The defective car was completely totaled and towed to a salvage yard by the police and the insurance company. Defect malfunction caused a collision which resulted in property damage and personal injury to three vehicles and three people. I mentioned the defect to my insurance company at the time of the accident. I do not know if they chose to follow up and inspect the vehicle. The problem occurred twice before ( not resulting in property damage or injury) and I took it to my Subaru dealer both times and they claim that they could not find anything wrong, but oddly did not note that they checked for the problem which I brought to their attention on the written summary/invoice for my service visit. We had to put an extension on generator compartment door just so we could keep it shut, it had come open while we were traveling. 8. Closet shelfs do stay, they fall out every time we travel. At first it was only the top shelf now the second shelf fell out on our last trip. 9. Slide floor broke and fell. This slide is also missing or broken off screw on the outside. 10. In front wardrobe closet the trim is pulling away from walls, and shelves have gaps against the walls. 11.Missing lag bolt 12. Incident happens every 1k miles or more. Car is in motion driving. Press down on throttle and transmission drops to 1st gear. A bunch clicking and thumping sounds are heard. Car doesnt accelerate(can be at 70mph), starts to slow quickly until gears change. Also 4H is jerky as well as sport mode. Very unsafe to be traveling on the highway trying to overtake a vehicle and then your car fails.

On 5/28/24, while pulling up the driveway, the car suddenly accelerated and it’s so fast that we couldn’t react. The car crashed into the garage, knocked down one wall along with the main electrical panel. It stopped because it got stuck on 10 in foundation’s of garage structure. ... crashing into the garage. Airbags on both driver and passenger sides did not deploy. Prior to this accident, the car already had issue with the driver’s profiles and the seat just moved back and stuck at the easy entry mode. The car became inoperable because the driver’s seat can’t be moved forward to a position where I could reach the gas and brake pedal. I contacted Tesla service and was told to delete the driver’s profile and create new one , power off the screen, power cycled the car. I’ve done all of that but the problem kept happening on and off. The car was scheduled to be looked at by Tesla on 6-20-24 but the accident happened on 5-28-24. We’d like to find out the root cause of the accident whether the car has any malfunction that could’ve triggered the sudden unintended acceleration. We’ve reported the accident to both Tesla and my insurance company.
