Model Y (VIN: [XXX]). On XXX, I brought my vehicle to Tesla Service for replacement of the front wheel hub assembly and paid approximately $1,000 for the repair. Shortly after picking up the vehicle, I noticed abnormal knocking and clicking noises while turning the steering wheel. ... y. On XXX, the independent shop inspected the vehicle and found that two wheel hub mounting bolts had not been properly tightened and that one mounting bolt was completely missing. The repair invoice states that the vehicle was being operated in an unsafe condition and that continued operation could have resulted in wheel hub failure and a serious accident. The shop corrected the condition by replacing the missing bolt and properly tightening the wheel hub assembly. I incurred an additional expense of $284.47, which would not have been necessary had the original repair been completed properly. I have received a dashboard warning from Telsa that there is a “Front passenger safety relevant safety issue”. Upon follow-up with the Tesla service team, I’ve learned that the sensor has failed and since they can only replace it with a new and different type of sensor, they also need to replace the air bag. Despite my intervention, the vehicle made contact with the boom barrier assembly. The impact caused significant damage to the windshield and driver's side door frame. Glass entered the passenger compartment. My infant child was in the vehicle at the time. Fortunately, no injuries were sustained. Tesla Dashcam footage of the incident was preserved and is available. A claim was opened with Tesla Insurance, and I was advised to submit a vehicle malfunction report for review of vehicle telemetry and FSD operation during the incident. After the collision I maneuvered the vehicle back into its lane, exited the toll highway and parked at a nearby shopping center to assess our injuries the vehicle. I was driving my Tesla in self drive mode, and ibecause it started raining. I reduce the speed to standard. I had a notification on the screen, an alert in red. That the tesla reached the maximum speed in that mode “standard”. I was surprised and decided ti canceled the FSD feature and took control of the vehicle. Drivers rely on immediate acceleration when crossing intersections, turning across traffic, or merging. A vehicle that unexpectedly limits acceleration can place occupants directly in the path of oncoming vehicles, creating a substantial risk of a severe collision, serious injury, or death. I reported this issue to Tesla Service and provided multiple timestamps documenting the events. Tesla was unable to identify any fault codes or provide a diagnosis and indicated that because no fault was recorded, the issue could not be confirmed. Through independent research, I found numerous reports from other Tesla owners describing substantially similar acceleration limitation events. Many reports specifically reference Obstacle Aware Acceleration as a possible cause, and some owners report that disabling the feature resolved the issue. If a safety system can falsely detect an obstacle and reduce acceleration while a driver is actively entering an intersection, the system itself may create a greater danger than the one it is designed to prevent. I respectfully request that NHTSA investigate whether Tesla vehicles are experiencing unexpected acceleration limitation or torque reduction during intersection entry and traffic merging situations, evaluate the role of Obstacle Aware Acceleration and related software systems, determine whether a broader pattern exists, and assess whether corrective action or a recall is warranted. Tesla does a warning if it can not detect your eyes on the road when using FSD (Full Self Driving). After few warnings on a trip it becomes a strike. I am reporting a safety concern involving a Tesla operating with Full Self-Driving Supervised active. On June 5, 2026, at approximately 7:50–7:55 AM Pacific Time, the vehicle was being driven with FSD Supervised engaged. The vehicle was in a turning maneuver when the driver mistakenly applied the accelerator. Vehicle data provided by Tesla appears to show ACTIVE_FSD shortly before the incident,
... a turning/steering angle, accelerator-pedal input/override, FSD becoming unavailable, and then crash-related/braking/tire-pressure signals shortly afterward. I am not claiming the vehicle accelerated on its own. The data appears to show driver accelerator input. My concern is that while FSD Supervised was active and the vehicle was already turning, the system allowed a strong accelerator override that rapidly worsened the situation. In this case, the vehicle went over a curb/sidewalk area and sustained significant damage. The safety concern I am reporting is whether FSD Supervised should better limit, warn, or manage strong accelerator override during low-speed turning maneuvers, especially where the vehicle may leave the roadway, cross a curb, or enter a sidewalk area. This causes two clear safety hazards while driving: 1 It blocks the screen: The popup actively covers up part of the navigation map and basic screen controls. If you are trying to navigate a tricky intersection or see your route right after taking over, you can't see the map clearly because this giant survey is blocking it. 2 It forces you to look away from the road: Because the prompt never goes away on its own, it forces the driver to physically look down, read the small text options, and tap the screen to clear it. Forcing a driver to do a multiple-choice survey while actively managing a manual takeover in traffic is incredibly dangerous. Tesla should not be allowed to use a mandatory, permanent UI popup to collect data while a vehicle is actively moving. It is a severe visual and cognitive distraction at the exact moment a driver needs to be 100% focused on the road. Please investigate this software design and require Tesla to add an automatic timeout or a quick-dismiss button that doesn't require driver interaction.Read more

The technician's inspection revealed that critical structural components were loose and failing. The technician explicitly warned me that this specific failure carries a direct risk of sudden wheel separation while the vehicle is in motion. This forced me to take immediate, emergency action right there at the dealership; leaving the shop without fixing it was simply not an option, as driving my car in that condition would have caused immediate and dangerous consequences on the road. A wheel separating or a suspension collapsing at highway speeds is an inherently fatal scenario that endangers my life, my passengers, and the public. This mechanical failure is highly similar to the defect described in NHTSA Number: 21V835000, which covered front suspension lateral links separating from the subframe and carries the same dangerous consequences. However, because my vehicle is a '22 model, Tesla refused to acknowledge the connection to that safety campaign and denied coverage under the drivetrain warranty. I was forced to pay over 800 out of pocket to resolve this hazard. While I could afford this, many drivers cannot. Tesla’s failure to expand their recall to the '22 model year means thousands of drivers are unwittingly operating a ticking time bomb.
