Date of Incident: [XXX] US Eastern Time Location: [XXX] Incident Type: Safety defect and vehicle malfunction due to Sudden Unintended Acceleration with multiple safety system failures On the date referenced above, while traveling at approximately 5 mph and preparing to park in a gravel parking lot, I removed my foot from the accelerator pedal to bring the vehicle to a complete stop using the n
... ormal deceleration process (with One-Pedal Mode disabled). Instead of the expected behavior, the vehicle: 1.Suddenly and violently accelerated without input 2.Drove straight into a parked vehicle at high speed, completely displacing it 3.Continued accelerating despite my attempts to control it 4.Required multiple brake applications before finally stopping 5.Traveled approximately 270 degrees across the parking lot while I desperately tried to avoid additional collisions 6.There were ~6 women that had been standing exactly where the incident happened only a few minutes before the accident, had I arrived earlier I could have hurt/killed these pedestrians Critical Safety System Failures During This Incident: •No pre-collision warnings or alerts activated •No collision avoidance sensors engaged •No automatic emergency braking deployed •No warning messages appeared on the vehicle display •No audible warning sounds of any kind •No air bag activation after my car rammed into the parked car This represents a catastrophic failure of multiple redundant safety systems that are fundamental to the Polestar 3's safety design. Since October 2023, my 2019 Toyota Prius has developed severe and unpredictable driving behavior after a regular brake service, which may have triggered or coincided with a software or ECU update. The car, which had been driving smoothly since I bought it in 2021, suddenly began showing false braking, steering interference, and lane instability — as if the car were trying to “correct” proble
... ms that didn’t exist. These interventions often happen on highways or during turns, creating serious safety risks for both driver and passengers. I’ve spent months and thousands of dollars replacing parts, performing alignments, and recalibrating sensors, including at Toyota dealerships. The problem persists because it is software-based, not mechanical. I discovered that in mid–2023, the NHTSA announced upcoming rules requiring Automatic Emergency Braking (AEB) with pedestrian detection by 2029. Around that same time, reports from multiple drivers (Toyota, Honda, Hyundai, Ford, Subaru) began to appear describing the exact same issues. It seems manufacturers began silently updating vehicles to meet anticipated compliance standards. However, this introduced overly sensitive and malfunctioning driver-assist systems. The most alarming part is that the driver cannot fully disable these features — the “off” button only reduces their intervention. This means when the system malfunctions, the driver is forced to operate an unsafe vehicle. As a professional rideshare driver, this problem destroyed my ability to work. Passengers reported me for unsafe driving because of the car’s erratic behavior. I eventually lost my vehicle and income. I believe this is a systemic safety defect linked to premature or unregulated implementation of AEB software. Drivers deserve the right to fully disable malfunctioning systems that endanger lives. This is not just a car issue — it’s a public safety crisis that must be urgently investigated.Read more

While there were no reported incidents, it appears there may be manufacturing defects with this vehicle. The rear seat safety belts are reportedly quite difficult to fasten, as the receptacle is positioned deeply within the seats, sometimes making it impossible to secure the belt. This then triggers an alarm, which can be quite bothersome for the driver. Furthermore, there seems to be an issue wit
... h the fuel gauge, as it may display an inaccurate reading. For instance, upon filling the tank, the meter might show 470 ml, but while still at the gas station, it could indicate 330 ml. Additionally, the reading can fluctuate after a trip. Finally, the air conditioning vent on the driver's side does not seem to be functioning correctly. In one instance, despite a minor road hazard that was not immediately noticed, the left rear tire was completely damaged. Despite multiple attempts to address these issues at the showroom and contacting Subaru headquarters, the problems persist. Note the purchase of vehicle forced upon me ,in addition Hallmark Subaru refuse to address the issues with vehicle. I have reported the matter to AL state Attorney general. Initial Oil Change Visit: Brought leased vehicle under full warranty to the dealership for its first oil change. No issues were mentioned at this time. First Return (After 5 Minutes Driving): After leaving the dealership, warning lights and unusual sounds began within 5 minutes of driving. Returned immediately. The service team claimed the issue was due to the 'oil gun not being recalibrated' and
... that they had topped the oil off using a jug. This was later revealed to be a lie. Second Return (After Another 5 Minutes Driving): The same warning lights and noises returned after driving only 5 minutes. Returned again to the dealership. This time, they admitted that no oil had ever been put into the vehicle after the oil was drained. The service manager stated that the engine was 'cooked' and would need to be replaced. Admission by Service Manager: The service manager personally drove me to the rental car location. During this time, he admitted that the technician had simply forgotten to refill the oil after draining it, stating they had 'just gotten busy.' He had no explanation for why the dealership had lied during the first return visit. Safety Concerns: This negligence could have had catastrophic consequences. If the engine had blown up while driving, I could have crashed, potentially injuring or killing myself, my pregnant fiancée who was with me, and our unborn child. This was not just an inconvenience but a severe, reckless breach of safety and trust. Dealer’s Resolution: The dealership later acknowledged fault and offered to replace the engine with a new one. However, given the severity of the negligence and the safety risk, we refused and ultimately walked away from the lease when they would not provide an equivalent vehicle under the same terms. They instead paid off the remaining lease balance ($42,286.94). -Sent certifed formal demand letters to dealer and manufacturer -sent follow up emails to both -have not received response to my complaints.Read more

The problem originates from a single faulty passenger seat airbag sensor, but Hyundai requires replacement of the entire seat cushion module, quoted at approximately $3,200 for the part alone and around $4,000 total repair cost. This creates the impression that a single failed sensor affects the entire airbag system. I was not provided with a definitive answer as to whether this fault impacts the
... driver airbag, which leaves a critical safety uncertainty. By designing the sensor as an inseparable, non-serviceable part of the cushion module, Hyundai forces consumers into an unreasonable expense. This situation puts drivers in the position of either paying thousands of dollars or risking driving with potentially compromised airbag safety. I strongly believe this constitutes a serious safety defect and should be subject to recall.Read more

Recent Maintenance Completed: New car battery (3 battery since purchase of car) New spark plugs (Replaced in June 2025) Regular oil changes (every 3000 miles) New engine air filter and cabin air filter (yearly) Additional Symptoms: The rear-view camera frequently times out or fails to display when reversing. The dealership has not been able to confirm the cause but has mentioned a potential error code P30DE00, suggesting alternator replacement. However, a second opinion found no active error codes, leaving the root cause unclear. I'm seeking a definitive diagnosis and a resolution plan, as the issue has become a safety concern.Read more
