While driving on the highway, a strong burning smell began inside the vehicle, similar to a brush or forest fire. At first it was unclear whether the smell was coming from outside, but when traffic slowed, smoke was seen coming from the driver-side wheel well and from under the front hood. No warning lights, alerts, temperature warnings, or malfunction messages appeared before or during the smoke.
... The vehicle was moved off the highway to a side road. Upon opening the hood, light but steady smoke continued from the engine area. The coolant reservoir was full and there were no visible leaks. The vehicle continued to smoke consistently for approximately 50 minutes while waiting for a tow truck. After the vehicle was loaded onto the flatbed and transported back toward the highway, an orange glow became visible inside the cabin, indicating an active fire had started inside the interior. The tow truck driver immediately pulled over, used a fire extinguisher, and unloaded the vehicle from the flatbed for safety. Smoke increased significantly from the cabin and wheels. Both white and black smoke were present. Police and fire department personnel responded to the scene and extinguished the fire. The vehicle was a nearly new plug-in hybrid with low mileage, and there were no dashboard warnings or prior symptoms of failure. The cause of the fire is unknown, but it appears related to a malfunction involving the electrical, battery, thermal-management, or propulsion systems. I discovered this issue while inspecting the frunk during routine cleaning and noticed that my four-year-old sat very comfortably inside the frunk listening to music while I cleaned the car. No incident has occurred yet, but the potential for harm is clear and warrants immediate investigation, as it violates basic entrapment-prevention standards observed in rear trunks and other vehicles. This defect compromises occupant safety and should be addressed through a recall or a retrofit that includes an illuminated emergency release. The selling dealer was required to inspect the vehicle before delivery and did not report any transmission or valve-body issues. On June 25, 2025, the vehicle received a multipoint inspection at Auto Angel in Santa Fe. No transmission failure, shifting issues, or valve-body concerns were found.
... The Jeep was not showing symptoms related to Recall U90 at that time. After this, I attempted to have Recall U90 (PCM/TCM reprogramming) performed. Lithia Chrysler in Santa Fe confirmed the recall by email and scheduled an appointment, but after towing the vehicle to them they refused to perform the recall and repeatedly stated “there is no recall,” forcing me to tow the vehicle home and then again two days later to a dealership in Albuquerque. At the Albuquerque dealership, I gave clear instructions not to perform any paid repairs—including a transmission valve-body replacement—until the U90 reflash was completed and confirmed. I never received confirmation. The dealership later claimed they followed protocol but refused to release the WiTECH session report or timestamps showing when the recall work occurred. They told me I could only see documentation after paying $2,600. I contacted FCA/Stellantis. The case manager said her supervisor stated the dealer “followed protocol,” but FCA admitted this was based solely on the dealership’s word. FCA could not confirm whether they reviewed the WiTECH diagnostic session or any recall repair records. Under 49 U.S.C. §30120(i) and 49 CFR Part 576, manufacturers and dealers must retain detailed recall records, including diagnostic data and timestamps, to verify proper recall completion. FCA’s inability or refusal to verify the recall using these required records raises concerns that Recall U90 may not have been performed correctly or in the proper sequence. The next day they called and informed me I had an internal coolant leak that had seized my spark plugs and destroyed my engine. This is a known issue, extremely common, and yet they refuse to do a recall on it and could provide me no help. I was quoted $16,000 for the repair (far more than the car is worth) and essentially just ended up having to buy a new car. The Ford will have to be scrapped for a few hundred dollars despite only being 8 years old with only 98,000 miles on it. I have records of my ownership and a written diagnostic report from Ford. From Ford: "Observed coolant intrusion into cylinder #2. Recommend replacement of the long block at this time. Coolant intrusion into long block at this time. Found the engine assembly has an internal coolant leak." Additionally, Ford has posted a service bulletin with recommended repair instructions for this known issue. The bulletin number is 9-2139 from 2019. There is also an active class action lawsuit against Ford being run by Kershaw Talley Barlow for this issue.Read more

Electrical and digital system malfunctions, including warning lights, dashboard errors, and intermittent failures of the infotainment system. Other mechanical concerns include inconsistent braking responses and occasional unusual vibrations from the suspension, as well as numerous other issues. The stop-sale record indicates a potentially serious defect that could put the driver or others at risk. I have not been able to confirm the exact nature of the defect or whether it has been repaired. However, there are still two campaigns/recalls associated with my VIN. I am submitting this report to NHTSA to request an investigation and ensure that the vehicle is safe for operation. My 2019 Toyota RAV4 Hybrid (VIN: [XXX] ) suffered sudden engine failure due to coolant intrusion. ... commending a replacement. This vehicle is part of the early 2019 Japan-built batch widely known for premature electric water pump and coolant-flow system failures. Internal impeller swelling, seal leakage, and bearing seizure can cause coolant pressure spikes and ultimately head-gasket failure with coolant entering the cylinders. This defect has been reported by thousands of RAV4 Hybrid owners (2019–2021). Toyota has addressed it quietly through internal TSBs such as XXX and XXX but no public recall has been issued. The safety risk is severe: the engine began emitting thick white smoke and ran roughly, risking sudden power loss, overheating, or engine seizure while driving. A stall at highway speed could easily cause a collision. Toyota USA opened Case #XXX but declined assistance despite the defect being well documented. This failure was sudden, with no warning lights prior to the incident. Given the widespread nature of this defect and its clear safety implications, I request that NHTSA open a formal investigation into the electric water pump/coolant system failures in 2019–2021 Toyota RAV4 Hybrids and consider requiring a safety recall. I was entering an intersection to make a left-hand turn. I was driving at a low speed. My foot was pressing the accelerator and the car entered the intersection. In the middle of the intersection, the car wheels suddenly stopped turning. There was nothing in front of the car. I was able to drive out of the intersection to a safe location and turned off the car engine. I restarted the car and drove immediately to the dealer where I purchased the car from for them to evaluate it. It is currently being submitted for diagnostics at the car dealer. The contact owns a 2021 BMW 330I. The contact stated while driving 50 MPH and coming to a stop at a red traffic light, the contact started smelling smoke. There was no warning light illuminated. The contact turned off the vehicle, but the vehicle failed to restart with a message that the vehicle was unable to be restarted displayed. ... in the middle of the road. The contact then noticed that there was smoke coming from the front end of the vehicle and immediately called 911. The contact stated that while a Metro service employee was attempting to assist the contact in towing the vehicle to the side of the road, the worker noticed that fire had started underneath the vehicle and called the Fire Department. The contact exited the vehicle and noticed that there was fire underneath the front end. The contact stated that the front end of the vehicle then burst into flames. The fire was extinguished by the Fire Department. A Fire Department report was filed. The front end of the vehicle was significantly burned. A Police Officer arrived at the scene but had not filed a report because there was no injury sustained. The Police Officer provided his badge number to the contact. The vehicle was towed to a tow lot, where it was being evaluated by the Insurance Company. The contact stated that the vehicle was a total loss; however, the Insurance Company had not yet confirmed that the vehicle was a total loss. The dealer and the manufacturer were not notified of the failure. The vehicle was not yet diagnosed or repaired. The failure mileage was approximately 30,000.Read more

It is not possible to comply with the requirement to park the vehicle away from structures because the residential community where the vehicle is located prohibits street parking and does not allow parking off the driveway. As a result, there is no safe or legal way to store the vehicle that satisfies the recall instructions. This creates an ongoing fire risk to nearby structures and poses a significant safety concern for the owner, neighboring homes, and the community. The recall has effectively made the vehicle unusable, unchargeable, and unsafe to store, with no remedy currently available. ... ce, the same or a very similar defect has also occurred in my 5.3L engine, which is currently not included in that recall. 2.Valve Lifter Failure: In addition, my vehicle has suffered from valve lifter collapse—a well-documented problem in 2021–2022 GM trucks and SUVs with both the 5.3L and 6.2L engines. This caused severe misfiring, drivability issues, and expensive repair needs. Why this is a concern: •Both failures occurred well before what would be expected of a modern engine. •The crankshaft failure raises a safety risk: sudden loss of propulsion while driving could lead to accidents. •The lifter issue is widely reported by other owners and has been the subject of GM service bulletins. •Taken together, these problems suggest systemic defects in the 2021 Tahoe’s 5.3L engine that have not been adequately addressed by GM. Requested Action: •I ask GM to extend recall and warranty coverage to include the 5.3L L84 engine in addition to the 6.2L L87. •I ask NHTSA to consider investigating whether the scope of the current 6.2L recall should be broadened to include 5.3L engines, given failures like mine. •At minimum, I request assistance in covering repair/replacement costs for both the crankshaft and lifter defects, which are clearly not due to normal wear and tear.
