... ersion 14.2.2.5 actively engaged. Three occupants were present in the vehicle, including two minor passengers. All occupants were properly restrained with seat belts. Component/System That Failed: The Full Self-Driving (Supervised) system, version 14.2.2.5, failed to safely detect and respond to a snow-covered road surface. The system lost control of the vehicle upon encountering a snow patch on the roadway, causing the vehicle to strike a tree. The FSD system is the primary failed component. The vehicle has been towed and is available for inspection upon request. How Safety Was Put at Risk: The FSD system was in active control of the vehicle's steering, braking, and acceleration at the time of the failure. The system provided no auditory alert, no visual warning, and no driver takeover request prior to the loss of control event. The failure occurred without any warning whatsoever, leaving insufficient time for the driver to intervene and prevent the collision, despite the driver actively supervising the system in full compliance with Tesla's own supervision requirements. "Shift to Park" alarm continues to appear when the car is in park. At times it does not clear for 10 minutes or so. On a couple of occasions I could not get it to go away for 30 minutes. While reversing into a parking space, the parking sensor system repeatedly indicated an obstacle on the left side of the vehicle even though no object was present. The system displayed a red warning on the left side and automatically applied the brakes very suddenly, causing the vehicle to jerk to a stop. Because the system continued to detect a phantom obstacle on the left side, it repeatedly interfered with maneuvering the vehicle while reversing. During this situation the vehicle ultimately made contact with a pole behind the car, which I did not initially realize because the braking intervention was so abrupt. The parking sensor was damaged in the impact. The vehicle was inspected by a BMW dealership, and they stated diagnostics showed no malfunction prior to the collision, only that the sensor malfunctioned after the impact. However, this was not the first time the vehicle’s parking system has falsely detected an obstacle and locked the brakes while reversing, which raises concerns about the reliability of the parking-assist automatic braking feature. The sudden braking and false obstacle detection create a potential safety risk, particularly if the system unexpectedly stops the vehicle or interferes with maneuvering in tight spaces. Back in December 2024 I had this exact issue of the car jolting and stalling. Kia said it was the transmission. I paid to have that replaced but it seems the vehicle is still having all of the same issues. My life was put at risk driving this 2021 Kia Seltos. I am very lucky that the cars around me were paying attention and did not crash into my car that came to a complete stop with vehicles going 65+ around me. ... erience loss of range and/or performance if the recall condition exists in the vehicle. The stated safety risk is that the vehicle may catch fire due to battery-related thermal propagation. I brought the vehicle to an authorized Volkswagen dealership for diagnosis and warranty/recall repair after learning of the recall. However, nearly two months after the recall was issued, the remedy is still not available. As a result, I remain in possession of and am expected to continue using a vehicle that is subject to a serious fire-related safety recall, without any available repair to correct the defect. This creates an ongoing safety concern. I do not feel safe continuing to drive or park the vehicle because of the possibility of a battery fire. My concern is heightened as warmer weather approaches, because heat may increase battery-related fire risks. Even if the event is described as rare, a fire risk involving a high-voltage battery is serious and presents an unacceptable safety concern for me as the driver and for others around the vehicle. The manufacturer has acknowledged the defect through the recall, but has not made a timely remedy available. I am asking NHTSA to note that consumers are being left without a repair for a serious recalled safety defect for an extended period of time. This incident could have resulted in losing control of the vehicle when the tires locked up or caused a fire when the oil hit the exhaust. It could have cause an accident if another vehicle was following. No lights were on until after the incident when the check engine and oil light came on. The vehicle became undriveable and could not be safely transported to a Ford dealership. It had to be repaired at a nearby independent shop (Auto Stop Elite of Purcellville, VA). The conductor plate was replaced on February 12, 2026, at a total cost of $1,689.59. Despite this being a well‑documented safety defect affecting many Ford F‑150 owners, no recall or service campaign was ever issued for my VIN. No warnings appeared on Ford’s website, Carfax, or through any Ford communication. Other owners have received reimbursement from Ford for the same failure, yet Ford provided no notice or remedy in my case. This defect placed my son’s life in danger and represents a clear safety issue that should be addressed. I am requesting that NHTSA review this incident as part of the ongoing pattern of conductor plate/lead frame failures in Ford’s 6R80 transmissions.
