On 5/28/24, while pulling up the driveway, the car suddenly accelerated and it’s so fast that we couldn’t react. The car crashed into the garage, knocked down one wall along with the main electrical panel. It stopped because it got stuck on 10 in foundation’s of garage structure. Prior to this accident, the car already had issue with the driver’s profiles and the seat just moved back and stuck at the easy entry mode. The car became inoperable because the driver’s seat can’t be moved forward to a position where I could reach the gas and brake pedal. I contacted Tesla service and was told to delete the driver’s profile and create new one , power off the screen, power cycled the car. I’ve done all of that but the problem kept happening on and off. The car was scheduled to be looked at by Tesla on 6-20-24 but the accident happened on 5-28-24. We’d like to find out the root cause of the accident whether the car has any malfunction that could’ve triggered the sudden unintended acceleration. We’ve reported the accident to both Tesla and my insurance company. Able to switch to neutral and push back into spot. Could have been a very dangerous situation if it had occurred while driving or with my children in tow. Car would not restart at that point. At this point the brake pedal was hard and would not move, my back left wheel was completely locked. I shut the vehicle down waited a few minutes and restarted it, all of the lights were still active but I was able to move the vehicle. I began traveling forward and the rear wheels alternated locking up. I started backing up to move to a safe position, the brake pedal became hard and I was unable to stop causing my vehicle to collide with a wall, speed was below 5 miles an hour but still caused damage to my left rear quarter panel and fiberglass top. After we assessed the damage, I put the vehicle in 2 Wheel Dr., disconnected the battery for 30 minutes, and was able to drive the vehicle to a parking lot in 2 Wheel Dr. After this point and not having phone service, use a satellite communicator to call a local off-road recovery nonprofit that I volunteer for, they were able to assist in bringing a trailer out to tow my vehicle home as it is unsafe to drive and my children were with me. Unloading the vehicle off the trailer all the lights were gone with the exception of a check engine light is now on. My vehicle may be under warranty still, but my life is not, neither is my husband and children's lives. This vehicle is how we get places as a family and it puts all four of us at risk of injury or death. This is a HIGH risk glitch. I contacted the Dan Cummins dealership in Paris KY and they said they had NEVER heard of this problem? But it's ALL OVER the internet? And it hasn't been recalled? They said it would be 2 weeks before they could look at it and acted like it wasn't a major issue. I am furious. This is people's LIVES at risk. This isn't a camera malfunction, or a tailgate that doesnt lock. This is the engine shutting down then the E-Break engaging when driving and potentially flipping and spinning my vehicle out of control. Not to mention other problems like my driver side window glitching and not rolling up until I hit it two and three times, or the AC shutting down and blowing out hot air when the automatic hybrid kicks in/or the truck is half shut down at lights. Investigation revealed a puncture in the tread of the tire, caused by a gravel stone still lodged in the tire. Based on the amount of protrusion of the stone and the lack of any visible wear on it, I concluded that the stone must hav
... e originated in my gravel driveway when returning home the night before. The tire had been installed on 6/20/2023 at 104513.5 miles, being in service for 1 year and 9293 miles, not even 20% into its expected life. Initial assessment of the damage to the tire, including the puncture size and longitudinal shape, suggests that repair will not be feasible and the tire will require replacement. While a flat tire due to foreign objects is commonplace, the safety concern here is that a simple small stone from a gravel driveway should not have been able to cause the failure of a 'Kevlar reinforced' all-terrain truck tire. I believe the materials or construction of these tires may be defective and may have insufficient tensile strength in the tread material, causing them to yield and fail at much lower forces than are to be expected from tires in highway use. At any rate a well-packed gravel driveway that has been in use for many years should not be a challenge to the safety and performance of a truck tire rated for highway use. This tire failure comes less than a month and 1000 miles after a more dramatic and catastrophic failure of another tire of the same model on the same vehicle. While coincidence cannot be ruled out, two failures of the same model tire, both related to weak or defective tread sections, points to possible defects that should be investigated. After I have already told them what the problem is and what the code is for the problem. Which is the same code i got from my $40 code reader is the same code he got from his $1500 code reader, so what can the tell me for $ 167 that i dont already know and have told them, and from further research on recalls for this vehicle, I have found that they already know this problem exist. Toyota has issued on previous Technical Service Bulletin (TSB) - T-SB-0111-22 - that Toyota states should resolve an issue with "Hesitation From Stop and Surge Concerns". The TCM -Transmission Control Module
... - logic has been modiWed to address these conditions. According to Toyota, this updated logic SHOULD keep this from happening. This TSB was released December 16, 2022. To date, this issue continues to occur. The only way to keep it from happening is to Reset the TCM Learning. This requires the battery to be unplugged for an extended period of time. There was a subsequent TCM logic update for Recall 24TA02 (NHTSA Campaign Number 24V125000). It would seem that this TCM Logic Update has actually caused this condition to be present more often. This is something that we have expressed to our Service Consultant at every visit and it is still occurring over two years later. We have had this happen multiple times while attempting to proceed through a 4-way stop, as well as trying to merge into tra`c in slower moving city streets. There are members of an online forum who have been able to record the data aow and show when and how the delay occurs. Other than December 2022 TSB, Toyota has not acknowledged this issue even though it continues to occur on the MY22+ Tundra/MY23+ Sequoia, as well as the MY18+ Lexus LS 500. I am lead to believe this is a result of the programming for the Direct Shift 10- AT from Aisin/Toyota and/or the V35A-FTS, both Type '17 (LS 500) and the Type '21 (Tundra/ Sequoia).Read more

They set up lane closures that change every week or so. They use a line of red poles along the middle of the road to block off a lane and direct both lanes of traffic into one lane. 2020 Escape has a turn style shifter that becomes stuck in park and will not shift into gear on hot summer days making the car inoperable leaving unsuspecting drivers stranded. The issue happens almost weekly to daily to my vehicle. The dealership has found no cause for this issue as when the shifter is acting up and is inoperable, the dealership has no opening to serivce vehicle.

A sensor, or trio of sensors, in the passenger side airbag system failed. Nissan/Infiniti says they cant be sure which of the three sensors is causing the problem and needs to replace all three. Infiniti is unable to tell me how the passenger side airbag will react in the event of an accident, so that leads me to beleive the vehible is possibly unsafe, and a risk to anyone riding in the front passenger's seat. The problem has been inspected and is documented with the Infiniti and the Sewell Dealership in Dallas, Texas. The vehicle has not been inspected by the police or insurance representatives, but rather the Sewell Infiniti Service Center. There is an airbag light flashing on the dashboard.
