Recurring camera and Autopilot defect on a 2023 Tesla Model Y (HW3 with in-cabin camera, in scope for NHTSA Recall 23V838). 7 service visits in 13.5 months and ~14,000 miles of my ownership. But Tesla's own VIN-linked telemetry, which Tesla provided me via Privacy Portal data request, contains: 28 Autopilot FAULT events on 14 of 27 retained days; 1 ABORTING event matching my report to the minute; and 17 highway-speed (>=50 mph) phantom-brake-class hard decelerations <= -0.20 g. The worst event: 4/28/2026 at 2:12 PM EDT, less than 90 minutes after Tesla returned the vehicle. Autopilot was in ACTIVE_NOMINAL state with cruise engaged at 63.6 mph. System produced an unprompted -0.252 g (-2.47 m/s^2) deceleration. Driver was not braking; system was driving. Additional phantom brakes: -0.297 g at 62.9 mph on 5/2/2026; -0.294 g at 70.0 mph on 5/3/2026. Multiple Take Over Immediately alerts without explanation. Camera Blocked alerts in clear conditions. Driver-monitoring system flagged 'hands NOT detected' on 339 of 684 monitoring detection events during the 27-day window despite hands being on the wheel — the recall remedy mechanism is failing on this vehicle. This vehicle is in scope for NHTSA Recall Query RQ24009 (open), investigating whether the 23V838 OTA remedy actually fixed the underlying Autosteer defect. I paid them and after a few weeks they claimed it was the Transmission and wanted more money which they were paid. Weeks went by and no updates from them and they weren’t responding to my messages. When they finally responded they started saying the starter was bad, the differential, the fly wheel, etc, Again, we paid them more money, and again weeks went by without any updates. We paid a total of $9000.00 Their last attempt to fix the vehicle also failed and I’m left with a vehicle with no transmission, the entire front end taken apart and scattered all over my property and this was their last response. “Oh lord. So the original place we got the transmission from denied the warranty, but at the time it wasnt a problem we have insurance. So we summited a claim and thats why it was going to be shipped from another place. Well they kept telling me that it was coming this day then that day. I got a letter denying the claim Instead. I have tried to fight it, but they ended up dropping us after that. We are willing to install one in for free if your able to get your hands on one, because this whole ordeal has been a nightmare, but we got denied due to mileage with the claim. There was a difference in our timestamped pictures and the adress of where it went once it left. So they said it was driven on with a problem, because it was moved to there with a problem thats what they are denying my claims to fight it. ... nction." Auto Hold and the electronic parking brake's hold function were disabled, requiring manual brake application to prevent rollaway. Mileage at incident: approximately 21,344 miles. The simultaneous failure of Electric Power Steering, Dynamic Stability Control, and the Electronic Parking Brake while the vehicle was in motion in traffic created an immediate safety hazard. No crash or injury occurred, but a recurrence at highway speed, on a wet road, or during an evasive maneuver could foreseeably cause loss of vehicle control, crash, injury, or death. The vehicle was taken to an authorized BMW dealer. The technician stated this failure is a known issue following a recent vehicle software update, that BMW has no repair available, and that the only action taken was to power-cycle the vehicle and clear stored fault codes. No software remedy, parts replacement, or TSB was provided. I was given no assurance the failure will not recur and no timeline for a fix. To my knowledge, no recall has been issued for this defect on the i5 platform. I request that NHTSA evaluate the scope of this issue across the i5 fleet and determine whether a defect investigation or recall is warranted.Read more

The radiator for the car gets fully clogged with debris very easily. There is no screen mesh to protect the radiator from all the debris. The radiator then becomes clogged with debris in the fins and stops cooling correctly. This is not a serviceable item that an owner could accomplish. On February 20, 2026, Porsche White Plains (an authorized Porsche dealer) performed an inspection but was unable to duplicate an active fault code and declined to generate a battery State of Health (
... SoH%) report. No warranty repair was authorized. Porsche Cars North America (PCNA), Case #XXX, has since moved to close my statutory warranty claim, citing the absence of a duplicated fault. However, under New York State's adopted CARB emission warranty standards (6 NYCRR § 218-9.1), high-voltage propulsion batteries in TZEV/PZEV vehicles are warranted for 10 years or 150,000 miles from the date of original sale. My vehicle was placed in service in 2018 and remains within that statutory coverage window through 2028. PCNA has not addressed this statutory obligation or provided written confirmation of coverage. I also have not received the inspection documentation I formally requested on March 17, 2026, despite no case closure having occurred at the time of that request. Owners across multiple years consistently report: • Black screens, freezes, and reboots • Loss of HVAC control and dual-zone functions • Backup camera not displaying • Vehicle/Apps tabs failing to open • Uconnect error messages • Smaller, harder-to-read gauges after updates • Wireless CarPlay/Android Auto failures • No option to decline or roll back OTA updates These failures distract the driver, remove access to federally required equipment (rear camera), and eliminate visibility and climate controls that directly affect safe operation. The system becomes unpredictable and unstable despite the vehicle being in motion. My vehicle experiences the same issues, including: Cameras not working, screen freezing or blacks out, navigation failing, carplay unstable won’t stay connected or randomly drops and more. Because this affects essential safety-related controls and visibility, I believe this is a safety defect in Ram/Stellantis Uconnect 4 and Uconnect 5 systems.
