What component or system failed or malfunctioned, and is it available for inspection upon request? -The issue involved the truck’s parking and collision avoidance systems, which suddenly shut off. A warning message appeared on the dashboard stating "Service Shifter." This issue may be related to the electronic shifter module or associated control systems. The vehicle is currently available for i
... nspection upon request. How was your safety or the safety of others put at risk? -The sudden deactivation of both the parking and collision systems while the vehicle was in motion created a serious safety hazard. Without collision avoidance functionality and the ability to securely park the vehicle, there was increased risk of unintentional vehicle movement or inability to avoid obstacles, which could have led to a crash or injury. Has the problem been reproduced or confirmed by a dealer or independent service center? -Yes, the truck was taken to an authorized dealer for assessment, and the issue was confirmed. Diagnostic tests found trouble codes starting with "U", which typically indicate a communication or control module fault within the vehicle’s network. The codes provided from the dealer are U0159, U0140, U0100, U0137, U0102, U10121, and U0101. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? -The issue has been inspected and documented by the dealership. There is no indication that police or insurance representatives have inspected it at this time, but the incident and diagnostics are recorded in the dealer’s service records. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? -Yes, a warning message "Service Shifter" appeared on the dashboard. This message coincided with the shutdown of the parking and collision systems. These symptoms appeared suddenly and without prior warning during regular vehicle operation.Read more

I have a 20202 Rav4 Hybrid and the engine short block has malfunctioned due to no error on my part, in my research there was a recall for this EXACTLY this issues in other cars of this year/make/model - although my vehicle initially did not qualify for this I believe that this in fact related and that Toyota should be responsible for the repair or replacement of the engine. The car is currently
... with a Toyota Dealership Service Center, Lake City Toyota in Seattle, WA but they have not been helpful. They have given me vague answers and have sent the engine offsite to a machine shop and keep requesting that I pour additional money into fixing an engine that clearly had a manufacturer defect. My car has always been serviced at Toyota dealerships, as suggested by Toyota. My car has never had previous issues until the engine light came on one day, i checked the coolant and it was empty - first replaced the fluid, let it sit overnight, then drove it around the block to circulate the coolant - when the reservoir emptied again I immediately made an appointment to take it in. The Dealership has not given me any paper invoices or written confirmation/notice of what is happening aside form continuing to ask me to pay more money to fix an engine that is clearly related to the national recall issued.Read more

Accident Description: 2025 Kenworth T380 Model: 2025 Kenworth T380 VIN: [XXX] Date of Incident: [XXX] Location: Ontario, Canada On [XXX], while operating a 2025 Kenworth T380 on a straight section of highway in Ontario, Canada, the steering system suddenly became unresponsive while traveling at approximately 64 mph. I believe this failure is directly related to the USK tie rod ends, which are a
... ssociated with two known open recalls concerning USK ball studs. At the time of the incident, no early warning indicators or mechanical symptoms were present to suggest imminent failure. Without warning, the vehicle veered sharply to the left, causing a complete loss of control. The truck subsequently rolled over multiple times, resulting in a serious multi-unit accident that blocked the highway. My drive truck, along with two additional trucks I was transporting, were all involved in the crash. I was entrapped inside the tractor cab and had to be extricated by emergency responders. I sustained significant injuries, including nerve damage and multiple cervical spine injuries, due to the violent nature of the rollover. After the incident, I observed indications that the vehicle may have been tampered with. I am uncertain about the current location of the vehicle or the whereabouts of critical front-end components. However, photographic evidence shows that the ball studs were removed post-accident. It is my belief that Paccar is not fully disclosing accurate information related to this incident and the condition of the steering components. This incident raises serious concerns regarding the vehicle’s safety and the handling of related recall information. Cam Phasers were recalled a couple years ago. I had the recall done and they were replaced. They are now worse than ever. I brought my vehicle from a used certified dealer at the time of test drive I asked why such a rough transition when going from gear to gear and I was told it was the ECOBoost engine packed with power. Fast forward after multiple repairs and recalls on this vehicle I’ve made it to my breaking point when my SUV shut down as I was turning onto a busy street coming from my son’s school. The engin
... e literally cut off almost causing a head on collision. This idle function is extremely dangerous. I had multiple belts repaired and many things checked after my car overheated and couldn’t find the culprit. One mechanic explained my fan wasn’t working to assist in cooling off the engine and the vents/grill in front of car was shut causing the engine to over heat. After opening the vents and connecting the car to diagnostic machine there were no codes to fix. Over the next month my suv would become significantly worse rattling when I stop at a red light almost drifting into a car in front of me. No longer overheating but now feeling the car physically shifting gears as if I drive a stick shift and I have an automatic transmission. After doing careful research I learned was some recalls on the fan and eco boost engine explaining the same issues that I’am currently having. Don’t mention fixing the recalls because all the certified dealers are 2-3 weeks behind on fixing and diagnosing these same vehicles. They won’t loan you a car while being repaired which is another cost associated with this vehicle. Please look into this ecoboost engine and overheating issues. Also I was told my engine would go soon do the oxygen intake not exchanging enough oxygen causing oil to push into spark plugs and other parts of the engine it doesn’t belong. This SUV is a complete lemon and should be recalled right away before someone dies from this awful feature. Just last week I saw someone on the interstate with my same vehicle stopping mid traffic. This is dangerous!!!Read more

Driving 60mph in 8th gear while raining, 40 deg F, in Minnesota. The truck shut off (I did not press the Start/Stop button) and dash said "Vehicle Not in Park." Went to pull over, put on turn signals, but they didn't work (it acted like the vehicle was completely shut off but was still rolling at 50+ mph in 8th gear - there was no indication on the dash that showed green light indicating turn sign

I pulled over to shoulder, slowed to a stop, and put vehicle into Park manually. Pressed Stop/Start button and vehicle started. No codes were thrown. Drove another 2-3 miles and similar issue happened again. Display flashed on and off (then completely turned off) when coming to a stop this time. Vehicle stayed running but again said "Vehicle Not in Park" on dash. No other indication on dash (no warning lights). Drove vehicle back to dealership to get inspected. Similar issue occurred 4 more times on the 12 mile drive to dealership. When I got the vehicle into the service shop, I put it into Park, then pressed the Start/Stop button, but the truck would not shut off. I left the vehicle with the dealer and they are currently diagnosing the issue. 105,000 miles on vehicle. First time this issue has ever occurred on the Ram since I bought it used with 67,000 miles on it. Scared the heck out of me - I had cars swerve around me as I pulled off to the shoulder without any indication - no brake lights (I assume), no turn signals. Very scary. I am worried about driving my family (wife and 2 young children) in this vehicle. What has scared me even more is I looked up this issue online and I have found 100s of examples in the forums of similar (or exactly the same) issue happening to them! I have had all recalls completed on my truck and keep meticulous care of it. I am an engineer and understand technical details well - this electrical issue scares me. Not safe.Read more

***THIS IS A RESUBMIT WITH CORRECTED INFORMATION*** I purchased a used 2016 Acura MDX from a family member, and shortly afterward began experiencing a rhythmic metallic knocking noise under load, particularly at low RPMs (e.g., accelerating in low gears or when the engine is bogged). The noise is not present at idle or during coasting, and it closely matches the symptoms described in recall 23V-7
... 51, which addresses premature connecting rod bearing wear that may lead to engine failure in certain Honda and Acura vehicles. However, my vehicle’s VIN is not included in the existing recall. The knocking has persisted across several thousand miles and multiple oil changes. I recently inspected the drained oil and found metallic sparkles/particles, which may indicate active bearing wear. There has been no Check Engine Light, and the vehicle has received on-time oil changes using high-quality synthetic oil, with immediate mechanical inspection when the noise first appeared. I know for certain the previous owner maintained the vehicle properly, as he is a family member and I occasionally helped with maintenance myself. Based on the symptom pattern, operating conditions, and consistency with the failure mode described in 23V-751, I believe this vehicle may be affected by the same underlying defect, but is excluded due to an overly narrow recall scope. I am submitting this complaint in support of Recall Query RQ24-013, which is currently under review to determine whether Honda failed to include all affected vehicles. I am concerned this poses a long-term safety and reliability risk, and that vehicles like mine are being improperly excluded from coverage. It is available for inspection. I am deeply concerned for my significant other to drive my vehicle in any capacity, and myself as well. I have videos as well. Please reach out if you need them because the bearing has not spun out yet.Read more
