Safety defect involving exterior rearview mirrors on Rivian R1S I am reporting a recurring and worsening safety issue involving the exterior rearview mirrors on my Rivian R1S Gen 2. Both side mirrors repeatedly lose their saved positions without driver input. More concerningly, on multiple occasions the mirrors have changed position while the vehicle was being driven, including at highway speeds.
... When this occurs, my rearward and side visibility is unexpectedly altered while driving, requiring me to readjust the mirrors and creating a significant safety concern. This problem has persisted across multiple Rivian software versions and has become progressively worse. I have already reported the issue to Rivian Service more than once. I have attempted to re-save the mirror positions, disable and re-enable Auto Tilt in Reverse, perform software resets, and recalibrate the system, without resolving the problem. The issue is not caused by Automatic Ride Height Adjustment, as it has occurred while that feature was disabled. I have requested that Rivian escalate the issue to its Engineering team and review vehicle logs, which should document the repeated changes in mirror position. Because the exterior mirrors can change position without driver input while the vehicle is moving, I believe this may constitute a safety-related defect affecting driver visibility. I am requesting that NHTSA review this issue and determine whether similar complaints exist involving Rivian R1S vehicles.Read more

I am reporting repeated suspension and structural failures involving my 2022 Rivian R1T. In April 2026, at approximately 90,537 miles, Rivian performed a comprehensive inspection and found both front lower control arm bushings torn, the right rear damper leaking, and the rear subframe cracked. Rivian’s own service documentation specifically stated that the vehicle was “unsafe to operate due t
... o damage to the rear subframe” and recommended that the vehicle be towed. Rivian subsequently replaced the rear subframe, both front lower control arms, the right rear damper, and serviced the hydraulic damper system. The April repair invoice totaled approximately $9,953. Approximately five months later, at approximately 96,956 miles, the vehicle developed another significant suspension problem. The front passenger side began sitting noticeably lower. The ride became extremely rough over bumps, I could feel significant vibration through the vehicle, and the vehicle repeatedly attempted to adjust its ride height. I also received warnings telling me not to move the vehicle while the air suspension was adjusting, and I was unable to properly adjust the ride height. I brought the vehicle back to Rivian in September 2026. The current Rivian service estimate calls for replacement of both front dampers, the air-suspension compressor/ASU, and the left and right front hydraulic suspension jounce lines, along with suspension diagnostics. The suspension repair estimate is approximately $7,121. I was informed that the ASU/compressor was tried as part of diagnosing the problem but did not resolve the condition, and the front dampers are now suspected. I was also informed that because the front suspension functions as a connected system and the replacement components are an updated design, both sides/components may need to be replaced. I am concerned about the repeated failure of safety-critical suspension and structural components on the same vehicle. In approximately five months .Read more

Rivian later stated it could not reproduce the concern. In March 2026, Rivian documented Limited Performance while driving, the vehicle coming to a stop, and multiple restarts before it would move. On September 4, 2026, the vehicle again entered Limited Performance and would not exceed approximately 20 mph. After a restart, it operated normally. Rivian received the vehicle for service on September 7. On September 9, Rivian stated that initial testing did not duplicate the Limited Performance telltale, that it had no confirmed repair, and that it proposed an extended approximately 100-mile test drive to try to reproduce the issue. I retained photos and videos of additional events involving reduced speed, warnings, restarts, and inability to move. The intermittent loss or reduction of propulsion while driving can affect my ability to maintain safe speed and creates a potential crash risk. No injuries occurred. I am reporting this because Rivian has previously disclosed to NHTSA (Recall 25V-585, XXX) a related defect in which its ADAS software misclassified a lead vehicle at low speed. My incident involved the same general failure pattern - the system not correctly responding to a slow-moving vehicle directly ahead at low speed. Additionally, the vehicle's onboard camera/recording system (Gear Guard) had been disabled by the manufacturer service center during a repair visit completed earlier that same day, and was not re-enabled before the vehicle was returned to me. As a result, no independent video record of the incident exists. At 56,083 miles, my 2024 Rivian R1S experienced a suspension warning while driving on the US-101 Freeway and entered reduced-performance/turtle mode, limiting the vehicle to approximately 50 MPH while surrounding traffic was traveling substantially faster. The sudden speed differential created a serious highway collision risk. Rivian inspected the vehicle and diagnosed fault P12DF7A, “Roll Cont
... rol Cross Circuit Static Leakage Check Failure.” The technician repeated the hydraulic leakage test to rule out a false positive. After draining the hydraulic fluid, Rivian found metal debris throughout the system and documented that it was causing system contamination. Rivian separately confirmed in writing that the source was an “internal mechanical failure in the system.” Rivian Repair Invoice INV_20260811-204135.pdf Rivian determined that the contaminated system required replacement of the hydraulic suspension manifold, hydraulic pump and all four dampers. Its repair procedure requires all four dampers to be replaced when metal debris contaminates the system. Rivian - Metal Debris Email.pdf Critically, Rivian advised me that even if turtle mode did not immediately recur, recurrence was “inevitable in the future” because of the metal debris and stated: “For your safety, we strongly advise against driving the vehicle in this condition prior to completing the full repair.” Rivian - Metal Debris Email.pdf I previously reported excessive suspension clicking to Rivian at approximately 44,000 miles while under warranty. I am aware of at least two potentially similar R1 failures, including another involving reported metal fragments and turtle mode. I request NHTSA investigate whether an internal mechanical failure can introduce metal debris into the interconnected R1 hydraulic suspension system, contaminate multiple components and cause reduced-performance mode at highway speed, and whether similar R1S/R1T incidents indicate a broader safety defect.Read more

We replaced the battery, and a few days later the car would randomly turn on and I could not use an electrical key to open the car door. Only way to open it is with the physical key. The battery had to be unplugged in order to stop the car for randomly turning on. All electrical systems would turn on in the car, including the alarm and radio, even if the radio had been off when the car was last turned off. The car has been inspected by Honda mechanics, who told me the electrical component is dying, but they don't know why, the car isn't that old, and it has very few miles on it.Read more

Over the past year (2025) we have noticed that our F150 King Ranch occasionally was having a rough or harsh shift between the 7-8 gear. We took the truck back to the dealer, Mullnax Ford of Central Florida, 1551 E Semoran Blvd, Apopka, FL 32703. We left the vehicle with the service advisor, Sam. Later that day the service manager Sam said the technician “test drove the truck and said needs a new transmission at a cost of almost $10,000.” Based on research I conducted I determined that there was an existing TSB 25-2475 recently issued October 25, 2025 regarding this harsh/delayed shifting issue on the 10R80 transmission and applicable to our Truck. We have taking the truck to 2 Ford dealers asking them to perform the diagnostic process outlined in TSB 25-2475 The harsh shifting is not only annoying, the sudden sharp shift especially at lower speed presents a possible significant safety risk. The harsh shift many times causes the vehicle to lurch unexpectedly. The truck is only 5 years old and has less than 100k miles We sent certified letters to the CEO of Ford and to the Ford Customer Relationship Center, with no response. This is a wide spread problem and is well known to Ford.Read more

To Whom It May Concern, I am writing to formally dispute a repair charge and report unacceptable service at Courtesy Kia (Mesa, AZ) regarding my vehicle. Timeline of Events: June 12, 2026: I brought my vehicle to Courtesy Kia for a mandatory software recall. Prior to this appointment, my fuel gauge worked perfectly. Immediately upon picking up the vehicle, the fuel gauge ceased to function.
... After calling customer service, I brought the vehicle back for inspection. The dealership kept my car for three days. Despite the issue being caused during their previous service, I was denied a loaner vehicle, forcing to come to take the car and use it during the weekend or I can wait until Monday 29 until the service manager back but I asked them I can’t use the car with this error as it’s not safe for me and my kids and that should be fixed by them since that’s their mistake and unfortunately I’ve had paid out-of-pocket for my family's transportation. June 26, 2026 (Diagnostic): The service department initially claimed the issue was a broken fuel pump and quoted me $1,210. I informed them this was impossible, as the fuel pump was completely replaced just 3 months ago. I requested to speak to the service manager, but was told he was unavailable until Monday. And the gentleman Michael who was taking care of my car said let me speak with the technician and back to me again with different quote The diagnosis was suddenly changed to a "fuel sending unit and gasket," with a new quote of $546?? But if I want I can wait until Monday to check with service manager. June 29, 2026: After receiving no follow-up call, I contacted the dealership myself. I spoke with Service Manager Shane Hackelton, who refused to take responsibility for the damage caused during the recall service and insisting that not related to recall software and should be fixed by myself. I look forward to your prompt response and a resolution to this matter.Read more

The rearview camera started acting funny around March 2026 and completely stopped working in April 2026. When I put the car in reverse it says “service vision system” I have been frequently checking the recall website and it says they are aware of the recall but do not have a fix. I have even called my local Chevy dealer and they had the same info. Not having a rearview camera is a big safety

Randomly but frequently, my car would turn off when coming to a stop and with the auto shut off - but the full car would turn off and to move, I would need to press the ignition to turn back on and drive. This even occurred while accelerating onto a highway. I took this to the shop and after 6 months of back and forth and paying for fuel injectors they 'repaired' it. Now, my car felt heavy with t

This is an EV . We use the regenerative braking to slow down. This car has both a front and rear dash cam. The car is about two and a half years old and has about 20,800 miles. We were approaching a red traffic light with several vehicles stopped in front of us. My partner was driving and had slowed to 8 mph, with her foot off the accelerator, advancing slowly towards the car in front of us when

Ten minutes later she was parking at our house, going 4 mph and it did it again, coming within four feet of the building. Neither time was she touching the accelerator, but the car accelerated like the accelerator was pressed to the floor. This is very dangerous.Read more

On 06/18/2026, the vehicle displayed multiple safety warnings, including ABS malfunction, dynamic stability control/traction control malfunction, Smart Brake Support and Forward Obstruction Warning disabled, possible forward sensing camera malfunction, and airbag/seat belt pre-tensioner malfunction. On 04/24/2025, the vehicle displayed similar warnings, including critical brake system/Electronic Brake Force Distribution malfunction, ABS malfunction, Smart Brake Support and Forward Obstruction Warning disabled, possible forward sensing camera malfunction, and keyless entry malfunction. The vehicle was towed at that time. The vehicle was inspected by an authorized Mazda dealer in June 2026. The dealer replaced the battery under warranty, but I was not provided diagnostic trouble codes, a root-cause explanation, or written confirmation that the braking, ABS, airbag/seat belt, stability control, Smart Brake Support, Forward Obstruction Warning, and forward sensing/camera systems were fully diagnosed and verified safe. The dealer inspection report also listed the mileage as “No Mileage.” The vehicle is available for inspection upon request. I do not feel safe driving it because the repeated warnings involve braking, collision avoidance, stability control, airbags, and seat belt systems. I am concerned that an electrical, braking, or safety-control defect may disable safety systems while driving.Read more

All ADAS functions stopped working on my 2019 Odyssey during a 6 hour drive to the beach with my family of four. It appears that the multipurpose camera has failed.
