Now 2026 recall shows with DMV and Honda is failing at providing a proof of correction after Inspecting the car and approving all the changes made to it. I am unable to get my car tags due to Honda failing to comply with what DMV is needing to complete this request for my tags.

I had to drive my husband and triplet grandchildren in the direction of the car, visible by the black smoke plumes, while not knowing if my daughter and her husband were alive. The fire department, a state trooper, and local police were on the scene and I am thankful everyone was safe. Needless to say, this situation was very traumatic for my family. My daughter reported the oil light and the engine coming on, as well as a dash symbol she didn't recognize, right before the car caught on fire. This created an immediate hazard as I was unable to keep up with the flow of traffic, forcing me to make an emergency exit across multiple lanes while moving significantly slower than other vehicles. The loss of motive power increased the risk of a high-speed rear-end collision. 3. Has the problem been reproduced or confirmed by a dealer? Yes. The problem was confirmed by Heritage Toyota Service. The technicians confirmed an internal transmission failure. A secondary consultation with a transmission specialist (AAMCO) also identified the failure as a known mechanical defect common to this transmission model. 4. Has the vehicle been inspected by the manufacturer... or others? Yes, it has been inspected by the authorized Toyota Dealership listed above. I have also reported the issue to Toyota Brand Engagement (National), but a corporate field inspection (DSPM) has not yet been performed. 5. Were there any warning lamps... or other symptoms? On 12/20/25, the "lane assist not available" and the "EPC" warning indicators lit up. I was in the car with my 11 year old, we pulled over and looked in the manual to understand EPC, and the manual said it "is possible to continue driving," so we continued. We got off at the next exit, drove to a parking lot and let the car rest for 10-15 minutes. When we turned it back on, the "Exhaust System Malfunction" light and "Central Warning Light" indicators were on. The lane assist not available indicator had lit up a day prior, as well. My safety and the safety of my child were at risk on the freeway. I called the dealership from which I had purchased the car 2 years earlier and which has ~11,280 miles. I was told by their service department to drive the car home, but not to drive it after that. However, they also told me to drive the car from my home to their shop (10 miles), but to avoid the freeway, in order to have it serviced. This suggestion also put me at risk, as it would have been unsafe to drive the car between my home and their shop. I did call back to say that I didn't feel safe driving it, at which point they looked up my car and saw that it is under warranty, which includes roadside assistance coverage. ... to driver visibility and road safety. The “Theater Screen” is a 31-inch ultrawide display that folds down from the roof for rear passengers. Upon activating this feature, the screen completely blocks all rear visibility through the rearview mirror. When I first engaged the system, I was immediately alarmed to find the rear view entirely obstructed, forcing me to turn it off and fold it back up to avoid driving blindly. In addition, the rear window roller shade, another optional feature I purchased, obstructs approximately 80% of the rear visibility when fully deployed. Both of these factory-installed and BMW-approved options create an extreme safety hazard, effectively disabling the driver’s ability to monitor traffic behind the vehicle — a fundamental requirement for safe operation on public roads. When I brought these concerns to BMW’s Customer Relations department, they dismissed the issue, stating that both features were “inspected and approved by the National Highway Traffic Safety Administration.” Frankly, it is difficult to believe that NHTSA would approve such features that blatantly compromise driver visibility and violate basic safety standards. I am therefore requesting that NHTSA urgently investigate this matter to determine how such equipment passed federal safety standards, and to ensure appropriate corrective actions are taken. Nature of the Defect: The exterior clear coat and base coat are separating from the underlying primer, resulting in large, unsightly patches of paint peeling and flaking off the vehicle's body panels. Manufacturer Acknowledgment: American Honda Motor Co., Inc. previously acknowledged this defect by issuing a Warranty Extension (Technical Service Bulletin 19-064, issued August 29, 2019) for specific models, including the 2016-2018 HR-V, extending the warranty coverage for this specific paint problem to 7 years from the original date of purchase with no mileage limit. The Current Issue: My vehicle has recently begun exhibiting this peeling condition, which is consistent with the defect covered under TSB 19-064. However, I have been informed by the dealer/Honda Customer Service that the 7-year warranty extension period has expired as of 2024, and they are refusing to cover the cost of the necessary repainting, which can cost thousands of dollars. Request for Action: The underlying problem is a manufacturing defect that exists for the life of the vehicle and is only now manifesting due to premature failure. Limiting the remedy to a seven-year period is inadequate for a defect that is inherent to the production process and directly affects the vehicle's structural integrity over time (potential for rust exposure). The recall repair 95 B is attributed to be causing Red Flag Dashboard Warnings to "service hybrid system". Obviously crazy things can happen when a car's computer shuts down while driving -- no one knows if my next Digital Crash could shut down my engine when I am in a multi-car passing situation on a 2 lane highway at 70 MPH and yet that scenario is happening to others. I have seen 3 jeep dealers and have documentation on those digital scans for my vehicle-- Jeep is aware of the problem but they have no-fix. There are over 500 complaints on the public jeep 4xe website across multiple conversation threads of people having this exact problem. You can see from the attachment that the dealers are telling owners that the cars are 'unsafe to drive' because they do recognize that the next digital crash of the computer will be a fatal accident. None of these problems existed before that recall was available in January. Something in that NHTSA 95 B recall for the Grand Cherokee PHEV was defective and is causing these digital crashes -- jeep does not dispute that, at least from what I have been told. Jeep has to be required to do another recall to fix the earlier 95 B recall. It has been close to 6 months now without a fix and nothing is happening. It is probably a software fix, but it needs to be done and at this point it appears that they must be forced to do so by NHTSA. ... ing engine shut downs on the highway -- see the attachments for 4 such cases downloaded from the Jeep website. Obviously crazy things can happen when a car's computer shuts down while driving -- no one knows if my next Digital Crash could shut down my engine when I am in a multi-car passing situation on a 2 lane highway at 70 MPH and yet that scenario is happening to others. I have seen 3 jeep dealers and have documentation on those digital scans for my vehicle-- Jeep is aware of the problem but they have no-fix. There are over 500 complaints on the public jeep 4xe website across multiple conversation threads of people having this exact problem. You can see from the attachment that the dealers are telling owners that the cars are 'unsafe to drive' because they do recognize that the next digital crash of the computer will be a fatal accident. None of these problems existed before that recall was available in January. Something in that NHTSA 95 B recall for the Grand Cherokee PHEV was defective and is causing these digital crashes -- jeep does not dispute that, at least from what I have been told. Jeep has to be required to do another recall to fix the earlier 95 B recall. It has been close to 6 months now without a fix and nothing is happening. It is probably a software fix, but it needs to be done and at this point it appears that they must be forced to do so by NHTSA. In December 2024, the engine lost power while driving. We were able to pull over on a busy road, where we were blocking traffic on a curve. We had it towed to the dealership. ... , 4 months later, the check engine light came on. I took it to the dealership, and another fuel injector needed to be replaced. The service manager stated that he requested that all of the fuel injectors be replaced because this is a known issue, but Hyundai denied the request, so they only replaced one fuel injector. I am expecting this to happen again, and I am especially worried that the vehicle could lose power while driving on the freeway. When I restarted the car after 5 minutes, the erratic RPM resumed, and then the engine stopped running, and my display screen showed the following message: “Engine Stopped, Steering Power Low”. The check engine, check steering, and check battery lights were also illuminated on the dashboard. The vehicle would not restart after several tries and ultimately ended up having the vehicle towed 30 minutes. The vehicle was taken to an independent mechanic where it was diagnosed with turbo failure. The vehicle was repaired; however, the failure persisted. On another occasion, the failure recurred, and the failure became more frequent.
