The contact owns a 2024 Kia Carnival. The contact stated that while driving at various speeds, an abnormal gas odor would emit through the vehicle's ventilation system. The contact notified a local dealer, and an appointment was made to have the vehicle serviced. The vehicle was inspected, and the contact was informed that there was likely bacteria growing inside the vehicle, which caused the odor
... . The contact disputed this claim and recommended that the air filters be replaced; the contact paid for the repair. Despite the repair, the contact stated that the fuel odor had become a chemical odor, and the vehicle was taken back to the dealer. The dealer confirmed that they had smelled a fuel odor inside the vehicle and informed the contact that she had overfilled her fuel tank with gas. While the contact also disputed this claim, the dealer later concluded that the dipstick inside the oil pan was not properly sealed, and the dealer repaired the defect. Despite the dealer's claims and repairs, the fuel odor had become stronger. The contact stated that the odor had become so severe that she went to the emergency room to be treated for nausea and lightheadedness. The contact was given medication for her symptoms. The manufacturer was notified of the failure, and the contact was given a case number. The vehicle had yet to be repaired. The failure mileage was unknown.Read more

The front windshield spontaneously developed a crack about 5 inches long which later suddenly increased to about 15 inches in length. It is our understanding that this problem has occurred in the windshields of other Subaru models. The internet states that Subaru windshields have had a tendency to crack without impact due to the windshield's angle or thinness to accommodate Eyesight technology.

My vehicle has repeatedly displayed a “low tire pressure” warning despite having been serviced twice for this issue. In addition, there is an open case with Subaru of America regarding ongoing performance concerns – fuel tank?! In addition, I have reported both verbally and in writing to the dealership that my car often loses power when driving uphill and, at times, runs unevenly or hesitant
... ly—similar to an engine stutter or sputter. Most recently, multiple problems occurred simultaneously. The low tire pressure warning remained on, my cell phone was no longer recognized or paired with the vehicle and would not charge, part of the clock display stopped illuminating, and then the vehicle suddenly began to shake. Within seconds, all dashboard warning lights turned on and the engine shut off entirely. This happened so rapidly that I had no time to react. Fortunately, I had pulled over the car; otherwise, this incident could have resulted in a serious crash with me behind the wheel. Although no prior warning lights had appeared, I was later informed that the vehicle had a cracked ignition coil and a dead battery—despite the car having relatively low mileage and there being two recalls related to this issue. The new manager went above and beyond to assist me during this ordeal and I am beyond grateful for everything – Subaru of America is lucky to have him on staff!! However, as of today, I still have not received a clear explanation of what caused this failure or what risks may remain if I continue driving the vehicle. My understanding is that ignition coils should run for 80K - 100K. I am aware that others have experienced similar problems, and I request that this matter be thoroughly addressed before it results in a potentially preventable, yet serious accident.Read more

The airbag system/clockspring assembly in the steering column appears to have failed. The airbag warning light turned on and all steering-wheel controls stopped working at the same time, which indicates that the driver airbag may not function in a crash. The vehicle can be inspected upon request. This is a major safety risk because the primary airbag might not deploy during a collision, putting t
... he driver at high risk of injury. The issue has remained constant since it first appeared. I had the vehicle inspected by a highly reputable independent repair shop, and their assessment was that the clockspring/airbag component has failed. They advised that the necessary replacement part is unavailable to order, leaving the vehicle without a functional airbag system. I have also seen many other owners of similar 2015 Passat models reporting the same failure online, which suggests this may be a widespread defect. The airbag light is the only warning I received. It turned on a few months ago and has stayed on continuously since. UNKNOWN if there are other related electrical faults.Read more

My 2024 Subaru Ascent developed a recurring clunk/thump noise from the steering system during low-speed turning (10–15 mph) and when turning the wheel at a stop. The noise was present daily and worsened over time. I took the vehicle to two Subaru dealerships. Sterling Subaru dismissed the steering noise completely and stated they could not duplicate it. Dulles Subaru also initially dismissed th
... e issue. Only when I returned to pick up the vehicle and insisted on demonstrating the noise did their technician acknowledge it. They compared my vehicle to another Ascent on the lot and confirmed the noise was abnormal. Dulles then replaced the steering gear box assembly (steering rack) under warranty. This confirms there was a mechanical defect in the steering system. Before this repair, I also reported excessive inner-edge wear on both front tires. The technician documented “excessive wear on both front inner tires” and recommended replacing all four tires and performing an alignment. Inner-edge wear on both front tires is not caused by lack of rotation—it is consistent with misalignment or steering geometry deviation, which matches the confirmed steering defect. Subaru America has refused to replace the damaged tires even though the tire manufacturer (Falken) stated in writing that irregular wear caused by a mechanical defect is the vehicle manufacturer’s responsibility. Safety concern: A defective steering rack caused abnormal steering behavior and front-end geometry issues, which led to rapid inner-edge tire wear. If unnoticed, this can result in loss of steering control or a tire failure at highway speeds. Both dealerships initially dismissing the issue is also concerning. I am filing this report so NHTSA can track potential steering-related defects and resulting tire safety issues on the 2024 Subaru Ascent. I can provide service records, photos, and Subaru correspondence upon request.Read more

Dear Toyota Customer Experience Team, I am writing to report a serious and potentially widespread safety issue with my 2018 Toyota Camry XSE. The automatic transmission has failed. The vehicle exhibits slipping, hard or delayed shifting, intermittent loss of drive, and most critically, the car completely shut off while I was driving on the highway, creating an immediate risk of a serious accident
... . The transmission is available for inspection upon request. These failures pose a direct safety hazard. The unexpected shutdown on the highway, combined with gear engagement problems, could easily lead to loss of control, collisions, or injury to myself, passengers, and other road users. The transmission failure has been confirmed by a certified independent transmission specialist, who stated that the transmission requires either a complete rebuild or replacement. The vehicle has been maintained according to Toyota’s recommended schedule. The vehicle has not been inspected by Toyota representatives. It has been inspected by the independent specialist for diagnostic purposes. Hard shifting and gear slipping began approximately 11/08/2025. The transmission warning light hasn't even come on. Sudden hesitation during acceleration or gear changes became increasingly frequent. The most severe symptom occurred when the car completely shut off on the highway which is super dangerous. Based on my observations and the independent specialist’s evaluation, I believe this failure may be related to known issues affecting 2018 Toyota Camry XSE transmissions, potentially involving internal clutches or valve body components. I respectfully request that Toyota consider offering a goodwill repair or replacement of the transmission for my vehicle. Provide guidance to owners of similar vehicles to prevent accidents or injuries.Read more

What component failed: The CVT on our 2015 Nissan Rogue failed while in operation. The vehicle is now inoperable. In 12/2024, at 84,000 miles, we brought the car to a Nissan dealership due to stalling and hesitation. The service record noted: “Felt trans judder during a slow turn,” which matches early CVT failure symptoms outlined in Nissan Technical Service bulletin SB‑10075090‑5448. Desp
... ite this, the issue was not flagged, and a transmission flush was performed as a precaution. Safety risk: The failure occurred while my 17‑year‑old daughter was driving on the expressway. The vehicle suddenly lost power in ACTIVE TRAFFIC and she had to coast to the shoulder. This placed her life, and other drivers around her, at risk of a rear‑end collision at highway speed. Sudden loss of propulsion in high‑speed traffic is a serious safety hazard. Importantly, this CVT issue has been and is the subject of multiple Technical Service Bulletins. Older vehicles like ours are commonly passed down to first-time drivers, especially teenagers. By refusing to address known transmission defects in these vehicles past arbitrary warranty thresholds, Nissan is knowingly putting thousands of young drivers at daily risk. Has the problem been reproduced / confirmed by dealer: The same drivability symptoms were reported to and documented by a Nissan dealership in December 2024 at 84,000 miles. They performed a transmission flush and stated the vehicle was fine. Less than 5,000 miles later, the CVT failed completely. Has the vehicle been inspected by others: The vehicle has been inspected by a Nissan dealership (12/2024). Nissan Consumer Affairs was contacted several times, but no remedy or goodwill inspection has been offered. Were there warning symptoms before failure: Yes. Stalling / hesitation while driving had been occurring the entire time prior to failure. The Nissan dealership documented “trans judder” in December 2024, which matches Nissan’s own TSB language for CVT failure symptoms.Read more

We were stopped on the highway >20min until police arrived to block the lanes so we could safely exit the vehicle and a tow truck could safely retrieve it. The ground strap underneath the vehicle that grounds the main engine harness was broken. As a result the electrical short resulted in burning/melting of sever
... al spots on the harness. There was no warning ahead of time and I’ve learned that this is a known issue. We are very lucky that this did not lead to a crash, injury or death.Read more

Check Engine Light appeared, and car seemed to sputter and had an odd chemical smell. Brought to mechanic who scanned PCM and found 3 misfire codes (P0300, O0301, P0316). They removed 1 spark plug and inspected cylinder and found it to be wet. Block test was performed, and fluid was found to be coolant. Engine head gasket requires replacement as well as repair or replacement of cylinder. Mechanic
... suggested seeking support from dealership to have the issue fixed because this is a known issue. A simple internet search confirmed that this make and model (2018 Ford Escape 4 Cyl Ecoboost) has this issue and it is well known and documented. Ford coolant intrusion is a known problem and design flaw. The service manager at the Flood Ford in Narraganset said that since there are no recalls and the warranty on the car has expired there is nothing that they can do as far as monetary support or compensation. They quoted a $9000 repair price. The manager then suggested speaking to Ford Customer Service directly. I spoke to a service rep followed by a supervisor (Reggie) at Ford Customer Service who both told me that there was nothing they could do because there were no active recalls or active customer service programs. The rep at Ford did tell me that CSP 19B37 was done on the vehicle in 2020, prior to my purchase. I was not told of this upon purchase or made aware in any way of a coolant issue. This issue causes major damage to the vehicle and is a safety risk.Read more
