significant safety concerns. Coolant intrusion into the turbocharger and combustion chamber may lead to engine misfire, loss of power, potential engine damage while driving, and possible sudden engine failure. Additionally, burning coolant may damage emissions components and create hazardous driving conditions due to reduced visibility from exhaust smoke. The fact that two separate engines in the same vehicle have experienced the exact same cylinder head failure suggests a potential design or manufacturing defect specific to the 2.7L EcoBoost engine. I respectfully request that NHTSA investigate this issue to determine whether this is a broader safety-related defect affecting other 2015 Ford F-150 vehicles equipped with the 2.7L EcoBoost engine. Please let me know if additional documentation, repair records, or diagnostic reports are required.Read more

I bought my vehicle used, in May 2025. Then, in October (if not earlier), a safety recall was issued relating to the fast charging of the EV battery. This recall tells me that I’m not to use the CHAdeMO charging port due to battery fire risk. Now, almost 6 months later, there still isn’t a remedy available. Not being able to fast charge puts a huge restriction on where I’m able to go in my
... car. NO ONE wants to sit around for hours waiting for their car battery to recharge in order to get to their destination. I have contacted Nissan’s corporate office to ask if they would offer me a loaner vehicle for my longer distance drives, but they said no and didn’t have any other assistance to offer until the remedy is available. I would like to know, are they given any sort of deadline to remedy such a HUGE safety recall, or what most people would consider a DEFECT? If so, what kind of penalty do they face if this deadline isn’t met? If not, is there anything I can do to hold them accountable for selling me a defective Electric Vehicle? There are two major safety issues that came with the vehicle upon the new car delivery. The problems existed even before the delivery as I found the first issue during the new car delivery and the second issue upon leaving the delivery center. First is the non-operational FSD (Full Self Driving). FSD never worked from the beginning. The Tesla advisor noticed it at the delivery center but insisted
... that it will work after 10 minutes of driving, needing calibration. However, after more than several hours of driving and 35 miles of driving, FSD still doesn't work and the display says FSD is non-functional. Second is the lane departure and lane approach sensor. When the neighboring car approaches from the side, there is no alert or notification. The blind spot sensor doesn't work either. This is a big safety issue and I almost got into an accident had I looked back and noticed a car approching from behind when I tried to move into the right lane. I expalined both of these two major issues with the vehicle and left the car at the service center in Vienna, VA, expecting the repair to be done within hours as promised. However, the service technician later sent a message via Tesla app that the part is missing from the car and the part needs to be ordered. It's been a full day and Tesla is still in the process of finding the issue, which means they still don't know what the issue is. I belive this is not the quality of product and testing that has been registered with Tesla in NHTSA. This surely represents the sub-par quality product and lack of full testing that needs to be done prior to the new car delivery. The service center employee responded via app that the car needs to in repair for 5 full days and may need more time if necessary. This is definitely not the quality and experience that are advertised by Tesla.Read more

Vehicle: 2023 Tesla Model Y VIN: [XXX] Mileage: Approximately 40,531 miles Software Version: 2025.45.9 FSD (Supervised) Version: v14.2.2.4 While driving on [XXX] near the [XXX] entrance in Mission Viejo, California, the vehicle was operating under Full Self-Driving (Supervised) mode. As the freeway lane ended/merged, the vehicle unexpectedly performed a sudden full stop at freeway speed. There w
... as no visible obstruction in front of the vehicle. Multiple vehicles behind me had to brake abruptly to avoid a rear-end collision. The system did not provide any audible or visual warning before the sudden braking. This has created a serious safety risk due to the high-speed freeway environment. This issue appears to be related to lane-ending or merge interpretation by the FSD system. I am concerned that this behavior could result in a rear-end collision. The front lower lateral link (rearward lower control arm) fastener/bolt completely backed out and fell off the vehicle while in motion, causing the lateral link to separate entirely from the subframe. This is the exact mechanical failure described in Tesla Recall Campaign 23V-235 (Service Bulletin SB-23-31-001), however, my specific VIN was not included in that recall. The vehicle and components a
... re currently at an independent EV repair facility and are available for inspection upon request. Photographic evidence of the missing bolt and separated control arm has been retained. The separation of the lower lateral link caused an immediate and violent collapse of the front passenger-side suspension geometry. The affected wheel was no longer anchored to the vehicle frame, causing it to fold backward and grind heavily against the wheel well. To keep the vehicle moving straight, the steering wheel had to be turned to an extreme 45-degree angle. This resulted in a near-total loss of steering control. Had this fastener failed while traveling at highway speeds instead of low speeds, it would have resulted in an uncontrollable, catastrophic collision. Yes. The problem was physically confirmed and documented by an independent, EV-certified repair facility (EVFixMe/Shocker Motors in Costa Mesa, CA) once the vehicle was placed on a lift. The vehicle was physically inspected by an independent service center. The manufacturer (Tesla) was contacted via the Tesla App; however, they refused to perform a physical diagnosis without pre-authorization of a $2,600+ comprehensive suspension replacement estimate and stated the vehicle was out of warranty with no open recalls. There were no dashboard warning lamps, digital alerts, or error messages from the vehicle's computer system prior to the failure. The failure presented suddenly as a loud, painful metal-on-metal grinding noise accompanied by immediate, severe steering misalignment. Currently waiting on invoice from indy shop.Read more

While on the road driving at 35 MPH, the vehicle started to "bog" and losing power and/or acceleration. Had to immediately drive off the road as there were several vehicles behind. As soon as driving off the road, the vehicle died down and would not start again. The check engine light came on. After several minutes the vehicle started again and drove to a secure area to call the tow truck. If the
... vehicle died down while in traffic, it would have caused an accident involving several vehicles. If it happened on the freeway, it would have caused a "major accident". The vehicle was towed to the vehicle dealership. It was determined by the vehicle dealership that the "high pressure fuel pump" was faulty and had to be replaced. It took four (4) months for the replacement part to arrive. Another similar dealer said that there were "55 other vehicle owners" waiting on the same part nationwide. It must be an "inherent problem" with the high pressure fuel pump. After replacing the high pressure fuel pump, the vehicle was operating normally.Read more

I was driving my vehicle after dropping my son off at school and my back windshield exploded without warning. I was unsure what to do and there was nowhere to pull over. I drove home and called my local Nissan dealer who I was leasing the vehicle from and they said the repair would not be covered. I used a local windshield replacement company and paid out of pocket to have the windshield replaced.

Component/System: Braking System (Caliper Bolt) and Suspension (Control Arm/Linkage Bolt). Description of Incident: While traveling on a tollway at 70 mph, I heard a faint noise followed by a severe metallic scratching sound coming from the front driver-side wheel. Upon inspection at a tire service center, it was discovered that a brake caliper bolt had fallen out, causing the caliper to displace
... and score the inner rim of the wheel. The vehicle was towed to a Tesla Service Center, where the caliper and wheel were replaced under warranty. Tesla could not identify a root cause but stated they performed a quality check on all remaining wheels. Subsequently, a second failure occurred on the front passenger-side wheel. A different bolt—this time related to the suspension—loosened or fell out. The vehicle produced a severe grinding noise and felt unstable, as if the wheel was about to dislodge. Safety Risk: Both incidents posed an immediate risk of catastrophic mechanical failure at high speeds. A detached brake caliper can lead to total loss of braking or wheel lock-up, while a suspension bolt failure can cause a complete loss of steering control. Either scenario could have resulted in a high-speed collision, endangering my life and the lives of my two children. Confirmation/Inspection: • Confirmed by Dealer: Yes, Tesla Service Center performed repairs on both occasions. • Available for Inspection: The vehicle has been repaired, but service records and photos of the damage/missing bolts are available. • Other Inspections: Initial driver-side failure was visually confirmed by Discount Tire. Warning Signs: There were no dashboard warning lamps or messages. The only symptoms were a brief "pop" or "click" followed immediately by loud metal-on-metal grinding and vibration.Read more

(FIX INFO BELOW) The Issue: The ignition intermittently fails to rotate to the "OFF" position, locking the key fob in the column despite the vehicle being in "Park." This is a known flaw in the Dart’s sensitive electrical/shifter interface that can lead to total battery drain and theft risk. <>Quick Fixes to Release the Fob<> I. The "Re-Park" Movement: Simply shifting gears is of
... ten not enough. Restart the engine, physically reverse the vehicle a short distance, then drive forward back into your space before shifting into park. The physical movement of the car is often required to reset the transmission sensor. II. The Twist: When you first park, try a quick, sharp twist to the "OFF" position immediately. Catching the system quickly sometimes prevents it from locking the fob. III. The Inward Firm Method: If already stuck, push the fob firmly inward toward the steering column (as if you are inserting your fob) while simultaneously twisting hard to the left. Repeat a few times before doing the re-park method to see if this will work for you first. Run/Acc>Off (be firm but if it doesn't work you're not trying to force/break it). <>Emergency Security<> If the tricks above do not work and you must leave your vehicle, you can still lock it and get back in. Make sure you apply your handbrake first just incase. - Physical Key Extraction: Locate the small release slide on the back of your key fob. You can slide the metal emergency key out even while the fob is stuck in the ignition. - Manual Locking: Use the door button, physical door locks or this metal key/a backup spare to manually lock the door/driver’s door from the in/outside. This prevents vehicle theft while the fob remains trapped. !!! Safety Conclusion!!! This issue highlights the volatile nature of the Dodge Dart's electrical system. While these workarounds are effective, the underlying sensor or shifter cable failure poses a recurring safety risk to owners.Read more

When driving at normal speed, my truck completely shut down with no warning. The instrument panel went dark and the engine shut off. I inspected the battery connection and nothing seemed out of place or degraded while trying to get the car out of the road. Highway Patrol also looked, but only was able to offer a tow truck as a solution. My truck has been at the dealer service for almost 2 days awaiting inspection at this point so I have no additional information on what caused this dangerous situation. There had been no indication prior, the truck had started and run normally and all scheduled services had been carried out by the dealer up to this point.Read more

car has 56K miles. went to dealer for the recall on the bushing and cap. left dealer and next couple days the car slid twice in my driveway a few feet even with parking brake on. went back to dealer and they said cant duplicate problem. Also the engine is NOT throwing a code. so dealer and another repair shop has charged 1600 for service and its still doing it!!!! Me researching seems to prove ford had transmission issues with this year of escape titanium and the edge etc.... why is there not a full recall on the transmission that they have to fix with such little mileage!Read more

While using Tesla's Full Self-Driving (Supervised) software on the Pacific Coast Highway (PCH) and other highways, the vehicle’s automated steering system repeatedly malfunctions by failing to maintain lane centering. Specifically, the car drifts or actively steers across double yellow lines into the lane of oncoming traffic. This issue is a persistent failure of the Advanced Driver Assistance S
... ystem (ADAS) and Lane Keeping Assistance. The vehicle has attempted this maneuver multiple times at [Approximate Speed, e.g., 40-60 mph], requiring immediate and forceful manual steering intervention to avoid head-on collisions with oncoming vehicles. The system provides no warning chimes, error messages, or "Take Over Immediately" alerts prior to crossing the lines; it appears to perceive the oncoming lane as a valid path or fails to recognize the double yellow boundary entirely. This has occurred during [mention conditions: e.g., bright daylight with high glare / night time]. The vehicle is equipped with Hardware 3.0 (HW3). I believe the 1.2-megapixel camera suite or the processing power of the HW3 computer is insufficient to safely navigate the geometry of the PCH or any roads within mountains/hills. I have attempted to "Clear Calibration" and performed system resets, but the life-threatening behavior persists. The vehicle and its internal data logs are available for inspection. I have also sent "Bug Reports" via voice command to the manufacturer immediately following these near-miss events.Read more
