To Whom It May Concern, I am writing to formally report a serious safety malfunction involving my 2025 Dodge Charger Daytona R/T Stage 1 EV. This incident has caused significant property damage. I arrived home and pulled into my driveway as I routinely do. I opened my garage door and waited for it to fully open before slowly pulling my vehicle into the garage. As soon as the car was positioned in
... the garage — a maneuver I have performed many times — the vehicle suddenly and unexpectedly experienced rapid acceleration without warning. The acceleration was forceful and immediate. The vehicle propelled itself through the back wall of my garage, destroying the wall and items stored there, went airborne, and landed further out into my yard. The entire incident occurred so quickly that I was stunned and had no opportunity to react. Fortunately, I was not physically injured, but the outcome could easily have been catastrophic. The rear wall of my garage was completely destroyed, and my vehicle sustained extensive body damage. My car has been at the dealership, for approximately six weeks awaiting replacement body panels. While my insurance company has handled the property repairs efficiently, I continue to make payments on a vehicle that experienced a frightening malfunction through no fault of my own. My insurance has agreed to the supplemental estimate and committed to payment. But there are body panels that don't even have an ETA. So, I may be waiting for and making payments on a vehicle that I won't feel safe driving. After the incident, once I had time to process what occurred, I researched similar events involving this same vehicle model. I found multiple reports describing nearly identical scenarios, including unintended rapid acceleration while preparing to park. One report described a Dodge Charger Daytona EV accelerating through hedges into the road causing a 3 car crash.Read more

Woke up one morning and noticed the b pillar almost completely detached. Called ford dealership in Musk*** MI. Subject: Excessive Engine Oil Consumption – 2016 Hyundai Sonata I am reporting an ongoing and excessive engine oil consumption issue with my 2016 Hyundai Sonata that I believe may represent a safety defect or other significant reliability concern. The engine is burning oil at a rate far beyond normal expectations (requiring frequent top-off well below recommended mileage intervals), resulting
... in dangerously low oil levels while driving. I have maintained the vehicle per manufacturer recommendations and documented oil add-ins and dealer visits, but the problem has worsened over time. Despite presenting this problem to my Hyundai dealership, they have declined to authorize engine cleaning or further corrective action due to the mileage. I am struggling with high oil consumption and potential risk of engine failure while driving. This issue is not unique to my vehicle — numerous similar complaints exist in Hyundai forums and in other owners’ submitted complaints (e.g., reports of more than 1 quart consumed every ~500–1,000 miles and sequences of denied repairs) which suggests a broader pattern of oil consumption failures for this model year. Auto Safety +1 I believe this may represent a safety defect because loss of engine oil can lead to engine seizure or failure without warning, increasing the risk of a crash or injury. Please review this complaint, add it to the NHTSA vehicle database, and contact me if further information is needed.Read more

My vehicle was serviced at an authorized VW dealer on Dec 12 for recall-related work & a diagnostic water test (fee waived; $0 paid). Before leaving the dealership, while still on property, I observed extensive interior damage not present prior to service: widespread staining/saturation of headliner, sunroof shade, interior pillars (A–D), plus damage to interior trim. Substance was not colorless
... or odorless; damage was systemic, not localized. I documented immediately w/ photos taken on dealership property. I requested help/loaner; denied; required to take vehicle. After leaving, additional wet areas appeared & later dried w/ visible staining, indicating prolonged saturation following service. Dealer service documentation states rear sunroof drains were leaking; however damage was present throughout interior, including areas inconsistent w/ a rear-only drain issue. Based on the location (headliner + pillars), this raises concern for potential impact to electrical components/wiring & airbag-related areas, plus latent issues due to moisture/chemical exposure. I requested confirmation of procedures/substances used during testing; info not provided. I requested a garage liability claim; declined. Dealer offered cleaning & later proposed replacing only the headliner. This issue occurred during recall-related service, raising concern about whether OEM-approved procedures were followed during diagnostic testing. The dealer declined to confirm what liquids or methods were used. Because the damage pattern contradicts the stated diagnosis, any repair based on that diagnosis would be incomplete, potentially allowing continued intrusion or latent electrical or airbag-related issues to develop over time. The vehicle was returned to me in this condition without corrective action, leaving the potential safety and electrical risks unresolved following recall-related service. No independent post-service inspection was offered to verify system integrity.Read more

I had to turn the truck completely off then start it again, after it started I made it completely onto the lot and parked it. When I went to start it again it wouldn’t turn over or startup. ... 3/9/25 the truck wouldn’t start again and I requested for another jumpstart. The engine light came on shortly thereafter saying that the AWD wasn’t available and that the FWD train was only available. I took it in for a battery replacement and that seemed to help for a couple of months, then my headlights started going out one at a time. The passenger side bulb went out first. I replaced it with a new one at Valvoline and then the driver side bulb went out two weeks or so later and I replaced it then the check engine light came back on and I eventually had to take into the dealership for repairs and maintenance. They said they fixed it. It worked for about a 12 days and then the headlights started dimming over time as the week went on the engine light came back on and the driver side bulb was out again. So from 12/23/25 to 1/5/2026 was the length of time the truck worked but still poorly performance wise. The gas goes very quickly still and I have to fill it way too often now in its condition. I am reporting a recurring safety defect involving the front bumper assembly on my brand-new Toyota Camry Hybrid. The front bumper repeatedly detaches, separates, and pulls away from the vehicle during normal driving conditions. This has occurred multiple times despite repeated repair attempts by the authorized Toyota dealership. Toyota and the dealership have acknowledged that this is a known is
... sue affecting other vehicles and have admitted that there is currently no permanent repair remedy available. Despite this, the defect has been misclassified as “cosmetic,” which is inaccurate and unsafe. The bumper is a critical safety component designed to absorb and distribute crash forces. A bumper that is already detached or partially unsecured compromises crash force pathways and reduces occupant protection in the event of a collision. If an accident occurs, the bumper cannot perform its intended safety function, increasing the risk of injury. Additionally, I can hear air intrusion entering the vehicle through the front area where the bumper is separating, confirming that the vehicle’s exterior seal and structural integrity are compromised while driving. This vehicle has never been in an accident and is operated under normal conditions. A front bumper should not detach on a new vehicle. This is not a cosmetic concern it is a safety defect that places occupants at risk. I am requesting that NHTSA review this issue as a potential pattern defect and require Toyota to address it as a safety matter rather than mislabeling it to avoid corrective action.Read more

On 01/04/2026, while operating a gas-powered 2024 Volvo XC40, the vehicle experienced a catastrophic Sudden Unintended Acceleration (SUA) event while stationary at a red light. With the service brake fully depressed, the internal combustion engine began to surge and rev uncontrollably, making loud noises. The engine torque increased in direct response to increased brake pressure, eventually overco
... ming the braking system and forcing the vehicle forward into the 3-way intersection. Technical Details: Brake-Throttle Override (BTO) Failure: The vehicle’s safety logic failed to prioritize the brake signal over the uncommanded throttle increase. Shifter Interlock Malfunction: During the surge, the electronic gear selector was unresponsive, preventing shifting into Neutral (N) to disconnect power. Shifting capability only returned once the vehicle was brought to a forced stop. Prior Symptoms: Intermittent "idle flaring" and surging were noted 48 hours prior to the failure. Safety Risk: The vehicle moved into traffic against my intent and physical input, posing a high risk of collision. The car was towed by Volvo Road Assistance on 1/4/26 to the Volvo dealership later that evening. This incident mimics the "Powertrain Control System" defects currently documented in NHTSA Campaign 25V392 and the Becker v. Volvo class action, suggesting the software defect extends to ICE (Internal Combustion Engine) models. The car is currently at the Volvo Dealership for diagnostics. The service intake person has not provided an update on whether they have been able to duplicate the issue. I do not know what diagnostics have been performed on the car at this point. A few months ago, I received an email from CarFax letting me know that my vehicle, a 2019 Infiniti QX50 with 2.0L Turbo Engine had a major recall (NHTSA Recall Campaign No. 25V-437). The letter indicated the recall was due to issues with the engine bearings which could lead to a catastrophic failure. I wasn't sure what this meant, so I took the car to Austin INFINITI. The technicians checked the o

My understanding is that the oil pan should have been removed, drained of all oil to ensure there were no issues that could lead to the catastrophic failure of the engine. None of this was done. I was given my car back and told all was okay. Unfortunately On the evening of November 10, 2025, my vehicle’s engine suddenly seized while driving at highway speeds. Fortunately, I was able to coast onto the Frontage Road. For my protection, the Police Department arrived quickly to park police vehicles in the front and back of my car with their lights on to avoid a nighttime collision or worse. I had no idea why my engine suddenly lost power while driving down the highway and continued to refuse to start despite repeated attempts to restart the vehicle, the engine would not start. I can't eat or sleep and experience nightmares that my car died, then I am hit by a car driving at highway speeds as I tried to exit the vehicle. My wife and I are both terrified of driving the car now. I tried to resolve the issue with my local INFINITI dealership and even contacted NISSA/INFINITI executives. I was met with disregard, disrespect and a $1,700 bill to replace the starter that burned out as we kept trying to start the car. The starter burnout was a secondary cause due to the primary issue of the engine seizure. Infiniti disagreed and insisted on the $1,700 for the starter.Read more

The letter indicated the recall was due to issues with the engine bearings which could lead to a catastrophic failure. I wasn't sure what this meant, so I took the car to Austin INFINITI. The technicians checked the o
... il pan to see if there were issues. I was scared to death and this situation has exacerbated my current diagnosis of PTSD for which I am currently in treatment. This was a horrifying event that could have cost me my life.

One morning the instrumental cluster display would not display mileage, fuel capacity, temperature, or any notifications. The dealership inspected it for a blow fuse and a computer reset and determined the display shorted and would need to be replaced. I called FCA regarding a recall for this exact problem on 2020 Jeep gladiators and that this was a known safety issue released by nhtsa. I was tol
... d by FCA that this trim level was not included in this recall. My jeep Overlook was overlooked. Trim level deals with extras like heated seats not foundation electrical installation. The recall 30B is as follows: 5/8/2024ELECTRICAL SYSTEM: INSTRUMENT CLUSTER/PANEL Summary Chrysler (FCA US, LLC) is recalling certain 2020-2024 Jeep Gladiator and 2018-2024 Jeep Wrangler vehicles. The instrument panel cluster may experience an internal short circuit and fail. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 101, “Controls and Displays.” Remedy: Dealers will replace the instrument panel cluster, free of charge. Owner notification letters were mailed between October 3, 2024, and January 23, 2025. Owners may contact FCA customer service at 1-800-853-1403. FCA’s number for this recall is 30B.Read more

REPORT OF FAILED RECALL REMEDY (RECALL 19V-149 / WUE-90) My 2015 Subaru Forester experienced a total recurrence of the defect addressed in 2019. The vehicle became trapped in 'Park' intermittently on several occasions before remaining immobilized. During one incident, the manual shift-lock override failed to function. While the brake lights functioned during these incidents, the failure to shift
... is a documented symptom of a failing brake light switch. I had the switch replaced by a technician who initially suggested a full shifter assembly replacement. However, replacing ONLY the brake light switch immediately and permanently resolved the issue. This confirms the recall-related part was the point of failure. Subaru of America was notified but declined to acknowledge this as a failure of the safety remedy. I am also reporting intermittent 'ghost' activations of the right turn signal stalk.Read more

The heating steering wheel had an electrical short in the heating element where it began to melt the rubber of the steering wheel and start smoking. This became very hot where it could burn my hand. I was forced to pull over immediately to diagnose the problem before anything else could burn. The smoking stopped when I tuned off the heated steering wheel.
