Component or system that failed: Electric Drive System / Charging System / Software – repeated failure displaying “EV System Off” message, rendering vehicle inoperable. Description of incident: This is the second time within 30 days that my 2025 Nissan Ariya (VIN: [XXX] ) has completely shut down and become undriveable. On [XXX], while attempting to use a DC fast charger at Electrify Americ
... a (1150 E Flamingo Rd, Las Vegas, NV), the vehicle failed to begin charging. After a few attempts, the dash displayed “EV System Off” and the car could not be restarted or shifted into gear. It had to be towed again — same as the incident on [XXX], which occurred under nearly identical circumstances. Safety risk: This failure left me stranded twice, with no warning or ability to drive. Had this happened on a freeway or in a rural area, it could have been dangerous. The dealer has only reset the vehicle each time with no resolution. Reproduction and inspection: The issue was inspected by Findlay Nissan Henderson. They stated that other Ariya owners have reported similar failures, especially after using Electrify America stations, and implied it may be a compatibility issue — yet this is not publicly disclosed by Nissan. I have charged successfully at Electrify America many times before, and the same charger worked immediately after my failure. Warning signs: No warning lights appeared before the failure. The “EV System Off” message appeared suddenly after connecting to the charger. Summary: This is a critical systems failure that recurs, impacts safety, and limits reliable use of the vehicle. I request this be formally investigated for a potential design or software flaw. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

ICCU (Intergrated Charging Control Unit) failed after two mandatory recalls were completed. Vehicle is not drivable as it’s awaiting for a replacement ICCU (backordered). The section where the vehicle shutdown was without cel

The component has been inspected by the manufacturer and dealer with a temporary remedy of replacement. Warning lights appeared after the failure occurred; essentially giving the driver no warning or time to react. The vehicle displayed a warning message to pull over and turn off vehicle immediately. Power was reduced to 15mph for about 15-20 minutes.Read more

I have a constant issue with the power steering. This issue makes it hard for my vehicle to turn properly as well as when you go to turn a corner you have to be a body builder to get the car to even turn. There has been times when I am driving my car that I have to pull over to the side of the road very carefully because I feel like my car is going to die and then when I turn it off and then back on it acts like it has absolutely no power. I haven't gone anywhere to get my vehicle checked yet. I am reporting a repeated and potentially dangerous transmission failure in my 2020 Mazda CX30. At 6 months old, the original transmission failed. Mazda confirmed it was defective and replaced it with a refurbished unit with 10,000 miles on it. That replacement is now also failing—within 4 years of the first failure. This means the car has experienced two transmission failures before reaching 5
... years of service. This raises concerns about: •The reliability of Mazda’s factory parts •The quality and safety of refurbished transmissions Mazda uses in warranty repairs •A potential pattern of premature transmission failures on this model Repeated transmission failure can cause loss of power and control, posing serious safety risks to drivers and passengers. Mazda has all service records confirming both incidents. I am submitting this to alert NHTSA to a possible defect pattern that may warrant investigation.Read more

I am reporting a potential safety and mechanical defect involving the cylinder head in the Mercedes-Benz GLA 250. Given the frequency and severity of this failure — and the high cost of repairs — I believe this warrants an official investigation and potential manufacturer recall. I urge the NHTSA to review similar complaints and initiate a formal inquiry into Mercedes-Benz’s awareness and handling of this defect. Thank you for your attention to this matter.Read more

I later discovered that this exact vehicle is subject to Reca
... ll 23V652000, related to an electrical short in the Hydraulic Electronic Control Unit (HECU) that can result in an engine compartment fire while the vehicle is parked or driving. As of now, the recall remains open and unrepaired under this VIN. I was never notified of this critical safety recall before the incident. Kia’s authorized dealer in Puerto Rico (MOTORAMBAR) confirmed the recall but declined responsibility for the damage already caused, offering only the standard fuse replacement. I have submitted a written complaint to Kia and am now documenting this safety failure with NHTSA. This fire was not caused by misuse or wear-and-tear — it occurred under normal conditions and clearly matches the exact language of the recall defect. I believe this represents a serious failure of notification and manufacturer accountability. I am seeking resolution and accountability through both Kia and the NHTSA. * The component that appears to be malfunctioning is the driveshaft and/or related transmission system. I have experienced repeated issues such as abnormal jerking when switching gears, grinding or clunking noises, and jerking through the drivetrain with erratic RPMs before the vehicle regains power. These symptoms are consistent with problems covered in recent recalls for the 2020 Ford Explorer.
... The vehicle is still in my possession and available for inspection upon request. *The issues have made it feel unsafe to drive the vehicle, especially with children in the car. The unpredictable jerking and loss of power present a serious danger, particularly when accelerating into traffic or driving at highway speeds. I am extremely concerned that a sudden failure could result in loss of control, a rollaway, or even a serious accident. Another concern is the defect involving the driveshaft or half shafts that might disconnect or stop functioning. This disconnection can lead to a loss of drive power or, if the parking brake is not applied, a possible vehicle rollaway. *While I have not yet had the issue diagnosed by a service center, I have filed a lemon law claim, and I’m in the process of addressing the matter formally. The symptoms I’m experiencing appear to match known issues that Ford has already recalled for the 2020–2021 Explorer models. *The manufacturer has not yet inspected the vehicle, and no inspections have been conducted by police or insurance representatives. I am open to having the vehicle inspected by Ford or an authorized party as needed. *There were no warning lamps or messages displayed prior to the issues occurring. The symptoms—jerking, clunking noises, and erratic RPM behavior—began gradually over time, with the first noticeable incident occurring a few months ago. Since then, the issues have become more frequent and severe.Read more

This is a second report of an issue that first happened in February 2025, the dealer replaced the battery and called it fixed. The dealer had it three days and replaced the brake master cylinder and called it fixed, I picked the vehicle up and on the way home I stopped for gas, after the gas station the dash displayed: Service brake assist, I turned right around and took it back to the dealer, they checked the code and said the brake assist module was drawing too much current, I picked the vehicle back up three days later, they said they bleed the air out of the brake system and called it fixed. After routine service at South Coast Toyota in Costa Mesa, CA, I unknowingly drove my 2023 Toyota RAV4 Hybrid for over a week with two missing lug nuts and one extremely loose lug nut on the front passenger wheel. The service performed on April 5, 2025, included an oil change, tire rotation, and full inspection. No issues were reported to me at pickup. Over the next several days, I heard increasin
... g thumping and metallic noises from the front right wheel area, especially at highway speeds. On [XXX], while driving with children in the car, the vehicle made loud metal screeching noises and the steering became impaired—I could not complete a basic turn. I limped the car home at 4 mph. Upon inspection, I found two lug nuts missing and one so loose I could unscrew it by hand. I had the car towed back to South Coast Toyota on April 18. The dealership admitted two lug nuts were missing but failed to apologize or fully document the issue. I was told via text that the only action needed was replacing the missing lug nuts. I received no formal inspection report or confirmation that the vehicle was safe. I paid out of pocket for a third-party inspection at a separate Toyota dealership, which confirmed that the previously recommended sway bar replacement was unnecessary and that my tires and components were in better condition than initially claimed. This was a clear service-related safety failure that could have led to wheel detachment and a serious accident. I drove over 200 miles on freeways with children in the vehicle. The dealership failed to follow basic safety protocols and has not taken responsibility. I am reporting this to NHTSA in the interest of safety and accountability. I am writing to formally request reconsideration for including my 2017 Kia Forte in the existing recall (NHTSA Recall No. 21V-260, Manufacturer Recall No. SC210) affecting similar 2017-2018 Kia Forte models. Despite my vehicle not being initially included, I believe it should be reconsidered due to the following reasons: 1. Similar Engine Issues: The recall pertains to certain 2017-2018 Kia Forte
... models manufactured at the Kia Mexico plant between June 5, 2017, and July 24, 2017, equipped with 2.0L Nu MPI engines. My vehicle, manufactured at the Kia Mexico plant, is equipped with the same 2.0L Nu MPI engine and has experienced similar issues as described in the recall, including abnormal engine noise and warning lamp illumination prior to engine failure . 2. Consistent Maintenance Record: I have maintained my vehicle consistently, adhering to the manufacturer’s recommended maintenance schedule. The Carfax records demonstrate regular oil changes, brake inspections, tire rotations, and other necessary maintenance performed both at authorized Kia service centers and independent shops. The most recent maintenance was conducted at 112,530 miles, just before the engine failure occurred . This demonstrates that the failure was not due to neglect or lack of proper maintenance. 3. Premature Engine Failure: My vehicle’s engine failed before reaching 116,000 miles, which is significantly premature for a well-maintained engine of this type. Given the similar defect characteristics — oil pressure issues caused by foreign particles in the oil pump variable relief plunger — I strongly believe that my vehicle’s failure aligns with the recall conditions described . 4. Safety Concerns: The defect described in the recall poses a significant safety risk due to the potential for loss of motive power while the vehicle is in motion. My experience aligns with this risk, as the engine failure occurred without sufficient warning, putting me and others at risk.Read more

[XXX] Here is a link that explains this further. The oil pump on this vehicle has gone out with 11,000 miles on it. This a known problem from GM and they will not fix it. I am being told my car is out of warranty so no luck. This is not right. The pump should not go out that quick. I am being thrown to the side because I cherished my GM vehicle and didn't drive it as much as others. It risks b

The vehicle rolled and killed the occupant because the roof caved in. The shoulder portion of the seat belt did not hold the occupant (only the lap portion) and the occupant was partially ejected out of the rear of the cab. The vehicle was inspected and purchased by State Farm insurance since the vehicle was totaled. The 2000's Ford F-150s have roofs that are too weak to support the weight of the
