The transmission is failing. It is a known issue. 2018 Ford F150 Lariat. The transmission shifts very hard; it's slipping out of gear. It is getting stuck in gears and not shifting as it should. It is flaring when shifting gears. My safety is at risk because I never know when the vehicle is going to move if I am stopped. It could happen in the middle of an intersection; it could go out when I'm go
... ing at highway speeds. Yes, the problem has been confirmed by a Ford dealer. The component has not been inspected by anyone on my personal vehicle except by the Ford technician at a local dealership. There were no warning lamps or messages. At first, when you would pull out for the first time of the day, it would have a little trouble shifting through 3rd and 4th gear for the first few minutes, then it was fine the rest of the day. Now it is slipping out of gears, shifting hard, missing shifts, banging when shifting gears, flaring when shifting gears, getting stuck in gears. I did not know this was an issue before buying the vehicle. After it started occurring, I looked it up, and this is a VERY widely known issue with this Ford 10R80 transmission in multiple models of their vehicles, and they REFUSE to recall them. This is an incredible safety issue, as the vehicle can slip out of gear or randomly change to the wrong gear at any time and make the driver lose control of the vehicle.Read more

While uninstalling a Chicco Fit360 ClearTex rotating convertible car seat, we discovered that the load-bearing webbing connecting the car seat base to a lower-anchor connector had completely separated. The car seat had not been involved in any crash or impact, and nothing struck or contacted the seat during removal. The separation was not visible during normal use. The seat had become loose, but
... we did not know that the lower-anchor webbing itself had failed. Because this webbing secured the base to the vehicle’s lower anchor, the failure severely compromised the installation while our child had been riding in the seat. In a collision, the restraint might not have remained secured to the vehicle. The car seat has been removed from use. It has not yet been inspected by the manufacturer, police, insurer, or another party. The seat, separated webbing, photographs, and video have been preserved and are available for inspection upon request. We purchased two identical seats and are concerned that other units may experience the same safety-critical failure. We request investigation as a potential safety defect.Read more

I am reporting repeated failures of the front safety camera / Collision Mitigation Braking System (CMBS) on my 2017 Acura MDX Tech (FWD). The vehicle has experienced intermittent and sudden loss of all ADAS safety functions, including CMBS, Lane Keeping Assist, Adaptive Cruise Control, and Road Departure Mitigation. The failures occur without warning, while driving at normal speeds. The dash disp
... lays “Safety Support System Problem,” “Collision Mitigation System Problem,” “Lane Keeping Assist Problem,” and related alerts. When this happens, all forward‑facing safety systems shut down, increasing the risk of collision. The issue is persistent and unpredictable. It has occurred in clear weather, dry roads, and normal driving conditions. The vehicle does not have the Advance package or surround‑view system; this is the standard Tech‑package front camera. Multiple Acura service departments have confirmed the problem is caused by a faulty front monocular camera module (part number 36160‑TZ5‑A36). Acura has superseded this part several times (A63, A64, A65, A66), indicating an ongoing design or reliability issue. Replacement cost is extremely high, and the failure disables federally‑relevant safety systems. This is a safety‑critical component. When the camera fails, the vehicle loses collision‑avoidance capability, lane‑keeping assistance, and adaptive cruise control. The failure happens while the vehicle is in motion, creating a risk of rear‑end collisions or lane‑departure accidents. I am requesting NHTSA review this issue for potential defects in the Acura MDX front safety camera system.Read more

My Honda Ridgeline has experienced a dangerous malfunction involving the Auto Idle Stop system. When the vehicle comes to a stop, the system automatically shuts off the engine as designed to conserve fuel. Normally, the engine should restart when the brake pedal is released. On multiple occasions, however, the engine has failed to restart when the brake pedal is released. A warning appears on the
... dashboard, and the vehicle becomes difficult to restart. This has left me unexpectedly unable to move the vehicle while stopped in traffic. This creates a serious safety hazard because the failure can occur at an intersection or in an active traffic lane. When traffic begins moving, the vehicle may remain stalled while surrounding drivers expect it to accelerate normally, creating a risk of a rear-end or other collision. The problem is unpredictable, and I am concerned that the Auto Idle Stop system can disable the vehicle during normal operation without adequate warning. I believe this condition should be investigated as a potential safety defect.Read more

I purchased this new 2026 Kia Seltos with approximately 32 miles. The vehicle now has fewer than 600 miles and has experienced two incidents of sudden, intermittent, complete loss of accelerator response. During one incident in highway traffic, the vehicle unexpectedly began decelerating and pressing the accelerator pedal produced no acceleration or meaningful propulsion. I could not maintain spe
... ed or accelerate to move out of surrounding traffic. The vehicle became unable to move normally until it was turned off and restarted. An oil-pressure warning light illuminated during the failure. There was no meaningful warning before the loss of acceleration. After the first incident, an authorized Kia dealer found diagnostic code U200300 only in the vehicle’s history. The dealer attributed the condition to a loose right battery cable, repaired or tightened the connection, and returned the vehicle as fixed. The same loss-of-acceleration condition occurred again after that repair. On July 13, 2026, an authorized Kia dealership technician personally reproduced the failure during a 30-mile test drive. The technician documented that the vehicle felt as though it died during acceleration and that pressing the accelerator would not make the vehicle go anywhere. No diagnostic trouble codes were generated or found during this dealer-confirmed failure. The dealer and Kia Techline inspected the vehicle. The dealer documented proper oil level, no obstruction in the air filter or throttle body, no visible leaks, approximately 50 psi of oil pressure at idle, and no visible corrosion or damaged connector pins. The diagnostic records provided to me end while Kia Techline was still asking about the oil-pressure sensor. I have not been provided with a confirmed root cause or documentation showing what specific repair permanently corrected the loss of propulsion. The failure creates a serious crash risk because the vehicle can suddenly lose all useful acceleration in highway traffic, lRead more

On multiple occasions, under different driving conditions, the vehicle has experienced significant and rapid deceleration without any warning lights, diagnostic codes, or dashboard messages. The deceleration has occurred unexpectedly and creates a dangerous situation, particularly in traffic where a car or semi behind me is not anticipating the sudden decrease in speed. This requires me to fully d
... epress the gas pedal which then creates a slingshot effect once acceleration takes hold. This situation was almost continuous on a recent long-distance drive and only stopped once I parked the car in my garage. The only “fix” for now is to drive in cruise control where speed will be maintained. Since then, I’ve only driven locally and it has happened almost daily but much more intermittent with local driving. It's happened at speeds from 12 mph-74 mph. This is NOT an adaptive cruise control issue and I drive with only my right foot.Read more

Nelson v. Ford Motor Company is a class action lawsuit targeting a defect in Ford's 2.3-liter EcoBoost engines. I have had my 2016 ford mustang ecoboost in and out of mechanic shops for months now accumulating well over $7K in out of pocket expenses. I have been to multiple mechanics trying to d
... iagnose and fix the problem but in the end it was a design flaw in the 2016 ford mustang ecoboost 4 cylinder which caused it to leak coolant, misfire, and overheat. I have been having this problem for some time now, this put my safety and the safety of others at risk because I had to slow down dramatically in order to pull over on the freeway due to the engine overheating from coolant loss. I've also experienced problems like trouble starting, rough idling, loss of coolant, and huge clouds of white smoke coming from the exhaust upon start. I have saved every single receipt and invoice breakdown since this problem began. My mechanic has also been documenting everything including taking notes and photographs of every issue. My car is currently in the shop right now being worked on. My mechanic says that he can temporarily patch the problem with a high performance head stud, copper sealant, and an ExtremeHeat metallic repair paste by JB Weld to momentarily solve the issue. He says that he does not know how long this patch will last so he is insisting I either get a new car or to see what my options are regarding the class action lawsuit filed against Ford.Read more

... prior to May 2026 but were undocumented. Occurs most frequently decelerating off highway exit ramps but also during general highway coasting in Houston, TX. A passenger was present during multiple incidents. Trailing vehicles are not anticipating the sudden speed reduction, creating rear-end collision risk. ISSUE 2 – FALSE BLIND SPOT / REAR RADAR ALERTS (ADAS): The Blind Spot Monitoring and Rear Radar Sensor systems generate daily false alerts — both audible and visual — when no vehicles or objects are present. Triggers randomly across varying conditions. False alerts have caused multiple aborted lane changes that were otherwise safe to execute. ISSUE 3 – LANE KEEPING ASSISTANCE (ADAS): The LKA system applies forceful steering corrections during intentional, signal-indicated lane changes, including in areas without lane markings. Occurs at highway speeds during on/off-ramp transitions. Corrections have triggered reactive maneuvers by surrounding drivers in dense traffic, causing near-collision situations. The leather-covered dashboard panel, in the approximately 4-6 inch section between the base of the windshield and the first seam across the dash, has shrunk, rippled, and pulled away from the underlying substrate. This area also houses the driver-assist camera housing and the defrost vent. The vehicle was purchased new and has been Ford-maintained, professionally detailed, and garage-kept througho

The vehicle's build date is January 2018, which falls within the range covered by Ford Technical Service Bulletin 18-2283 ("Instrument Panel Sagging at the Windshield Glass"), issued for vehicles built between October 13, 2014 and February 2, 2018, describing the instrument panel sagging and separating near the windshield. I am filing this complaint because this appears to be a documented, recurring defect pattern affecting this vehicle's dash generation, rather than isolated cosmetic wear. Ford Customer Relations has twice declined goodwill assistance, citing the vehicle's age (just over 8 years) as the sole basis, without physical inspection or evaluation of the underlying condition. An independent repair estimate to replace the dash panel and remove/reinstall the windshield for access was quoted at approximately $3,100.Read more

To be clear, the front lower lateral links and compliance arms all had to be replaced.

I am reporting a serious safety concern with my 2024 Jeep Wagoneer. For approximately six months, I have repeatedly taken the vehicle to an authorized Viva Jeep dealership in [XXX], because of transmission and acceleration concerns. The dealership has attempted to diagnose the issue, including keeping my vehicle for approximately one month, but the problem has not been resolved. On XXX, while dri
... ving, the vehicle unexpectedly accelerated without my intention or command and reached approximately 90 mph. I was able to regain control and safely stop the vehicle. The following day, I experienced additional abnormal behavior. When pressing the accelerator, the RPM and shifting did not respond normally, and the vehicle appeared to remain in an incorrect gear. I have repeatedly reported concerns about the transmission/powertrain for months, but the problem continues and has now escalated into a serious safety concern. I do not feel safe operating this vehicle. I am requesting that NHTSA document and review this incident as a potential vehicle safety defect involving unintended acceleration and transmission/powertrain malfunction. We were driving from [XXX], to [XXX], on XXX. That morning we'd taken the Honda Pilot in to Xpresso Lube and got an oil change and all fluids checked out. Eight hours into the drive we heard a little clanking and started to pull off the freeway. As we did so, a Low Oil Pressure light came on for a few seconds and then we heard a loud bang and then very loud, rhythmic, clanking noises in the engine
... as it was losing power. The first mechanic shop we had it towed to diagnosed engine failure, so we had it towed to a Honda Service shop 30 miles away. They said it was a rod bearing and the entire engine needed replacing. Our mileage at the time it entered the Honda shop was 72, 275K. We have done regular, consistent oil changes and maintenance on this car that we bought new in 2019 (a 2018 model). We at first wondered if the wrong oil was put in or if it was a faulty oil filter, but Honda said there was plenty of oil in the engine. We did get a sample of the oil that came out. The filter will be sent in to the manufacturer for quality assessment. Our safety was at risk to some degree, but we were able to pull off the road. The Honda dealership is holding the engine for me and could be inspected.. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Multipurpose Camera Failure causing multiple displays on dashboard
Incident date: Jul 21, 2026
I am reporting a product safety defect regarding an aftermarket dashboard cover manufactured by Seatz Manufacturing ([XXX]) for the 2018 Nissan NV200. The company's official website explicitly admits that their pattern covers the passenger airbag deployment zone, claiming the airbag will simply 'explode and blow through' the fabric to push it out of the way. However, their physical pattern feature
... s pre-sewn Velcro loop tabs directly along this deployment seam, and the instructions mandate applying heavy adhesive fasteners to lock them down. Relying on an airbag to rip through anchored, industrial Velcro tape creates a severe mechanical obstruction and a dangerous deployment delay. This design directly violates Nissan's strict factory safety mandates to never place objects over the airbag inflation zone." CORRECTION TO NHTSA COMPLAINT 11754243 (8/2/2026). That complaint was submitted late at night with assistance; several fields were entered wrong and the electrical failure history was omitted. Apologies for the error. Please associate this with XXX. CORRECTIONS: Vehicles towed - YES, TWO, not "No"; my Genesis is still at the dealer it was towed to in [XXX]. Injured - THREE, not 1. Police report -
... YES, not "No". Mileage about 23,500. Speed at T0 per the EDR - 23 km/h, about 14 mph. OMITTED AND MATERIAL: Before this crash the vehicle STARTED ITSELF TWICE WITH NO HUMAN COMMAND - no key press, no remote start, nobody in it. Separately the entire system went completely black, causing TOTAL ECU AND ECM FAILURE. The vehicle lost electrical integrity. WHY THAT MATTERS: Genesis Service Campaign 914G is "ECM Update - P030X Misfire"; its remedy is "Update the ECM (Engine Control Module) software." It covers 2022-2023MY GV70 (JK1) built 3/29/2021-3/4/2025, VIN starting KMU. My VIN is [XXX], a 2023 GV70. Owner notice was due September 2025. The campaign was still open and unremedied when the car crashed 12/3/2025. THE EVENT: Exiting a highway at low speed with no impact, collision or obstacle of any kind, the airbags deployed spontaneously. The deployment caused loss of control and a three-vehicle collision. The deployment caused the crash. No warning lamp or symptom preceded it. FAULT HISTORY: B139500 cross coupling (SRS firing-loop interconnection fault), B1650 (airbag control unit fault history), and multiple LOW VOLTAGE codes. An electrical system that starts an engine uncommanded and blacks out an ECU can fire a restraint with no crash. EDR: The report states the EDR is part of the Air Bag Control Unit and that if ACU power is lost during an event, all or part of the data may not be recorded. Manufacturer closed its claim with no repair or explanation. Vehicle and ACU remain available; no dealer diagnostic was done on the ACU. Went to get into my 2024 Acura MDX parked car this morning temperature around 68 degrees and noticed the back window of my 2024 MDX had shattered out. Driver seat feels like something is loose in the back and is popping randomly almost like somebody’s kicking you in the back. It can be very distracting while driving. This happened on the 2026LE I had that I turned in because of that problem. I’ve had it seen several times by the dealership for this issue and there’s no solution. There’s also an issue with the wiper blades going extremely

This may seem like a time in which you should just pull over, but I was born and raised driving in South Florida in the rain and I’ve had multiple vehicles that I’ve never had this issue before. Electronic system can be fidgety. The rear cross traffic warning stopped working. I was not aware and did not realize I had somebody in close proximity that could happen accidentally hit.Read more
