How was your safety or the safety of others put at risk? My safety was significantly at risk. I had no control over stopping the vehicle despite repeated brake attempts. To avoid a direct collision, I was forced to swerve at the last second, hitting the rear edge of the truck ahead and deploying my airbag. Had I not swerved, the impact could have been more severe and potentially endangered the other driver. Has the problem been reproduced or confirmed by a dealer or independent service center? Not yet. My insurance is working to determine whether an inspection is possible due to the extent of the damage. I intend to have the braking system inspected to confirm the failure if it can be safely accessed. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? The accident has been reported to my insurance, and they are aware that I believe the brakes failed. I’m waiting to hear whether an inspection by their representatives or a third party will be conducted. To my knowledge, the manufacturer has not inspected the vehicle. Were there any warning lamps, messages, or other symptoms of the problem prior to the failure, and when did they first appear? No brake-related warning lights or messages appeared before or during the failure. The turbocharger failed on my 2017 Hyundai Santa Fe Sport 2.0T. It is still on the vehicle and available for inspection upon request. The turbo failure caused a severe loss of engine power, making acceleration dangerously slow and unpredictable, especially when merging onto highways or passing vehicles. ... avoid faster-moving traffic. Yes. The failure was confirmed by two independent service centers: Mavis Tires and Brakes, and Tires and Trust in Pensacola, FL. Both observed loss of turbo boost and excessive oil consumption consistent with turbocharger failure. Yes. The vehicle was also inspected by Allen Turner Hyundai in Pensacola, FL. They did not detect the failure during their inspection, but the issue persisted and was documented by the other two shops. The check engine light came on around 118,000–119,000 miles. After that, the turbocharger lost significant power, making the vehicle unable to accelerate normally. At the same time, the engine began consuming oil at an abnormally high rate, requiring frequent top-offs between oil changes. This suggests internal oil seal failure in the turbo or related component issues. These symptoms are consistent with failed turbocharger bearings or seals, oil starvation, or blocked oil lines causing overheating and damage. Such failures can release oil and metal debris into the intake and exhaust systems, risking further engine damage or even fire. Hyundai previously issued a software update (KSDS) to detect early bearing wear in these engines, which my vehicle received in 2019 (Service Campaign 953). Despite this, the turbo failure still occurred, suggesting the underlying design or manufacturing issue may not have been fully resolved. On Sunday, July 13, 2025, I received a warning that my EyeSight was disabled in the middle of driving. This meant that I was unable to utilize cruise control or any driver assistance functions. Cruise control and driver assistance features were unavailable. On the morning of Tuesday, July 15, I came out to a 7-8 inch crack in the windshield, on the passenger side. No impact had occurred to cause this crack and it was not present the previous days. EyeSight is still disabled due to this and the windshield needs a full replacement. All driver assistance/collision prevention features are not available until the windshield is replaced and a recalibration is completed. Advantage Auto Glass of Rockford, IL will be inspecting the glass and can make it available for NHTSA evaluation. However, given that I have owned two Subaru Outbacks now, windshield weakness has been an issue with this vehicle, especially in warmer weather.Read more

I explained I rely on this vehicle for work and couldn’t leave it that long without transportation. I was dismissed without any explanation of the warning. Despite reporting a system warning related to crash prevention, the dealership allowed me to continue driving. No inspection was done, and I received no help understanding the message or resolving the issue. On May 8, 2025, I missed a stop sign and collided with another vehicle. The crash caused major damage and totaled my car. The Forward Collision System never activated—no alerts, no automatic braking. The vehicle was advertised to include safety features like collision prevention that would warn or stop automatically. This failure contributed to the crash. I was cited for failing to stop and driving without insurance. I believe the system malfunctioned and that the dealership acted negligently by not inspecting the vehicle promptly. The issue has not been confirmed by the dealer because they refused to inspect it. The vehicle may still be available for inspection, and I plan to have the black box (EDR) data analyzed. Upon further research this appears to be a known issue that has been brought up to Mazda from other owners and it has been dismissed by Mazda as a safety feature designed to prevent theft. Apparently this is by design, the app will not let you unloco the car if the vehicle is running with the keys in it. Personally, the theft of the vehicle is nowhere near as important as the health and safety of my child. We were extremely fortunate that the air conditioning was on as the temperatures in our town are hitting triple digits right now. What if it had not been on? This could have had a much worse outcome. Additionally, how in the world does this deter theft? So what Mazda is basically saying is that somebody would get my phone, access my password/fingerprint protected app, unlock my car from my phone, then get in and drive off. The likely hood of this happening is way lower than a curious toddler or rambunctious dog accidentally getting locked inside the running vehicle. Additionally, if my wife locked her keys in the car, and isn't home to get the spare, or im at work and can't bring her the spare right away, she's [XXX]. While driving on I-17 Southbound near mile marker 311 at approximately 75 mph, I experienced a sudden and dangerous malfunction of the Lane Keep Assist (Auto-Steer) system in a 2021 Subaru Forester. The feature was active at the time and unexpectedly veered the vehicle sharply toward the concrete wall on the left shoulder of the highway. ... of the malfunction, I overcorrected and collided with the wall on the rear driver side. There were no vehicles in the immediate vicinity, or this could have resulted in a multi-vehicle collision at high speed. I hit my head during the impact but did not require emergency medical services at the scene. Road conditions were dry and clear, with no visible debris or extreme weather factors. There were no warning lights or error messages from the vehicle prior to or during the incident. The Auto-Steer system did not disengage on its own before veering off course. This was the first time driving this particular vehicle, which was a company car, and I immediately reported the incident to my employer. The damage was reported and the vehicle is seemingly available for inspection. At the time of this report, the issue has not yet been formally diagnosed by a dealer or service technician. The failure of this driver-assist feature significantly endangered my safety and potentially the safety of others had there been nearby traffic. This has been an ongoing concern, reported from the first service (10k) on my truck, which now has 45k miles. I received the remedy engine at 37k miles under the safety recall. The contact stated that the VIN was previously included in the recall; however, the recall was marked as complete after the vehicle passed the inspection test. The contact stated while driving 20-25 MPH, the failure recurred. The vehicle was towed to the dealer, where the contact was informed that the vehicle could not be repaired under the recall. The vehicle was then towed to a second dealer, Keyes Hyundai of Mission Hills(10240 Sepulveda Blvd, Mission Hills, CA 91345), where it was diagnosed that the power relay assembly (PRA) located underneath the rear seat and the 12-Volt battery had failed and needed to be replaced; however, the part was on backorder. The manufacturer was notified of the failure and advised the contact that a goodwill discount could be provided at the dealer's discretion. A case was filed. The contact stated that after the manufacturer contacted the dealer, the dealer doubled the cost of the repair and provided a goodwill discount. The vehicle was not repaired due to the cost. Emission system control error please see your dealer do not accelerate at high speeds, ACC control, module error, brake system failure, error, lane control, assistance, failure, numerous lights illuminate on dashboard 7 to 10 error codes all at once. Vehicle was taken to Diagnosed and found turbo Charger for engine acceleration was faulty, spark plugs changed, 4 coils and cam shift sensors2 total.

Vehicle is getting errors for Emssions controls, parking brake, and sh-awd system faults all at the same time. Car will still function normally and codes will go off and come back intermittently. Also while reading the codes it will get a U3006-16 code for low voltage on the ecm. Confirmed battery is good under load test and alternator is charging at 14.2 volts.
