While driving on the highway at 75mph, with "Traffic Aware Cruise Control" (aka, "Adaptive Cruise Control") enabled, a white napkin had blown into the lane ahead. This caused the car to slam on the brakes, and rapidly reduce speed to 20 mph, before I was able to re-accelerate back up to speed. I consider this a dangerous malfunction of the Adaptive Cruise Control system (and possibly the Automatic

Ford Motor Company has had a known issue with the truck frames cracking on the F-550 chassis. They acknowledged it by doing a recall, which either replaced or repaired the chassis frame or installed reinforcement braces if no cracks were visible. The recall expired in September of 2023. In October 2023 we noticed a crack on the frame just outside the recall reinforcement brace that Ford-certifi
... ed technicians installed. We found another in November (reinforcement brace installed) and another in January (with Ford's new and improved frame produced due to a recall issue). Ford has denied making the repairs since the original warranty expired, even though this was the same related issue. We have been dealing with Ford Motor Company since October in hopes of resolving the issue but they continue to give us any resolution. The problem has not only caused a burden on our workforce but also caused great safety concerns. With Ford avoiding responsibility, they are allowing trucks to operate daily on our highway system, which becomes a safety concern for not only the driver but for many other motorists. The problem existed, but the issue was not resolved, all we are asking is that they take responsibility for making the repairs and permanently resolving the issue for the safety of all motorists.Read more

Incident date: Apr 2, 2024
While driving normally, an error warning was received that stated "Front Left Safety Restraint System Error" and the icon for an airbag fault would come in simultaneously. This error would disappear and reappear, sometimes up to 10 times per minute, creating a driver distraction due to the loud error chime. The error code identified for the fault when checked on the Tesla service screen correspond
... ed to pretensioner wire harness. The pretensioner harness connects the seat sensors for driver weight and positioning to the car's airbag control system, which then classifies passengers and ensures that the airbags fire correctly in the event of a collision. For example, if a smaller female driver was operating the car but was incorrectly classified as male, the airbag could fire with excessive force and cause injury. The set of symptoms occurs commonly in Tesla Model 3s and are caused by the wear over time of a poorly designed wiring harness, which chafes as the seat is moved forward and backward until it breaks. This is a known problem by Tesla and they have a specific service bulletin designed to address it, which I have attached below. When taken in to address the issue, Tesla performed the procedure in the attached bulletin and identified a damaged harness, which was subsequently replaced. I retained the damaged harness for inspection. The invoice for this service is attached. No other defects were identified that could have caused the issue. This is a safety related issue occurring due to a manufacturing related defect and should be a mandatory recall with complimentary repair.Read more

Our car’s Android Auto navigation system is failing. This is not just a navigation failure. When we drive, especially at night, and our navigation system fails, it becomes a security and safety hazard. The problem: 1. After connecting a phone, TCL A30 Android 11, via the USB cable to the car navigation system and entering an address, frequently, the system gets frozen. After a few tries at physi
... cally connecting and disconnecting the cable, we were able to get the system going. 2. While driving, the navigation system frequently fails. Occasionally, it points in the wrong direction, freezes, or shows that the navigation position is lagging by miles from our actual position. Disconnecting and reconnecting the USB cable tends to resolve the problem. When we use the Subaru navigation system and, at the same time, concurrently employ a second phone navigation system, that standalone phone navigation system does not experience the failures we are seeing with the Subaru navigation system. 2. Frequently, we get a message about a lost GPS signal. This is an erroneous message. We know that because a second phone, not connected to the car navigation system, does not give GPS error messages at the time the car navigation system announces GPS errors. We did the following tests to verify that the navigation failures we observed were not phone or USB cable issues. We tested the car navigation system with two different phones and observed the same failures with both phones. We tested the system with two USB cables, and the failures persisted. We tested the USB cables for data transfers by downloading files from the phones to a computer. The file transfers from the phone to the computer were quick and accurate. It appears that the communication protocol that links the phone with the car’s navigation system is broken. We communicated our concerns to Subaru customer support. To the present time, we have received no response from Subaru.Read more

Incident Tesla Model S was up to date on recalls as far as we can tell, including the recent autopilot update via Tesla internet vehicle update releases. While driving in autopilot mode on the freeway in the lane next to the HOV lane, the autopilot got confused as to which lane it should keep and failed to maintain its lane. Incident vehicle drifted across the double lines into the HOV lane, where
... another Tesla Model S was driving. Both cars were damaged in the ensuing side bump impact, then the incident vehicle made a slight impact to another car rear bumper in front of the incident vehicle. No injuries were reported. A subsequent Traffic Crash Report was filed as required with CA Highway Patrol. We are awaiting this report in the mail. Our safety and others was put at risk due to the failure of the autopilot to maintain its lane. Insurance was contacted. The insurance company, USAA, has deemed the incident vehicle (our vehicle) damage too expensive to fix due to the very high cost of replacement/repair parts and has deemed our vehicle a "total loss." We have not been able to access the software of the vehicle. Since USAA has deemed the vehicle a total loss, they are selling the car for salvage and thus we have no further access to the vehicle for legal purposes. As far as I know, USAA has not pulled the vehicle software records nor has requested such records and has completely ignored the autopilot issue. That is part of the reason for our complaint here as well. Since there were no injuries, legal representation in this case has not been forthcoming. We ask the NHTSA to investigate since we can get no help from Tesla, USAA or any other legal advocate.Read more

Over the last 4 years I have had 2 prior occurrence where I've lost power turning or stopping, causing the engine to stall. Both times the vehicle was taken to our mechanic and he replaced the connectors to the battery. On January 18th, 2024, while turning off of a rural highway onto the street we live on I again lost power and the engine stalled and smelled a faint electrical smell. I called my m
... echanic and he said he could get it in on January 22nd. We checked all connections to the battery, made sure the battery terminals were clean. The morning of January 22nd, while on my way to the mechanic the vehicle in front of me stopped quickly. I started to stop a little quicker than normal, causing the front of the vehicle to tip down. I immediately lost power and the engine died, causing me to rear end the vehicle in front of me. The collisions was at slow speed, but it also caused disproportionate damage to my vehicle. Over the weekend of January 20th, we became aware if a recall for 2013 to 2018 Rav4s regarding the tops of newer batteries being smaller and causing problems with the battery shifting causing loss of power and potential fires. We strongly believe this recall needs to be extended to the 2012s.Read more

Later they stayed blinking fast and tail lights out again after stopping starting several times. Continental 235/40/r19 This is what I sent to Consumer Affair website: My personal experience reflects exactly the same as the poor rating of this brand. Tire has separation at 32k miles on the Tesla 3. I first noticed a thumb thumb sound. I ignored but it got worse then the vibration inside cabin started to occur. I pulled the tire off and inspected. My God i have a tire separation with steel

I got mad after i found out that Tesla wants nothing to do with it (heck they don't even a have no. to call for an expensive car, shame on you Tesla) and Continental does not have a warranty. Spending time searching for another tire to buy i find out it is expensive for EV tire. I was frustrated and angry that i have to deal with a defective tire with no warranty from Tesla and Continental for low miles (plenty of thread left on tire). I decided to call Continental. The experience was horrible with them hung up on me and a run around. They told me to take to Discount tire to have it inspect. Dt agreed tire is defective and expect me to pay $250 with pro rate covered by Continental. I already found the same tire for $225 so i tell D.t take a hike. I called Continental back and [XXX] broke loose. I got a lot of run around, hung up with the rep. then manager and they won't do the tire pro rate more than 30%. I got fed up with lot of cussing I went back to D.Tire and settled with another Continental tire that is cheaper. I was pushing my luck driving with the tire that will blow anytime. The [XXX] Tesla does not have a spare tire on an expensive car (shame on you Tesla). I had no choice to replace a tire out of my pocket of $150 for the same brand as that is the only one with similar thread pattern very reluctantly as I had NO other choice or take a risk driving and NO spare tire. While driving CA-17, as is frequently the case, I had to disengage automatic steering several times to correct its poor lane keeping. Much to my surprise, the vehicle stopped maintaining speed, creating a dangerous situation with vehicles behind me as the uphill road caused the vehicle to rapidly decelerate the second I turned the wheel. After experimentation, I discovered that Tesla has complet
... ely removed cruise control capabilities from these vehicles unless you completely disable the FSD beta functionality, which makes this vehicle highly unsafe to drive in any environment where autosteer is inadequate, including CA-17. Further, this isn't just unsafe on weird highways. This poor decision on Tesla's part could very easily cause rear-end collisions on freeways. Nobody in their right minds would expect a vehicle to suddenly massively decelerate just because you correct its poor steering. This is just plain DANGEROUS. This so-called "Full Self Driving (Supervised)" version needs to be recalled immediately, and Tesla MUST be forced to make cruise control fallback available regardless of the FSD state. Until they do, it is fundamentally unsafe to allow Tesla vehicles to remain on public highways and freeways with FSD (Supervised) enabled.Read more
